        견고 구외 골신장술을 이용한 구순구개열 환아의 치험례 : CASE REPORT

        구순구개열 환자에서는 이른 시기에 시행된 수술로 인한 구순이나 구개부의 반흔 형성으로 섭식장애나 발음장애를 동반한 상악의 열성장이 나타나게 된다. 때로는 집중적인 교정치료 후에도 심한 상악골 저형성증을 보이며, 이러한 경우 성장이 완료된 후에 골이식을 동반한 악교정 수술로 상악골을 전방이동시켜 안모의 개선을 도모하기도 한다. 그러나 이러한 상악골의 전방이동은 연조직의 과도한 신장으로 인한 술후회귀현상, 추가적인 골이식이 필요하다는 한계를 가지고 있다. 골신장술은 이러한 한계점을 극복하는 최신 치료방법으로 구순구개열 환자, 두개골 융합증을 나타내는 환자 등에서 상악골을 포함한 두개 안면골의 개선에 많이 이용되고 있다. 특히 구외장치를 이용하는 골신장술은 골신장기간 중에 견인 방향의 조절이 보다 쉬우며, 충분한 양의 골신장이 가능하여 보다 좋은 결과를 얻을 수 있다. 본 증례는 상악 열성장을 보이는 구순구개열 6세 7개월의 여자 환아로, 변형된 구내 프로씨 1형 골전단술(modified Le Fort I osteotomy) 후 두개골을 고정원으로 이용하는 견고 구외 골신장술을 통해 상악골을 전진시켜 양호한 결과를 얻었기에 이를 보고하는 바이다. Patients with cleft lip and palate present severe maxillary hypoplasia due to scar of lip and palate, often accompanied by compromised mastication, speech abnormalities. Sometimes maxillary hypoplasia persist even though active orthodontic treatment was done. In theses cases, patients born with cleft lip and palate will be potential candidates for maxillary advancement with bone grafting after growth to correct the functional deformities and improve aesthetic facial proportions. But, maxillary advancement using standarf surgical approaches has several limitations: increased relapse tendency after maxillary advancement, necessity of additional bone graft and mandibular setback surgery. Distraction osteogenesis is current treatment modality to overcome these limitations, thus has become popular for treatment of maxillary hypoplasia associated cleft lip and palate, craniosyntosis. Especoally, rigid external distraction, contrary to internal device, has adbantages; better vector control of osteotomized segment, effective traction of the bony segments, the ease of the application and removal the distraction device. This study showed that relatively successful result could be generated by using rigid external distraction osteogenesis(RED) in the case of cleft lip and palate with severe maxillary hypoplasia, 6 years 7 months old.

      • 한국인 제2형 당뇨병 환자에 있어 지속적 피하 인슐린 주입법 치료시 Lispro insulin와 Regular insulin의 효과 비교

        Objective: The Lispro insulin has faster onset of effects and shorter duration than the human regular insulin, an analog of human insulin. The differences in the pharmacodynamics between the two types of insulin in the continuous subcutaneous insulin infusion(CSII) with insulin pump for treating the Korean Type 2 DM patients were analyzed. The effects on the regulation of blood sugar, the amount of insulin injected, and the frequency of hypoglycemia were compared between the lispro insulin and the regular human insulin. Method and Material: One hundred and three Korean Type 2 DM patients were randomly divided into two groups, 49 Lispro insulin and 54 Regular insulin.(Table 1) Both groups were treated for 14 days. Also at a set time before meal, insulin was injected as bolus, using a portable insulin pump, in both groups, 15 minutes before for lispro insulin and 30 minutes before for human insulin The HbA1C, C-peptide, AC/PC, height and weight were measured for each patients at the time of admission. The peripheral blood sugar level and insulin injection profile were recorded 7 times per day for each patients. Results: After treating for 7 days and 14 days, the blood sugar level normalized in both groups. There weren't any significant differences in blood sugar levels in both groups at point of the study, and also there weren't any differences in the frequency of hypoglycemia. In order to maintain the normalized blood sugar level, the lispro insulin group had higher total daily insulin requirement than the regular insulin group after 14 days of treatment (0.90±0.35 vs 0.73±0.39u/kg/day, p<0.05) This difference is probably due to the difference in basement insulin requirements rather than prandial insulin requirements (0.32±0.1 vs 0.43± 0.09u/kg/day, p<0.001). Discussion: In the hospitalized Korean Type 2 DM patients with CSII treatment, continuous subcutaneous insulin infusion therapy using lispro insulin has convenience in usage compared to the regular insulin. However, there were not any differences in the blood sugar levels. Also lispro insulin requires more insulin in order to maintain normal blood sugar level, which is probably due to the increase in the basal requirement. Also there were not any difference in the frequency of hypoglycemia.

        Prevotella nigrescens의 용혈특성에 관한 연구

        세균의 용혈활성은 세균이 숙주 내에서 생존하기 위해 필요한 철을 획득하기위한 특성이며 기능면에서 볼 때 숙주에 대한 중요한 독력인자로 간주될 수 있다. 본 연구에서는 괴사치수 및 치근단 치주염으로 진단된 환자의 근관에서 분리한 Prevotella nigrescens의 용혈활성을 다양한 조건 하에서 측정하여 그 특성을 규명하는 것을 목적으로 한다. 1. 임상에서 분리한 P. nigrescers와 표준균주인 P. nigrescers ATCC 33563에서 모두 용혈활성이 나타났다. 2. 사람, 면양 및 말 세 가지 종에 대해 용혈활성을 비교한 결과 사람의 적혈구에서 가장 강한 용혈활성을 나타내었다. 3. 용혈소 억제제인 NaN₃와 dithiothreitol (DTT)는 농도의존적으로 P.nigrescers의 용혈활성을 감소시켰다 (p<0.05). 4. P. nigrescers가 최대 용혈활성을 나타내는 최적의 pH는 4이었으며, 50℃이하의 온도에서는 용혈활성을 보였으나 95℃에서 급격히 감소하였다. 5.배양조건에 따른 P. nigrescens의 용혈활성을비교한 결과 10% CO₂배양기에 배양한 경우혐기성 조건에서 배양한 것보다 더 높은 용혈활성을 보였다. Hemolytic property is a specific feature of bacteria to obtain iron which is essential for its survival in host tissues. Therefore, it is thought to be one of several factors of virulence. The purpose of this study was to investigate the hemolytic properties of Prevotella nigrescens isolated from the teeth diagnosed as pulp necrosis and apical periodontitis under the presence of hemolysin inhibitors such as NaN3 and dithiothreitol, heat, various pH and cultural conditions. The results were as follows; 1. Clinically isolated P. nigrescens strains and standard P. nigrscens ATCC 33563 showed hemolytic activity. 2. P. nigrescens showed higher hemolytic activity against human erythrocytes than sheep or horse erythrocytes. 3. NaN₃ and dithiothreitol (DTT) reduced the hemolytic activity of P. nigrescens in a dose dependent manner (p<0.05). 4. Optimal pH for the maximum hemolytic activity of P. nigrescens was 4.0 and the hemolysin was stable under the 50℃, but the hemolytic activity was significantly decreased at 95℃. 5. P. nigrescens cultured in 10% CO₂ condition showed higher hemolytic activity than the bacteria cultured in the anaerobic condition.[J Kor Acad Cons Dent 30(4):335-343,2005]

        With the widespread use of fertility medications and techniques of in vitro fertilization, multiple gestation has become epidemic in modern obstetric practice. Delivery of the initial fetus in a multiple gestation usually is followed by delivery of the subsequent fetus or fetuses shortly thereafter. However in rare circumstances, the delivery is delayed for days due to disappearance of uterine contraction after delivery of the first fetus. When uterine activity ceases after the delivery of the first neonate, a policy of nonintervention may be considered in case of an immature or very premature delivery, in order to achieve a gestational age for the remaining fetus(es) more compatible with neonatal survival. We report a case of twin gestation with prolongation of the delivery interval between the twins for 46 days.

        Comparison of Bradykinin - and Platelet - Derived Growth Factor - Induced Phosphoinositide Turnover in NIH 3T3 Cells

        Phosphoinositide turnover in response to platelet-derived growth factor, epidermal growth factor, and bradykinin was evaluated in NIH 3T3 cells. Platelet-derived growth factor and bradykinin induced a significant increase in incorporation of ^(32)P into phosphatidylinositol (PI), phosphatidylinositol 4-monophosphate (PIP), and phosphatidylinositol 4,5-bisphosphate (PIP₂) in serum-starved NIH 3T3 cells. However, epidermal growth factor increased incorporation of BHP into these phosphoinositides by only a small amount. Stimulation with platelet-derived growth factor, not bradykinin, caused a rapid elevation of PI and PIP kinase activities that were maximally activated within 10 min. The maximal levels of their elevation in cells with plateletderived growth factor stimulation were 3.2-fold for PI kinase, and 2.1-fold for PIP kinase. Short term pretreatment of NIH 3T3 cells with phorbol 12-myristate 13-acetate, activator of protein kinase C, caused an approximately 60% decrease in platelet-derived growth factor-induced PI kinase activities, indicating the feedback regulation of phosphoinositide turnover by protein kinase C. These results suggest that although the enhancement of phosphoinositide turnover is a rapidly occurring response in platelet-derived growth factor- or bradykinin-stimulated NIH 3T3 cells, phosphoinositide kinases may be associated with initial signal transduction pathway relevant to platelet-derived growth factor but not to bradykinin.

      • 좌측흉수로 발현된 Boerhaave 증후군 1예

        구토 후 발생한 토혈을 주소로 내원한 환자에서 좌측 늑막 삼출 동반 시 식도 파열의 가능성을 고려하여 흉수 천자 및 지속적인 흉부 단순 촬영의 추적관찰이 필요하며 식도 파열이 배제된 후 환자의 생체 징후 및 토혈 양을 파악하여 내시경 검사 시기를 신중히 결정하는 것이 좋을 것으로 생각된다. 본 증례는 토혈을 주소로 내원하여 Mallory-Weiss 증후군 의심 하에 내시경을 통한 치료 시행 후 추적 관찰에 의해 진단된 Boerhaave 증후군을 경험하였기에 보고하는 바이다. Boerhaave's syndrome is a spontaneous perforation of the esophagus most commonly resulting from a sudden increase in intraesophageal pressure combined with negative intrathoracic pressure caused by straining or vomiting. Surgery should generally be performed within 24 hours because mortality approached 30 to 50 percent in which surgery was delayed. Therefore, early diagnosis and prompt surgical repair are critical for survival. We have experienced a case of Boerhaave's syndrome in a patient presenting with hematemesis with left pleural effusion without any signs or symptoms of pneumomediastinum or pneumothorax.

      • Enhanced Extraction of trans-Resveratrol from Polygonum cuspidatum by a Combination of Thermal and Enzymatic Treatment

        Resveratrol (RES), a naturally occurring phytoalexin, has been known for health beneficial functions, such as cardioprotective, antihypertensive, and antiaging properties. The root of Polygonum cuspidatum Sieb. et Zucc. (P. cuspidatum) is extensively used as a good source of trans-RES, the most bioavailable form of RES. Our previous study showed an improved conversion rate of piceid (PD) present in grape peel into RES by pre-heating at 95°C for 10 min, followed enzymatic hydrolysis at 50°C for 60 min. In this study, we examined if combined thermal and enzymatic treatment could maximize extractability of PD and RES from P. cuspidatum. Results from this study showed that the treatment of P. cuspidatum with Viscozyme dramatically increased the conversion of PD into RES as well as the entire extractability of RES and PD among several enzymes tested. Furthermore, 60% and 80% ethanolic solvents showed the best extraction yield of RES. The extraction method established in this study is a low-cost green processing strategy and could be applied to maximize extraction of bioactive trans-RES from P. cuspidatum.

      • 간경변 환자에서 Oro-cecal Transit Time의 측정

        Background: Neurotransmitter like substances in the gut have been proposed to contribute to the overall portal systemic encephalopathy (PSE). The source of the ammonia in patients with PSE is mainly the small bowel, as a result of the conversion of glutamine in the diet to glutamate and ammonia. PSE is highly responsive to changes in diet, to antibiotic therapy and to ingestion of nondigestible disaccharides such as lactulose. This study was performed to evaluate the relationship between PSE and oro-cecal transit time. In addition, performed to evaluate the effect of severity, cause, complications of liver cirrhosis on oro-cecal transit time. Methods: Between August 2006 and February 2007, 48 cirrhotic patients consecutively was enrolled in study their oro-cecal transit time after informed consent was obtained. A non-invasive hydrogen breath test was used to study oro-cecal transit time (OCTT). Result: The OCTT were delayed in cirrhotic patients with PSE compared without PSE (123.3 ± 42.7 min vs 170.0 ± 58.3 min, p<0.05). OCTT tended to prolongate further as the damage in the liver got worse, nevertheless the result was statistically meaningless (p<0.111). Neither presence of ascites or cause of PSE had any influence on gastrointestinal transit. Conclusions: Cirrhotic patients with PSE have more delayed OCTT than without PSE. Further research is needed to find out the relationship between OCTT and severity of liver cirrhosis.
