        Escherichia coli 패혈증 환자에 합병된 대칭적 하지 말단 괴사증 1예

        We have encountered a rare case of symmetrical peripheral gangrene complicating Escherichia coli sepsis in a 47-years-old male. He was successfully treated with antibiotics, anticoagulants, and vasodilator. To our knowledge, this is the first report on symmetrical peripheral gangrene complicating E. coli sepsis in Korea.

        돈분과 피마자박의 혼합비율에 따른 퇴비화 연구

        This study was conducted to estimate the humificating grade according to experiment of physico-chemical characterization and phyto-toxicity during composting of using with pig manure and castor meal. The material ratios of the compost which was mixed with pig manure(P), castor meal(C), and saw dust(S) were 5 : 1 : 4(PCS-I), 5 : 2.5 : 2.5(PCS-II) and 5 : 4 : 1(PCS-III) by volume to volume, and they were decomposed for 60 days. In the result, the changes of temperature in all treatments during composting were rapidly increased more than 68℃ at the incipient stage, and gradually decreased within 39℃ at 60 days the after treatment. pH was slowly increased from 7.5 to 7.7, and the C/N ratio was 13∼14 at the final composting stage. The low C/N ratio value in this compost was caused by the castor meal contented high nitrogen level(T-N 5.7 %). G.I.(germination index) was showed 73 to 78 range in all treatments at the 60^(th) day. Among all treatments PCS-I was appeared to be the best condition for composting.

        12주간 태권도 수련프로그램이 비만 초등학생의 혈중지질과 성장호르몬에 미치는 영향

        This study aims to examine the effects of 12weeks taekwondo training program for 12 weeks in obese children. 20 boys in the 4th~6th grades of an elementary school located in S-gu, Gwang-ju served as the subjects of the study. The following selection criteria were used: a student who did not take regular exercise and was judged as obese with BMI of 25 kg/m2 or above in a physical examination. 12 children were randomly assigned to the taekwondo training program group and 10 children were done to the control group. The taekwondo group was trained at 4~5 pm, three times a week(Monday, Wednesday, and Friday) for 12 weeks. Measurement factors included body composition(weight, BFP), body lipid(TC, TG, HDL-C), and Growth hormone. Means(M) and standard deviation(SD) of each group were computed using SPSS(Version 17.0) program and represented by a chart. Two-way ANOVA with repeated measure was conducted to test difference between groups and between measuring times. After exercise was conducted for 12 weeks, the following results were obtained. First, for body composition changes, the taekwondo training group showed significant decrease in weight and bodyfat suggesting positive effects of taekwondo training. Second, for blood lipid changes, the taekwondo training group showed significant decrease in TC and TG, or increase in HDL-C, suggesting positive effects of taekwondo training. Third, for Growth hormone changes, the taekwondo training group showed significant increase, suggesting positive effects of taekwondo training. Based on the results, regular taekwondo training decreased weight and bodyfat and prevented metabolic syndrome in obese children. It also decreased TC and TG and increased HDL-C and Growth hormone, suggesting a positive effect to prevent cardiovascular diseases. Further study should develop a variety of exercise programs for obese children to enjoy exercise.

      • ATSC DTV 시스템에서 수신 파워 레벨 보상을 이용한 타이밍 동기 기법

        ATSC DTV시스템에서는 심볼타이밍 동기를 위해서 ATSC규격에 소개되어있는 77.3 ㎍ 마다 반복적으로 삽입되어있는 세그먼트 싱크를 이용하는 세그먼트 동기 방법 또는 QAM과 같은 다중레벨을 가지는 신호에 일반적으로 사용되는 가드너(Gardner)방법을 사용한다. 이중 가드너 방법은 매심볼마다 타이밍 에러성분을 추출하므로 다중경로 채널에서 타이밍동기를 추적하면서 유지하는데 유리한 방식이어서 일반적으로 사용한다. 가드너 방법을 이용하는 ATSC DTV시스템에서 가드너 방법에 에러를 검출하기 위해 사용되는 가드너 타이밍 에러 검출기(Timing Error Detector)는 수신단의 파워레벨이 기준 파워레 벨에서 크게 벗어날 경우 에러를 검출 할 수 없는 문제점을 가지고 있다. 이를 해결 하기 위해 가드너 타이밍 에러 검출기 블록 앞에 송신파워 레벨과 수신파워 레벨의 비를 이용하여 정상적인 수신 파워 레벨로 수신할 수 있도록 보정하는 블록을 추가하여 전체적인 동기성능을 향상시키는 알고리즘을 제안한다.

      • Synergistic mucus secretion by histamine and IL-4 through TMEM16A in airway epithelium

        <P>Histamine is an important mediator of allergic reactions, and mucus hypersecretion is a major allergic symptom. However, the direct effect of histamine on mucus secretion from airway mucosal epithelia has not been clearly demonstrated. TMEM16A is a Ca2+ -activated chloride channel, and it is closely related to fluid secretion in airway mucosal epithelia. We investigated whether histamine directly induces fluid secretion from epithelial cells or submucosal glands (SMG) and mechanisms related, therewith, in allergic airway diseases. In pig airway tissues from the nose or trachea, histamine was a potent secretagogue that directly induced strong responses. However, gland secretion from human nasal tissue was not induced by histamine, even in allergic rhinitis patients. Histamine type 1 receptor (H1R) and histamine type 2 receptor (H2R) were not noted in SMG by in situ hybridization. Cultured primary human nasal epithelial (NHE) cells were used for the measurement of short-circuit current changes with the Ussing chamber. Histamine-induced slight responses of anion secretions under normal conditions. The response was enhanced by IL-4 stimulation through TMEM16A, which might be related to fluid hypersecretion in allergic rhinitis. Pretreatment with IL-4 augmented the histamine response that was suppressed by a TMEM16A inhibitor. TMEM16A expression was enhanced by 24-h treatment of IL-4 in human nasal epithelial cells. The expression of TMEM16A was significantly elevated in an allergic rhinitis group, compared with a control group. We elucidated histamine-induced fluid secretions in synergy with IL-4 through TMEM16A in the human airway epithelium. In addition, we observed species differences between pigs and humans in terms of gland secretion of histamine.</P>

      • 적응적 루프필터의 지터 변화율을 이용한 ATSC DTV 시스템의 심볼 타이밍 동기 성능 개선

        ATSC 지상파 DTV 시스템에서 심볼 타이밍 동기 성능 개선을 위한 알고리즘을 제안한다. 일반적으로 심볼 타이밍 동기를 위해 사용되는 가드너 방법은 다중 경로 페이딩 환경에서 성능이 좋지만 지터에 의해 성능 열화가 발생한다. 지터량는 루프 필터 대역폭이 작을수록 작아지지만, 수렴속도는 느려지게 된다. 수렴속도는 빠르면서 수렴 후 지터량를 감소시키기 위해 일정시간마다 루프필터의 출력 값을 평균하고 이 평균값을 이용하여 옵셋량을 추정한 후 추정된 옵셋의 변화율에 따라 루프 필터의 대역폭을 줄여 지터의 크기를 줄이는 알고리즘을 제안한다.

      • CT 조영검사 시 적정한 조영제 온도 설정을 위한 온장고 온도 및 가온시간에 관한 연구

        목적 : 예측 가능한 조영제 부작용 중 하나인 조영제 용액 온도와 관련하여 온장고의 설정 온도와 가온시간을 변화시켜 인체의 온도와 비슷하게 가온되는 각각의 기준에 대하여 알아보고자 하였다. 대상 및 방법 : 조영제 보관시 사용되는 온장고(FINE HNC)를 37℃, 42℃, 47℃ 변화시켜 설정하고 30분 단위로 디지털 온도계를 이용하여 용기 안에 담긴 조영제의 온도를 측정하였다. 먼저, 제조회사가 다르고 요오드 함량이 같은 조영제 Iobrix350 150ml, Xenetix350 150ml, Omnihexol350 150ml, Bonorex350 150ml를 사용하여 온도를 비교 측정 하였다. 두 번째 측정은 요오드 함량이 다른 조영제 Optiray320, Omnihexol350. Pamiray370, Iomeron400을 사용하여 온도를 비교측정 하였다. 세 번째 측정은 Ultravist 370을 보관 용기의 용량을 100ml, 150ml, 500ml 달리 하여 온도를 비교측정 하였다. 마지막 측정은 체온과 비슷한 적정온도의 조영제를 상온 22℃에서 장시간 노출시 온도변화를 측정하기 위해 30분 단위로 500ml용기에 담긴 조영제의 온도를 측정하였다. 결과 : 측정결과 온장고의 설정온도는 42℃로 설정하고, 조영제 온장고 보관시간은 100ml, 150ml용기에 담긴 조영제는 3시간, 500ml 용기에 담긴 조영제는 4시간 지나야 체온 36~37℃와 비슷한 적정 온도에 도달하였다. 적정온도의 500ml 용기에 담긴 조영제의 상온 22℃ 장시간 노출시 1시간이 지나면 3℃ 낮아졌다. 조영제 제조회사별, 요오드 함량별 온도변화 차이는 크지 않았다. 결론 : 체온과 비슷한 조영제 용액의 온도를 맞추기 위해 온장고의 설정 온도 및 가온시간을 적절히 조절한다면 환자의 혈관통 감소 및 고온의 용액 사용으로 인한 부작용 등을 줄일 수 있어 CT 조영검사의 만족도 향상에 기여가 될 것으로 사료된다. Purpose : To propose safe and proper standard of storage and use for contrast by measuring time that temperature reaches to 36℃~37℃ when storing contrast media in a contrast warmer, and checking whether the temperature presented in the contrast warmer and the real temperature is same. Material and method : We established the temperatures like 37℃ 42℃ and 47℃ differently using the contrast warmer(FINE HNC) for contrast media which is now using and measured the temperatures for thirty minutes on a case-by-case using a digital temperature indicator. First, temperature were measured and compared in the iodine content of the different maker Iobrix350 150ml, Xenetix350 150ml, Omnihexol350 150ml, Bonorex350 150ml. Second, temperature were measured and compared in the Iodine content of different contrast agents Optiray320, Omnihexol350, Pamiray370, lomeron400. Third, temperature were measured and compared in capacity of the storage containersuch as 100ml container, 150ml and 500ml using Ultravist370. Lastly, We measured temperatures When exposed for a long time at room temperature 22℃, 500ml contrast media which similar to a proper temperature. Result : The temperatures of contrast media were the closest 36℃~37℃ when setting 42℃ by contrast warmer, to use 100ml, 150ml containers in contrast media should be kept before 3 hours, and 500ml should be kept before 4 hours. After 1 hours at room temperature 22℃ when exposed to lower 3℃ in capacity of the storage container 500ml. There was no difference in the iodine content of the different maker and the Iodine content of different contrast agents. Conclusion : The use of contrast media in proper temperature would be very helpful to reduce side effects by providing viscosity and to minimize the impact on the human body

      • Clinical Study on the Iron Absorption from Heme-Iron Polypeptide and Nonheme-Iron

        Iron bound to heme appears to be more bioavailable than iron salts. A clinical study was performed to investigate the absorption efficiency of heme-iron and iron-salt products available. Heme-iron and nonheme-iron supplements have become available in Korea. We performed iron absorption studies to compare the absorption of heme-iron polypeptide (HIP) products made from digested hemoglobin, produced in Korea (HIPk) and imported from Japan (HIj), with that of iron salts. In the study, 80 subjects were divided into 5 groups (n=56): placebo group; 12 mg glucose, HIPk group; 12 mg iron as HIPk, HIj group; 12 mg iron as HIj, iron-salt group 1; 12 mg of iron as ferrous aminoacetate, and iron-salt group 2; 100 mg iron as ferrous aminoacetate. Changes in serum iron levels were measured at 3 and 5 hours post ingestion. Absorption of iron in HIPk was higher compared to HIj, iron-salt or placebo. There was a significant inverse correlation between low serum iron levels $(<80{\mu}g/dl)$ and iron absorption from HIPk. These results demonstrated that HIPk was more bioavailable, even taken with a meal, and would have potential advantages over iron salt or HIj as an iron supplement. Our results indicate that heme-iron absorption is regulated by iron status through a heme receptor, whereas iron-salt absorption is unregulated.
