        Carbohydrate and fat ingestion during exercise are essential nutrient, but a number of opinion is different from their contents of food. In this point of view, to study how high-fat dietary contents affects glycogen utilization, 48 female Sprague-Dawley rats were trained on the treadmill trained for 4wk with consuming two high-fat diet and one high-carbohydrate diet. All animals were fed 79%high-fat diet(n=16, 79F), 40% high-fat diet(n=16, 40F), 70% high-carbohydrate diet(n=16, 70C). After the final day of training, one-half of rats in each the three groups were run to exhaustion (E), whereas the remaining rats served as rested controls (R). All animals were trained on a motorized rodent treadmill. The animals ran for 30min/d at 28m/min(0% grade), analysis items were glycogen, body weight and %Fat were measured. All data were examined by means of one-way ANOVA and significance was set at the p<0.05. The Duncan method was used for a post hoc test. Body weight were similar among the 79F, 40F and 70C groups. %Fat was significantly greater in animals that consumed the 79F group than in those on the 70C group (P<0.05), even though similar body weight each other. Generally liver and muscle glycogen concentration had more in 79FR, on the other hand, had less in 79FE compared with the others. The mean overall glycogen utilization rate was depressed in muscle and liver after animals were on the 79% F diet. In summary, the present study has suggested that consumption of 79% F diet during endurance training can lead to an increase in exercise performance in female rats. Some of the consequences training on F diet are an increase in body fat deposition and slower utilization of glycogen during exercise. These data indicate that rats exposed to a high-fat diet are capable of prolonging exercise in spite of limited glycogen stores. This improved capacity for exercise appears to be partially the result of muscular adaptations to the diet, which apparently increase the ability to oxidize fat and concomitantly spare glycogen.

      • Robot assisted laparoscopic myomectomy, an alternative to laparotomy for numerous myomas (over 10)

        목적: To evaluate the feasibility of robot-assisted laparoscopic myomectomy in multiple myomas over 10 in number and to compare outcomes with open myomectomy. 방법: This study is a retrospective chart review of 216 patients who underwent robot assisted laparoscopic myomectomy by a single operator at St. Mary's Fibroid Center in Seoul between October 1, 2010 and January 31, 2014; in the selected patients, 10 or more uterine myomas were removed. We reported the characteristics of the removed myomas including the with maximum diameter of the myomas, the sum of the diameter of each myoma, and the types of the combined surgeries. we organized surgical outcomes that were measured including the operative time, estimated intraoperative blood loss, length of hospital stay, and perioperative complications. And we compared these surgical outcomes with cases of open myomectomy. 결과: A total of 13 women underwent the removal of 10 or more uterine myomas by robotics. The patient age was 37.6±3.6 years, and all of the patients were nulliparous. The operation time was 389.8±100.6 min. The number of removed myomas for each case was 13.1±3.8 (range 10-20). The myoma with the maximum diameter was 6.5±0.7 centimeters (range 5.0-8.0 centimeters). The sum of the diameters was 34.9 ±9.6 centimeters (range 20.0-54.5 centimeter. The Estimated blood loss during operation was 219.2±158 milliliter. . The postoperative hospital stay was 2.6±0.6 days. Compared to laparotomic adenomyomectomy, robotic surgery has strengths that enables less blood loss, shorter hospital stay and decreasing risk of blood transfusion although it takes more operation time. 결론: This report is the first to describe the feasibility of robot-assisted laparoscopic myomectomy in multiple myomas over 10 in number. In robot- assisted laparoscopic myomectomies, supplementary techniques including preoperative MRI assessment, intraoperative sonographic navigation, and palpation by an assistant to localize the myomas are helpful in overcoming the absence of haptic perception. This technique is considered to be an alternative to laparotomy for more than 10 myomas.

        Pulse Oximetry and Skin Temperature Gradient as Diagnostic Tools of Successful Caudal Block

        Background: Though caudal block is a relative simple technique, it has not been widely used in adults because of a high failure rate. We assumed that any tests to quantify the changes of sympathetic tone in the affected areas would be excellent indicators of successful block. We tested the usefulness of two candidates (pulse oximetry plethysmographic waveform amplitude measured at 5th toe and calf minus 5th toe skin temperature gradient) as indicators of successful caudal block. Methods: In 45 adult patients undergoing anal surgery with caudal block, these two variables were simultaneously measured at 2-min intervals for 20 min. A two-fold increase in the plethysmographic waveform amplitude from baseline and skin temperature gradient of 0°C were predefined as test criteria of successful block. Results: While the sensitivity, specificity, positive predictive value, and negative predictive value of the skin temperature gradient test were 45.9%, 100%, 100%, and 9.1%, those of the plethysmographic waveform test were 86.5%, 100%, 100%, and 28.6%. The plethysmographic waveform test showed a significantly higher discriminative capacity than the skin temperature gradient test (94.9% vs. 48.7%, P < 0.05) Conclusions: Unlike the skin temperature gradient test, the plethysmographic waveform test showed a considerably high validity in detecting successful block. Considering its simple and real time monitoring potentials together with a high failure rate of caudal block in adults, we cautiously recommend it as a supplemental diagnostic tool to predict successful block, especially when verbal communication with patient is difficult.         백서의 심실세동성 심정지 모델에서 해마신경세포 손상에 대한 단백질 합성저해제의 효과

        Background: The goal of successful resuscitation is not only to stop the process of ischemia as soon as possible but also to overcome the secondary injury process after resuscitation, which involves a complex interplay of mechanisms. Brain damage accompanying cardiac arrest and resuscitation is frequent and devastating. Cells die by one of two mechanisms: necrosis or delayed neuronal death. Delayed neuronal death may require protein synthesis. Neurons in the CA1 subfield of the hippocampus are selectively vulnerable to death after injury by ischemia and reperfusion. Death of these neurons occurs after an interval of 1 or 2 days. We assessed the effects of a protein synthesis inhibitor, cycloheximide(CHX), on hippocampal neuronal death of rats by using the ventricular fibrillation cardiac arrest(VFCA) model. Methods: The effect of CHX(3 mg/kg, s.c.) on hippocampal neuronal death was studied in two groups of 18 rats each, one group being subjected to a 2-min VFCA and the other to a 3-min VFCA. Each group was divided into three subgroups: control(groupⅠ,Ⅱ) without subcutaneous injection of CHX, "esp-12" of group Ⅰ/Ⅱ treated with CHX 12 hours after return of spontaneous circulation (ROSC), and "exe-24" of group Ⅰ/Ⅱ treated with CHX 24 hours after ROSC. The coronal sections of the hippocampus levels were stained with hematoxylin-eosin after 72 hours of survival. The histologic damage score(HDS) was used to assign a score to the total number of damaged neurons counted in each of the hippocampal CA1 subfields. Results: 1. There were not significant differences in heart rates, blood pressures, blood sugar, and blood gas in group I & Ⅱ during the pre-arrest steady state or at 5 min and 30 min after ROSC. 2. In group I & Ⅱ, the HDS, were significantly reduced in rats(I exp-12, 1.1 ±0.6;Ⅰexp-24, 1.3 ±0.5;Ⅱ exp-12, 1.4±0.7; and Ⅱ exp-24, 1.8±0.8) treated with CHX 12 hours or 24 hours after ROSC than control rats(1,2.5 ±0.9;Ⅱ,2.9±0.8)(p<0.05). Conclusion: These results suggest that delayed hippocampal neuronal death from ischemic insult after ventricular fibrillation cardiac arrest followed by resuscitation can be prevented by a protein synthesis inhibitor, CHX. Further experimental studies of the action mechanism of protein synthesis inhibitors to delayed neuronal death and clinical applications are required.

      • 흰쥐에서 탄수화물 종류가 운동시 당 및 지질대사에 미치는 영향

        The purpose of this study was to investigate the effects of glucose, sucrose, and glucose polymer(GP) ingestion on the carbohydrate and lipid metabolism during treadmill running in rats. Rats were randomly assigned to normal, control and carbohydrate groups. Carbohydrate group was divided into glucose, sucrose and GP ingested rats. The experiment was carried out after overnight fasting. Rats were administrated either normal saline(control) or carbohydrates solutions of formulated glucose(6%), sucrose(6%) and GP(6%) via polyethlene tube to stomach before treadmill run. Exercise was loaded by a treadmill run at the speed of 1 km/hour for 30, 60 and 120 minutes. The results of experiment were as follows: Plasma glucose levels were decreased in proportion to the exercise time in control, but increased at treadmill run 30 minutes and then gradually decreased with the lapse of the exercise time in carbohydrates compared with normal rats. It decreased in control, glucose, sucrose and GP compared with normal rats in decreasing order at treadmill run 120 minutes(% change of plasma glucose level of control, glucose, sucrose and GP compared with normal rats were 67, 74, 87 and 91 in plasma glucose level at treadmill run 120 min., respectively). Plasma lactate levels were increased by the exercise loading compared with normal in control and carbohydrates ingested rats. It increased in control, sucrose and GP in increasing order compared with normal rats, at treadmill run 120 minutes(% change of plasma lactate level of control, glucose, sucrose and GP compared with normal rats were 167, 185, 149 and 114 in plasma lactate level at treadmill run 120 min., respectively). Plasma FFA levels had tendency to increase by the exercise loading compared with normal in control and carbohydrates ingested rats. It showed increase in the sucrose, glucose, GP and control rats in Increasing order compared with normal rats at treadmill run 120 minutes(% change of plasma FFA level of control, glucose, sucrose and GP compared with normal rats were 151, 153, 166 and 152 in plasma FFA level at treadmill run 120 min., respectively). Plasma acetoacetate levels were higher in control compared with carbohydrates ingested rats at treadmill run 30, 60 and 120 minutes. Plasma beta-hydroxybutyrate levels increased by exercise compared with normal rats in control and carbohydrates ingested rats at treadmill run 30, 60, 120 minutes. Plasma insulin levels were decreased in proportion to the exercise time in control compared with normal rats, but in carbohydrates ingested rats increased at treadmill run 30 minutes and then gradually deceased with the lapse of the exercise time compared with normal. In carbohydrates ingested rats, plasma insulin levies were higher than control. Hematocrit did not show any difference between control and carbohydrates rats at treadmill run 30, 60 and 120 minutes. From these experimental results, it is suggested that the exhaustion of skeletal muscle glycogen, the hypoglycemia, the overproduction of lactate and dehydration is delayed in carbohydrates ingested rats. GP of the carbohydrates is considered as the most effective for these effects. Therefore, intake of carbohydrates before exercise, especially GP seems to increase performance in the prolonged moderate exercise.

        인체혈장 중 에탐부톨의 HPLC 분석법의 검증 및 단일용량 투여에 의한 약물동태 연구

        An HPLC method was employed for the determination of ethambutol in human plasma. After addition of internal standard (IS, octylamine, 2 μg/mL) and alkalinization of the plasma with 5 M sodium hydroxide, the drug and IS were extracted into the mixture of chloroform and diethyl ether (40:60, v/v). Following a 15-min vortex-mixing and a 10-min centrifugation, the organic phase was spiked with 100 pL of phenylethylisocyanate (2000 μg/mL) for chemical derivatization, mixed for 5 min and evaporated to dryness under a stream of nitrogen. The residue was reconstituted with 100 μL of mobile phase and 20 pL was injected into Cl8 column with a mobile phase consisting of methanol:water (70:30, v/v). The samples were detected utilizing an ultraviolet detector at 200 nm. The method was specific and validated with a limit of 0.15 μg/mL. Infra- and inter-day precision and accuracy were acceptable for all quality control samples including the lower limit of quantification. The applicability of this method was demonstrated by analysis of human plasma after oral administration of a single 1200-mg dose to 20 healthy subjects. From the plasma ethambutol concentration vs. time curves, the mean AUC was 9.61 ± 1.64 μg hr/mL. and Cmax of 2.68 μg/mL reached 2.73 hr after administration. The mean biological half-life of ethambutol was 3.46 ± 1.21 hr. Based on the results, this simple and validated assay could readily be used in any pharmacokinetic studies using humans.

        中學 科學敎育의 探究學習에 대한 評價問項 開發 (Ⅱ)

        This study was accomplished to develope the evaluation items for inquiry learning in the 2nd grade Middle School Science for the consecutive study of the evaluation items for inquiry learning in the 1st grade Middle School Science(U-Hang Ki et al, 1984). In this study, paper and pencil test items and performance test items are made by analyzing the abilities of inquiry according to the contents and four basic experiments from each unit in the 2nd grade Middle school science. These evaluation items were applied to tke five classes of the 2nd grade of middle school to test their validity. It is desirable that performance test schuld be used for the evaluation for the abilities of inquiry which can not be evaluated by paper and pencil test. In the evaluation methods of performance test, tester evaluation, peer evaluation and self-evaluation can be applied to the science class in a multi-student class. In higher grade, however tester evaluation is more desirable than peer and self evaluation. It is found that peer evaluation and self-evaluation make possible the perfect study by feedback.

        다발성 연조직 종괴를 동반한 급성 림프구성 백혈병의 증례 보고

        A 15-year-old patient, who had been diagnosed and treated as Burkitt cell type acute lymphoblastic leukemia (ALLL3) already, visited our department. He complained of gingival enlargement and loosening teeth 1 month ago. The clinical examination revealed anterior open bite, gingival enlargement, and nontender swelling particularly in molar regions of both jaws. Deep periodontal pockets and severe mobility was shown on most of the teeth. The panoramic radiographs showed severe bone destruction and extrusion of the molars. The contrast enhanced CT showed multiple enhanced mass and bone marrow obliteration in both jaws. Chemotherapy was done and the swelling was subsided at 1 month later. In conclusion, radiologic findings of leukemia with soft tissue mass, known as chloroma or granulocytic sarcoma, mimic those of lymphoma, so blood test may be needed for the final diagnosis.

        정신간호조무사 양성소 교육과정 전후의 정신질환에 대한 태도조사

        이 연구는 정신질환에 대한 태도 변화가 적절한 교육에 의하여 변화할 수 있다는 관점에서, 정신질환자 치료에 일익을 담당하게 정신간호조무사를 양성하는 국립서울정신병원 부설 정신 간호조무사 양성소의 학생을 대상으로 정신질환에 대한 태도 조사를 하여, 정신과 교육이 정신질환에 대한 태도에 미치는 영향을 보았다, 연구 방법은 1993년과 1994년에 양성소 교육을 받고 전과정을 수료한 39명을 대상으로, 정신질환에 대한 태도를 묻는 설문지를 사용하여 정신과 교육전, 후의 정신질환에 대한 태도의 변화를 보았으며, 그 결과는 다음과 같다. 먼저 양성소 교육이 정신질한에 대한 태도에 미친 영향을 보면 다음과 같다. 1) 정신병 환자에 대한 개념 및 태도는 긍정적인 방향으로 변하였다(p<0.05). 2) 정신병원에 입원한 환자에 대한 치료 전망, 개방병동에 대한 태도의 변화는 부정적인 방향으로 변화하였다(p<0.05). 3) 나머지 신경증에 대한 개념, 정신질환에 대한 부정적 편견, 정신질환의 치료 예후에 대한 태도, 입원치료와 가정내 보호에 대한 태도, 정신병동의 기능에 대한 태도는 통계적으로 유의한 수준의 변화가 없었다. 이 연구의 조사 대상군의 특성을 알아보기 위해 동일한 설문을 사용하여 조사한 일반인 군, 의과 대학생 군과 비교해 보면 다음과 같은 결과를 얻었다. 1) 이 연구의 교육전 대상군은 정신과 실습을 받지 않은 의과대학생군과 정신질환에 대한 태도를 묻는전체 문항에서 통계적으로 유의한 차이를 보이지 않았다. 2) 이 연구의 교육적 대상군은 일반인보다 신경증에 대한 개념, 정신질환의 치료 예후에 대한 태도, 정신병원에 입원되어 있는 환자의 치료전망에 대한 태도를 묻는 문항에서 일반인보다 부정적으로 대답하였다. Objective : This study was designed to evaluate the effect of the psychiatric nursing assistant education done at Seoul National Psychiatric Assistant School on the attitude toward mental illness. Methods : The subjects were 39 graduates from Seoul National Psychiatric Assistant School, during 1993- 1994. They were assessed during pre- and post-education periods with Scale for Mental Health Attitude(Star 1952). Chi-square test was used for statistical analysis. Results : 1) Statistically significant changes were shown in concepts and attitudes toward mentally ill patients, therapeutic prospect to inpatient, and attitude toward open ward (p<0.05). 2) Statistically significant changes were not shown in concepts of neurosis, negativistic bias toward mental illness, therapeutic prognosis of mental illness, attitude toward inpatient treatment and care in home, and attitude toward function of psychiatric ward. Conclusion : We conclude that psychiatric nursing assistant education needs further improvement.
