Locally advanced breast cancer (tumor > 5 cm, widespread infiltration of the skin and muscle, or metastases to lymph nodes) is difficult to resect by surgery, and even when it is resectable, there is a high probability of local recurrence and distant metastasis. Therefore, systemic therapy should be administered first. However, as cutaneous infiltration progresses, the patient's quality of life is impaired by pain, bleeding, presence of exudates, and a foulsmelling odor. Treatment with Mohs paste with systemic therapy can control symptoms associated with skin infiltration and can also be expected to decrease tumor volume. Herein, we report a case in which a tumor was resected following Mohs paste and systemic chemotherapy administration, and the skin defect was reconstructed with a latissimus dorsi myocutaneous flap. We also review the literature for previously reported cases of breast cancer involving Mohs paste.
Pitfalls in the Management of Atlanto-Occipital Dislocation

Atlanto-occipital dislocation (AOD) is rarely seen in clinic because it is characteristically immediately fatal. With recent progress in the pre-hospital care, an increasing number of AOD survivors have been reported. However, because the pathophysiology of AOD is not clearly understood yet, the appropriate strategy for the initial management remains still unclear. We report a case of successful AOD treatment and describe important points in the management of this condition. It is important to note that abducens nerve palsy is a warning sign of AOD and that AOD can result in a life-threatening distortion of the arteries and the brain stem. We recommend the application of a halo vest to protect the patient’s neural and vascular competence as the immediate initial step in the treatment of AOD. Horn’s grading system is useful in assessing indications for surgery. Finally, when performing posterior fixation, C2 should be included because of the anatomy of the ligamentous architecture.
Drop Metastasis of Adrenocorticotropic Hormone-Producing Pituitary Carcinoma to the Cauda Equina

The diagnosis of pituitary carcinoma cannot be made easily histologically, and most cases of pituitary carcinoma are diagnosed only after the clinical detection of metastasis. Distant metastasis of pituitary tumor occurs in 0.1% to 0.2% of cases and has been reported in the liver, bone and central nervous system, with only one case of metastasis to the cauda equine reported. This study describes a rare case of the drop metastasis of adrenocorticotropic hormone-producing pituitary adenocarcinoma to the cauda equina, causing cauda equina syndrome.
Effect of Pressure on the Field-induced Ordered Phase in the Heavy-fermion Compound YbCo2Zn20

The effect of pressure on the field-induced ordered phase for H k h111i in the heavy-fermioncompound YbCo2Zn20, which is presumably a field-induced antiferro-quadrupolar (FI-AFQ) phase,was investigated in the pressure range up to 4.5 GPa and under magnetic fields up to 80 kOe. When pressure is applied, the metamagnetic-like transition at Hm =6 kOe shifts to lower fieldsand disappears around the quantum critical pressure Pc 1.8 GPa. A pressure-induced antiferromagnetic(PI-AFM) phase appears at pressures above about 2 GPa, and the critical field Hc ofthe PI-AFM phase increases with increasing pressure. On the other hand, the transition field HQof the FI-AFQ phase decreases gradually without showing any anomalous behavior around Pc andbecomes obscure around 4 GPa, where Hc of the PI-AFM phase and HQ of the FI-AFQ phasebecome comparable. The magnetic field versus pressure, H-P, phase diagram for H k h111i at0.1 K was constructed in the pressure range up to 4.5 GPa.
Tetraspanin CD9 modulates ADAM17-mediated shedding of LR11 in leukocytes

LR11, also known as SorLA or SORL1, is a type-I membrane protein from which a large extracellular part, soluble LR11 (sLR11), is released by proteolytic shedding on cleavage with a disintegrin and metalloproteinase 17 (ADAM17). A shedding mechanism is presumed to have a key role in the functions of LR11, but the evidence for this has not yet been demonstrated. Tetraspanin CD9 has been recently shown to regulate the ADAM17-mediated shedding of tumor necrosis factor-a and intercellular adhesion molecule-1 on the cell surface. Here, we investigated the role of CD9 on the shedding of LR11 in leukocytes. LR11 was not expressed in THP-1 monocytes, but it was expressed and released in phorbol 12-myristate13-acetate (PMA)-induced THP-1 macrophages (PMA/THP-1). Confocal microscopy showed colocalization of LR11 and CD9 proteins on the cell surface of PMA/THP-1. Ectopic neo-expression of CD9 in CCRF-SB cells, which are LR11-positive and CD9-negative, reduced the amount of sLR11 released from the cells. In contrast, incubation of LR11-transfected THP-1 cells with neutralizing anti-CD9 monoclonal antibodies increased the amount of sLR11 released from the cells. Likewise, the PMA-stimulated release of sLR11 increased in THP-1 cells transfected with CD9-targeted shRNAs, which was negated by treatment with the metalloproteinase inhibitor GM6001. These results suggest that the tetraspanin CD9 modulates the ADAM17-mediated shedding of LR11 in various leukemia cell lines and that the association between LR11 and CD9 on the cell surface has an important role in the ADAM17-mediated shedding mechanism.
Real-Time, in Situ Monitoring of the Oxidation of Graphite: Lessons Learned

<P>Graphite oxide (GO) and its constituent layers (i.e., graphene oxide) display a broad range of functional groups and, as such, have attracted significant attention for use in numerous applications. GO is commonly prepared using the 'Hummers method' or a variant thereof in which graphite is treated with KMnO4 and various additives in H2SO4. Despite its omnipresence, the underlying chemistry of such oxidation reactions is not well understood and typically affords results that are irreproducible and, in some cases, unsafe. To overcome these limitations, the oxidation of graphite under Hummers-type conditions was monitored over time using in situ X-ray diffraction and in situ X-ray absorption near edge structure analyses with synchrotron radiation. In conjunction with other atomic absorption spectroscopy, UV vis spectroscopy and elemental analysis measurements, the underlying mechanism of the oxidation reaction was elucidated, and the reaction conditions were optimized. Ultimately, the methodology for reproducibly preparing GO on large scales using only graphite, H2SO4 and KMnO4 was developed and successfully adapted for use in continuous flow systems.</P>

Objective: Rikkunshito, an herbal medicine, is widely prescribed in Japan for the treatmentof anorexia and functional dyspepsia, and has been reported to recover reductions in foodintake caused by cisplatin. We investigated whether rikkunshito could improve chemotherapyinducednausea and vomiting (CINV) and anorexia in patients treated with cisplatin. Methods: Patients with uterine cervical or corpus cancer who were to receive cisplatin (50 mg/m2day 1) and paclitaxel (135 mg/m2day 0) as first-line chemotherapy were randomly assignedto the rikkunshito group receiving oral administration on days 0–13 with standard antiemetics,or the control group receiving antiemetics only. The primary endpoint was the rate of completecontrol (CC: no emesis, no rescue medication, and no significant nausea) in the overall phase(0–120 hours). Two-tailed p<0.20 was considered significant in the planned analysis. Results: The CC rate in the overall phase was significantly higher in the rikkunshito groupthan in the control group (57.9% vs. 35.3%, p=0.175), as were the secondary endpoints:the CC rate in the delayed phase (24–120 hours), and the complete response (CR) rates(no emesis and no rescue medication) in the overall and delayed phases (63.2% vs. 35.3%, p=0.095; 84.2% vs. 52.9%, p=0.042; 84.2% vs. 52.9%, p=0.042, respectively), and time totreatment failure (p=0.059). Appetite assessed by visual analogue scale (VAS) appeared to besuperior in the rikkunshito group from day 2 through day 6. Conclusion: Rikkunshito provided additive effect for the prevention of CINV and anorexia
