      • 제2형 당뇨병 환자의 치료에서 복약순응도가 혈당 조절에 미치는 영향

        Abstract: As known in clinical trial like DCCT, UKPDS, tight glycemic control is essential to prevent complications from diabetes mellitus(DM). In type 2 diabetes taking multiple medication, nonadherence to medications is common problem. The aim of our study is to investigate adher-ence rate of patients with type 2 DM and to evaluate association between many factors including adherence rate and HbA_(1C). 441 patients with type 2 DM who visited Busan National University Hospital endocrine internal department from Aug 13 to Sep 2, 2008(3weeks) were enrolled. We conducted a man-to-man questionnaire survey about general knowledge about diabetes mellitus, medicine and adherence, and measured HbA_(1C). Among them, 305 patients taking oral hypo-glycemic agent(OHA) were analyzed to investigate correlation between adherence rate and glycemic control. We studied 305 patients(male 126, female 179, average age 61.2±9.8 years, DM duration 8.6±6.4 years, average adherence 88.8±16.2%, average HbA_(1C) 7.03± 1.18%). HbA_(1C) showed significant relation with DM duration(r=0.137, p=0.017), number of aHA in regimen (r=0.135, p=0.018), dosage frequency(r=0.132, p=0.026), Medication Refill Adherence(MRA)(r=-0.124, p=0.030) and adherence(r=-0.168, p=0.003). Adherence had significant relation with MRA(r=0.148, p=0.010) but didn't have statical corelationship with DM duration, number of aHA and dosage frequency. Patients that their adherence rate exceed 90% took fewer OHA(p=0.011) and went on a better dietary treatment(p=0.007). In patients controlling diabetes mellitus with aHA, when adherence rate and MRA was higher, HbA_(1C) was lower. As a result, to improve adher-ence rate, it is necessary to enhance patient education, do careful consideration about prescrip-tion medicine number and dosage frequency

        Distribution and Potential Toxicological Effects of 2,2',4,4'-tetrabromodiphenyl Ether (BDE-47) as a Endocrine Disrupting Chemical in Human and Animals

        Polybrominated diphenyl ethers (PBDEs) are a class of "brominated" (bromine containing) man-made chemicals used as flame retardant additives in plastics, foams, and textiles. PBDEs are found in various environmental contaminants in air, soil, sediment, and water, and 209 individual forms (congeners) of PBDE exist. Among these, 2,2',4,4'-tetrabromodiphenyl ether (BDE-47) is the dominant congener found in the environment. Exposure to BDE-47 is now worldwide, and levels of BDE-47 have been detected in the blood of animals, including humans. BDE-47 can adversely affect the developmental system in both humans and animals. BDEs have structural similarities to polychlorinated biphenyls and thyroid hormones. However, recent studies have shown that BDEs may act as hormonal disrupting chemicals with detrimental effects. Therefore, a reliable assessment of BDE-47 toxicological action is required to understand the detrimental impacts of BDE-47 on human health. In this review, we overview recent studies on the distribution and potential toxicological effects of BDE-47 in humans and animals.

      • 면역학적 특성에 따른 yeast cytochrome c의 구조에 관한 연구

        The antiserum against yeast cytochrome c and horse heart cytochrome c was reacted immunologically with those from various species. The results were as followings; 1. The antiserum against horse heart cytochrome c was reacted immunologically with heart cytochrome c of horse, cow, rabbit, mouse, chicken, pigeon, frog, tuna, and guanaco, and were not with yeast and neurospora cytochrome c. 2. However, antiserum against yeast cytochrome c was only reacted immunologically with yeast cytochrome c, and was not mammals and vertebrates horse, cow, rabbit, mouse, chicken, pigeon, frog, tuna, guanaco, even neurospora 3. CNBr-digested peptides of horse heart cytochrome c were obtained 5 peaks, Ⅰ(1-80), Ⅱ(1-65), Ⅲ(66-104), Ⅳ(81-104), Ⅴ(66-80), and other peptide (1-50). Among them peptides Ⅰ and Ⅱ were weakly reacted immunologically with antiserum against horse heart cytochrome c. CNBr-digested peptides of horse heart cytochrome c were not reacted with antiserum against yeast cytochrome c. 4. From the above, the immunologically positive cross reaction was thought to be related with Ω loop D.

        약물사용 고위험군 선별척도의 표준화 연구(Ⅰ)

        One of the screening tests used for identifying the high risk group of adolescent drug users is High Risk Group of Adolescent Drug Users Screening Test (HIRIGADUST) developed by Kyung-Bin Kim (1977) that consist of six subscales and 70 items. The Purpose of this study was to examine the reliability and construct validity of HIRIGADUST. HIRIGADUST was administered to 370 third grade middle school students. In the reliability test, the Cronbach’s α coefficients were 0.70-0.89 in five subscales, but the Cronbach’s α coefficient was 0.53 in ‘Personal experience and general environment’ subscale. The resulting of testing the construct validity of five subscales of HIRIGADUST by factor analysis provided evidence for the validity of the each subscales. We used T score 65 as a cut-off point. The cutting scores were 12 for ‘Peer relationships and school performance’ subscale, 14 for ‘Personality and value system’ subscale, 14 for ‘Psychological problems and health concerns’ subscale, 8 for ‘Family history and characteristics of parents’ subscale, and 14 for ‘Conserns about parents’ drinking subscale.

      • 아메바성 대장염과 연관된 Henoch-Scho¨nlein Purpura 1예

        Henoch-SchonIein purpura is a common form of vasculitis primarily involving the skin, the gastrointestinal tract, the joints, the kidneys, and the nervous system. The etiological investigations are essential, because triggering factors are found in approximately half the patients such as viral, bacterial and parasitic infections, drugs, toxins, systemic diseases and cancer. But it's the correlation between Henoch-SchonIein purpura and Entamoeba histolytica has been rarely reported. We report a 29-years-old woman with Henoch-SchoIein purpura complicated by the lower gastrointestinal bleeding caused by the amoebic colitis. She complained of cramping abdominal pain, lower extremity purpura and hematochezia. Cysts and trophozoites of Entamoeba histolytica were detected in her stool. Skin biopsy demonstrated the leukocytoclastic vasculitis. She was administered metronidazole 1,500㎎ a day for the treatment of amebic colitis. Her hematochezia was improved, but the abdominal pain and purpura were not. The subsequent corticosteroid therapy improved her symptoms.

      • 스트레스관리 프로그램이 만성정신분열병환자의 증상과 삶의 질에 미치는 효과

        This study examined the effects of stress-management programs on the symptoms and quality of life in the chronic schizophrenics. The subjects were the chronic schizophrenics who were registered and managed by one of the public health care centers in Seoul. The stress-management programs were provided 10 times. Positive and negative syndrome scale (PANSS) and quality of life were surveyed before and after the stress-management programs. The results were as follows. 1. The differences of positive symptoms, negative symptoms and general psychopathological symptoms before and after stress-management program were significantly different between the group which were provided with stress-management programs and the group which were not provided. 2. The differences of the quality of life before and after stress-management program were significantly different between the group which were provided with stress-management programs and the group which were not provided. These results indicate that stress-management programs have effects on relieving the symptoms and improving the quality of life of schizophrenics. Ultimately these results also suggest that stress-management programs be used as the supplementary means to treat schizophrenics.

      • 인삼액이 알코올 투여 흰쥐의 효소활성에 미치는 영향

        The S.D. rats were classified with four groups: ethanol-treated group, ginseng-treated group which was pretreated with ethanol, ginseng-treated group and ethanol-treated group which was pretreated with ginseng. Then enzyme activities such as γ-GTP, Alk phosphatase and amylase had been measured in serum, hepatic and renal tissue. The descriptions were based on the analysis of samples collected by 5 days interval for25 days. The following results were obtained. 1. The changes of serum γ-GTP, Alk phosphatase and amylase were significantly increased in ethanol-treated group, decreased in ginseng-treated group which was pre treated with ethanol except amylase, amylase activity was especially decreased in ginseng-treated group. 2. The changes of hepatic γ-GTP, Alk phosphatase and amylase were significantly increased in ethanol-treated group, decreased in ginseng-treated group which was pretreated with ethanol except amylase, amylase activity was especially decreased in ginseng-treated group. 3. The changes of renal γ-GTP, Alk phosphatase and amylase were significantly increased in ethanol-treated group, decreased in ginseng-treated group which was pretreated with ethanol except amylase, amylase activity was especially decreased in ethanol-treated group which was pretreated with ginseng. Based on these results, it suggested that ethanol increased the concentrations of γ-GTP, Alk phosphatase and amylase but ginseng suppressed the hepatotoxic effect by ethanol.

        심정지환자에서 epinephrine투여시 혈중 catecholamine의 변화

        Background : Cardiac arrest is a potent stimulus for the release of endogenous catecholamine and high plasma catecholamine concentration has been reported during cardiopulmonary resuscitation(CPR). Few data were present about the clinical effect of high plasma catecholamine and endogenous catecholamine response to exogenous epinephrine administration during CPR. Purpose : This study was designed to evaluate endogenous catecholamine response to cardiac arrest and exogenously administered epinephrine during CPR. Method : In 21 cardiac arrest victims, blood sample for epinephrine and norepinephrine was drawn before and 1 minute after each epinephrine administration during CPR. 1mg of epinephrine was administered immediately after intravenous access and 3 minute interval during CPR. Plasma catecholamines were quantitated by high-performance liquid chromatography. Result : Baseline plasma epinephrine concentraqtion was higher in patients with cardiac arrest than normal controls (985±1627 vs 194±173 pg/ml, p<0.05). Baseline plasma norepinephrine concentration was also elevated in patients with cardiac arrest than normal controls, although statistically insignificnat(1965±4915 vs 360±250 pg/ml). Plasma epinephrine concentration in cardiac arrest patients was significantly elevated after exogenous administration of epinephrine, but its response was blunted after the third does of epinephrine. Plasma norepinephrine concentration in cardiac arrest patients was also elevated, but its response was observed only after the first epinephrine administration. Plasma epinephrine concentration was persistently higher in patients with return of spontaneous circulation (ROSC) than patients without ROSC. there was no significant difference of plasma norepinephrine concentration whether spontaneous circulation was restored or not. Endogenous epinephrine response to exogenous epinephrine was not observed in patients having prolonged arrest time over 20 minutes. Conclusion : Results of this study suggests that significant amount of catecholamine is released by exogenous epinephrine administration in spite of high plasma catecholamine concentration, and degree of endogenous catecholamine response influences resuscitation outcome.

      • 알코올 및 인삼액이 효소활성도에 미치는 영향에 관한 연구

        This study was investigated for the change of r-GTP, CPK activity and CPK-isoenzyme by alcohol and Ginseng administration, The normal white S.D rats were classified with four groups; ethanol treated was group A, ethanol pretreated group combined pretreated with Ginseng extract was group B, Ginseng extract treated was group C, and Ginseng extract treated group combined pretreated with ethanol was group D. The white rats of four groups were treated with 20%, 30%, 40% ethanol and/or 1% Ginseng extract. Then enzyme activities such as r-GTP, CPK and CPK-isoenzyme had been measared in serum, hepatic, and renal tissues. The sample was collected by 5 day intervalus for 25 days. The following results were obtained. 1. The changes of serum r-GTP activities were significantly increased in ethanol-treated group, decreased in Ginseng-treated group. 2. The changes of hepatic r-GTP activites were significantly increased in ethanol-treated group, decreased in Ginseng-treated group. 3. The changes of renal r-GTP activites were significantly increased in ethanol-treated group, decreased in ginseng-treated group. 4. The changes of serum CPK activites were significantly increased in ethanol-treated group, decreased in ginseng-treated group. 5. The changes of hepatic and renal tissue CPK activites were not significant. 6. The changes of serum and hepatic tissue CPK-isoengyme were significantly increased in ethanol-treated group, decreased in ginseng-treated group combined pretreated with ethanol. 7. The changes of CPK-isoengyme, CPK 1 and CPK 2 were not significant.
