A successful treatment of infants and children with congenital heart defects depends upon several factors: diagnosis must be accurate, the operative technique, including techniques of perfusion and myocardial preservation, must be suited to the needs of patients and intensive cares, including accurate anesthesia, must be applied through all stages of investigation and treatment, i.e. before, during and after the operation. Recently we performed a open heart surgery successfully for a six-years old girl who had suffered from frequent upper respiratory tract infection and mild to moderate exertional dyspnea with congenital heart defects (ventricular septal defect, Kirklin type II, with patent foramen ovale). The operation and postoperative course were very smooth. And now we report this with review of literatures.

Urine certified reference material(CRM) has been developed to help clinical labolatories control analytical accuracy. Two levels of freeze-dried urine were prepared. The low level CRM was made from normal urine and the abnormal level CRM was prepared by spiking th normal urine with As, Cd, Cr, Cu, Hg, Ni and Pb. Urine reference materials were analyzed by graphite furnace atomic absorption spectrometry(GFAAS) and inductively coupled plasma mass spectrometry(ICP-MS). Analyte elements were separated from matrix elements by using ion exchange resin and hydride generation. Isotope dilution method was employed to enhance analytical accuracy. Round robin test results are also presented which were carried out with 5 clinical laboratories.

purpose: This study was conducted to describe qualitatively the entities of nurse's experiences in general hospitals and to suggest basic data guiding research on developing Standards of clinical nursing practice in Korea. Method: Fourteen nurses working at general hospitals with over 300 beds in Seoul were interviewed in-depth until saturation using tape-recorders and transcription. Result: The central theme of clinical nursing practice experienced by subjects was "being with clients" that means accepting client's personal character, solving client's needs and providing client-centered nursing. A also "being with clients" was felt to be the responsibility of nurses which was learned from their nursing schools. The nursing strategies performed in order to be with patients were proving skillful nursing techniques, accepting, educating, emotional support, advocating, and self-reflecting, the subjects experienced somewhat problematic affects such as difficulties in interpersonal relationship, work overload, negative image of nursing, deficit of self-confidence for nursing actions, poor working conditions, and unfair treatment. Nurses at the hospital practiced with pride when they felt that they were accepted by clients. Conclusion: Further research is needed to analysis problems in clinical practice and the comparison of nurses' experiences of clinical practice, with nurses' experiences in various settings.
라미실 정(테르비나핀 125mg)에 대한 미코실 정의 생물학적 동등성

Terbinafine has a primary fungicidal action mediated by squaleue epoxidase inhibition. Treated fungi accumulate squaleue while becoming deficient in ergosterol, an essential component of fungal cell membranes. Bioequivalence of two terbinafine tablets, Lamisil^(TM) (Novartis Korea Ltd., Seoul, Korea) and Mycosil^(TM) (Daewon Pharmaceutical Co., Ltd., Soul, Korea), was evaluated according to the guidelines of Korea Food and Drug Administration (KFDA). Sixteen normal male volunteers (20∼29 years old) were randomly divided into two groups and a randomized 2×2 cross-over study was employed. After oral administration of Mycosil^(TM) or Lamisil^(TM) (125 ㎎ terbinafine), blood samples were taken at predetermined time intervals and the serum terbinafine concentrations were determined using an HPLC method with UV/VIS detector. The pharmacokinetic parameters (AUC_t, C_(max) and T_(max)) were calculated and ANOVA was utilized for the statistical analysis. The results showed that the differences in AUC_t, C_(max) and T_(max) between two tablets based on the Lamisil^(TM) tablet were -2.24%, -7.68% and 2.92%, respectively. The powers (1-β) for AUC_t, C_(max) and T_(max) were 87.11%, 95.36% and 99.99%, respectively. Minimum detectable differences (Δ) and 90% confidence intervals were all less than ± 20%. All these parameters met the criteria of KFDA for bioequivalence, indicating that Mycosil^(TM) tablet is bioequivalent to Lamisil^(TM) tablet.
