        Malignant peritoneal mesothelioma is a rare neoplasm that arises from the mesothelium of a serosal cavity and is a rapidly fatal disease with a median survival of 4 to 12 months for untreated cases. Recently, we experienced a case with malignant peritoneal mesothelioma who was suspected hepatocelluar carcioma by abdominal CT scan and was confirmed by biopsy including immunohistochemical stain(calretinin) after surgery. We performed tumor excisions and wedge resection of the liver(segment Ⅷ)and inserted Tencoff catheter in abdominal cavity at 25th day of post-operation. We treated with intraperitoneal paclitaxel(25mg/m^(2)/day for 5 days) six courses monthly. She was well tolerable and is still living without any evidence of recurrence for 14th month of post-operation.

        비만 개선 효과를 지닌 탄수화물 및 지방 흡수 억제 기능성 식이조성물 개발

        In a previous study, we developed a new food additive as an egg yolk antibody (IgY) against carbohydrate digestion enzymes for the regulation of blood glucose level and weight control. The IgY delayed and decreased the increment of blood glucose level after administration of sucrose in human being by 30% in 20∼30 min. We also developed a lipase inhibitor as a water extract of two kinds of herb, Platycodon grandiflorum and Solanum melongena. Twenty three volunteers were subjected to the intake of the egg yolk IgY plus the herbal extracts for 50 days. In average, the treated subjects appeared to lose 1.96 ㎏ of body weight and 3.4 ㎏ of body fat mass during the treated period. Furthermore, Panniculus adiposus and breech size were significantly decreased during the experimental period. Above results suggested that the administration of the dietary additives composed of egg yolk IgY and natural herbal extract improve the obesity by the decrement of body weight and body fat mass.

        This paper presents a two dimensional analytical method of a permanent Magnet Linear Synchronous Motor(PMLSM) based on the concept of space harmonic The air gap flux densities due to permanent magnets and primary current are calculated separately, and the resultant air gap flux density is obtained by superposition of those, Maxwell's stress tensor is used to calculate thrust and normal force. The static thrutst and normal force according to exciting current and power angle are calculated, and the harmonics of the thrust are analyzed using discrete Fourier transform In order to verify the validity of the proposed analytical method, we make a test facility and compare the analysis results with the experimental ones.

        응급의료센터로 내원한 급성심근경색증 환자

        Early reperfusion can prevent myocardial necrosis, and clinical trials with reperfusion therapy demonstrating a significant reduction in acute myocardial infarction(AMT) mortality have dramatically changed the treatment paradigm for AMI. All patients with symptoms and ECG findings suggestive of AMI should be considered for treatment with thrombolytic agents. However, only a minority of AMI patients actually receive a thrombolytic therapy. Many patients are often excluded from treatment because they do not meet the criteria for age, duration of the chest pain, and a qualifying ECG. And late arrival at the hospital is a frequently cited reason for not giving a thrombolytic agent. To confirm the reason for not receiving an early reperfusion therapy, we obtained the data for the clinical characteristics, the time intervals between the symptom onset and the start of a thrombolytic agent infusion, the method of reperfusion therapy, the reason for not giving a thrombolytic agent and overall outcomes by retrograde chart review. During the 12-month study period between July 1994 and June 1995, 113 patients were finally diagnosed to AMI, who presented to Emergency Center of Ajou University Hospital in total 30,819 patients. The results were followings: 1. The average age was 59±12 years old, the ratio of male to female was 3.2:1. The direct visited patients to our hospital were 31 and the transfered were 82. 2. The chief complaints were chest pain(86.7%), dyspnea, dyspnea, and mental change. The common preceding diseases were angina pectoris(10 cases), old myocardial infarction(9 cases), congestive heart failure(2 cases) and typical chest pain but not diagnosed(23 cases). The risk factors were smoking(81 cases), hypertension (46 cases) and DM (22 cases). 3. 75 patients had arrived within 12 hours from symptom onset and 38 patients after 12 hours. 45cases (54.7%) were performed the reperfusion therapy ; 31 patients were taken the thrombolytic therapy by tissue-type Plasminogen Activator. 4. The most common reason for not receiving a reperfusion therapy was the time delay and the main was the patient/bystander factor. 5. 75.2%(85 cases) of the patients discharged with or without complications, 7.1% (8 cases) died, 10.6% (12 cases) discharged moribundly, and 7.1% (8 cases) discharged against advise. In conclusion, the time delay was the first reason for not receiving a reperfusion therapy in AMI patients. And the education for the AMI symptom and BLS (Basic Life Support) to the people, good EMS(Emergency Medical Service) system, early definite diagnosis and aggresive therapy may decline the mortality rate.

        주성분 회귀분석 및 인공신경망을 이용한 AE 변수와 응력확대계수와의 상관관계 해석

        AE 신호와 재료의 기계적 물성과의 관계를 정량적으로 제시할 수 있는 방법을 개발하였다. 재료의 여러 가지 기계적 성질들 중 피로균열 거동에 관련된 응력확대계수를 중심으로 AE 신호와 같은 다변량 데이터의 처리에 많이 사용되고 있는 주성분 회귀분석과 비선형적 문제 해결에 적합한 신경회로망 기법을 이용하였다. 이를 위하여 강교량 부재인 SWS490B 강에 대한 피로균열전파 실험을 수행하였으며 표준 CT 시편에 대한 피로균열진전 시 발생하는 AE 신호의 각 변수와 응력확대계수와의 관계를 고찰하였다. 통계분석 방법인 변수선택법을 적용한 결과 AE 카운트(RC), 에너지(EN), 신호지속시간(ED)의 각각에 대한 유의성이 높은 것으로 나타났으나 전반적으로 전체 AE 변수를 모두 이용할 경우 통계적 유의성이 높은 것으로 나타났다. 부재의 반복하중 시 발생하는 피로균열진전을 정량적으로 도출할 수 있는 응력확대계수 추정모델을 개발하고 평가하였다. 미지 시료에 대하여 개발된 모델의 응력확대계수 예측 성능을 분석한 결과 주성분 회귀모델과 인공신경망 모델 모두 우수한 예측성능을 나타내었으나 전반적으로 인공신경망 모델이 주성분 회귀모델보다 다소 양호한 것으로 분석되었 다. The aim of this study is to develop the methodology which enables to identify the mechanical properties of element such as stress intensity factor by using the AE parameters. Considering the multivariate and nonlinear properties of AE parameters such as ringdown count, rise time, energy, event duration and peak amplitude from fatigue cracks of machine element, the principal component regression(PCR) and artificial neural network(ANN) models for the estimation of stress intensity factor were developed and validated. The AE parameters were found to be very significant to estimate the stress intensity factor. Since the statistical values including correlation coefficients, standard error of calibration, standard error of prediction and bias were stable, the PCR and ANN models for stress intensity factor were very robust. The performance of ANN model for unknown data of stress intensity factor was getter than that of PCR model.

      • 소아에서 전신마취후 기관내 튜브의 발관방법에 따른 말초동맥혈 산소포화도의 변화

        Changes of peripheral arterial oxygen saturation(SaO_2) were compared according to the methods of extubation after general anesthesia in 44 pediatric patients in TaeJon ST.Mary's Hospital between March and May in 1992. Patients were divided in two groups. Group Ⅰ (n=22); endotracheal extubation was performed with suctioning by catheter after one or two endotracheal suctions. Group Ⅱ (n=22); endotracheal extubation was performed wit positive pressure administration of 100% oxygen by reservoir bag simultaneously after one or two endotracheal suctions. All patients were administered 100% oxygen for one minute after extubation and transferred to recovery room and administered oxygen 3ℓ/min by face mask for three minutes. The results are as follow: 1) SaO_2 was decreased significantly (p<0.05) after extubation in group Ⅰ compared with control and group Ⅱ. 2) The number of patients whose SaO²bellow 95% are increased in group Ⅰ(77%) compared group Ⅱ(5%) after endotracheal extubation. 3) SaO_2 of all patients were recovered to over 99% after administration of oxygen for three minutes in recovery room.

      • Carbofuran이 흰쥐의 간조직과 Cytochrome P-450효소계에 미치는 영향과 Phenobarbital-Sodium의 보상효과에 관한 연구

        In order to investigate the toxicity of carbofuran and compensatory effects of phenobarbital sodium(PB) against carbofuran toxicity on hepatic tissues and cytochrome P-450 enzyme system of the rat. Sprague-Dawley male rats were administered carbofuran only, PB only or combination of carbofuran and PB. Carbofuran was severely damaged in hepatic tissues by dose-or time-dependent manner and increased the level of cytochrome P-450 enzyme system at 6hrs or 12hrs after administration. PB were not damaged in hepatic tissues, but the level of cytochrome P-450 enzyme system was increased the same as that of carbofuran. Histopathlogical changes were observed on hepatic tissues at 12hrs after carbofuran and PB combination f treatment, but were already recovered at 48hrs. PAS-positive cells were increased at 12hrs after carbofuran only administration and most hepatic cells of carbofuran and PB combination treated group showed PAS-positive reaction, but hepatic cells were normally recoverd. Also, glycogen granule were increased at 12hrs after carbofuran only administration, but glycogen granules were decreased at 12hrs. After carbofuran and PB combination treatment in best camine stain. The level of cytochrome P-450 enzyme system after carbofuran and PB combination treatment were more increased than that of carbofuran only or PB only treatment. The results indicate that PB decreased carbofuran toxicity by the elevation in the level of cytochrome P-450 enzyme system.

        응력확대계수와 음향방출 변수를 이용한 피로균열 거동 연구

        응력확대계수와 음향방출 발생 관계를 이용해 균열 길이 및 균열 검출 능력을 평가할 수 있는 새로운 접근 방법을 파괴역학적인 관점에서 제시하였다. 이를 위해 강 교량 부재인 SWS 490B 강의 피로 균열 활동도를 음향방출시험을 통해 관찰하였다. 표준 CT 시편에 대한 피로균열 진전 시 AE 특성을 관찰하는 실험을 진행하였으며, 기존의 AE 파라미터 분석은 물론 응력확대계수와 AE 발생거동 사이의 관계를 논의하였다. CT 시편 실험 결과에서 균열 성장 길이, AE 최대진폭, AE 에너지, AE hit 발생율 등과 같은 특정 변수들은 응력확대계수 값이 증가함에 따라 이들값도 증가하는 경향을 나타내었다. 또한 피로사이클 수에 따른 AE 에너지의 변화는 균열진전에 대한 활동성을 평가하는데 사용되는 효과적인 변수중의 하나임을 보여 주었으며, AE 에너지 분석은 균열 활동도와 AE 변수 사이의 관계를 평가하는데 사용될 수 있을 것으로 판단된다. By using the relation between stress intensity factor and AE parameter, new approach method for assessing the crack length and detectability of crack was proposed. Laboratory experiment was carried out to identify AE characteristics of fatigue cracks for compact tension specimen. The relationship between a stress intensity factor and AE signals activity as well as conventional AE parameter analysis was discussed. As a result, the features of specific parameters such as the length of crack growth, the AE energy, the AE peak amplitude, and the cumulative AE hits, showed the almost same trend in their increase as the number of fatigue cycle increased. From the comparisons of peak amplitude and AE energy with stress intensity factor, it was verified that the higher stress intensity factors generated AE signals with higher peak amplitude and a larger number of AE counts. If we can get more reliable database for the relation between AE parameters and stress intensity factor, this approach will provide a good information for evaluating both the existence of crack and the minimum detectable size of crack.
