저자들은 태생학적 기원, 면역조직화학적, 분자생물학적으로 완전히 별개의 암종인 갑상선 수질암종과 유두암종이 동시에 동일한 갑상선에 발생하는 매우 드문 사례를 경험하였고 이러한 현상이 우연한 동시 발생인지, 공통의 줄기세포 기원인지 혹은 공통의 암 유발인자에 의한 것인지는 여전히 논쟁의 여지를 남겨두고 있다. It is generally believed that the origins of medullary and papillary carcinoma of thyroid are embryologically different. So mixed medullary-papillary thyroid carcinomas are rare. The two separate tumors of the present case may be coincidental double primaries. It can be assumed that at least some of the mixed papillary and parafollicular cell carcinomas are results of collision of carcinomas derived from two embryogenetically different cell types. Recently, we experienced a case of mixed medullary-papillary thyroid carcinomas. A 50-year-old woman was found to have mixed medullary thyroid carcinoma and papillary thyroid carcinoma on the same lobe with lymph node metastases. We report a this case with literature review
조선업 도장작업자의 유사노출군과 노출변이; 크실렌 노출을 중심으로

목적 : 조선업 도장 작업자의 크실렌 노출을 평가하기 위하여, 수행직무 및 밀폐특성에 따라 유사노출군으로 구분하여 노출 변이 및 동질성을 파악하고자 하였다. 방법 : 3회 이상 반복측정이 이루어진 근로자 중동일공정에서 직무상 변동이 없는 근로자 총 28명을 대상으로 하였다. 노출농도 값을 대수전환(lOgtransformation)하여 근로자간, 근로자내 노출 분산요소을 추정하기 위하여, nested randomeffects ANOVA model을 활용하였다. 선정된 유사노출군 내 구성원의 측정 평균이 동일하다고 할수 있는지를 검정하기 위하여 분산요인(9f)을 산출하였고, 쿨즈0.031일 경우 동일하다고 생각하였다.또한 노출 값을 대수전환하여 95 %의 근로자(b-R9s)가 2배수 범위 내에 측정값을 가질 경우 동일하다고 생각하였다. bwRSS의 값을 유사노출군내의 근로자들간(between worker)의 기하표준편차(bwsy)를 구한 후 exp(3.92 bwsy)로 계산하여 산출하였고, 그 값이 2이하이면 동일하다고 생각하였다. 결과 : 일반적으로 유사노출군은 노출농도가 비슷한 것이라는 생각과는 달리, 직무에 근거한 유사노출군은 동질성 검정상 이질적이었다. 분무도장과 붓도장 모두 bwRss은 2를 초과하였고, 룰 또한 0.031를 초과하여 변이 폭이 컸다. 그러나 밀폐특성에 따른 노출 분포의 변이성은 직무만으로 구분했을 때의 변이지표 보다 그 수치가 낮아 밀폐특성으로 직무의 변이성을 설명 할 수 있었다. 결론 : 직무에 근거한 유사노출군이 동질적이지알다는 결과는 유사노출군으로 구분된 근로자의 업무특성이 하나의 유사노출군으로 설정할 수 없다는 점을 시사한다. 따라서 향후 작업환경 측정 시 직무뿐만 아니라 특정 업무(specific task) 또는 세부작업현황까지 파악할 필요성을 제시하고 있다. Objectives : This study was conducted to assess the concentration of xylene exposure and exposure variability of a similar exposure group (SEG) in accordance with job title and confined space. Method : The study subjects included 28 workers working in the painting process in the ship-building industry. Measurement of subjects were repeated three times. Within and between-worker exposure valiance components were estimated from log-transformed exposure concentrations employing a one-way nested random effects analysis of variance model. A uniformly exposed group was defined as one where the between-worker variance(σB2) was less than or equal to 0.031 or bwR95 ≤2, where bwR95 =exp(3.92 bwSy) , representing the radio of the mean exposures of the 97.5th and 2.5th percentile groups of the workers sampled. Result : The results indicated that, contrary to popular belief, similar exposed groups based on job title were heterogeneous. The between-worker variance(σB2) is large(≥0.031) and the ratios of the 97.5th and 2.5th percentile workers of the log normally distributed exposures of each group of worriers was more than 2. However dividing job title into confined space (in-block, out-block, in/out-block) decreased the between-worker exposure variability. Conclusion : This study focused on the quantitative aspects of exposure in painters in the ship-building industry. We cannot accept the assumption that SEG are uniformly exposed, as a major component of variation in exposure relates to differences in tacks or other factors. Therefore, the observational schemes can be improved by investigating Job-specific task and practice.
Changes in the Swine Gut Microbiota in Response to Porcine Epidemic Diarrhea Infection

<P>The gastrointestinal tract of mammals is a complex ecosystem with distinct environments and comprises hundreds of different types of bacterial cells. The gut microbiota may play a critical role in the gut health of the host. We herein attempted to identify a microbiota shift that may be affected by porcine epidemic diarrhea (PED). We observed significant differences in microbiota between the control and PED virus (PEDV)-infected groups at both the phylum and genus level. Most commensal bacteria (<I>i.e. Psychrobacter</I>, <I>Prevotella</I>, and <I>Faecalibacterium</I>) in the healthy gastrointestinal tract were decreased due to dysbiosis induced by PEDV infection.</P>

A Gram-stain-positive, oxidase- and catalase-positive, motile, aerobic, and rod-shaped bacterial strain, designated as DCT-5T, was isolated from a native plant belonging to the genus Campanula at Dokdo island, Republic of Korea. Growth of the strain DCT-5T was observed at 15–37°C (optimum 30°C) on R2A broth, pH 6.0–8.0 (optimum 7.0), and 0–5% (w/v) NaCl concentration (optimum 0%). The 16S rRNA gene sequence analysis revealed that strain DCT-5T was most closely related to Arthrobacter silviterrae KIS14-16T, Arthrobacter livingstonensis LI2T, Arthrobacter stackebrandtii CCM 2783T, Arthrobacter cryoconiti Cr6-08T, Arthrobacter ramosus CCM 1646T, and Arthrobacter psychrochitiniphilus GP3T with pairwise sequence similarities of 98.76%, 97.47%, 97.25%, 97.11%, 97.11%, and 97.00%, respectively. The DNA G+C content of strain DCT-5T was 64.7 mol%, and its DNA–DNA relatedness values with A. silviterrae KIS14-16T, A. livingstonensis LI2T, A. stackebrandtii CCM 2783T, A. psychrochitiniphilus GP3T, A. ramosus CCM 1646T, and A. cryoconiti Cr6-08T were 32.57 ± 2.02%, 28.75 ± 0.88%, 31.93 ± 1.15%, 34.73 ± 1.86%, 29.12 ± 1.56%, and 27.23 ± 0.88%, respectively. The major quinone was MK-9(H2) and major fatty acids were anteiso-C15:0, anteiso-C17:0, iso-C15:0, and iso-C16:0. The polar lipids were diphosphatidylglycerol (DPG), phosphatidylglycerol (PG), phosphatidylinositol (PI), unidentified glycolipid (GL), two unidentified aminophospholipids (APLs), and three unidentified lipids (Ls). The peptidoglycan type was A3α. On the basis of phenotypic, phylogenetic, genotypic, and chemotaxonomic characteristics, strain DCT-5T represents a novel species of the genus Arthrobacter, for which the name Arthrobacter dokdonellae sp. nov. is proposed. The type strain is DCT-5T (= KCTC 49189T = LMG 31284T).

<P>This retrospective study investigated the clinical characteristics and outcomes of second malignant neoplasms (SMNs) in survivors of childhood cancer from multiple institutions in Korea. A total of 102 patients from 11 institutions who developed SMN after childhood cancer treatment between 1998 and 2011 were retrospectively enrolled. The most common primary malignant neoplasms (PMNs) were central nervous system (CNS) tumors (n = 17), followed by acute lymphoblastic leukemia (n = 16), non-Hodgkin lymphoma (n = 13), and osteosarcoma (n = 12). The most common SMNs were therapy-related myeloid neoplasms (t-MNs; acute myeloid leukemia [AML], 29 cases; myelodysplastic syndrome [MDS], 12 cases), followed by thyroid carcinomas (n = 15) and CNS tumors (n = 10). The median latency period was 4.9 years (range, 0.5–18.5 years). Among 45 patients with solid tumors defined as an SMN, 15 (33%) developed the lesion in a field previously subjected to radiation. The 5-year overall survival (OS) rate of patients with an SMN was 45% with a median follow-up time of 8.6 years. Patients with AML, MDS, and CNS tumors exhibited the poorest outcomes with 5-year OS rates of 18%, 33%, and 32%, respectively, whereas those with second osteosarcoma showed comparable outcomes (64%) to patients with primary counterpart and those with second thyroid carcinoma had a 100% OS rate. Further therapeutic efforts are recommended to improve the survival outcomes in patients with SMNs, especially in cases with t-MNs and CNS tumors.</P>

Neurilemmoma, also called schwannoma, neurinoma and perineural fibroblastoma is a rare benign tomor orginated from the schwann cell of nerve sheath. They occur commonly at 20-50 aged person and involved the head and neck and flexor surface of the extremity. Histologically, they are characterized by Antoni A and Antoni B tissue composed of high and low cellularity. Primary neurilemmoma involved in chest wall is very rare and have not been reported previously. We experienced chest wall neurilemmoma in a 72 year old man. The pulmonary radiology showed the mass in the chest wall, and percutaneous transthoracic needle lung biopsy was performed to confirm the schwannoma.
