압박 고 나사로 치료한 대퇴골 전자부 골절 : 고정 실패의 인자에 대한 분석 Analysis of Factors associated with Failure of Fixation

목 적 : 압박 고 나사로 치료한 대퇴골 전자부 골절의 고정 실패에 영향을 미치는 요소에 대하여 분석하고자 하였다. 대상 및 방법 : 1995년 5월부터 2000년 7월까지 대퇴골 전자부 골절을 압박 고 나사를 이용하여 치료하고 1년 이상 추시가 가능했던 97예를 대상으로 하였다. 골절의 분류는 Jensen 분류법을, 골다공증의 정도는 Singh 지수를 이용하였다. 술후 방사선 검사에서 경체각, 정복 상태, 지연 나사의 대퇴골 두 내 위치, tip-apex distance(TAD) 및 지연 나사의 활강 거리를 측정하였고 고정 실패에 미치는 영향을 통계학적으로 분석하였다. 결 과 : 97예 중 17예(17%)에서 고정 실패가 발생하였고 그 중 지연 나사의 과도한 활강으로인한 경우가 15례(15.4%), 골두 천공 1얘, 10도 이상의 외반 변형 1예였다. 고정 실패의 발생과 80세 이상의 고령, 불안정성 골절, telescoping 정복, 원위 골편의 내측 혹은 전방 전위, 대퇴골 두 내 지연 나사의 위치와 밀접한 관계가 있었다. 결 론 : 압박 고 나사를 이용한 대퇴골 전자부 골절의 치료에서 고정 실패를 방지하기 위해서는 정확한 정복이 필요하고 대퇴골 두 내에서 지연 나사의 전방 위치를 피하는 것이 좋을 것으로 사료된다. Purpose : This study was performed to analyze the significant factors that may affect failure of fixation in trochanteric fractures of the femur treated with the compression hip screw. Materials and Methods : From May 1995 to July 2000, the authors analyzed 97 cases of trochanteric fracture of the femur treated with the compression hip screw and followed more than one year. We classified the fracture type by Jensen's method. We used Singh index for the degree of osteroporosis. In the post-operative radiograph, we checked neck-shaft angle, state of reduction, position of the lag screw within the femoral head, tip-apex distance, and sliding distance of the lag screw. The relationship between these factors and failure of fixation was statistically analyzed. Results : There were 17 cases(17.5%) of failure of fixation ; 15 cases(15.4%) of excessive sliding of the lag screw, 1 case (1%) of cutting out of the lag screw, and 1 case (1%) of valgus malunion. There were significant relationship between failure of fixation and old age over 80 , unstable fracture, telescoping reduction, anterior or medial displacement of the distal fragment, and anterior placement of the lag screw within the femoral head. Conclusion : Accurate reduction and avoidance of the placement of the lag screw in the anterior part of the femoral head were important factors to prevent failure of fixation in trochanteric fractures of the femur treated with the compression hip screw.
Squalene Adjuvant를 이용한 단백질 항원의 세포성 면역반응 유도

To investigate the role of adjuvant for cytotoxic T lymphocytes (CTL) induction, squalene-based adjuvant was studied its capability eliciting to the induction of cellular immunity as well as humoral immunity to exogeneous proteins. Ovalbumin (OVA) was used as model proteins. It was demonstrated that antigen formulation consisted of metabolizable oil squalene mixed with Tween 80 and pluronic L121 (S/TJ121) could induced the antigen specific CTL responses and antibodies irrespective of immunization routes in mice. We also demonstrated that this antigen formulation was a inducer of CD8+, major histocompatibility complex(MHC) class I-restricted, and antigen-specific C;1'Ls. These data suggest that the squalene-based emulsion system is a potent adjuvant inducing cellular immunity as well as humoral immunity.

Inhibition of acetylcholinesterase (AChE) is one of the important modes of action available for the control of insects. An assessment was made of the electric eel AChE inhibitory activity of 55 endophytic fungi isolated from the leaves of Huperzia serrata collected in Eunshi, Hubei Province, China. Of 55 mycelial extracts and 55 ethyl acetate-soluble fractions from liquid culture of the fungi, the ethyl acetate-soluble fraction of strain ES056 exhibited strong AChE inhibitory activity (88%) at a concentration of 100 μg/mL. Based on morphological characteristics and nuclear ribosomal DNA ITS sequence analysis, this fungus showed greatest similarity to members of the order Eurotiales and was shown to be most closely related to members of the Aspergillus aculeatus group. It was designated Aspergillus sp. MY056. This strain merits further study as a potential anti-AChE product.

<P>This paper describes a primary study for a new method of enhancing measurement with using an impedance analysis method that is applied to structural health monitoring. The goal of this research is to show basic information that is used to evaluate the structural health condition with using piezoelectric sensors. We present a study for the development of a practical and quantitative technique for the assessment of the structural health condition with using a piezoelectric impedance-based technique that is associated with longitudinal wave propagation. The natural frequency has a tendency of frequency shifting with respect to the hole size that corresponds to the real structure’s crack size. To numerically estimate the damage condition, we suggest the Cov (Covariance) and CC (Correlation Coefficient) evaluation methods.</P>

With the acid of PCR, we used two intragenic factor VIII gene RFLP markers(BclI-intron 18 and XbaI-intron 22) and one extragenic marker(St 14 VNTR) to estimate allele frequencies of these markers from 100 Korean individuals and to analyze eight Korean hemophilia A families. Allele frequencies for the presence of the enzyme sites and heterozygosity expectations were 86 and 24% for BclI-intron 18 RFLP marker, and 62% and 47% for XbaI-intron 22 RFLP marker, respectively. When BclI-intron 18 was homozygous, 31.5% of women was predicted to be heterozygous for XbaI-intron 22. Number of repeats in St14 VNTR loci from Korean males differed from those previously reported for Caucasian males. 68% of Korean women was estimated to be heterozygous for St14 VNTR marker. Analysis of hemophilia A families with BclI-RFLP and St14 VNTR proved to be informative for six out of eight families tested.

This study was undertaken to compare the complications of outside-in transobturator tape procedures (TOT) and inside-out transobturator tape procedures (TVT-O) with concomitant gynecologic surgery for the treatment of female stress urinary incontinence (SUI). A retrospective review of 206 consecutive patients who underwent either TOT or TVT-O with concomitant gynecologic operations between March 2008 and February 2011 was conducted. The incidence of perioperative complications was compared. For statistical analysis, chi-squared tests were used. There were no reports of intraoperative complications such as vaginal injury or bladder perforation. Postoperative complications were noted in 23 procedures (11.2%). These included 6 cases of urinary retention (2.9%), 2 cases of vulva hematoma (1.0%), 7 cases of urinary tract infection (3.4%), 4 cases of de novo urgency (2.9%), and 4 cases of vaginal erosion (2.9%). There were no significant differences in complication rates between the two groups. Our results suggest that inside-out and outside-in procedures are simple and safe techniques that may have a low rate of complications when used with a concomitant gynecologic operation.

A 73-year-old female with diabetes admitted for treatment of an intertrochanter fracture of the femur and a urinary tract infection (UTI) with Escherichia coli developed thrombosis in her right azygos vein, which was thought to be associated with antiphospholipid and immunoglobulin M anticardiolipin antibodies. After antibiotic therapy, antiphospholipid antibody was undetectable, and a repeat chest computed tomography showed complete resolution of the azygos vein thrombosis. A wide variety of infections can be associated with thrombotic events in patients with transient antiphospholipid syndrome (APS), and this case serves as a reminder that the possibility of transient APS should be considered in patients with venous thrombosis in the setting of a UTI. <P><B>Abstract</B></P> <P>The rapid detection of biological contaminants such as worms in fresh-cut vegetables is necessary to improve the efficiency of visual inspections carried out by workers. Multispectral imaging algorithms were developed using visible-near-infrared (VNIR) and near-infrared (NIR) hyperspectral imaging (HSI) techniques to detect worms in fresh-cut lettuce. The optimal wavebands that can detect worms in fresh-cut lettuce were investigated for each type of HSI using one-way ANOVA. Worm-detection imaging algorithms for VNIR and NIR imaging exhibited prediction accuracies of 97.00% (RI<SUB>547/945</SUB>) and 100.0% (RI<SUB>1064/1176</SUB>, SI<SUB>1064-1176</SUB>, RSI-I<SUB>(1064-1173)/1064</SUB>, and RSI-II<SUB>(1064-1176)/(1064+1176)</SUB>)<SUB>,</SUB> respectively. The two HSI techniques revealed that spectral images with a pixel size of 1×1mm or 2×2mm had the best classification accuracy for worms. The results demonstrate that hyperspectral reflectance imaging techniques have the potential to detect worms in fresh-cut lettuce. Future research relating to this work will focus on a real-time sorting system for lettuce that can simultaneously detect various defects such as browning, worms, and slugs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> VNIR- and NIR- HSI algorithms to detect worm on fresh-cut lettuce were developed. </LI> <LI> The optimal wavebands to discriminate worm were investigated by the one-way ANOVA. </LI> <LI> The worm detection algorithm achieved the accuracy of 100% for NIR-HSI. </LI> <LI> The best pixel size of spectral images to detect worm were 1×1 or 2×2mm. </LI> </UL> </P>
