        Hypertriglyceridemia: A Neglected Risk Factor for Ischemic Stroke?

        Hypertriglyceridemia is caused by defects in triglyceride metabolism and generally manifests as abnormally high plasma triglyceride levels. Although the role of hypertriglyceridemia may not draw as much attention as that of plasma cholesterol in stroke, plasma triglycerides, especially nonfasting triglycerides, are thought to be correlated with the risk of ischemic stroke. Hypertriglyceridemia may increase the risk of ischemic stroke by promoting atherosclerosis and thrombosis and increasing blood viscosity. Moreover, hypertriglyceridemia may have some protective effects in patients who have already suffered a stroke via unclear mechanisms. Therefore, further studies are needed to elucidate the role of hypertriglyceridemia in the development and prognosis of ischemic stroke.

        Investigation of the SHM-oriented model and dynamic characteristics of a super-tall building

        Shanghai Tower is a 632-meter super high-rise building located in an area with wind and active earthquake. A sophisticated structural health monitoring (SHM) system consisting of more than 400 sensors has been built to carry out a long-term monitoring for its operational safety. In this paper, a reduced-order model including 31 elements was generated from a full model of this super tall building. An iterative regularized matrix method was proposed to tune the system parameters, making the dynamic characteristic of the reduced-order model be consistent with those in the full model. The updating reduced-order model can be regarded as a benchmark model for further analysis. A long-term monitoring for structural dynamic characteristics of Shanghai Tower under different construction stages was also investigated. The identified results, including natural frequency and damping ratio, were discussed. Based on the data collected from the SHM system, the dynamic characteristics of the whole structure was investigated. Compared with the result of the finite element model, a good agreement can be observed. The result provides a valuable reference for examining the evolution of future dynamic characteristics of this super tall building.

      • Seismic response of combined retaining structure with inclined rock slope

        A gravity wall combined with an anchoring lattice frame (a combined retaining structure) is adopted at a typical engineering site at Dali-Ruili Railway Line China. Where, the combined retaining structure supports a soil deposit covering on different inclined rock slopes. With an aim to investigate and compare the effects of inclined rock slopes on the response of combined retaining structure under seismic excitation, three groups of shaking table tests are conducted. The rock slopes are shaped as planar surfaces inclined at angles of 20°, 30°, and 40° with the horizontal, respectively. The shaking table tests are supplemented by dynamic numerical simulations. The results regarding the horizontal acceleration response, vertical acceleration response, permanent displacement mode, and axial anchor force are comparatively examined. The acceleration response is more susceptible to outer structural profile of combined retaining structure than to inclined angle of rock slope. The permanent displacement decreases when the inclined angle of the rock slope increases within a range of 20°-40°. A critical inclined angle of rock slope exists within a range of 20°-40°, and induces the largest axial anchor force in the combined retaining structure.

      • Extraskeletal Ewing Sarcomas in Late Adolescence and Adults: A Study of 37 Patients

        Background: Extraskeletal Ewing sarcoma (EES)/primitive neuroectodermal tumours (PNET) are rare soft tissue sarcomas. Prognostic factors and optimal therapy are still unconfirmed. Materials and Methods: We performed a retrospective analysis on patients to explore the clinic characteristics and prognostic factors of this rare disease. A total of 37 patients older than 15 years referred to our institute from Jan., 2002 to Jan., 2012 were reviewed. The characteristics, treatment and outcome were collected and analyzed. Results: The median age was 28 years (range 15-65); the median size of primary tumours was 8.2 cm (range 2-19). Sixteen patients (43%) had metastatic disease at the initial presentation. Wide surgical margins were achieved in 14 cases (38%). Anthracycline or platinum-based chemotherapy was performed on 29 patients (74%). Radiotherapy was delivered in 13 (35%). At a median follow-up visit of 24 months (range 2-81), the media event-free survival (EFS) and overall survival (OS) were 15.8 and 30.2 months, respectively. The 3-year EFS and OS rates were 24% and 43%, respectively. Metastases at presentation and wide surgical margins were significantly associated with OS and EFS. Tumour size was significantly associated with OS but not EFS. There were no significant differences between anthracycline and platinum based chemotherapy regarding EFS and OS. Conclusions: EES/PNET is a malignant tumour with high recurrence and frequent distant metastasis. Multimodality therapy featuring wide surgical margins, aggressive chemotherapy and adjuvant local radiotherapy is necessary for this rare disease. Platinum-based chemotherapy can be used as an adjuvant therapy.

        Characterization and expression of calcium-activated potassium channels (Slo) in ovary of the mud crab, Scylla paramamosain

        Large conductance calcium-activated potassiumchannels (Slo) play important roles in controllingneuronal excitability. At present, very little is known aboutthe function of Slo channels on ovarian development. Wecloned the SPSlo gene from the mud crab, Scylla paramamosain. This gene shows 91 and 93 % sequence identityto PISlo from the spiny lobster, Panulirus interruptus andCBSlo from the jonah crab, Cancer borealis, respectively. We isolated six variants of the SPSlo cDNA within S. paramamosain ovary tissue. Sequence analysis indicatedthat there were at least seven alternative sites in SPSlo,each with multiple alternative segments. Real-time PCRshowed that the SPSlo gene was expressed in various tissues,and highly expressed in brain and ovary. In addition,the expression of SPSlo changed throughout ovariandevelopment, highest at the early-developing stage (StageII) followed by a slow decrease in subsequent stages. Theseresults suggested that SPSlo channels may be implicated inthe ovarian development of the mud crab.

        A Prediction Model of the Sum of Container Based on Combined BP Neural Network and SVM

        The prediction of the sum of container is very important in the field of container transport. Many influencing factors can affect the prediction results. These factors are usually composed of many variables, whose composition is often very complex. In this paper, we use gray relational analysis to set up a proper forecast index system for the prediction of the sum of containers in foreign trade. To address the issue of the low accuracy of the traditional prediction models and the problem of the difficulty of fully considering all the factors and other issues, this paper puts forward a prediction model which is combined with a back-propagation (BP) neural networks and the support vector machine (SVM). First, it gives the prediction with the data normalized by the BP neural network and generates a preliminary forecast data. Second, it employs SVM for the residual correction calculation for the results based on the preliminary data. The results of practical examples show that the overall relative error of the combined prediction model is no more than 1.5%, which is less than the relative error of the single prediction models. It is hoped that the research can provide a useful reference for the prediction of the sum of container and related studies.

      • Transrectal Real-time Tissue Elastography - An Effective Way to Distinguish Benign and Malignant Prostate Tumors

        Background: To investigate the relationship between extracellular matrix parameters and texture of prostatic lesions evaluated by transrectal real-time tissue elastography (TRTE). Methods: 120 patients suspicious for prostate cancer underwent TRTE. Targeted biopsies were carried out after 12-core systematic biopsy. Epithelia were stained with hematoxylin-eosin, and Victoria blue and Ponceau S were used to stain elastic-collagen fibers, and picric acid-sirius red for visualization of collagen type I (Col1) and III (Col3). Smooth muscles were visualized by immunohistochemistry. All image analyses were performed in a blind manner using Image Pro Plus 6.0, and the area ratios of epithelium, elastic fibers, collagen fibers and Col1/Col3 were determined. Results: 42 patients with typical elastograms were included in the final data analysis. Significant differences were detected between the benign and malignant groups in the area ratios of epithelium (P = 0.01), smooth muscles and Col1/Col3 (P = 0.04, P = 0.02, respectively). There were no significant differences in the area ratios of epithelium, smooth muscle and elastic fibers between the stiff and soft lesion groups. The area ratio of Col1 was ($0.05{\pm}0.03$) in the stiff group, and ($0.02{\pm}0.01$) in the soft group (P= 0.00). However, the area ratio of Col3 was ($0.03{\pm}0.02$) in the stiff group, and ($0.05{\pm}0.04$) in the soft group (P = 0.16). Col1/Col3 in the stiff group ($1.99{\pm}1.59$) was greater than in the soft group ($0.71{\pm}0.64$) (P = 0.01). Conclusions: Tissue hardness of prostatic tumors was mainly dependent on the Col1 content, Col1/Col3 being higher in malignant than in benign lesions, so the prostate tissue texture can be used as a target for distinguishing between the two with TRTE.

        Ginsenoside Rg5 overcomes chemotherapeutic multidrug resistance mediated by ABCB1 transporter: in vitro and in vivo study

        Background: Multidrug resistance (MDR) to chemotherapy drugs remains a major challenge in clinicalcancer treatment. Here we investigated whether and how ginsenoside Rg5 overcomes the MDR mediatedby ABCB1 transporter in vitro and in vivo. Methods: Cytotoxicity and colon formation as well as the intracellular accumulation of ABCB1 substrateswere carried out in MDR cancer cells A2780/T and A549/T for evaluating the reversal effects of Rg5. Theexpressions of ABCB1 and Nrf2/AKT pathway were determined by Western blotting. An A549/T cellxenograft model was established to investigate the MDR reversal activity of Rg5 in vivo. Results: Rg5 significantly reversed ABCB1-mediated MDR by increasing the intracellular accumulation ofABCB1 substrates without altering protein expression of ABCB1. Moreover, Rg5 activated ABCB1 ATPaseand reduced verapamil-stimulated ATPase activity, suggesting a high affinity of Rg5 to ABCB1 bindingsite which was further demonstrated by molecular docking analysis. In addition, co-treatment of Rg5 anddocetaxel (TXT) suppressed the expression of Nrf2 and phosphorylation of AKT, indicating that sensitizingeffect of Rg5 associated with AKT/Nrf2 pathway. In nude mice bearing A549/T tumor, Rg5 and TXTtreatment significantly suppressed the growth of drug-resistant tumors without increase in toxicitywhen compared to TXT given alone at same dose. Conclusion: Therefore, combination therapy of Rg5 and chemotherapy drugs is a strategy for the adjuvantchemotherapy, which encourages further pharmacokinetic and clinical studies.

        Subdivision of Nasopharyngeal Carcinoma Patients with Bone-Only Metastasis at Diagnosis for Prediction of Survival and Treatment Guidance

        Purpose The purpose of this study was to subdivide M1 stage nasopharyngeal carcinoma (NPC) patients with bone-only metastases for prognosis prediction while identifying the treatment effect of locoregional radiotherapy (LRRT) and metastasis radiotherapy (MRT) among patients with different risk. Materials and Methods From November 2006 to October 2016, a total of 226 patients with bone-only metastasic NPC were retrospectively enrolled. All patients developed distant lesions before receiving treatment. All potential prognostic factors were considered and the correlation of the M1 subdivisions with overall survival (OS) was determined by Cox regression hazards model. Kaplan-Meier curves were used to appraise survival condition and log-rank testing was used to compare the differences. Results The median follow-up time was 33.9 months (range, 3 to 126 months). According to multivariate Cox proportional hazard analysis, the number of metastatic lesions and Epstein-Barr virus (EBV) DNA status after palliative chemotherapy (PCT) were independent prognostic factors for OS. Thus, we subdivided patients into three risk groups according to these two factors. Systemic chemotherapy combined with LRRT may benefit patients in low- and intermediate-risk groups but not in the high-risk group. Further aggressive MRT based on systemic chemotherapy showed no survival benefit in any risk group. Conclusion The stratification of NPC patients with bone-only metastasis based on EBV DNA after PCT and the number of metastatic lesions provided promising prognostic value and could aid clinicians in person-specific treatment.
