        원위부 후하소뇌동맥류 2례 : Case Report

        Authors reported two cases of aneurysm of distal posterior inferior cerebellar artery(PICA) which arose from the televelomedullary segment and one of them was associated with AVM fed by the tonsillo-hemispheric branch of PICA. Aneurysms were successfully obliterated through the bilateral suboccipital approach and the AVM was completely removed in same operative procedure in the case with combined lesions. Immediate postoperative course was uneventful in both cases but the patient combined with AVM. developed postoperative meningitis and was discharged in moribund state on POD 42.

        가토 측뇌실내 Diphenhydramine과 Ranitidine의 혈압상승 효과

        H1-receptor antagonist인 diphenhvdramine (DPH)과 H2-receptor antagonist인 ranitidine (RAN)의 측뇌실내 투여가 혈압에 어떻게 영향을 미치는지를 알아보기위해 urethane 마취가토를 사용하여 실험하였다. 1) DPH와 RAN은 측뇌실내 투여로 모두 혈압상승을 일으켰으나 정맥내 투여로는 혈압에 거의 영향을 미치지 못했다. 측뇌실내로 투여한 DPH의 승압효과는 용량 의존적이었으나 RAN의 승압효과는 그렇지 못했다. 2) 뇌실내 DPH 1㎎ 투여에 의한 승압효과는 뇌실내 phentolamine 250, 500㎍, 정맥내 chlorisondamine 0.1, 1㎎/Kg, phenoxybenzamine 1㎎/Kg 처리후에는 유의하게 감약되거나, 강압효과로 역전되었으며 척수이단가토에서 DPH는 혈압상승을 일으키지 못했다. 3) 뇌실내 RAN 1㎎ 투여에 의한 승압효과는 뇌실내 phentolamine 500㎍, 정맥내 chlorisondamine 1㎎/Kg, phenoxbenzamine 1㎎/Kg 처리에 의해 영향 받지 않았으며 척수 이단 가토에서도 이 RAN의 승압효과는 그대로 나타났다. 이상의 실험 결과로 가토에서 H1-receptor antagonist인 DPH는 뇌내 α-adrenoceptor의 흥분을 통한 중추성 교감신경계 긴장도의 증가에 의해 혈압상승을 일으키며 H2-receptor antagonist인 RAN은 중추성으로 혈압상승을 일으키는 어떤 체액성인자의 분비를 촉신시켜 혈압상승을 일으킨다고 추론하였다. This study was undertaken to observe the effects of centrally administered antihistamines on the blood pressure. Diphenhydramine(DPH), a H1-receptor antagonist, and ranitidine (RAN), a H2-receptor antagonist were administered intracerebroventricularly(icv) on urethane-anesthetized rabbits. 1) Both DPH and RAN administered intracerebroventricularly increased blood pressure, however the intravenous(iv) adminstration of them did not affect blood pressure. The pressor response to icv DPH was dose-dependent of them did not affect blood pressure. The pressor response to icv DPH was dose-dependent, but that to icv RAN was not 2) The pressor response to icv DPH(1㎎) was either markedly attenuated or reversed to depressor response by the pretreatment with icv phentolamine(250, 500㎍), and iv chlorisondamine(0.1, 1㎎/㎏). And iv phenoxybenzamine(1㎎/㎏). In cord-sectioned rabbits. Icv RAN) 1㎎) did not produce pressor response. 3) The pressor responsr to icv RAN(1㎎) was not affected by the pretreatment with icv phentolamine(500㎍), iv chlorisondamin(1㎎/㎏) and iv phenoxybenzamine(1㎎/㎏). And iv phenoxybenzamine(1㎎/㎏). RAN also produced pressor response in cordsectioned rabbits. These results suggest that the pressor response to icv DPH is elicited by increasing peripheral sympathetic ton via the stimulation of central α-adrenoreceptors and the pressor response to icv RAN is produced by releasing some humoral factor which can increase blood pressure.

        순환신경망을 이용한 태양광 발전량 예측 모델 설계

        The Smart Grid predicts the power generation amount of renewable energy and enables efficient power generation and consumption. Existing PV power generation prediction studies have rarely applied and compared recurrent neural network techniques that are superior to time series. Furthermore, in the reported studies, there is no consideration of the length of past data used for learning, leading to lowered prediction performance of the model. In this study, we used the embedded variable selection techniques to find the factors influencing PV power generation. Subsequently, experiments were carried out to insert various past data length into the recurrent neural networks (RNN, LSTM, GRU). We found the optimal prediction factors and designed a prediction model based on the outcomes of the experiments. The designed PV power generation prediction model shows better prediction performance compared to other factor settings. In addition, better performance based on the prediction rate is confirmed in the present study as compared with the existing researches. 스마트그리드는 태양광 발전을 포함한 신·재생에너지의 발전량을 예측하고 이를 기반으로 효율적인 전력 생산과 소비를 가능하게 한다. 기존 태양광 발전량 예측 연구들은 시계열에 뛰어난 순환신경망 기법들을 적용 및 비교한 연구가 거의 없다. 또한 학습에 사용되는 과거 데이터의 길이에 대한 고려가 없어 모델의 예측 성능이 떨어졌다. 본 연구에서는 임베디드 변수 선택 기법을 이용하여 태양광 발전에 영향을 미치는 요인을 찾아내고, 시계열 순환신경망 기법들(RNN, LSTM. GRU)에 다양한 과거 데이터 길이를 넣는 실험을 진행하였다. 이 과정에서 가장 뛰어난 성능을 보이는 예측 요인들을 찾고 예측 모델을 설계하였다. 설계한 태양광 발전량 예측 모델은 다른 변수 설정을 사용할 때와 비교하여 더욱 뛰어난 예측 성능을 보이는 것을 확인하였다. 또한 기존 연구들과의 비교를 통하여 본 연구에서 개발한 태양광 발전량 예측 결과가 더 뛰어난 성능을 보이는 것을 확인하였다.

        Simulation of Modeling Characteristics of Pumping Design Factor on Vacuum System

        Recently, with the development of advanced thin film devices comes the need for constant high quality vacuum as the deposition pressure is more demanding. It is for this reason our research seeks to understand how the variable design factors are employed in such vacuum systems. In this study, the effects of design factor applications on the vacuum characteristics were simulated to obtain the optimum design modeling of variable models on an ultra high vacuum system. The commercial vacuum system simulator, VacSim(multi), was used in our investigation. The reliability of the employed simulator was verified by the simulation of the commercially available models of ultra high vacuum system. Simulated vacuum characteristics of the proposed modeling aligned with the observed experimental behavior of real systems. Simulated behaviors showed the optimum design models for the ideal conditions to achieve optimal pressure, pumping speed, and compression ratio in these systems.

        Choroid plexus papillomas are uncommon neoplasms of the central nervous system, accoun- ting for 0.4% to 0.1 % of all intracranial tumors. We present a choroid plexus papilloma in the trigone of the lateral ventricle in an 8 months old infant with hydrocephalus We successfdly controlled hydrocephalus and subdural fluid collection afier total removal of the mass.

        금속판 고정술을 이용한 하부경추부 전방접근법의 임상적 고찰

        The surgical approach to the lower cervical lesions, especially in traumatic injuries, has been controversial. Some authors advocated posterior fusion for such lesions, while others disagreed and reported good operative results with anterior approach using several types of cervical plates. We analysed 28 patients with lower cervical spine traumatic as well as pathological lesions who underwent 32 anterior surgical interventions with cervical plates during the period of August, 1991 and December, 1993. A successful postoperative vertebral stability was obtained in 25 patients(89.3%) and in 19 patients(87.5%) who had predominent posterior ligamentous injuries. Serious complications such as esophageal perforation and postoperative angulation were seen in 5 patients(17.3%) that were related to the process of reduction. With our clinical experiences, we support anterior fusion with cervical plates particularly for lower cervical lesions even though posterior fusion has been preferred for traumatic posterior ligamentous complex injuries.
