        "본 논문은 대한내과학회지 2006년 제70권 제3호에 실렸던 논문으로 대한내과학회 편집위원회의 승인을 득하고 본 협회지에 게재함. Background : Diabetes mellitus is a major independent risk factor for atherosclerosis. In recent years non-invasive high resolution B-mode ultrasound methods have been developed to measure the intima-media thickness(IMT) of the carotid artery as an indicator for early atherosclerosis. Itis known that obesity plays a role in the development of type 2 diabetes and cardiovascular disease, and it has also been reported that not only the amount but also the distribution of body fat is important. This study investigated the relationship between obesity and the development of carotid atherosclerosis in type 2 diabetic patients. Methods: Carotid IMT was measured by ultrasound B-mode imaging in 144 patients with type 2diabetes mellitus. All subjects underwent assessment for the degree and distribution of obesity, the presence of coronary artery disease risk factors, and the presence of diabetic complications. Resuts: Carotid IMT was increased in the abdominal obese group defined by waist circurference. However, there was no significant difference in carotid TMT between the non-obese group and obese group as defined by body mass index, waist to hip ratio, and total body fat percent measured by bio electrical impedance analysis. There were positive correlations between carotid IMT and age, duration of diabetes, systolic blood pressure, and waist circumference. Multiple linear regression analysis revealed the variable that interacted independently with carotid IMT was age in type 2 diabetic patients. Carotid IMT was significantly increased in type 2 diabetic patients with macrovascular complications and microvascvlar complications .Conclusion: This study suggested that abdominal obesity rather than general obesity was associated with carotid atherosclerosis reflected by increment of carotid IMT in type 2 diabetic subjects.

        Recently, as the industrial structure and working environment change due to the structural deepening of low birthrates and aging and the rapid development of information technology in our society, discussions on the reemployment of retirees can be very important. Therefore, we sought to carry out empirical testing on the path that leads to the re-employment intentions from the positive psychological capital of the retiree in this study. It was shown in the research result that as positive psychological capital, self-efficacy and hope have a positive impact on career planning and self-efficacy and optimism have a positive impact on career commitment relationships. In addition, career planning was shown to have a positive impact on career commitment and re-employment intentions. And in the relationship between career planning and career commitment and re-employment intentions, social support was shown to have a moderating effect. Through this study, we intend to provide theoretical support and foundation for the proactive and systematic career development and the resulting career success of the retiree.

        자세 변화와 도구 사용 시 최대 들숨과 날숨 시 호흡근의 근활성도 차이

        Purpose : In this study, we aimed to measure how respiratory muscle activity changes as posture changes. We also aimed to measure the inspiratory sternocleidomastoid (SCM) and upper trapezius (UT) muscle activities as well as expiratory rectus abdominis (RA) and oblique abdominis (OA) muscle activities in healthy male and female university students in their twenties. Methods : Using the percentage of reference voluntary contraction (% RVC) with electromyography, we measured the SCM, UT, RA, and OA muscle activities in 16 healthy male and female university students in their 20s according to position change and breather tool use. Results : We found that the average value of inspiratory and expiratory muscle activities by posture was highest when using the breather tool. When using the tool with 10-mm and 2-mm straws for standard breathing at rest in the sitting position, a statistically significant increase in SCM, UT, and OA activities occurred (p < 0.05). For standard breathing at rest in the supine position, a statistically significant increase in SCM, RA, and OA activities occurred (p < 0.05). Conclusion : In clinical application, it may be better to use a respiratory muscle-strengthening tool to increase the activity of inspiratory and expiratory muscles.

        LNG 저장탱크용 멤브레인재(STS 304강)의 강도 및 파괴인성에 미치는 저온효과

        Tensile and fracture toughness tests of the cold-rolled STS 304 steel plate for membrane material of LNG storage tank were performed at wide range of temperatures, 11 IK(boiling point of LNG), 153K , 193K and 293K(room temperature). Tensile strength significantly increases with a decrease in temperature, but the yield strength is relatively insensitive to temperature. Elongation at 193K abruptly decreases by 50% of that at 293K, and then decreases slightly in the temperature range of 193K to 111K. Strain hardening exponents at low temperatures are about four times as high as that at 293K. Elastic-plastic fracture toughness($J_c$) and tearing modulus($T_{mat}$) tend to decrease with a decrease in temperature. The $J_c$ values are inversely related to effective yield strength in the temperature range of 111K to 293K. These phenomena result from a significant increase in the amount of transformed martensite in low temperature regions.

        Isolation of Bacteria with Protease Activity from Cheonggukjang and Purification of Fibrinolytic Enzyme

        혈전용해효소를 생산하는 균주 분리를 위해서 우선 한국, 일본 등지에서 모은 21개의 청국장 시료를 준비하였고 총 268개의 균주를 분리하였다. 이 중에서 1% skim milk가 포함된 nutrient agar 배지에서 protease를 생산하는 bacteria를 분리하였고, 이 결과로 22개의 균주가 분리되었다. 균주들은 apiweb을 통해 생화학적 특성에 근거하여 동정하였다. 또한 세균동정을 위해 16S rRNA 염기서열 분석을 수행하였다. 분리된 대부분의 균주는 Bacillus subtilis와 Bacillus amyloliquefaciens였다. 혈전용해효소의 활성은 fibrin plate 방법에 의해 측정되었고 A2-14, A2-20, C1-05, C1-09, F2-01로 명명된 다섯 균주가 선택되었다. 이중에서 A2-20 균주는 강한 혈전용해 활성을 보였고 동정결과 Bacillus amyloliquefaciens에 가까웠다. A2-20 균주에 의해 생산되는 혈전용해효소는 균 상등액을 이용한 gel filtration과 ion exchange chromatography를 거쳐 부분정제 되었다. 부분 정제된 효소의 최적 pH는 7.0이었고, 최적 온도는 35℃였다. 정제된 단백질의 분석은 SDS-PAGE와 zymography로 이루어졌다. 이와 더불어 혈전용해효소의 유전자적 분석도 수행되었으며 A2-20 균주가 생산하는 혈전용해효소의 부분적인 염기서열과 유전적 상동성을 보이는 서열을 규명하였다. To isolate the fibrinolytic enzyme, 268 strains from 21 samples were morphologically isolated from Cheonggukjang collected from Korea and Japan. Among the 268 strains, protease-producing bacteria were isolated in nutrient agar medium including 1% skimmed milk. As a result of this, 22 strains were isolated. Apiweb site was used to identify these strains based on their biochemical properties. In addition, 16S rRNA sequencing was performed to identify the strain. Most of the identified strains were Bacillus subtilis and B. amyloliquefaciens. Fibrinolytic enzyme activity was measured with the fibrin plate method. Five strains were finally selected: A2-14, A2-20, C1-05, C1-09, and F2-01. Of those five strains, the A2-20 strain, which is close to B. amyloliquefaciens, showed the strongest fibrinolytic activity. The fibrinolytic enzyme produced by the A2-20 strain was partially purified from culture supernatant by gel filtration and ion exchange chromatography. The optimal pH and temperature values of the partially purified enzyme were 7.0 and 35°C, respectively. Purified protein analysis was carried out with SDS-PAGE and zymography. A genetic analysis was also conducted by PCR based on the consensus sequence of fibrinolytic enzyme. Corresponding genes with a partial sequence of the A2-20 strain were identified.

        인슐린 비의존형 당뇨병 환자에서 혈장 호모시스테인 농도와 만성 합병증과의 관계

        Background : Elevated plasma homocysteine is a recently-recognized independent risk factor for cardiovascular disease. In patients with non-insulin dependent diabetes mellitus (NIDDM), plasma homocysteine concentration was reported to be elevated in association with nephropathy. However, inconsistent results were reported about the association with other microvascular complications. Methods : To determine the relationship between plasma homocysteine and the development of chronic diabetic microvascular complications, fasting plasma homocysteine, glycemic control, lipid and lipoprotein levels, vitamin status, renal function test, and chronic diabetic microvascular complications were assessed in 101 patients with NIDDM in the present study. Results : There was no difference in the plasma levels of homocysteine by sex, age, status of sugar control, duration of diabetes, other cardiovascular risk factors. Patients with diabetic microangiopathy had higher plasma homocysteine concentrations than those without diabetic microangiopathy. Moreover, there was a significant correlation between amount of urinary albumin excretion and plasma homocysteine level (p=0.004, r=0.357). However, multivariate analysis showed that only serum creatinine (β=0.635) was independently associated with plasma homocysteine level in NIDDM patients. The increase in plasma homocysteine was not shown to increase the risk of diabetic microvascular complications independently on multiple logistic regression analysis. Conclusion : In conclusion, decrease of renal function is an independent determinant of plasma homocysteine level and higher plasma homocysteine is associated to diabetic microangiopathy. But an increase in plasma homocysteine in patients with NIDDM is not independently associated with diabetic microvascular complications considering renal function. Therefore, the renal function should be considered in study about relationship between plasma homocysteine level and the development and/or progression of chronic diabetic microvascular coplications in diabetic patients.(Korean J Med 59:174-182, 2000)
