병원 호스피스센터-보건소 연계를 통한 지역사회 재가암환자 관리 프로그램 평가

목적: 본 연구는 말기 재가암환자 관리를 위한 병원호스피스센터와 보건소 간의 연계 프로그램을 평가함으로써 보다 효율적이고 효과적인 재가암환자 관리체계를 구축하고자 시도되었다. 방법: 호스피스센터-보건소 재가암관리 연계 사업은 1) 협약체결, 2) 재가 말기암환자 발굴 및 등록, 3) 가정호스피스 방문 서비스 제공, 4) 만족도 조사의 단계로 전개되었다. 일 호스피스센터와 지역의 3개 보건소가 협약을 체결하였고, 2009년 2월 1일에서 12월 31일까지 11개월 동안 43명의 환자에게 605건의 가정호스피스 방문을 실시하였다. 방문기록지 분석을 통해 서비스 대상자의 특성과 제공된 서비스의 종류와 내용을 분석하였고, 이 중 20명에게 서비스 만족도 조사를 실시하였다. 결과: 대상자의 76.7%가 60세 이상이었고, ECOG 전신 수행상태 점수는 0점과 1점이 각각 37.2%, 39.5%이었다. 환자가 병식이 있는 경우는 90.7%, 호스피스 동의서에 서명한 경우는 62.8%였다. 초기방문 시 환자의 주 호소는 전신쇠약감(86.0%)과 식욕부진(72.1%)이 가장 많았다. 평균 총 서비스 기간은 144.42일이었고, 총 605건의 방문 중 간호사 방문이 371회로 가장 많았다. 각 방문중 정서적 지지와 건강상담이 가장 빈번하게 제공되었고, 서비스 전반에 대한 만족도는 5점 척도로 측정하였을 때 평균 4.45점이었다. 결론: 본 연구는 지역사회 내에서 실제적이고 체계적인 재가 말기암환자 관리 프로그램 구축을 위한 중간평가로서의 의미가 있다. 본 연구에서 의뢰된 대상자는 입원형 호스피스보다 기능 상태가 양호하고 서비스 제공기간이 길었으며, 서비스에 대한 만족도, 특히 정서적지지와 환자상태 설명에 대한 만족도가 높았다. 그러나 방문횟수와 빈도, 임종기 관리, 병원입원 재입원하는 환자관리에 대한 기준과 서비스 표준마련은 앞으로의 과제로 제시되었다. Purpose: The purpose of this study was to evaluate a community-based cancer patient management program (CBPCMP) which was collaborated between a hospice center and public health centers. Methods: The CBPCMP proceeded on four steps; 1) Signing agreements with three public health centers, 2) Enrolling the domiciliary terminal cancer patients, 3) Providing home hospice service, and 4) Inquiring patient's level of satisfaction. From February 1 to December 31 in 2009, 43 terminal cancer patients were referred and provided with home hospice service. The hospice team made a total of 605 visits. Medical records for each visit and data from satisfaction surveys were analyzed. Results: 76.7% of patients were older than 60 years, and 90.7% of the patients were alert. The level of functional status for 76.7% of patients rated as lower than ECOG grade 1. 62.8% of the patients or their caregivers signed hospice service agreements. On the initial evaluation, the most frequent reasons for referral were general weakness (86.0%), followed by anorexia (72.1%). Nurses visited the patients' most frequently (371 visits), followed by volunteers (216 visits). Nurses provided emotional support and health promotion counseling on 95.1% and 22.9% of visits, respectively. The mean satisfaction score rated by patients and their family was 4.45 out of 5. Conclusion: This study tested CBPCMP in collaboration with hospice centers and public health centers. CBPCMP showed a possibility to improve the quality of end of life care. To insure the quality care, however, the guidelines for home hospice service should be developed.
피부미용사의 건강기능식품 섭취 경험에 따른 섭식행동, 피부미용영양지식 및 건강기능식품의 인식에 관한 비교연구

The purpose of this study was to identify the status of health functional food intake and related factors in order to contribute to public health improvement in aestheticians at skin-care shops. Of 430 questionnaires, 394 were returned. A total of 390 follow-up lists of questions which were answered honestly, and were analyzed. The subjects were divided into two groups. One was the intake experienced group, which consisted of 236 people. The other group consisted of 154 people, and called non-experienced group. Through a questionnaire, we surveyed the general information of respondent, the situation of health funtional food, eating habit, and the perception of skin care and nutrition. The intake experienced group`s mean age(p<0.000), education level(p<0.01), a career (p<0.01), exercise (p<0.01), the score of eating habits(p<0.000) and ratio of married(p<0.000) were higher than the non-experienced group. But, the perception of skin care and nutrition was higher in non-experienced group. As a result, the aestheticians working at skin-care taked health functional food to maintain and promotion the health. Accordingly that we will pay more attention to health promotion by establishing the social system so that the consumer choose right health functional food.

To estimate the accuracy, efficiency and reproducibility of the test results of clinical bacteriological laboratory, we evaluated the performance test and sterility test for the Media and identification reagents from May 1007 through December 1007. The results were; 1. By the sterility test for media, no contaminated organisms were isolated. 2. By the performance test of the enrichment media, selective differential media, biochemical media and identification reagents for the positive and negative control organisms, the acceptable data were yielded. 3. It should be boiled for 60 minutes that the best effective result of MacConkey agar, xylose lysine desoxycholate(XLD> agar and salmonella-shigella(S-S) agar can be obtained and for the growth on the media bacterial colony count must be more than 3 x 1Q3-4/mL. 4. The MacConkey agar showed the best growth of the three selective medium and XLD agar showed better detection for salmonella than S-S agar. 5. It showed that UV -light and dry media make the bacterial growth to be inhibited. For the best isolation and identification of the bacteria from the samples, we should always perform the quality control activites in clinical bacteriological laboratory.

Cervical cancer is one of the leading causes of female death from cancer worldwide with about 500,000 deaths per year, A strong association between certain human papilloma viruses (HPV type 16 and 18) and cervical cancer has been well known. An extract of Saururus chinensis, named as PE-46, has been used to investigate whether this agent has the ability of inhibiting the oncogenes E6 and E7 of HPV type 16, PE46 inhibited the proliferation of human cervical cancer cell lines in a dose response manner, PE46 also inhibited the in vitro binding of E6 and E6AP which are essential for the binding and degradation of the tumor suppressor p53. In addition, PE-46 inhibited the in vitro binding of E7 and Rb which is essential tumor suppressor for the control of cell cycle. The levels of MRNA for E6 and E7 were also decreased by PE-46. SiHa cells treated with PE46 induced GO/G1 arrest, resulting in inhibition of growth. Our study showed that the PE-46 can inhibit the cervical carcinomas via both inhibition of bindings between oncogenes and tumor suppressors, and inhibition of G1->S S transition. PE-46 inhibited the oncogenecity of E6 and E7 of HPV 16 type, thus could be used as a putative modulating agent for the treaoent of cervical Carcinomas caused by HPV.
수용성 β-1,3-glucans의 면역 활성 효능에 대한 연구

Background: $\beta$-1,3-glucans are well known to enhance the immune reactions, resulting in antitumor, antibacterial, antiviral, anticoagulatory and wound healing activities. $\beta$-1, 3-glucans have various activities depending on molecular weight, degree of branching, conformation, water-solubility and intermolecular association. However, the $\beta$-1,3-glucan linked backbone structure is essential and $\beta$-D-glucopyranosyl units are required for immuno-potentiating activities. Result: In this study, we tested the immunophamacological activities of soluble $\beta$-1,3-glucans and confirmed the following activities: (1) $IFN-{\gamma}$ production in PBMCs in the presence or the absence of PHA, LPS, or IL-18; (2) induction of various cytokines in the spleen and thymus; (3) adjuvant effect on the antibody production; (4) nitrogen oxide synthesis in macrophages; (5) the cytotoxic and antitumor effects on cell lines and ICR mice. Conclusion: These results strongly suggested that $\beta$-1,3-glucans possessed various immuno-pharmacological activities.
어린이집 어린이의 식생활 실태 조사를 통한 영양 개선 방안 연구

A survey study was carried out to investigate dietary practices of preschool children aged 1 to 6 in order to present concrete nutritional improvement proposals. Two kinds of questionnaires were completed by 250 mothers and 50 menu-planners at 50 day care centers. The contents of questionnaires were composed of eating behaviors and dietary patterns of children at day care centers and at home. nutrition attitude of menu-planners and the mother, practices of nutrition education, and food preference and food intake frequencies of children. Descriptive analysis were applied to the continuous data as well as categorical data. The maximum error ratio of 5% was selected for testing of the hypothesis. The results are summarized as follows: Over 90% of personnel in charge of the menu-planning were nonprofessional workers who had insufficient knowledge in nutrition and foods. 63% of children exhibited some dietary problems. Food prejudices which showed the highest score was vegetable. Rice, soup, and vegetable dishes were eaten les frequently than snack foods and beverages at both day care centers and home. To solve the problem of skipped breakfast, nutritional improvement of breakfast and snacks at the day care centers and nutritional education about regular eating behavior for children or teachers were needed. According to children's RDA of nutrition for each age group, menu planning and improved diets for children at day care centers centered around taste, portion size, and shapes of foods that are appropriate for children are needed. The followings are nutritional improvement proposals for preschool children; For good dietary habits of preschool children, development & implementation of efficient nutritional education programs and the introduction of nutritionally balanced traditional korean foods are recommended at day care center. Dietitians should be in charge of developing a well-balanced menu reflecting children's food preferences.
