한국인 역류 식도염 환자의 식도 상피 세포 사이 간격 확장과 Esomeprazole 투여 후의 변화

목적: 서구에서 역류 식도염 환자의 식도 상피는 세포 사이 간격이 확장되어 있으며, 이는 프로톤 펌프 억제제 치료 후 호전된다고 한다. 이번 연구의 목적은 우리나라 역류 식도염 환자에서 식도 상피 세포 사이 간격이 확장되어 있는지 여부와 프로톤 펌프 억제제 치료 후 세포 사이 간격이 호전되는지 알아보았다. 대상 및 방법: 역류 식도염 증상이 있으며 상부위장관내시경 검사에서 점막 손상이 있는 역류 식도염 환자 10명과 위식도역류의 증상이 없고, 상부위장관내시경 검사에서 점막 손상이 없으며 24시간 보행 식도 산도 검사에서 음성인 정상인 10명을 대상으로 하였다. 모든 대상자에서 상부위장관내시경 검사를 시행하였으며, 위식도 경계부 5㎝ 상방에서 4군데 조직을 생검하여 전자 현미경으로 세포 사이 간격을 측정하였다. 모든 식도 조직의 전자 현미경 소견에서 100 번씩 세포 사이 간격을 측정하였다. 역류 식도염 환자는 8주간 esomeprazole로 치료한 후 내시경 검사를 하고 식도 상피 조직을 생검하여 세포 사이 간격을 다시 측정하였다. 결과: 역류 식도염 환자의 세포 사이 간격(평균 1.49±0.39 ㎛)은 대조군의 식도 상피 세포 사이 간격(평균 0.49±0.10 ㎛)보다 통계적으로 유의하게 확장되었다(p<0.01). 역류 식도염 환자에서 esomeprazole로 치료 후 세포 사이 간격(평균 0.56±0.19 ㎛)은 호전되었다(p<0.01). 결론: 우리나라 역류 식도염 환자의 식도 상피 세포 사이 간격은 정상인에 비해 확장된 소견을 보이며, 프로톤 펌프 억제제 치료 후 호전되었다. Background/Aims: It has been demonstrated that dilated intercellular spaces of esophageal epithelium are a marker of tissue injury in Western gastroesophageal reflux disease (GERD); which heals after proton pump inhibitor (PPI) treatment. The aim of this study was to determine whether dilated intercellular spaces are also a feature of acid damage in Korean GERD patients and are also healed after PPI treatment. Methods: Ten GERD patients with heartburn and esophageal mucosal breaks on endoscopy, and 10 controls with no symptoms, normal mucosa on endoscopy and negative 24-hour pH monitoring were enrolled. During upper gastrointestinal endoscopy, four biopsies from normal mucosa were taken within the lower 5 ㎝ of the esophagus for transmission electron microscopy (TEM) analysis. One hundred computer measurements were taken on TEM photomicrographs of the specimens in each patient. After eight weeks of esomeprazole 40 ㎎/day, patients with erosive esophagitis had another endoscopy with biopsies and the intercellular spaces were measured again. Results: The mean intercellular space diameter, in the erosive esophagitis patients, was greater than in controls (1.49±0.10 ㎛ vs 0.49±0.10 ㎛, p<0.01). The mean intercellular space diameter, in erosive esophagitis patients, improved after PPI treatment. Conclusions: Korean erosive esophagitis patients have dilated intercellular spaces of esophageal epithelium and dilated intercellular spaces can be reversed after esomeprazole treatment. B형 간염바이러스 중합효소의 역전사효소 활성을 억제하는 인간 단세포군 항체

목적: 간경변과 간암의 주 원인 중 하나인 B형 간염바이러스(hepatitis B virus, HBV)에 대한 새로운 치료법의 개발의 일환으로 phage display 기법을 이용하여 바이러스 복제에 있어 중요한 P 단백의 역전사효소 활성을 억제하는 인간 단세포군 항체를 제작하여 protein based 유전자 치료 가능성을 분석하고자 하였다. 대상 및 방법: HBV P 단백의 역전사효소 기능 부위인 RT/POL 기능 부위를 pMAL-C 벡터에 클로닝하고 maltose binding protein과 융합 단백의 형태로 재조합 단백으로 발현시켰다. 1.1×10(10)의 인체 single chain Fv phage antibody library를 사용하여 RT/POL 재조합 단백에 대한 항체를 BIAcore로 선택하였다. 선택된 항체의 역전사효소, RNA-dependent DNA 중합효소 기능 정도를 분석하였으며 BIAcore로 항원에 대한 친화도를 그리고 염기서열 분석을 통하여 항원 결정 부위를 확인하였다. 결과: 대장균에서 발현 정제된 RT/POL 기능 부위 재조합 단백은 역전사효소 활성을 유지하고 있었으며, 1μg의 재조합 단백은 5 unit의 MMLV 역전사효소와 동일한 효소 활성을 가지고 있었다. BIAcore를 이용한 RT/POL에 대한 항체의 선택으로 POL-A5, POL-B8 그리고 POL-B12의 3개 클론을 확보하였다. 선택된 클론들의 역전사효소 활성 억제능은 각각 82- 52%였으며, 항원에 대한 친화도는 8.15×10(-8) M to 1.75×10(-6) M이었다. 결론: 이번 연구를 통하여 제작된 인간 단세포군 항체는 항원에 대한 친화도가 낮았으나 in vitro에서 효과적으로 역전사효소 활성을 억제하여 protein based gene therapy로서 세포 내 항체로 발현한다면 HBV P 단백의 기능을 억제하여 바이러스의 복제를 억제할 것이다. Background/Aims: To develop a novel treatment method for hepatitis B virus (HBV) infection, we aimed to make a human monoclonal antibody inhibiting reverse transcriptase (RT) activity of P protein which was important in HBV replication by using phage display technique. Therefore, we analysed the usability of human monoclonal antibody as a protein based gene therapy. Methods: Reverse transcriptase/polymerase (RT/POL) functional motif of P protein of HBV was cloned in pMAL-c vector and expressed as maltose binding fusion protein form. The RT/POL recombinant protein (pMRT/POL) was purified by amylose resin column. Using human single chain Fv phage antibody library with 1.1×10(10) size, human antibody against pMRT/POL was selected with BIAcore panning. Selected antibody fragments were analyzed for the activity of RT inhibition. Finally, they were analyzed for the affinity with BIAcore and the complementarity determining regions with nucleotide sequencing. Results: pMRT/POL recombinant protein expressed in E. coli showed RT activity, 1㎍ of recombinant protein had an activity equivalent to 5 unit of MMLV RT. By BIAcore panning, we could select 3 clones; POL-A5, POL-B8 and POL-B12. Each clone`s RT inhibiting activity were 52-82%, affinity against antigen were 8.15×10(-8) M to 1.75×10(-6) M. Conclusions: Human monoclonal antibodies produced in this study showed low affinity, but efficiently inhibited the activity of RT in vitro. If POL-A5, POL-B8, and POL-B12 can be converted to intracellular antibody form, it can be used for protein-based gene therapy by inhibiting the replication through the neutralization of polymerase protein of HBV. Primary non-Hodgkin`s lymphoma of the extrahepatic bile duct presenting as obstructive jaundice is extremely rare. A 60-year-old man was admitted due to suddenly developed jaundice. Computerized tomography and endoscopic retrograde cholangiopancreatography showed a tumor at the proximal common hepatic duct. These clinical and radiologic findings resembled those of Klatskin tumor. The resection of the common hepatic duct tumor, lymph node dissection, and Roux-en-Y hepaticojejunostomy were carried out. There was no regional lymph node metastasis and no residual tumor at the resection margins. Histology and immunohistochemistry of the resected specimen confirmed a diffuse large B-cell malignant lymphoma involving the common hepatic duct. The patient is scheduled to receive adjuvant chemotherapy. In summary, primary non-Hodgkin`s lymphoma of the extrahepatic bile duct, despite its rarity, should be considered in the differential diagnosis of causes for obstructive jaundice. An accurate histopathologic diagnosis and surgical resection combined with chemotherapy may be the approach to offer a chance for cure. (Korean J Gastroenterol 2009;54:191-195)

Afferent loop syndrome is an uncommon complication of a gastrectomy and Billroth Ⅱ reconstruction. It may cause symtoms at any time from the first postoperative day to many years after the gastrectomy. Afferent loop syndrome is characterized by abdominal pain, vomiting and elevation of serum amylase. Thus, it is difficult to differentiate afferent loop syndrome from other cause of acute pancreatitis. However, the history of gastrectomy can be an important clue for diagnosing afferent loop syndrome. We experienced one case of chronic afferent loop syndrome with acute pancreatitis. After appropriate management, the abdominal pain disappeared and serum amylase level decreased. We report this case with a review of relevant literatures.

Carcinoid tumors have been reported in a wide range of organs but most commonly involve the lungs, bronchi and gastrointestinal tract. Within the gastrointestinal tract, appendix is the most common location for carcinoid, followed by the distal small intestine, the rectum, and the stomach. Among these, primary involvement of the ampulla of Vater is extremely rare. We report a case of carcinoid tumor of ampulla of Vater. A 62-year-old man presented with epigastric soreness. Gastroduodenal endoscopy showed hyperemic bulging mass on ampulla of Vater and diagnosed as carcinoid tumor by histologic finding, with immunohistochemical study. In ^(111)In-octreoscan, tumor had regional lymph node metastasis, but no evidence of distant metastasis. Pancreatoduodenectomy with lymph node dissection was performed.

Radiofrequency ablation(RFA), as a form of minimally invasive therapy for hepatocellular carcinoma, has become an important treatment modality. Because of limitation of surgery, RFA has become standard therapy for hepatocellular carcinoma in some situations. But there are some complications of RFA such as bleeding, infection, hematoma, adjacent organ thermal damage including intestinal perforation, needle track seeding, and so on. There are few reports in the literature that systematically evaluate the incidence of needle tract seeding and its associated risk factors. And only 2 cases of needle tract seeding of hepatocellular carcinoma after RFA were reported in Korea. We report a case of needle tract seeding of hepatocellular carcinoma after RFA.

Fungus such as Candida albicans is a normal flora that is frequently found in the oral cavity, gastrointestinal tract, genitourinary tract, vaginal mucosa in a normal person. However, candida can cause opportunistic infection in an immune compromised host. Candidiasis has broad spectrum of disease from mucocutaneous infection to invasive or disseminated infection. But, it is rarely reported that candida is associaed with gastrointestinal tract disease in a healthy adult. The case of gastric ulcer associated with candida particularly in a health adult is reported with relevant literature.
