        Terpenoid constituents from the aerial parts of Asplenium scolopendrium

        중환자실 임상약사의 약물처방 검토 시 필요한 필수점검 항목 개발

        Background and purpose : Clinical pharmacists are core members of a multidisciplinary team in critical care, playing a pivotal role in improving patient treatment. The responsibility of pharmacists for pharmacotherapeutic outcomes in South Korea has increased over the years. However, different interventional approaches or points of view in regard to patient medication exist among pharmacists, especially new team members. The purpose of this study was to set up principles of medication interventions, to develop an evidence-based checklist to minimize the difference of intervention levels, and to standardize tasks in interventions provided by pharmacists. Methods : After a comprehensive review of the literature, guidelines and protocols, the relevant items were identified. They were then structured in the form of a checklist. The consistency of the checklist was checked by a group of pharmacists involved in critical care. Cronbach’s alpha, a measure of internal consistency, was used to validate the checklist. Pharmacists’satisfaction with the checklist was assessed using Likert scales as a questionnaire survey. Results : We established an evidence-based checklist composed of seven categories for use in pharmaceutical interventions in critical care. A total of 69 pharmacists completed a questionnaire surveying satisfaction with the checklist. Consistency between two groups of pharmacists was 99.3 percent. The reliability of the checklist was Cronbach’s alpha 0.899, which validated the checklist. There was high satisfaction (4.32 on the Likert scale) with the checklist among pharmacists. Conclusions : The checklist may provide a useful tool for clinical pharmacists to conduct a highly-specialized critical care pharmaceutical intervention with evidence-based reliability.

        Low Correlation Between Vancomycin Area Under the Curve Over 24 Hours to the MIC (AUC/MIC ratio) and the Trough Concentration at Steady State in Methicillin-Resistant Staphylococcus Aureus Pneumonia

        Background : Vancomycin is a mainstay of treatment of serious methicillin-resistant Staphylococcus aureus (MRSA) infections. The recently published guideline on vancomycin monitoring recommends that a vancomycin loading dose can be considered and an individualized target AUC/MIC ratio of 400-600 and should be advocated to achieve clinical efficacy while improving safety in patients with the suspected or definitive MRSA infections. Methods : To evaluate the correlation between the vancomycin trough concentration and the area under the curve over 24 hours to the MIC (AUC/MIC) ratio according to the guideline recently published. A retrospective cohort study was conducted on adult patients treated for MRSA proven pneumonia with the vancomycin. We enrolled patients administered with the vancomycin loading dose before the maintenance dose and the monitored vancomycin trough concentrations at the steady state in medical intensive care units April 2017-March 2020. We calculated the vancomycin AUC/MIC ratio based on the trough concentration measured for each patient using the Bayesian software. The difference between the microbiological cure, all-cause mortality, and nephrotoxicity for each group was studied. Results : There was low correlation between the vancomycin AUC/MIC and trough concentration in MRSA pneumonia. A total of 35 patients were included. Thirty-one patients (88.6%) were above the AUC/MIC ratio 400, while 19 patients (54.3%) were in the trough range above 15 mcg/ ml. Although there was a weak correlation between the vancomycin trough concentration and the AUC/MIC ratio (r=0.427, p=0.010), the vancomycin through concentrations at the steady state within the therapeutic ranges did not mean the vancomycin AUC/MIC ratio within the therapeutic ranges (p=0.580). No significant difference was observed in the microbiological cure, all-cause mortality, and incidence of acute kidney injury in each group. Conclusion : The trough level-guided monitoring of the vancomycin should be replaced by the AUC/MIC ratio-guided monitoring in the clinical practice in Korea.

        고혈당증 환자의 내당능에 미치는 Lipid - Based TPN의 유효성에 관한 후향적, 대조군 비교시험

        Hyperglycemia is one of the most important adverse effect in patients with diabetes, chemical diabetes or persistent hypermetabolic state who receiving dextrose-based TPN (D-TPN). A retrospective study was conducted to evaluate the effects of lipid-based TPN (L-TPN) versus D-TPN on the blood glucose level in adult hyperglycemic patients in Samsung Medical Center. As a study group, 31 patients were randomly selected among 53 hospitalized hyperglycemic patients from October, 2003 to August, 2004 who received L-TPN. As a control group, 88 patients were also randomly selected among 107 patients from March, 2001 to September, 2003 who received D-TPN. By reviewing medical records, the blood glucose levels before and after TPN administration were compared in both groups to evaluate the effects of L-TPN and D-TPN. Administrated insulin doses were checked in unit per day. Serum albumin levels were also checked as a nutritional assessment. Wilcoxon rank sum test was used to determine the differences. In the study group, the mean ± SD blood glucose level (mg/dl) after TPN administration was significantly lower than before administration (167.90 ± 46.79 vs. 233.95 ± 48.08, p = 0.001). The mean ± SD blood glucose levels in the control group were 262.24 ± 54.85 and 247.83 ± 89.23, before and after administration, respectively (p >0.05). The mean ± SD of before and after insulin administration doses (unit/day) in the study group were 20.10 ± 25.29 and 10.50 ± 17.71. The mean ± SD of before and after insulin administration in the control group were 17.09 ± 23.60 and 30.19 ± 24.27. Both groups showed similar nutritional improvement. Since the serum albumin levels were elevated. While there were no significant differences in blood glucose levels between before and after administration of D-TPN, even though insulin administration doses were significantly high after D-TPN, L-TPN had significant differences in blood glucose levels that came down to normal levels. Therefore, L-TPN was more effective in managing blood glucose level in hyperglycemic patients.

        다학제 중환자 팀의료에서의 중환자약료 담당약사 역할에 대한 중요도 및 수행도 평가 설문연구 : 국내 임상의 및 약사 측면

        Background : The objective of this study was to survey critical care pharmacists’ role by importance- performance analysis in multidisciplinary ICU teams. Methods : This multicenter prospective cross-sectional questionnaire study was conducted through direct mailing. Questionnaires and return envelopes were mailed to heads of hospital department of pharmacy. These questionnaires were designed to request for importance, performance of fundamental, desirable, and optimal services of pharmacy services for ICU pharmacists and satisfaction for clinicians. Data were subjected to importance-performance analysis. Results : Forty-one item pharmacy questionnaires and 25-item for clinicians were developed and distributed to ICU pharmacist and clinicians. Forty-seven (90%) pharmacists and 51 (66%) clinicians returned questionnaires. Clinicians generally rated satisfaction of pharmacy services more favorably than pharmacists in clinical function in both patient care and non-patient care. Means ± standard deviations (SDs) of importance and performance were 6.20±0.17 and 5.37±0.38, respectively, for the evaluation of clinical activities of patient-care by critical care pharmacist. In the case of pharmacists’ evaluation of non-patient care, means ± SDs of importance and performance were 4.86±0.70 and 4.78±0.21, respectively. The evaluation of importance for non-patient care tasks was significantly lower than that of patient-care tasks (p=0.001). The IPA analysis of clinician’s scores of importance and performance were 6.27±0.25 and 6.11±0.21, respectively for the evaluation of clinical activities of patient-care by critical care pharmacist. In the case of clinician’s evaluation of clinical activities of non-patient care, means ± SDs of importance and performance were 5.49±0.45 and 5.74±0.27, respectively. Conclusion : In conclusion, it was evaluated as important or appropriate performance and highly evaluated for satisfaction with the clinicians for critical care pharmacists’ role on importance- performance in ICU multidisciplinary team activity.

        심장외과 중환자실에서 임상약사의 약처방 중재 활동 분석

        Background : It has been reported that clinical pharmacists’ participation in the multidisciplinary intensive care unit (ICU) team reduces the number of medication errors and improves the clinical outcome. However, studies assessing the role of pharmacists in a cardio-surgical ICU (CSICU) are limited. The aim of this study was to analyze pharmacologic interventions and identify pharmacologic problems that require pharmacist intervention in a CSICU. Methods : The research was conducted by retrospectively reviewing the electronic medical records of patients aged 18 years or older who were admitted to the CSICU of Samsung Medical Center from April 1 2019 to June 30 2019. Results : During the study, the clinical pharmacist monitored 12,021 drug prescriptions for 205 patients and conducted 379 pharmacologic interventions. The most common pharmacologic intervention recommended by the pharmacist was nutritional support (19.0%), followed by therapeutic drug monitoring (TDM) (18.5%) and provision of drug information (14.8%). The most frequently mediated group of drugs was antibiotics (31.1%), and the causes for pharmacologic intervention of antibiotics were mostly TDM (51.7%) and inappropriate dosage (20.3%). The intervention rate of ‘pain, sedation and delirium-related drugs’ was 17.2%, while the intervention rate of ‘antiarrhythmics, anticoagulant, and antiplatelet agents’ was 13.2%. The most common reason for intervention in both groups was missed prescriptions, followed by adverse drug events and inappropriate dosages. The acceptance rate of intervention was 92.2%. Conclusion : Clinical pharmacists’ participation in the ICU has enabled active drug prescription monitoring, proper nutrition support, and TDM. The acceptance rate of intervention is high. The CSICU requires pharmacologic intervention of pain, sedation and delirium-related drugs specific to critically ill patients, as well as drugs indicated for cardio-surgery.

        블렌디드 러닝을 활용한 중학교 건강 달리기 수업의 설계와 실천: 체육수업과 방과후 신체활동의 연계를 중심으로

        본 연구는 코로나19 전·후로 지속되고 있는 학생들의 운동부족 및 건강위기에 대한 문제의식을 바탕으로 블렌디드 러닝에 기반한 건강달리기 수업을 설계·실천하고, 이를 통해 건강영역 체육수업에서 블렌디드 러닝의 적용 가능성 및 역할을 탐색해보고자 실시된 연구이다. 이를 위해 블렌디드 러닝에 기반하여 대면수업, 원격수업, 방과후 신체활동을 연계하였으며, 애플리케이션, 온라인 플랫폼 등의 교수학습매체를 접목하여 수업을 설계하였다. 설계된 수업은 서울 소재 S 중학교 1학년 학생 133명(5개 혼성학급)을 대상으로 실시되었고, 이 중 5명의 학생을 주요 연구참여자로 선정하였다. 수업의 성과와 한계를 분석하기 위해 5명의 연구참여자에게 면담을 실시하였으며 수집된 자료를 나선형 자료 분석의 절차에 따라 분석하였다. 분석결과, 첫째, 주요 교수학습매체로 활용된 Nike Running Club에서 제공되는 운동정보를 통해 학생들은 달리기 운동에 몰입할 수 있었고, 수업에서 제시된 도전적, 경쟁적 과제는 자율적인 달리기 운동 참여를 이끌어 낸 동기로 작용하였다. 둘째, 공간과 매체가 혼합되는 블렌디드 수업에서 학생들은 등교제한, 방과 후 활동 등 동일 공간에 교수자가 부재한 상황에서도 교수자와의 적극적인 의사소통과 학습관리 속에 달리기 운동에 지속적으로 참여할 수 있었고, 셋째, 운동 참여에 대한 순환적 반복을 거듭하며 달리기와 함께하는 일상의 변화를 경험하게 되었다. 마지막으로 과도한 운동과제, 신체활동의 다양성 부족, 학습관리의 사각지대 등은 향후 개선되어야 할 수업의 문제점으로 지적되었다. 상기의 연구결과를 바탕으로 교육용 운동 애플리케이션의 개발 및 세련화, 블렌디드 러닝 기반 체육수업 연구의 확장, 현장 교사에 대한 테크놀로지 활용 교육연수 강화를 제언하였다. This study was attempted to explore the applicability, achievements and limitations of blended learning for P.E class by designing and practicing a health running class based on blened learning for solving insufficient exercises and health problems of middle school students. So, this study was to design health running class based on blended learning by linking face-to-face class, online class and after-school physical activity, and using moblie applications and eduacationl online platforms. This class was implemented in the 1st grade of S middle school of 133 students(5 coeducationl classes), and five students of them participated in this study as interviewees. For analysing the achievements and limitations of this class, five participants were interviewed, and the collected data was analyzed using Creswell's spiral data analysis methodology. The results of this study were as follows. First, through the exercise information provided by Nike Running Club, students were able to immerse themselves in the running exercise, and the challenging and competitive tasks presented in the class acted as a motivator for autonomous running and running exercise participation. Second, students were countinued to participate in runing exercises communicating with the teacher in blended classes, although the teacher was in the same place. Third, the students experienced the changes in their daily life with running by repeating the cyclical repetition of running exercise. Lastly, excessive exercise tasks, lack of diversity in physical activity, and blind spots in learning management were pointed out as problems of this class to be improved in the future. Through these results, this study suggests develpoing and improving the mobile execrise application for students, extending scope of research on P.E classes based on blended leraning, strengthening teacher training for technology application.

        소프트 다트를 활용한 체육수업에서 중학생의 경험과 교육적 의미

        본 연구의 목적은 학교 현장에 적용 가능한 소프트 다트 수업을 설계하고 수업에 참여한 학생들의 경험에 대한 교육적 의미를 탐색하는 것이다. 이를 위해 본 연구에서는 구성주의 철학에 근거하여 수업을 설계하고 S중학교 2학년 학생 184명(8개 혼성학급)을 대상으로 소프트 다트 수업을 실천하였다. 수업에 참여한 학생들의 교육적 경험을 탐색하기 위해 주요 연구참여자로 8명의 학생을 선정하였으며, 개방형 설문지, 인터뷰, 관찰일지 등을 지속적 비교분석법에 따라 분석하였다. 수집된 자료의 분석결과, 첫째, 수업에 참여한 학생들은 소프트 다트를 직·간접적으로 경험하면서 스포츠에 대한 재고(再考)의 기회를 갖게 되었으며, 둘째, 신체적, 인지적, 정서적 역량이 내면으로 집중되는 수렴적 움직임을 경험하였다. 셋째, 소프트 다트의 색다른 경기방식으로 인해 섬세한 움직임과 전략적 사고의 조화를 경험하였고, 넷째, 불평등과 소외가 만연했던 체육수업에서 새로운 관계 형성의 계기를 갖게 된 것으로 나타났다. 한편 듀이의 경험교육 철학에서 주장하는 상호작용성과 계속성의 원리에 근거하여 연구결과에 대한 교육적 의미를 논의한 결과, 소프트 다트는 현재 학생들의 교육적 요구 및 관심사에 부합되면서도 발전적인 후속 경험을 지속적으로 이끌어내기에 유리한 종목적 특성을 갖추고 있어 교육적 가치가 담보된 수업 활동임을 확인하였다. The purpose of this study is to design soft darts lesson which is applicable for PE classes, and explore educational meaning of students who were participated in soft dart lesson. To achieve this purpose, soft darts lesson was designed based on constructivism. and implemented in the 2nd grade of S middle school located in Seoul of 184 students(8 coeducational classe). Data collected through opening questionnaires, observation diary and In-depth interviews(8 students as research participants) were analysed using constant comparison method. The analysis results were as follows: First, students had an occasion to reconsider the meaning of sports experiencing soft darts directly and indirectly. Second, they experienced convergent movement that physical, cognitive, and emotioncal competence were concentrated on inner side of human mind. Third, they experienced the harmony between the elarborate movement and strategic thinking by game rules of soft darts. Fourth, they had an opportunity to establish interpersonal relationship between students in PE class where the alienation and inequality is rife. In conclusion, soft darts guarantees the educational value considering the experiences of the soft darts lesson based on a principle of continuity and interaction of John Dewey’s Educative Experience.

      • 국내산 어류 3종의 콜라겐 제조를 위한 전처리 비교

        본 연구는 한국에서 소비되는 tilapia(Oreochroma niloticus), flounder(Paralichthys olivaceus), red sea bream(Pagrus major) 3가지 어피의 특성을 비교하고 이를 이용하여 고순도 고분자 콜라겐을 제조하기위해 원재료의 불순물을 제거할 수 있는 최적의 방법을 개발하고자 하였다. 3종의 어피의 특성을 비교한 결과, red sea bream의 조지방이 29.93 %으로 가장 많았고, 단백질 함량은 flounder가 90.52 %로 가장 많은 단백질을 함유하였다. 비콜라겐성 단백질을 제거하고자 0.1 N NaOH와 5 % NaCl을 1, 5, 10일간 처리하여 단백질 제거율을 확인한 결과 세 어종간 차이는 없었으며, 두 용매 간에는 특이적인 차이는 없었다. 처리 기간이 길수록 단백질 제거율은 높았다. 탈지 단계에서는 10 % butanol, 25 % isopropylalchol, 50 % ethanol을 처리하였다. 지질 제거율은 3종 모두 10 % butanol 처리시에 가장 높은 제거율을 보였다. 콜라겐의 주요 불순물은 지질과 비콜라겐성 단백질이며 지금까지 여러 콜라겐 추출연구가 있었으나 어피의 불순물을 제거하는 구체적인 방법은 확립되어 있지 않다. 본 연구의 결과는 생선 껍질의 불순물을 제거하기 위한 효율적인 용매와 기간을 선정하는데 의미가 있다.
