        Cytoplasmic Anti-Neutrophil Cytoplasmic Antibody Positive Diffuse Alveolar Hemorrhage Associated with Methimazole

        Diffuse alveolar hemorrhage (DAH) is a life-threatening condition associated with many disorders. Here, we report a case of 59-year-old female who had diffuse alveolar hemorrhage associated with methimazole. She had been treated with methimazole for two weeks due to the recurrence of Grave`s disease, before visiting the emergency room. She had to be intubated on the 3rd day of hospitalization because of unabated massive hemoptysis and rapid progression of diffuse alveolar infiltration on chest radiographs. Since her clinical condition improved substantially after cessation of methimazole and steroid pulse therapy, she was extubated on the 9th day of hospitalization and then discharged. After discharge, DAH did not recur with cessation of steroid and she had radioactive iodine therapy for her Grave`s disease. This was a rare and interesting case of life-threatening DAH associated with cytoplasmic-antineutrophil cytoplasmic antibody and methimazole. (J Rheum Dis 2017;24:236-240)

        토기의 귀 연골 세포 배양에서 여러 가지 성장 인자의 효과

        Cartilage is used frequently as a graft material in plastic & reconstructive surgery. Nowadays, many investigations such as allograft of cartilage, effects of various growth factors in cultured chondrocytes, and cartilaginous tissue replacement for bony defects are being performed in many countries. In this study, we isolated chondrocytes from ear cartilage of the New Zealand white rabbits weighing 200 to 250 g. We performed high density culture using penicylinder and observed the effect of various growth factors on cell proliferation(DNA synthesis), protein synthesis(leucine uptake), collagen synthesis(proline uptake) and matrix secretion(glycosaminoglycan synthesis). Optimal concentrations of the growth factors, were determined from dose-response studies. The transforming growth factor(TGF)-β and basic fibroblast growth factor(bFGF) administered on the 3 day of culture showed the effect to increase the DNA synthesis. TGF-βand bFGF showed additive action when administered together in high density culture and showed synergistic action in lower density culture. And bFGF showed inhibitory effect in matrix synthesis when administered alone or with TGF-β, which means bFGF has dedifferentiation effect. The chondrocytes culture technique using the rabbit ear cartilage was established in this study and administration of both TGF-βand bFGF was found to be very helpful in the chondrocyte culture.

        Uncoupling Protein 3의 골격근 세포내 과발현이 OLETF 백서 및 배양된 골격근 세포에서 포도당대사에 미치는 영향

        연구배경:Uncoupling protein(UCP)는 미토콘드리아의 내막에 위치하는 단백질로 세포내의 과다한 에너지를 열로 발산시키는 기능을 가진다. 최근 동물의 갈색지방조직에만 존재하는 UCP와 유사성을 가진 아형들(UCP2,3)이 사람에게도 존재함이 알려져 큰 관심을 끌도 있는데 이중 UCP3는 그 발현이 골격근세포와 갈색지방조직에만 국한된다. 본 연구에서는 UCP3가 체내 인슐린 감수성을 결정하는데 가장 중요한 조직인 골격근에 국한되어 발현되는 점에 착안하여 UCP3를 골격근세포에 과발현시켰을 때 포도당 대사에 어떠한 영향이 나타나는 지를 조사하였다. 방법:25주령의 8마리의 OLETF 백서를 대상으로 하여 4마리는 골격근에 adenovirus 2mL(1×10¹²pfu/mL)를 주사하여 대조군으로 하였고 4마리는 골격근에 재조합법으로 제작된 adenovirus­UCP3 2mL(1×10¹²pfu/mL)를 주사하였다(UCP3 과발현군). UCP3를 투여한 백서에서 먹이섭취가 증가하는 경향이 있어 그 전날 대조군이 먹은 야의 먹이만큼 투여하였다. 골격근에 adenovirus를 주사한 10일 후에 euglycemic hyperinsulinemic clamp를 시행하였다. Adenovirus­UCP를 C2C12 골격근 세포에 transfection시켜 UCP3를 C2C12 골격근 세포에 transfection시켜 UPS3­C2C12를 만들고 C2C12 골격근 세포와 UPS3­C2C12 골격근 세포에서 포도당 수송 및 당원합성을 측정하였다. 결과:UCP3 과발현 OLETF에서 체중이 감소하는 경향을 보였고 인슐린 감수성이 증가하였다. C2C12세포에서 기저상태 포도당 수송은 1.28±0.17μmol/L/min였고 100nM 인슐린으로 2시간 처리한 후 2.67±0.20 μmol/L/min로 증가하였다. UCP3­C2C12 세포에서는 기저상태 포도당 수송이 3.98±0.13μmol/L/min로 증가되었고 인슐린 처리 후 5.74±0.44μmol/L/min로 증가하였다. 인슐린을 처리한 UCP3­C2C12 세포에 P13K 억제제인 wortmannin을 첨가하였을 때 포도당 수송활성이 3.81±0.20μmol/L/min로 감소하였다. 기저상태 당원합성은 C2C12 세포에서 0.25±0.01μmol/L/min였고 인슐린 처리 후 0.45±0.01μmol/L/min로 증가하였다. UCP3­C2C12 세포에서는 기저상태 당원합성이 0.62±0.01μmol/L/min였고 인슐린 처리 후 1.26±454μmol/L/min로 증가하였다. UCP3­C2C12세포에 wortmannin을 첨가하였을 때 당원합성율이 0.80±0.04μmol/L/min로 감소하였다. 결론:UCP3 과발현이 OLETF 백서에서 인슐린 감수성을 증가시켰고 골격근세포에서 포도당 수송 및 당원합성을 증가시켰다. wortmannin을 첨가하였을 때 포도당 수송 및 당원합성이 감소함으로 보아 이 과정이 인슐린 신호전달체계인 P13K에 일부 의존함을 알 수 있었다. Background : UC P3 is a mitochondrial membrane protein expressed selectively in the skeletal muscle and brown adipose tissue. Since the skeletal muscle is the main organ determining insulin sensitivity in the body, it was hypothesized that UCP3 overexpression in skeletal muscle cells would improve glucose metabolism. Methods : An adenovirus-UCP3 was produced by a recombinant DNA method. OLETF rats were divided into 2 groups. Four rats were injected with the adenovirus-UCP3 (UCP3 group) and others were injected with the adenovirus(control group) in the skeletal muscle. The UCP3 group was provided with the same quantity of food as that consumed by the control group on the previous day. Insulin sensitivity was evaluated by the euglycemic hyperinsulinemic clamp method. In a separate experiment, glucose transport and glycogen synthesis we evaluated in C2C212 cells transfected with ether an adenovirus or the adenovirus-UCP3. Results : The insulin sensitivity improved significantly and the body weight decreased in the UCP3 group. The glucose transport and glycogen synthesis were higher in the UCP3-C2C12 skeletal muscle cells at the basal state. After insulin treatment, glucose transport and glycogen synthesis were also higher in the UCP3-C2C12 cells but the increments were reduced after treatment with wortmannin, a PI3K inhibitor. Conclusion : Insulin sensitivity was higher in the UCP3-overexpressed OLETF rats in the in vivo study. UCP3 transfection also increased glucose transport and glycogen synthesis in the cultured skeletal muscle cells by a PI3K dependent mechanism(J Kor Diabetes Asso 25 :460~468, 2001).

      • Poster Session : PS 0354 ; Epidemiology and Public Health : Prevalence and Associating Factors with Atypical Femoral Fractures: An Asian Single Center Based Case-Control Study

        Background: Current evidence suggests that there is an association between bisphosphonate use and atypical femoral fractures (AFFs). However, the extent of this risk remains unclear, especially in Asian population. In this study, we aimed to estimate the proportion of AFFs among total patients with femoral fractures and to compare the characteristics of patients with AFFs with that of patients with classic femoral fractures (CFFs). Methods: A total of 578 female patients with low-energy femoral fractures who had been hospitalized at an Asian single university hospital were retrospectively enrolled. Patients were classifi ed into two groups according to the site of fracture: AFF group for patients with subtrochanteric or diaphyseal femoral fractures and CFF group for patients with intertrochateric or neck fractures. We assessed the association of bisphosphonate use and AFFs with using multivariate logistic regression analysis. Results: Twenty-seven patients (4. 7%) with AFFs and 551 patients (95. 3%) with CFFs were identifi ed. Of the patients with AFFs, 11 (40. 7%) had been treated with bisphosphonates compared with 40 (7. 3%) in the CFF group. With adjusting the age, body mass index, types of injury (slip or fall), and history of rheumatoid arthritis, bisphosphonate was the only predictor for atypical fractures (OR 9. 8, CI 3. 7-26. 4). Among the patients with using bisphosphonate when they fractured (n=44), the proportionof AFFs was nearly 21% (n=9). The proportions of AFFs among femoral fractures were increased according to the duration of bisphosphonate; 15. 4% (n=6) in patients with <5 years and 60% (n=3) in patients =5 years, respectively. Conclusions: The proportion of AFFs was around 5% among the patients with femoral fractures and AFF were associated with bisphosphonate use in Asian ethnicity. Longer duration of treatment resulted in augmented risk, though any period in bisphosphonate use could cause atypical femur fracture.

      • Self-assembled, highly crystalline porous ferroelectric poly(vinylidene fluoride-co-trifluoroethylene) interlayer for Si/organic hybrid solar cells

        <P><B>Abstract</B></P> <P>Ferroelectric polymers can effectively improve the photovoltaic performance of solar cells, inducing an electric field to promote the dissociation of electron-hole pairs, with the thus generated charges collected from open pores. Since such performance enhancement requires materials with a unique porous crystalline structure, we herein present a novel route to highly crystalline and porous poly(vinylidene fluoride-co-trifluoroethylene) (P(VDF-TrFE)) thin films utilizing a <I>modified</I> breath figure method based on spin coating. The key feature of the above method is the addition of small amounts of water to the acetone/P(VDF-TrFE) solution to produce porous ferroelectric thin films which have significantly higher crystallinity values than nanostructures or films prepared by other methods. Furthermore, <I>n</I>-Si / poly(3,4-ethylene dioxy thiophene):poly(styrene sulfonate) hybrid solar cells with porous P(VDF-TrFE) interlayers are demonstrated to exhibit spontaneous polarization sufficient for increasing their open circuit voltages and fill factors. Finite-difference time-domain simulation reveals that the electric field due to the above spontaneous polarization increases the built-in electric field of the Schottky junction between <I>n</I>-Si and poly(3,4-ethylene dioxy thiophene):poly(styrenesulfonate) and reduces the reverse leakage current of the Schottky diode. Thus, the organic ferroelectric thin films with controlled porosity proposed in this study are well suited for a broad range of optoelectronic applications.</P> <P><B>Highlights</B></P> <P> <UL> <LI> High crystalline and self-assembled porous thin films are fabricated by simple and facile method. </LI> <LI> The addition of small amounts of water to the acetone/P(VDF-TrFE) solution to produce porous ferroelectric thin films. </LI> <LI> Photovoltaic parameters are enhanced due to spontaneous polarization of the P(VDF-TrFE) interlayer. </LI> <LI> FDTD simulation reveals that the spontaneous polarization increases the built-in electric field of solar cells. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>Self-assembled porous ferroelectric Poly(Vinylidene fluoride-co-trifluoroethylene) thin films are fabricated using a simple spin coating method. The power conversion efficiency of optimized device is 10% higher than the device without the ferroelectric layer.         A Study on Pressure Loss by the Material of AHU-linked Vegetation Bio-filter and its Operational Energy Efficiency

        Recently, the air quality issue came to the fore to the occupants of indoor areas with the detection of a large amount of indoor air pollutants such as formaldehyde that causes headache and atopic dermatitis. In order to address this issue, the use of indoor air purifying plants is considered positively as an ecological improvement option. However, the objective performance verification on indoor air-conditioning air volume has not been sufficient. This study aims to verify possible linkage with a building’s air conditioning equipment in order to optimize indoor air-conditioning effects by vegetation bio-filters. To this end, 4 different types of air filter material and AHU (Air Handling Unit) system were linked under air conditions of total wind volume of 400, 600, 800, 1,000 CMH and pressure loss by material was monitored objectively. Finally, material-specific power consumption for system operation was calculated to review energy efficiency. As for pressure loss by material, in terms of total wind volume of 800±1.8 CMH, Pre filter was lowest at –11.69 mmAq and LMF-based vegetation mat was highest at –219.94 mmAq. Based on this, as for material-specific power consumption, the Pre filter, which has the lowest pressure loss, was expected to have power consumption 94.7% lower than the LMF-based vegetation mat.

        류마티스관절염(Rheumatoid arthritis)은 만성질환이며 병의 완치보다는 질병을 적절히 관리하여 일상 속에서 기능을 증진시키고 삶의 질을 향상시키는 것에 목적을 둔다. 류마티스관절염의 모니터링에는 ICT(Information Communication Technology)활용이 용이하며 환자들의 사용의지도 높다. 이에 본 연구는 포커스그룹 인터뷰를 통해 핵심 기능과 UI (User Interface)고려사항을 도출하여 류마티스관절염 관리를 위한 모바일 어플리케이션을 개발하였으며, 환자들에게 어떠한 영향을 미치는지 살펴보고자 한다.
