Trans-umbilical Single Port Laparoscopic Appendectomy: Initial Experience

Purpose: Laparoscopic appendectomy is one of the most commonly performed minimally invasive surgeries worldwide. In recent years, successful attempts to reduce the number of conventionally-used three ports have been reported. Specifically, two-port techniques, hybrid approaches and single-port assisted techniques have been described. In this article, we report on the feasibility, safety and cosmesis of trans-umbilical single port laparoscopic appendectomy. Methods: The procedure was performed using a single 15-mm-diameter umbilical incision. Under general anesthesia,an Alexis O wound retractor (Applied medical resources Co.,Ltd., rancho Santa Margarita) was inserted through the umbilicus. After the appendix and meso-appendix were dissected with a Harmonic scalpel (Ethicon Endo-Surgery, Inc.,USA), the base of the appendix was ligated with two Endo-loops (SEOJONG medical Co., Ltd., Korea). The appendix was withdrawn into the wound protector and removed from the abdominal cavity. Results: This retrospective study enrolled 70 patients (36females and 34 males with a mean age of 29.0±15.0 years). The mean operative time was 49.0±13.9 min. There was no mortality. There were five complications in five patients (7.1%)and the median hospital stay was 3.5±1.4 days. Conclusion: This is a safe, minimally invasive procedure with excellent cosmetic results. Compared with other trans-umbilical appendectomy techniques, single port laparoscopic appendectomy has the advantages of feasibility without extensive endoscopic skills and an acceptable operative time. Technical refinements and accumulated experience probably will enable its wider use for more patients with acute appendicitis.
Policy support to the Small and Micro firms of Korea

It is not difficult to understand why small and medium firms receive so much attention. Because SME provides about 80 percent of private sector employment so SME performance is an important economic and social factor. Despite this, it is subject to higher risk and mobility than those at the large firms. This paper is analyzes the relationship between firm growth measured as growth in employment, sales and production and firms age, size and R&D investment. The growth and its relationship with the determinants is linked to industrial policy in Korea. Empirical results are based on an unbalanced panel data covering period 1999-2002. Results show significant relationship between growth, size and age of firm.

>Researches concerning prediction of real estate price usually use VAR, ARIMA model and suggest prediction value in terms of national scope. It seems that Korean real estate market shows localization and regionalization so that construction of new methods and variables are desirable in order to find out more realistic explanatory factors. Recent forecasting researches have shown some discrepancies between prediction values and real values, while Johansen cointegration test shows that employment activity has more greater effect on house prices of Gangnam region (Gangnam, Seocho, and Songpa Gu) than the macroeconomic variables such as CPI, production index and interest rate. So this study suggests that more realistic prediction for real estate market depends on considering suitable regional variables. >부동산가격의 예측과 관련된 기존의 연구들은 대부분 VAR, ARIMA 모형을 이용하고 있으며 전국단위의 예측치를 제시하고 있다. 그러나, 거시경제 변수를 활용한 전국단위의 기존 예측모형은 양극화국지화되어 가는 부동산시장 예측에 한계를 보이고 있다. 지역부동산 시장의 예측에 적합한 변수의 개발이 시급한 것으로 보인다. 본 연구에서는 지역부동산시장으로서 강남지역(강남구, 서초구, 송파구) 부동산시장에 주목하고 이들 지역의 주택가격 변화의 인자로서 고용활동 변수의 영향력을 검증하였다. 요한젠 공적분검정 결과 강남지역의 집값 변화는 소비자물가지수 등 전통적인 시계열분석에서 활용되는 거시변수보다는 강남지역 신규구인인원이라는 지역적 변수에 의해 더 많이 설명되었으며 이는 향후 지역 부동산가격의 변화요인으로 국지적이고 지역특수적인 변수를 고려해야 함을 시사하고 있다.

Minimizing the total number of setup changes of a machine increases the throughput and improves the stability of a production process, and as a result enhances the product quality. In this context, we consider a new product-mix problem that minimizes the total number of setup changes while producing the required quantities of a product over a given planning horizon. For this problem, we develop a mixed integer programming model. Also, we develop an efficient heuristic algorithm to find a feasible solution of good quality within reasonable time bounds. Computational results show that the developed heuristic algorithm finds a feasible solution as good as the optimal solution in most test problems.
Frequency Reassignment Problem in Code Division Multiple Access Networks

In this paper, we present a frequency reassignment problem (FRP) that arises when we add new base stations to resolve hot-spots or to expand the coverage of a code division multiple access (CDMA) network. For this problem, we develop an integer programming (IP) model along with some valid inequalities and preprocessing rules. Also, we develop an effective heuristic procedure that solves two sub-problems induced from the original problem in repetition. Computational results show that the proposed heuristic procedure finds a feasible solution of good quality within reasonable computation time. Also, the lower bound by-produced from the heuristic procedure is quite strong.
