Background: Malnutrition is a common and crucial problem because it is associated with higher mortality in chronic dialysis patients. Subjective global assessment (SGA) has been proven as a clinically adequate method for assessing nutritional status in dialysis patients. We explored the relationships between initial and transitions of SGA and all-cause mortality. Methods: A nation-wide prospective observational cohort study included adult chronic dialysis patients. We selected patients who had baseline and 12 month follow-up SGA. SGA-7 score was categorized as well-nourished (SGA 6~7) and malnourished (SGA 1~5) groups. The changes of SGA were categorized by 4 groups (well-maintained, poorly-maintained, improving, and worsening group). Results: A total of 2,600 patients were selected and mean age was 55.7±14 years old and 57.2% of patients were male. The mean follow-up period was 28.5 months. During the follow-up period, 202 patients (7.8%) were died. The mortality rate in malnourished patients was higher in both incident (5.2% vs. 13%, p<0.001) and prevalent patients (6.3% vs. 20.4%, p<0.001). The survival rate was signifi cantly higher in well-nourished patients in 12 month follow-up (p<0.001). Moreover, in terms of SGA transition, improving and well-maintained groups had better survival rate than worsening group and poor-maintained group. These fi ndings were reproducible even afteradjustment of age, gender, smoking, BMI, comorbidity, serum albumin, cholesterol and hs-CRP (HR 2.913, 95% CI, 1.856-4.573, p<0.001). Conclusions: Malnutrition assessed by SGA was an independent predictor for allcause mortality. Furthermore, the transition of SGA was also correlated with mortality in chronic dialysis patients.

Purpose: The number of children in Korea whose language environment is bilingual or multilingual has increased dramatically since the early 2000s. The purpose of this study was to explore the university students" perception of children with culturally and linguistically diverse backgrounds in Korea. Methods: For this study a questionnaire consisting of participant characteristics, perception of CLD children"s language development and CLD mothers and children"s language use was distributed to university students in national wide. One hundred students participated in the survey through Google docs and the data was analyzed. Results: The results suggested that university students thought that children with CLD have language development problem due to bilingual environments. And they showed that mother with CLD have to communicate with their children using both Korean and mother"s native language. Conclusions: The results suggested that students specializing in speech and language pathology perceived children with CLD various aspects and other various factors to be considered together in the future studies.

Ectopic pregnancy accounts for approximately 2% of all pregnancies and is the most common cause of pregnancy-related mortality in the first trimester. Initial evaluation consists of β-hCG and pelvic ultrasonography. The fallopian tube is the most common location for an ectopic pregnancy. Other types of ectopic pregnancy include cornual, ovarian, cervical, scar, and abdominal pregnancy. In very rare cases, the abdominal pregnancy may be retroperitoneal. The diagnosis is seldom established before surgery and therapy is surgical resection of the ectopic mass. A 26-year-old woman visited our emergency department with sudden massive vaginal bleeding. She had undergone curettage 3 weeks before. But the transvaginal sonogram of the cul-de-sac revealed no fluid collection. She failed variable-dose methotrexate therapy. On laparoscopic operation, we found a cystic, conception-like structure in the retroperitoneum. Histology of resected structure showed chorionic villi. We describe this case with a brief review of the literature.
표준화된 간호진단 및 문제와 NANDA-I 교차분석: 4개 상급종합병원 사례를 중심으로

Purpose: To explore the scope and method of applying standardized nursing terminologies to nursing diagnosis and problems used in nursing practice. Methods: A descriptive study was done with a retrospective analysis of the nursing records of 141,420 patients that were hospitalized in 4 tertiary hospitals. The nursing diagnosis and problems collected from the records were standardized, and the standardized nursing diagnosis and problems cross mapped with NANDA-I, confirmed in a nursing focus group. Results: 65 (67.7%) of the 96 standardized nursing diagnosis and problems were equal with NANDA-I and included in the 10 domains of NANDA-I. Among 86 nursing diagnosis and problems excluded from the cross mapping with NANDA-I, the 63 terms (73.3%) related to surgery/procedure were the most common. Conclusion: It is meaningful that multi-tertiary hospital nursing diagnosis and problems were standardized and cross mapping with standard nursing terminologies was performed. As for the method of applying standardized nursing terminologies in nursing practice, it is appropriate to use several standardized nursing terminologies complementarily.
대통령기록물을 활용한 유비쿼터스 기반의 교육서비스 모형 개발

본 연구는 국내 기록관 중 대통령 기록관에서 제공하는 교육서비스를 해외 기록관과 비교?분석하여 문제점을 발견하였다. 이를 해결하기 위해 유비쿼터스 환경의 기술을 접목하여 대통령기록물활용 교육서비스 모형을 개발했다. 유비쿼터스 기술이 필요한 이유는 기관의 협업을 유기적으로 이루기 위함이며, 이를 정보활용능력을 위해 개발된 BIG 6 모형을 변형해(BIG 8 모형) 적용하였다. 유비쿼터스 기술이 기록관의 교육서비스에 미치는 기대 효과는 교육적 측면, 콘텐츠 개발 측면, 서비스 개발 측면으로 나누어 살펴보았다. This study proposes the formation of education service models using presidential archives based on ubiquitous technology. To accomplish these, this study analyzed various education programs of presidential archives in Korea and other countries. The reason behind the use of ubiquitous technology is to allow interinstitution cooperation and create synergy effects. With this, this technology is grafted onto the modified BIG6 Model, which is developed for information literacy. This study can be useful and valuable in the aspects of education, contents development, and service development.

Adipose tissue in the loin muscle area of beef cattle as a marbling factor is directly associated with beef quality. To elucidate whether properties of proteins involved in depot specific adipose tissue were sex-dependent, we analyzed protein expression of intramuscular adipose tissue (IMAT) and omental adipose tissue (OMAT) from Hanwoo cows, steers, and bulls of Korean native beef cattle by liquid chromatography-tandem mass spectrometry (LC-MS/MS)-based proteomic analysis, quantitative polymerase chain reaction (PCR) and western blot analysis. Two different adipose depots (i.e. intramuscular and omental) were collected from cows (n = 7), steers (n = 7), or bulls (n = 7). LC-MS/MS revealed a total of 55 and 35 proteins in IMAT and OMAT, respectively. Of the 55 proteins identified, 44, 40, and 42 proteins were confirmed to be differentially expressed in IMAT of cows, steers, and bulls, respectively. In OMAT of cows, steers, and bulls, 33, 33, and 22 were confirmed to be differentially expressed, respectively. Tropomyosin (TPM) 1, TPM 2, and TPM3 were subjected to verification by quantitative PCR and western blot analysis in IMAT and OMAT of Hanwoo cows, steers, and bulls as key factors closely associated with muscle development. Both mRNA levels and protein levels of TPM1, TPM2, and TPM3 in IMAT were lower in bulls compared to in cows or steers suggesting that they were positively correlated with marbling score and quality grade. Our results may aid the regulation of marbling development and improvement of meat quality grades in beef cattle.
효소가수분해 및 한외여과 처리에 따른 대두단백 펩타이드의 품질 특성 및 항산화 활성

본 연구는 제한된 조건에서 대두 단백질의 효소가수분해로 생성된 저분자 펩타이드의 물리화학적 특성 및 항산화 활성을 분석하였다. 대두단백을 0.1% 및 0.3% protease(w/w)로 처리한 다음 각 가수분해물을 3 kDa 및 10 kDa의 한외여 과막을 이용해 여과하였다. 가수분해물의 pH 값은 효소 농도가 높을 때 유의하게 낮았으나 분자량에 따른 차이는 없었다. 염도는 모든 처리구에서 유의적인 차이를 보이지 않았으며 총 용존 고형물은 효소 농도 0.3%, 10 kDa 분획물에서 가장 높은 함량을 나타내었다. 가수분해물의 색도에는 펩타이드의 분자량보다 효소 농도가 더 큰 영향을 미치는 것으로 나타났다. DPPH 및 ABTS 라디칼 소거 활성 측정을 통해 가수분해물의 항산화 활성을 분석한 결과 10 kDa의 분자량을 나타내었으나 3 kDa 여과물에서는 효소 농도의 영향이 크게 나타났다. 전자혀 데이터의 주성분 분석 결과 효소 농도에 따라서 저분자 펩타이드의 맛의 변화를 확인하였으며 효소 농도 0.1%에서는 3 kDa 및 10 kDa 여과물의 맛 패턴이 구별되었다. 효소를 기질 대비 0.3% 첨가하여 반응하였을 때 항산화성이 뛰어난 가수분해물을 제조할 수 있을 것으로 기대되며, 대두단백을 활용한 기능성 천연조미소재 개발에 대한 기초결과로 활용이 가능할 것으로 판단된다. This study aimed to analyze the physicochemical properties and antioxidant activity of low molecular weight peptides produced by the enzymatic hydrolysis of soy protein. Soy protein isolate was treated with 0.1% and 0.3% protease (w/w), and then each hydrolyzate was ultrafiltered using 3 kDa and 10 kDa membranes. The pH value of the hydrolyzate was significantly lower at higher enzyme concentrations, but there was no difference according to the molecular weight cut-off. The salinity did not show a significant variation in any of the treatment groups, and the total dissolved solids were the highest at 0.3% in the 10 kDa sample. The enzyme concentration of hydrolyzate affects its color more significantly than the molecular weight cut-off. The DPPH and ABTS radical scavenging activities were the highest in the sample, with 0.3% enzyme concentration and 10 kDa cut-off. Peptides under 3 kDa also had a significant effect on the antioxidant activity. The principal component analysis of the electronic tongue (e-tongue) data significantly distinguished 3 kDa and 10 kDa fractions with a 0.1% enzyme concentration. Based on these results, using the 0.3% enzyme for the enzymatic hydrolysis of soy protein can be an efficient method to produce seasoning materials that have functional properties from plant protein.

The analytical method for the determination of minerals (sodium, calcium, potassium, phosphorus, magnesium, iron, copper, manganese, and zinc) in both infant and adult nutritional formulas, using inductively coupled plasma optical emission spectrometry, was validated in terms of precision, accuracy, and linearity. Certified reference material 1849a, purchased from national institute of standards & technology, was used for the infant and adult nutritional formula. All calibration curves exhibited strong correlation linearity (r 2=0.9999) within the tested ranges. The recovery values of mineral compounds ranged from 90.35 to 110.63%, and the limit of detection and of quantification was less than 0.1005 mg/kg and 0.3351 mg/kg, respectively. The inter-day precision (n=12) values ranged from 1.19 to 18.19%, and intra-day precision (n=3) values ranged from 0.09 to 4.80%. Therefore, microwave digestion and inductively coupled plasma optical emission spectrometry were useful analytical methods for 9 types of minerals in food. The analytical methods verified in this study may be used to build up a food composition database for national health promotion.

이에 본 연구에서는 비침습적인 방식으로 국소적 부위인 경혈과 Trigger Point에서 경결된 근조직과 신경조직을 자극할 수 있는 자기장 자극 시스템을 제작하였다. 시스템을 이용하여 가정용 저주파치료기기와 동일한 조건에서 상완 이두박근의 근피로 유발 후 근피로 회복을 평가하였다. 경혈과 Trigger Point의 해부학적 위치가 동일한 청영혈(HT2)를 자극위치로 선정하였다. 등척성 운동을 통하여 25명의 일반인에게 근피로를 유발시켰으며, 무자극 5명, 저주파 치료기기의 전기자극 10명, 자기장 자극 10명으로 분류하였다. 자극시간은 5분으로 하여 자극하였으며, 5일동안 EMG를 측정하여 관찰하였다. EMG를 주파수영역에서 분석해본 결과, 무자극의 근피로 회복률이 가장 저조했으며, 저주파치료기기의 전기자극보다 자기장 자극이 근피로 회복에 효과적인 것으로 확인되었다. The aim of this study was developing the non-invasive focused magnetic stimulation system(NI_FMSS) that can stimulates acupoints which are small parts and knotted muscular tissues. To evaluate pain treatment effect of the NI_FMSS, we caused muscle fatigue to 25 subjects in biceps of the arm. Then, we stimulated acupoints(trigger points) HT2 using low frequency stimulator(10 subjects) and NI_FMSS(10 subjects). The other 5 subjects had not been stimulated. We analyzed muscle fatigue recovery with median frequency, RMS and median power in frequency domain for 5 days. We checked the magnetic stimulation effect on acupoint by evaluation of muscle fatigue recovery. Therefore, we identified that the NI_FMSS was more efficient system to relieve muscle pain than electric-stimulation system.
