저자들은 56세 여성 환자의 왼쪽 쇄골 하 정맥에 시행한 이식형 약제주입기 카테터의 삽입 후 발생한 비정상 위치에 대한 증례를 보고하고자 한다. 카테터의 끝은 환자의 늘 어진 가슴, 체위, 신체의 움직임, 카테터의 유동성 등의 영향으로 천자된 정맥 내에서 견인되고 뒤쪽으로 이동하였다. 환자는 어떠한 증상도 호소하지 않았지만 비정상 위치의 카테터는 심각한 합병증을 유발할 수 있기 때문에 제거되었다. 따라서 시술자는 약제주입기 삽입 전에 환자의 신체 상태와 단순 흉부 촬영상을 확인하고 카테터의 비정상 위치가 발생할 수 있는 위험 징후를 알아차려야 하겠다.

Objective The purpose of this study was to examine the regional cerebral blood-flow (CBF) difference between boys with pure ACHD and normal controls before and after methylphenidate treatment, and also we examined the association between rCBF changes and cognitive characteristics of boys with pure ADHD before and after methylphenidate treatment. Methods The rCBF of 5 boys with pure AOHD and 5 control boys were examined by Tc-99m ECD brain SFECT and cognitive characteristics by ADS and every 5 ADHD boys took MPH for 12 to 16 weeks After that all ADHD boys took Tc-99m ECD brain SPECT and checked ADS again rCBF difference and changes before and after MPH treatment were quantified and analyzed by using SPM And associations between types of cognitive changes measured by ADS and rCBF changes were examined and quantitative associations between them were also analyzed by using Pearson's coefficient correlation. Results 1) Before MPH treatment, rCBF of ADHD boys were significantly decreased in left orbitofrontal area and left caudate nucleus compared to normal controls 2) After MPH treatment. rCBF of the same regions were significantly increased. 3) There was positive correlation between the degree of commission error(impulsivity disinhibition) among the ADS variables and the degree of blood-flow decrease in above two areas before MPH treatment but after MPH treatment, only with the degree of rC3F increase in Lt orbitofrontal area. Conclusion : These results suggest that the pathophysiology of impulsivity of disinhibition of the characteristic symploms of AOHD boys IS associated with hypoactivties of left orbibfrontal area and left caudate nucleus, and MPH pharmacodynamics in ADHD boys is associated to their actions on these two areas.

2001년 4월부터 2002년 9월까지 소양호의 어류군집 동태를 조사한 결과는 다음과 같다. 조사된 어류는 총 11과 38종이었다. 한국 고유종은 P. tenuicorpus, C. splendidus, S. gracilis majimae, S. japonicus coreanus, H. mylodon, G. brevibarba, M. yaluensis, M. longidorsalis, C. koreensis, C. rotundieaudata, S. microdorsalis, P. koreanus, L. andersoni, C. herzi, O. patycephala 등 15종(40.54%)이었다. 우점종은 Z. platypus(39.60%), H. olidus(39.47%), S. gracilis majimae(12.24%), S. japonics coreanus(2.11%), P. fulvidraco(1.48%), L. macrochirus(1.40%) 등이었다. 채집된 어종들의 생체량을 보면 Z. platypus가 19.48㎏, H. olidus로 8.75㎏, C.cuvieri 7.42㎏, L. maeroehirus 7.39㎏, H. labeo 6.85㎏의 순으로 나타났다. 소양호에서 확인된 도입종은 A. japonica, H. eigenmanni, H. olidus, P. altivelis, C. urotaenius, R. giurinus, T. brevispinis, C. carpio(Israeli type), c. euvieri, Onchorhynchus mykiss, L. macrochirus, M. salmoides, I. puntatus 등 13종이었다. 이 중 C. carpio(Israeli type), C. cuvieri, Onchorhynchus mykiss, L macrochirus, M. salmoides, I. puntatus는 국외종이며, A. japonica, H. eigenmanni, H. olidus, P. altivelis, C. urotaenius, R. giurinus, T. brevispinis 등은 국내 다른 하천ㅇ로부터 유입된 종이다. The dynamics of the fish community in Lake Soyang, Korea, was investigated from April 2001 to September 2002. During the surveyed period 38 species belonging 11 families were collected. There were 15 Korean endemic species 39.47%, including Pseudopungtungia tenuicorpus, Coreoleuciscus splendidus, Squalidus gracilis majimae, S. japonicus coreanus, Hemibarbus mylodon, Gobiobotia brevibarba, Microphysogobio yaluensis, Microphysogobio longidorsalis, Iksookimia koreensis, Iksookimia rotundicaudata, Silurus microdorsalis, Pseudobagrus koreanus, Liobagrus andersoni, Coreoperca herzi, and Odontobutis platycephala. Dominant species were Zacco platypus (39.60%), Hypomesus olidus (31.70%), Squalidus gracilis majimae (12.24%), Squalidus japonicus coreanus (2.11%), Pseudobagrus fulvidraco (1.48%) and Lepomis macrochirus (1.40%). The biomass of collected fishes were Zacco platypus (19.48 ㎏), Hypomes olidus (8.75 ㎏), Carassilus cuvieri (7.42 ㎏), Lepomis macrochirus (7.39 ㎏) and Hemibarbus labeo (6.85 ㎏). Of the 13 introduced fishes in Lake Soyang Cyprinus carpio (israeli type), Carassius cuvieri, Onchorhynchus mykiss, Lepomis macrochirus, Micropterus salmoides and Ictalurus puntatus were originated from foreign countries but Anguilla japonica, Hemiculter eigenmanni, Hypomesus olidus, Plecoglossus altivelis, Chaenogobius urotaenius, Rhinogobius giurinus. Thridentiger brevispinis were introduced from other native river systems.
지도부딘 투여력이 없는 Human Immunodeficiency Virus Type 1 감염환자에서 지도부딘 내성 발현율

Backgroud : Drug resistant virus has been seen with all effective antiretroviral drugs in current practice. The isolation of resistant virus from subjects is associated with more rapid clinical outcome. The presence of resistance-conferring mutations in a patient in whom therapy is foiling indicates that other antiretroviral drugs should be considered. So, the identification of drug resistant Human Immunodeficiency Virus (HIV-1) mutations is of increasing importance. Zidovudine monotherapy for HIV-1 infection has been widely used in Korea since 1990. So, the prevalence of zidovudine resistant virus has been expected to be high. The purpose of the present study is to determine the prevalence of zidovudine resistant HIV-1 among zidovudine-naive Korean patients. Methods : We tested drug susceptibility of HIV-1 isolated from 19 zidovudine-naive Korean patients. We amplified a portion of gene reverse transcriptase of HIV-1 by RT nested PCR method. The susceptibility of HIV-1 to zidovudine was determined by sequencing the DNA base pairs of gene reverse transciptase. Results : Two (10.5%) of 19 HIV-1 isolates from zidovudine naive patients showed resistance conferring mutations to zidovudine. Two (2.1%) of 95 codons analysed from zidovudine naive patients showed mutations conferring resistance to zidovudine. Conclusion : The prevalence of zidovudine resistant HIV-1 was 10.5% among zidovudine-naive Korean patients. (Korean J Infect Dis 33:40~45, 2001)
Squalene Adjuvant를 이용한 단백질 항원의 세포성 면역반응 유도

To investigate the role of adjuvant for cytotoxic T lymphocytes (CTL) induction, squalene-based adjuvant was studied its capability eliciting to the induction of cellular immunity as well as humoral immunity to exogeneous proteins. Ovalbumin (OVA) was used as model proteins. It was demonstrated that antigen formulation consisted of metabolizable oil squalene mixed with Tween 80 and pluronic L121 (S/TJ121) could induced the antigen specific CTL responses and antibodies irrespective of immunization routes in mice. We also demonstrated that this antigen formulation was a inducer of CD8+, major histocompatibility complex(MHC) class I-restricted, and antigen-specific C;1'Ls. These data suggest that the squalene-based emulsion system is a potent adjuvant inducing cellular immunity as well as humoral immunity.
Mycobacterium mageritense에 의한 당뇨병성 족부 감염증 1예

Diabetic foot infection is one of the important complications in patients with advanced diabetes mellitus. Limb threatening infections such as osteomyelitis, abscess, and necrotizing fasciitis are frequently accompanied by the disease. Non-tuberculous mycobacterium (NTM) is a rare causative organism of diabetic foot infection. Thus, if one is not suspicious or meticulous, infection due to NTM will be easily overlooked and this will result in delayed diagnose and treat. Therefore, it is necessary to consider NTM as the causative organism if the wound does not respond to the conventional antibiotic treatment and the culture from the adequately obtained specimen reveals atypical acid-fast bacilli. We present a case of diabetic foot infection with osteomyelitis and abscess due to Mycobacterium mageritense, one of the rapid growing mycobacteria, that was successfully treated with surgical debridement and appropriate antibiotic treatment.
