      • Rat의 복강내에 투여한 5-fluorouracil-polyglycolic acid 제형이 장기에 미치는 영향에 관한 연구

        For more effective intraperitoneal chemotherapy, the authers made 5-fluorouracil-polyglycolic acid composite disks(FU-PGA disks) with 5-fluorouracil and polyglycolic acid, a biodegradable polymer. We inserted the FU-PGA disk into the peritoneal cavites of Sprague-Dawley male rats, the control and three experimental groups; one FU-PGA disk insert group(300mg/kg), two FU-PGA disk insert group(600mg/kg), and three FU-PGA disk insert group(900mg/kg). The control group received a similar number of PGA disk inserts. A pharmacokinetic study was performed to measure the 5-fluorouracil concentration in the peritoneal fluid, blood, and tissues(liver, kidney and heart) at 24 hours, 72hours and 168 hours after insertion of the FU-PGA disk. Light and electron microscopic studies were done. The results were as follows: 1) The number of white blood cells and platelets decreased after FU-PGA insertion. The degree was proportional to the duration and amounts inserted. The change in the number of red blood cells varied slightly. 2) Light microscopically, slight changes were noted at 168 hours in the 3 disk insert group (900mg/kg). Mild fatty change and hepatocyte degeneration around the central veins of the liver were noted, with vacuolar degeneration in the proximal convoluted tubules of the kidney. 3) Electron microscopically,the liver showed focal increases of lipid droplets in the hepatocytes, and irregular nuclear membrane with focal nucleolar segregation of the fibrillar and granular elements. Also the double membranous structure of the hepatocytes and Kupffer cells was widened with bleb formation. The kidney showed wide separation of the nuclear double membrane in the proximal convoluted tubular cells and mesangial cells, with vacuolation and myelin figure formation in the mitochondria of the proximal convoluted tubular cells, at 72 hours and 168 hours in the 2 to 3 FU-PGA insert groups. The heart showed focal loss of cristae,vacuolation and myelin figure formation in the mitochondria of the 2 to 3 FU-PGA insert groups. In conclusion, despite the large amont of FU-PGA inserted, the histopathological changes in the liver, kidney and heart were slight, and consonant with the very low amount of 5-fluorouracil concentrations detected in the liver, kidney and heart. The above results suggest that the FU-PGA composite can serve as a new device for releasing drugs in a controlled manner and easily targeted to intraperitoneal organs. This device can improve the efficacy of intraperitoneal chemotherapy for gastric cancer.

        Interferon-γ Inhibits in vitro Mobilization of Eosinophils by Interleukin-5

        <P><I>Background:</I> Th2 cytokines play pivotal roles in allergic inflammation, including eosinophilia, and their actions are antagonized by Th1 cytokines, conferring them therapeutic potential. <I>Methods:</I> In this study, we examined the ability of a number of cytokines to suppress the activation of eosinophils that function as effector cells for allergic airway diseases. <I>Results:</I> Interleukin (IL)-5, IL-6, and tumor necrosis factor (TNF) induced an eosinophil shape change, whereas interferon (IFN)-γ significantly inhibited the shape change. Other cytokines, including IL-1β, IL-4, IL-10 and IL-13, had little or only slightly enhancing or reducing effects on the shape change. We further analyzed the IFN-γ effect, showing that pretreatment with IFN-γ strongly suppressed IL-5-induced eosinophil shape change, and cycloheximide (CHX) abrogated the suppression by IFN-γ, suggesting that new protein synthesis is required for the inhibitory effect by this cytokine. In agreement with these results, IFN-γ blocked the eosinophil migration and ERK phophorylation induced by IL-5, and the addition of CHX restored eosinophil chemotaxis. <I>Conclusions:</I> Collectively, IFN-γ may attenuate eosinophilic inflammation by directly negating eosinophil mobilization.</P><P>Copyright © 2005 S.         만성 췌장염에 의한 십이지장 협착 1예

        저자들은 만성췌장염환자에서 지속적인 오심과 구토를 주소로 내원하여 십이지장협착으로 진단된 1예를 경험하였기에 보고하는 바이다. Duodenal stenosis resulting from fibroinflammatory process is a rare complication of chronic pancreatitis. The mechanism of duodenal stenosis seems to be related to ischemia and edema caused by persistent inflammation, which results in irreversible change of duodenal wall structure. We experienced a chronic alcoholic male patient with duodenal stenosis who had a history of choledochojejunostomy to relieve distal bile duct stenosis several years ago. He had suffered persistent postprandial nausea and vomiting for several months. Barium study of upper gastrointestinal tract and repeated endoscopy show narrowing lumen of duodenum with coarse nodular surface due to reactive inflammatory change.

        유년기 Ⅲ급 부정교합자에서 이모장치의 치료 효과에 관한 연구

        본 연구는 유년기의 하악 전돌성 III급 부정교합자에게 이모장치를 착용하여 치료하였을 때 나타나는 변화를, 보다 적절한 대조군을 이용하여 알아보고자 시행하였다. 이모장치를 이용하여 치료한 하악전돌의 골격성 III급 부정교합자중에서 치료 시작 시기가 약 7세경인 29명(남자 14명, 여자 15명)을 실험군으로 하였고, 비슷한 연령으로 골격적 상태가 유사하며 어떤 형태의 교정치료도 받지 않은 III급 부정교합자 21명(남자10명, 여자11명)을 대조군으로 하였다. 2년의 관찰 기간을 거쳐, 초기 및 2년 후의 두부방사선사진을 계측 분석하여 그 변화량을 비교한 결과 (t-test, p<0.05) 다음과 같은 결론을 얻었다. 1. 이모장치를 착용하지 않은 대조군에서는 악골의 부조화가 개선되지 않고, 심화되는 방향으로 성장하였다. 즉, 치료하지 않은 하악 전돌성 III급 부정교합자는 성장에 의해 개선이 일어나지 않음을 나타내었다. 이모장치를 착용한 실험군에서는 전후방 골격 부조화가 감소하였으며, 하악 전돌감이 개선되었다. 2. 두개저와 상악골의 성장은 이모장치에 의해 유의한 변화를 보이지 않았다. 3. 이모장치의 하악골 성장억제 효과를 인정할 수 없었으나, 하악골의 성장 방향의 변화 및 성장 형태의 변화가 관찰되었다. 4. 이모장치 착용시 수직적 성장 경향이 증가하였다. 5. 연구 성적을 종합적으로 검토한 결과, 이모장치의 주된 치료 기전은 하악골의 후하방 회전과 변위이며, 성장 방향과 형태의 변화가 부분적으로 기여한다고 볼 수 있었다. 위의 결과를 종합해 보면 이모장치의 치료효과는 하악 골격 성장의 억제효과라기 보다는 하악골의 후방 회전과 변위에 의한 것이며 성장기 하악 전돌성 III급 부정교합자에서 골격 부조화의 개선 및 전돌감 해소에 유용한 장치라고 생각된다. The purpose of this study was to investigate the changes in the craniofacial skeleton subsequent to chincap therapy in the juvenile skeletal Class III malocclusion with more appropriate control samples. The experimental group consisted of 29 Korean children(14 males, 15 females) who had skeletal Class III malocclusion with prognathic mandible and were undergone chincap therapy from the beginning of treatment. The control group was composed of 21 Korean children(10 males,11 females) who had no orthodontic treatment, but with similar skeletal discrepancies to experimental group. Lateral cephalometric radiographs at the age of 7, and 2 years later were analyized and compared with student`s t-test(p<0.05). The results of this study were as follows; 1. The control group without chincap therapy had not shown any improvement of the skeletal discrepancies, but had grown to be much severe. This means that the untreated Class III patient with prognathic mandible would not be corrected by growth. For the experimental group with chincap therapy, the anterior-posterior skeletal discrepanices and mandibular prognathism were both improved. 2. Neither significant restraint nor acceleration of growth was found in the cranial base and maxilla by chincap treatment. 3. The inhibition of mandibular growth could not be accepted, but the changes of the direction of growth and morphological changes were found. 4. Vertical growth tendency was increased with chincap therapy. 5. When putting together the results of the analyses, it seems to be the rotation and displacement of the mandible that the major treatment effects of chincap are. The changes of the direction of growth and the morphological changes also seems to contibute to the treatment effect partly. In summary, the chincap doesn`t restrain the mandibular growth. But, it is considered as a useful treatment modality for correction of skeletal discrepancies and reduction of mandibular prognathism in growing Class III patients with madibular prognathism.

        Background/Aims: Westernization in many Asian countries have changed lifestyles and diets, so once rare diseases have now become prevalent. The aim of this study is to investigate the changing pattern of digestive and liver disease in Korea, from 1990 to 2006. Methods: We extracted data specific gastrointestina (GI) disease based on the International Classification of Diseases code from the in-patients records at the Kangnam St. Mary`s Hospital in 1990, 1996 and 2006. This hospital is a tertiary-care hospital in Seoul, Korea, which has a capacity of 800 beds. Results: The admission rate for GI disease increased between 1990 and 2006. Overall in-patients were 1,623 persons in 1990, 2,368 persons in 1996 and 4,166 persons in 2006. The mean age of in-patients increased as time went by. A stomach cancer was the most common diagnosis during all periods, but its prevalence has decreased. Colon cancer ranked the 7th in 1990, but markedly increased and now ranks the second. The third was a hepatocellular carcinoma. The bile duct and gallbladder cancer, pancreas and esophageal cancer ranking followed with little interval change. In cases of cancer patients, a regular admission dramatically increased for chemotherapy. The number of patients admitted with pre-malignant neoplasm of stomach and colon increased remarkably with the development of endoscopy. The liver transplant, inflammatory bowel discase, and reflux esophagitis emerged form mid-1990s with greater frequencies, yet much below the levels found in the West. The admission rate for peptic ulcer, especially ulcer bleeding remained relatively stable, despite a decreased rate for ulcer perforation. Liver cirrhosis, hepatitis, cholelithiasis with cholecystitis-cholangitis, appendicitis, hemorrhoid and gastritis all decreased. Conclusions: The stomach cancer is the leading cause of admission, despite a recent decline. Colon cancer showed a marked rise. The liver transplant, inflammatory bowel disease and reflux esophagitis were new diagnosis with an increased tendency.

        To evaluate the in-vivo effects of mitomycin-C-polyglycolic acid composite(MMC-PGA) the disk: shaped composite were implanted into the peritoneal cavity of the Sprague-Dawley rats. The pathologic changes were examined at various time points up to 12 weeks. Initially the soft tissue around the inserted disks showed capillary congestion at 3 days. Inflammatory infiltrates with foreign body giant cells appeared from the 2nd week and reached peak response at 6-8 weeks. These reaction diminished prominently at 12 weeks. No specific pathologic change was found in the liver, the kidneys, and the heart. The above results suggest that the MMC-PGA composite can serve as a new device for intraperitoneal chemotherapy of various types of cancers.

      • cis-Diamminedichloroplatinum (Ⅱ)이 Hamster 근위곡요세관 상피세포의 미세구조에 미치는 영향

        cis-Diamminedichloroplatinum (II) (cis-DDP), a metallic compound, similar to bifunctional alkylating agent and has been known as an effective anticancer drug. Cis-DDP is known to inhibit cell division and is so effective to suppress the solid tumors that it has been used in the therapy of acute ovarian tumor, acute epithelial tumor in the neck, bladder cancer and thyroid cancer. cis-DDP reacts with guanine bases of DNA in the cell that it inhibits DNA synthesis, cell division and protein synthesis consequently. It exhibits no specificity with regard to type of cells that it acts on the normal cells as well as cancer cells during therapy. So the author undertook the present study to pursue the effect of cis-DDP on the cytoplasmic organelles of epithelial cell in kidney. Hamster was used as an experimental animals. The experimental animals were killed at 12, 24 and 48 hours after administrations of 6 mg/kg of cis-DDP. The specimen obtained from the renal cortex were fixed and stained with uranyl acetate and lead nitrae. And these preprations were observed with the JEM 100B electron microscope. The results obtained were as follows. 1. The cisternae of rough endoplsmic reticulum were sacculated and membrane bound ribosomes were detached at the epithelium of proximal convoluted tubles in the 24 and 48 hours cis-DDP treated hamster group. 2. Mitochondria were swollen and cristae of mitochondria were not seen at the epithelian cells of proximal convoluted tubules in cis-DDP 48 hours treated hamster group. 3. Vesicles and vacuoles increased in number with time at the epithelium of proximal convoluted tubules in cis-DDP treated hamster groups. 4. Microvilli were segmented at the epithelium of proximal convoluted tubules in cis-DDP 48 hours treated hamster group. Consequently it is suggested that cis-DDP induces the damage in the epithelial cells of proximal convoluted renal tubule.

        Trilateration survey with electro magnetic distance measuring instrument is now widely used as a method of determining horizontal positions. Since the degree of freedom in surveying is equal to the number of redundant observation equations, larger degree of freedom may be preferred for more accurate network design. The present study shows the optimum degrees of freedom for the reliablity-based network design.

        Waterfront area provides us with the importent elements for our urban life. Especially, this area gives us not only the functions of transportation, habour, fishery but also the elements of amenity, relaxation and recreation for reproduction. The purpose of this study is to analyze the landscape of waterfront area. we have assorted and classificated the characteristics of landscape in the waterfront area from psychological, physical and topographical points of view, and analyzed types of esch charateristic groups. And finally we have estimated suitability on the height and distance of building group in the this area using the method by unspecialist group.

      • 數値地形模型을 利用한 土工量計算에 관한 硏究

        The us of digital terrain model has been enlarged in calculating the earthwork due to the development of aerial photogrammetry. The calculation of earthwork plays a major role in plan or design of many civil engineering projects, and thus it has become very important to advance the accuracy of earthwork calculation. In this study, I have derived to algorithm and a theoretical study and practical applications to earthwork calculation and practical application show the following conclusions. Practical tests show that derived equation can give higher accuracy than the method (Simpson 1,2 and Trapezoid) for the same amount of field work, and nonlinear sport formula should provided a better accuracy 2-3% than the linear spot formula currently in use.
