        가시오갈피의 생리·생태적 특성 (Ⅰ) : 산지별 순광합성, 기공증산, 기공전도도 Net Photosynthetic Rates, Stomatal Transpirations, Stomatal Conductances of Leaves

        이 연구는 가시오갈피 엽의 생리적 특성을 밝히고자 광합성, 기공증산, 기공전도도의 생리반응을 측정한 것으로 결과를 요약하면 다음과 같다. 1. 광보상점은 양엽이 약 30 μ mol m^(-2) s^(-1), 음엽이 약 15 μ mol m^(-2) s^(-1) 이었다. 2. 광포화점은 양엽이 약 1,000 μ mol m^(-2) s^(-1), 음엽이 약 300 μ mol m^(-2) s^(-1) 이었다. 3. 우리나라 및 중국산 가시오갈피 상엽의 순광합성속도는 약 8.0-8.8 μ mol m^(-2) s^(-1) 로 산지간에 유의차가 인정되지 않았다. 제주산 섬가시오갈피 상엽은 약 6.9 μ mol m^(-2) s^(-1) 로 다소 낮았다. 4. 모든 산지에서 중위엽의 순광합성속도는 상엽의 40-65%, 하위엽은 상엽의 30% 정도이었다. 그러나 섬가시오갈피 하엽은 상엽의 약 71%로 다소 높았다. 5. 상엽의 기공증산속도는 1.1-1.4 mmol H₂O m^(-2) s^(-1), 중위엽은 0.7-1.0 mmol H₂O m^(-2) s^(-1), 하위엽은 0.5-0.6 mmol H₂O m^(-2) s^(-1)로 산지별로 유의차가 없었다. 6. 기공전도도는 상엽이 약 70-90 mmol H₂O m^(-2) s^(-1)로 산지별 유의차가 인정되지 않았다. 그러나 제주 섬가시오갈피 상엽은 약 380 mmol H₂O m^(-2) s^(-1)로 타산지보다 현저히 높았다. This study was made on the physiological reactions of photosynthesis, stomatal transpiration, stomatal conductance of Acanthopanax senticosus of leaves. The resuits obtained are as follows : 1. The light compensation points were approximately 30μ㏖ m^(-2)s^(-1) in sun leaves and 15μ㏖ m^(-2)s^(-1) in shade leaves. 2. The light, saturation points were approximately 1,000μ㏖ m^(-2)s^(-1) in sun leaves and 300μ㏖ m^(-2)s^(-1) in shade leaves. 3. There was no significant between various mountains distributed, net photosynthetic rates were approximately 8.0 to 8.8μ㏖ m^(-2)s^(-1) in upper leaves. However, net photosynthetic rate in upper leaves of Acanthopanax koreanum in Jeju island was slightly low about 6.9μ㏖ m^(-2)s^(-1). 4. Net photosynthetic rate in middle leaves of all mountains was ranged from 40% to 65% of upper leaves, and that in lower leaves was approximately 30% of upper leaves. But, the net photosynthesis in lower leaves of Acanthopanax koreanum in Jeju island was slightly high about 71% of upper leaves. 5. There was no significant between various mountains distributed, stomatal transpirations were ranged from 1.1 to 1.4 mmol H₂Om^(-2)s^(-1) in upper leaves, 0.7 to 1.0 mmol H₂O m^(-2)s^(-1) in middle leaves, and 0.5 to 0.6 mmol H₂0m^(-2)s^(-1) in lower leaves, respectively. 6. There was no significant between various mountains distributed, the stomatal conductance in upper leaves was ranged from 70 to 90 mmol H₂O m^(-2)s^(-1) . However, stomatal conductance in upper leaves of Acanthopanax koreanum in Jeju island was approximately 380 mmol H₂O m^(-2)s^(-1), its remarkedly higher than any other mountain.

        사례보고 : 라미부딘 내성을 보인 HBe 항원 음성 만성 B형간염 환자에서 페그인터페론 알파 치료 1예

        라미부딘은 부작용이 거의 없으며 비교적 저가라는 장점이 있어 만성 B형간염에 대한 항바이러스요법제로서 선호되지만, 장기간 투여할 경우 이 약물에 내성을 가진 변이종이 흔히 출현하여 이에 대한 적절한 치료법의 개발이 절실한 실정이다. 저자들은 이러한 변이종 치료의 한 방법으로 라미부딘 내성이 발생한 만성 B형간염 환자에서 PEG-IFN α-2a를 주 1회 12개월간 사용하여 특별한 부작용 없이 생화학적 간기능 호전 및 바이러스 억제효과가 관찰되고 치료 이후 24주 이상의 경과관찰에서 지속적인 치료반응을 보인 1예를 경험하였기에 보고한다. 라미부딘 내성을 가진 만성 B형간염 환자의 치료에 있어서 새로 개발된 경구용 nucleoside/nucleotide analogue와 함께 PEG-IFN도 변이종에 대한 대체치료의 한 방법으로 고려될 수 있을 것이다. The wide use of lamivudine in chronic hepatitis B has produced a monotonic increase in patients with lamivudine resistance. Therefore, treating lamivudine resistance in chronic hepatitis B is a major concern in clinical practice for the treatment of hepatitis B virus (HBV). There is conflicting evidence on the outcome of pegylated interferon alpha (PEG-IFN α) therapy against lamivudine-resistant HBV, which is due to mutations in the YMDD motif. We experienced a patient with chronic hepatitis B who was successfully treated with PEG-IFN α-2a after the development of virologic and biochemical breakthrough during lamivudine therapy. Virologic breakthrough was associated with the emergence of YMDD mutants 48 months after starting lamivudine therapy. Treatment with PEG-IFN α-2a for 12 months resulted in an undetectable serum level of HBV DNA and the resolution of hepatitis, and the virologic response was maintained over 16 months after cessation of PEG-IFN α-2a.         Cardiac Computed Tomographic Angiography for Detection of Cardiac Sources of Embolism in Stroke Patients

        <P>BACKGROUND AND PURPOSE: We assessed the diagnostic performance of 2-phase 64-slice cardiac computed tomographic angiography (CCTA) for the detection of a cardiac source of embolism in stroke patients using transesophageal echocardiography (TEE) as the reference standard. METHODS: We selected 137 patients who had experienced a recent episode of stroke and had undergone both 2-phase 64-slice CCTA and TEE within a period of 5 days. A potential cardiac source of embolism detected at both CCTA and TEE was recorded, and echocardiographic findings were categorized into high- and medium-risk sources based on the TOAST (Trial of Org 10172 in Acute Stroke Treatment) classification. RESULTS: Of 137 patients, 100 abnormal findings in 91 patients were found on TEE, and 46 patients had no abnormal finding on TEE. The overall sensitivity, specificity, positive predictive value, and negative predictive value of the 64-slice CCTA for detecting cardiac sources of embolism were 89% (95% CI, 82%, 95%), 100% (95% CI, 90%, 100%), 100% (95% CI, 95%, 100%), and 81% (95% CI, 70%, 92%), respectively. TEE detected a total of 47 high-risk sources of embolism, whereas CT detected 44 lesions. For medium-risk sources of cardiac embolic stroke, TEE detected a total of 53 abnormal findings, whereas CT detected 44 abnormal findings. Of 53 lesions, there were 8 false-negative results on CT (5 patent foramen ovale and 3 atrial septal aneurysm). CONCLUSIONS: Two-phase 64-slice CCTA is a noninvasive and useful modality for detecting high-risk cardiac sources of embolism in stroke patients.</P>

        The development of renal cell carcinoma in auto-sornal dominant polycystic kidney disease hemodialyzed was rarely found. A sixty-six year old man has been hemodialyzed for four years. His end-stage renal failure was caused by autosomal dominant polycystic kidney disease. He presented the sudden onset of gross hematuria which does not cause any pain. Radi-ological examination showed the possibility of renal cell carcinoma in autosomal dominant polycystic kidney disease. The renal cell carcinoma was confirmed through nephrectomy and pathological examination. In this article, we present a case of renal cell carcinoma in autosomal dominant polycystic kidney disease hemodialyzed.

        Left atrial appendage thrombi in stroke patients: detection with two-phase cardiac CT angiography versus transesophageal echocardiography.

        <P>PURPOSE: To assess the diagnostic accuracy of two-phase 64-section cardiac computed tomographic (CT) angiography for detection of left atrial appendage (LAA) thrombi and differentiation between thrombus and circulatory stasis in patients with stroke, with transesophageal echocardiography (TEE) as the reference standard. MATERIALS AND METHODS: This study was institutional review board approved, and all patients gave written informed consent. Fifty-five consecutive patients (36 men, 19 women; mean age, 61 years) who had recently experienced a stroke, had high-risk factors for thrombus formation, and underwent both two-phase 64-section cardiac CT angiography and TEE up to 5 days apart were examined. Agreement between CT and TEE for detection of thrombus was assessed with kappa statistics. For quantitative analysis, the LAA-ascending aorta attenuation ratio (LAA/AA, in Hounsfield units) was measured on early- and late-phase CT images. The significance of differences in CT attenuation measurements were assessed by using the Student t test. RESULTS: A total of 14 thrombi were detected in the 55 patients at TEE. With TEE as the reference standard, the overall sensitivity, specificity, and positive and negative predictive values of cardiac CT angiography for the detection of thrombus in the LAA were 100% (14 of 14 patients), 98% (40 of 41 patients), 93% (14 of 15 patients), and 100% (40 of 40 patients), respectively. Concordance between cardiac CT angiography and TEE for the detection of thrombus in the LAA was high (overall kappa = 0.953). Mean LAA/AA values were significantly different between thrombus (0.29 HU +/- 0.12 [standard deviation]) and circulatory stasis (0.85 HU +/- 0.12) on late-phase CT images (P < .001). CONCLUSION: Two-phase 64-section cardiac CT angiography is a noninvasive sensitive modality for detecting LAA thrombi and differentiating thrombus from circulatory stasis in stroke patients.</P>

        Sex Differences in Hippocampal Neuronal Sensitization by Nicotine in M. gerbils

        We studied the sex different nicotine effect on evoked population spike amplitudes (ePSA) and connexin (Cx) expression in the hippocampus CA1 area of gerbils. Acute doses of nicotine bitartrate (0.5 mg/kg: NT-0.5) slightly reduced ePSA in males but markedly augmented that in females. Acute NT (5.0 mg/kg) markedly increased the ePSA in all gerbils. Unlike acute NT-0.5, repeated NT-0.5 injection (twice a day for 7 days) significantly increased the ePSA in males and slightly affected the NT-0.5 effect in females. The Cx36 and Cx43 expression levels as well as Cx expressing neuronal populations were significantly increased by repeated NT-0.5 in in both male and female gerbils, and particularly, Cx43 expression was somewhat prominent in females. These results demonstrated a sex difference with respect to the nicotine effect on hippocampal bisynaptic excitability, irrelevant to connexin expression.

        Combined Orbital Fractures: Surgical Strategy of Sequential Repair

        Background Reconstruction of combined orbital floor and medial wall fractures with a comminuted inferomedial strut (IMS) is challenging and requires careful practice. We present our surgical strategy and postoperative outcomes. Methods We divided 74 patients who underwent the reconstruction of the orbital floor and medial wall concomitantly into a comminuted IMS group (41 patients) and non-comminuted IMS group (33 patients). In the comminuted IMS group, we first reconstructed the floor stably and then the medial wall by using separate implant pieces. In the non-comminuted IMS group, we reconstructed the floor and the medial wall with a single large implant. Results In the follow-up of 6 to 65 months, most patients with diplopia improved in the first-week except one, who eventually improved at 1 year. All patients with an EOM limitation improved during the first month of follow-up. Enophthalmos (displacement, 2 mm) was observed in two patients. The orbit volume measured on the CT scans was statistically significantly restored in both groups. No complications related to the surgery were observed. Conclusions We recommend the reconstruction of orbit walls in the comminuted IMS group by using the following surgical strategy: usage of multiple pieces of rigid implants instead of one large implant, sequential repair first of the floor and then of the medial wall, and a focus on the reconstruction of key areas. Our strategy of step-by-step reconstruction has the benefits of easy repair, less surgical trauma, and minimal stress to the surgeon.

      • Differential effects of 5-fluorouracil on glucose transport and expressions of glucose transporter proteins in gastric cancer cells

        Although 5-fluorouracil (5-FU) is a widely used chemotherapeutic agent in the treatment of gastric cancer, the underlying mechanism for 5-FU resistant phenotype, has yet to be elucidated. We hypothesized that the sensitivity of gastric cancer to 5-FU treatment might be related to the rate of glucose transport (GLUT), and investigated the expressions of GLUT1, 2, 3, and 4 in two different gastric cancer cells (SNU-216, moderately differentiated gastric adenocarcinoma; and SNU-668, signet ring cell gastric carcinoma). Immunohistochemistry of GLUT1 and GLUT4 and immunoblot analysis of glycogen synthase kinase 3 were also performed. Hexokinase activity was measured. We found that 5-FU suppressed glucose uptake in SNU-216, while it stimulated GLUT in SNU-668. Further analysis revealed that 5-FU decreased the expression levels of GLUT1, 2, and 4 in SNU-216 cells and increased the expression levels of GLUT1, 2, and 4 in SNU-668 cells. Consistent with GLUT expression levels, immunohistochemistry analysis showed that 5-FU increased GLUT1 and GLUT4 levels in SNU-216 and decreased GLUT1 and GLUT4 levels in SNU-668. We also observed that glycogen synthase kinase 3 activity was decreased in SNU-216 and increased in SNU-668 with 5-FU treatment. No significant difference in hexokinase activities was observed with 5-FU treatment. Taken together, these results suggest that 5-FU exerts differential effects on GLUT depending on gastric cancer cell types, which may indicate a possible explanation, at least in part, for the differing responses to 5-FU chemotherapy in gastric cancer.
