        The aim of this reports is helped to blood bank of others hospital that introduction to experienced the while and using computerized systems of blood bank in Samsung Medical Center. Utilization of the systems 1. We could be confirmed to final results when input, to prevent make an error only the same with the both results that after each inspector is in put result to the other computer. 2. We prevented to make an error once input a test result which accumulated the past a test result table and then compared to each result for a new perscription as tests. 3. We had been inputed after compared with result of stock specimens on the history table that to protect an error when registrated of stock specimens to the blood types. 4. All entry or delively procedures to be used to barcode scanner for a minimization to error. 5. The stock place is automatic registrated from 1 to 700 for a protect duplication of stock places. 6. Did no deliveried the prescribed blood and a kind of different bloods. 7. It`s establization system the publication sticker to patient`s information and blood delivery information for spend of time, minimization an error. 8. Get it to caution message to user before blood delivery when a ordered of filter or irradation. 9. Can`t delively if a different blood with patient`s blood type, but incase of BMT or exchange transfusion are able to delivery taken a caution message for user. 10. We established systems to shown when blood delivery, if have been the donation blood, automatic inquired to donation table. 11. We able to deliveried after easily to inquired to the past transefusion history of patient`s that use a function key before blood delivery. 12. We established to systems of various that a statistical bloods ,statistical tests.

        팔로사징후를 동반한 Smith-Magenis증후군 환아 2예: SMS에서 FISH검사의 진단적 의의

        Smith-Magenis syndrome (SMS) is characterized by distinctive facial features, developmental delay, cognitive impairment, and behavioral abnormalities and associated with interstitial deletion of chromosome 17p11.2. We report 2 cases of SMS with tetralogy of Fallot. The first patient was reported having a normal conventional karyotype 7 years ago. However, as she grew up, she showed more compatible findings with SMS in behavior and phenotype. On the second cytogenetic study, interstitial deletion of 17p11.2 was detected by conventional banding technique which had 550 band resolution and it was confirmed by metaphase fluorescence in situ hybridization (FISH) using D17S258 SMS probe (Oncor, Gaithersburg, MD, USA). The second patient showed subtle phenotypic feature except microcephaly and cardiac anomalies was confirmed as SMS by cytogenetic analysis and FISH. We suggest that FISH should be performed not to overlook the submicroscopic deletion when SMS is clinically suspected, even though cytogenetist can not detect any anomalies on the conventional cytogenetics. A confirmatory diagnosis using FISH would be helpful in terms of guiding medical management and leading to proper genetic counseling. SMS는 안면기형과, 발달지연, 인지장애 그리고 행동장애 등을특징으로 하는 증후군으로 17번 염색체의 단완의 p11.2부분의 결손으로 인하여 발생한다. 본 증례는 팔로사징후를 가진 10세 된여아와 4개월 된 여아로, 첫 번째 환아는 7년 전에 시행한 염색체배양검사에서 정상 핵형으로 보고되었다. 점차 자라면서 SMS에특이적인 행동 및 안면 이상소견을 보여 다시 시행한 염색체 배양검사에서 17p11.2부위에 부분적 결실이의심되는 소견이 보여, 이부위에 특이적인 D17S258 SMS probe (Oncor)를 사용하여 FISH를 시행하여 한쪽 17번 염색체에서 결실을 확인하였다. 두 번째환아는 소두증과 선천성 심기형 외에 뚜렷한 SMS에 합당한 형태학적 소견을 찾을 수 없던 환아로, 고해상도의 염색체 배양검사와FISH를 시행하여 17p11.2부위의 결실을 확인하였다. 임상적으로SMS가 의심되는 환자는 염색체 검사에서 확인이 되지 않았더라도 FISH를 반드시 시행하는 것이 필요하리라고 사료된다.

        RHD 유전자검사로 확진된 Partial D type VI-3 증례

        Weak D and partial D result in quantitative and qualitative changes in RhD protein expression respectively. It is difficult to discriminate weak D from partial D by serological tests alone. RHD genotyping is a useful method that complements serological results. A 64-year-old woman visited our hospital for microvascular decompression surgery. Her blood type was O, D negative by manual tube test and as per auto analyzer results (QWALYS-3 system; DIAGAST, France). Weak D and partial D tests were performed by using two different monoclonal anti-D reagents (Bioscot; Merck Millipore, UK; Bioclone; Ortho Clinical Diagnostics, USA) and a panel of nine monoclonal antibodies, including anti-D IgM and IgG (D-Screen; DIAGAST, France). However, these serological tests could not confirm the subtype of partial D. Therefore, sequencing of RHD exon 1 to 10 was additionally performed for the patient and the case was revealed to be partial DVI type 3. 약-D형은 적혈구에 표현되는 D 단백질의 양적 감소를, 부분-D형은 질적 결손을 유발한다. 이들은 혈청학적 방법만으로는 정확한 감별이 어려운데, RHD 유전자검사는 이런 혈청학적 검사를 보완할 수 있는 유용한 검사다. 본 증례는 64세 여자 환자로 미세혈관감압술을 위해 내원하였다. 환자는 수기튜브법과 자동혈액형검사장비(QWALYS-3 system; DIAGAST, France)에 의해 O형, D 음성이었고, 두 종류의 단클론성 항-D 시약(Bioscot; Merck Millipore, UK 및 Bioclone; Ortho Clinical Diagnostics, USA)을 이용한 약-D 검사와 9개의 단클론성항체(항-D IgM & IgG)로 구성된 시약(D-Screen; DIAGAST, France)으로 실시한 부분-D 검사로도 부분-D형의 세부 유형을 결정할 수 없었다. 따라서, RHD 유전자의 엑손 1에서 10까지 염기서열분석법을 추가로 실시하였고, 혈청학적으로 약한 D 표현형의 본 환자는 부분-D type 3형으로 확인되었다.

      • 인간 거대세포 바이러스 감염 진단을 위한 CMV 항원혈증검사의 유용성 평가

        Early detection and treatment of human cytomegalovirus(CMV) infection is very important because CMV infection is major cause of morbidity and mortality after organ transplantation. CMV antigenemia assay has been reported to be very sensitive and specific for detection of CMV infection among many laboratoη methods. We compared the results of CMV antigenemia(Clonab CMV kit; Biotest AG, Landsteinerstrasse, Dreiceich, Germany), CMV PCR, CMV culture and CMV IgM to evaluate their clinical usefulness. We compared 620 s없nples for CMV antigenemia tests, 2,272 samples for CMV culture, and 423 samples for CMV PCR tests between July 1997 and April 1998 in samsung medical center clinical microbiology For CMV test, the sensitivity of CMV culture, CMV antigenemia, and CMV PCR were 4.3%, 16.6% and 31.0%, respectively. Sixty seven patients were classified CMV antigenemia positive. The results of compared for CMV culture, CMV PCR, and CMV IgM antibody. For CMV antigenemia, the sensitivity of CMV PCR and CMV IgM antibody were 85.3%, 93.5% respectively. CMV antigenemia is thought to be useful for early diagnosis and follow- up of anti띠ral treatment as a quantitative and highly specific method. The sensitivity and specificity are very comparing to CMV antigenemia.

        환기정도에 따른 수술실용 신발 종류가 수술실 오염에 미치는 영향 : How to Choose Suitable Shoes?

        Background : Various types of protective footwear have been used to minimize bacterial contamination in operating rooms. In recent years, debate has arisen concerning the need for use of such protective footwear. This study was designed to provide useful data about choosing shoes most suitable for the surgical environment. Methods : Between November, 1999 and January, 2000, we performed this experimental study by comparing effect of three types of shoes (i.e., disposable shoescover, operating room-restricted shoes, and ordinary shoes) on bacterial contamination of operating rooms equipped with two different ventilation systems (i.e., high air-change, low air-change) respectively. Data were collected during two-hour sham operations in which subjects and their activities were strictly standardized. Bacterial flora were sampled from the study area floor and air colony-counts were measured. Results : In experiments involving high air-change ventilation system, there was a significant difference of floor contamination between three types of shoes, but no difference of air contamination. Under low air-change system, there was a significant difference of both floor and air contamination between three types of shoes. Conclusion : The results show that protective footwear would be unnecessary in the operating room with high air-change ventilation system, but it is important to choose suitable shoes carefully under low air-change system. Therefore, the use of outdoor shoes can be considered under high air-change system, but it would seem sensible to apply their first use in less bloody operations at the day surgery center or out-patient department to prevent transfer of body fluid into the outside environment.
