간질발작후 MRI 영상에서 뇌 종창을 보인 3예 보고

We report seizure induced brain swelling in three patients. MRI revealed a cortical, subcortical(3/3), and basal ganglia(2/3) high signal intensity on T2- weighted and FLAIR images. The lesion shows low signal on T1-weighted image, and enhanced after contrast administration. Follow up study after 27 days in one case revealed complete resolution of the lesion. The reversible MRI findings may be the results of a local brain swelling, breakdown of blood-brain permeability, and a defect of cerebral autoregulation during seizure activity. The transient nature of such findings have to be considered in the differential diagnosis such as reversible ischemia, migraine, inflammation and infection.

Noradrenaline이 근방추의 흥분성에 미치는 영향

The present experiment was done to determine the effects of noradrenaline on the muscle spindle in cats. The reaction site was determined by recording the changes of impulses from group Ia and group Ⅱ muscular afferents of peripheral sensory nerves and dorsal roots. The results are as follows: 1. 36 group Ia muscular afferents were recorded according to extracellular recording and 5 out of 36 showed enhancement of impulse discharge after 1-2 ㎍ noradrenaline injection, 8 showed decrease and 20 showed initial enhancement followed by decrement, but 3 gave no response at all. 2. The administration of KCI solution did not give nay influences to impulse changes of group Ia muscular afferents activated by noradrenaline, however,the injection of bradykinin diminished the decreasing effect of noradrenaline. 3. A small amount of noradrenaline slightly increased the systemic blood pressure but this kind of blood pressure changes was indifferent to the change of the excitability of group Ia muscular afferents from muscle spindle. However, a large amount of noradrenaline increased the blood pressure about 50 ㎜Hg above normal level which influenced the excitatory effect of the muscle spindle. 4. 22 out of 25 group Ⅱ muscular afferents did not show any responses to noradrenaline. Summerizing above results, it could be assumed that the initial excitatory response of group Ia muscular afferents following the injection of noradrenaline was not attributed to vasoconstriction but to direct effects on muscle stretch receptor, and its depressive response were due to the asphyxia resulted from vasoconstriction. And also, it could be suggested that noradrenaline acts on equator region of the muscle spindle, that is, the distribution area of group Ia muscular afferents.

In order to screen the new β-lactam antibiotics for clinical use. we were compared MIC values of various β-lactam antibiotics. We also tested the influence of pH, inoculum size, stability of antibiotics in medium and some kinds of media on MIC value. As a result, cefotaxime, cefoperazone, piperacillin, cefamandole, cephaloridine, cephapirin, cefazolin, cephalothin, cephaioylycn were scored in antimicrobial activity of MIC data.

사업장 근로자의 건강위험 평가도구 개발

Health Risk Appraisal(HRA) is usually defined as a process by which we expect of individual's chances of death or acquiring specific diseases within a defined period of time. The concept of worksite as an area of health maintenance and promotion is newly developing. Our movement for health promotion of employees has been increasingly known in the worksite. The aim of this study is to develop health risk appraisal tools about health promotion at the worksite for employees. We performed this study by two steps; one step was to develop a HRA questionnaire of worksites in Korea, another was to evaluate the reliability of the questionnaire. For developing HRA questionnaire, we reviewed scientific examples at first, and then weighted the score by delphi. To evaluate the reliability of developed questionnaire, we carried out survey by test-retest method. A total of 131 employees completed HRA questionnaire on two times. The results are as follows. The total score of health risk in unhealthy workers was higher than that of healthy workers. The range of test-retest reliability of responses to the questionnaire was 0.57-0.94. Therefore it seems that this questionnaire was very suitable to assess the health behavior of workers. In conclusion, the developed HRA questionnaire can be used as a tool for evaluating health behavior and for providing health counseling materials.

소아 급성 부비동염의 진단에 Water's view촬영이 충분한가?

The purpose of this study was to examine the utility of the Water's view in the diagnosis of acute sinusitis in children. The records of all pediatric patients who underwent sinus radiography for suspected acute sinusitis between Oct. 1995 and Apr.1996 was reviewed. All radiographs were reviewed by an attending radiologist and the interpretation of the Water's(occipitomental) view alone was compared to that of a 3 view PNS series(AP Caldwell, lateral, Water's view). 86 PNS series were obtained on 76 pediatric patients during the study period. When compared to 3 view series, the single Water's view had a sensitivity of 84%, specificity is 79%, positive predictive value of 87%, negative predictive value of 84%, overall accuracy of the Water's view in diagnosing childhood acute sinusitis was 85%. The authors conclude that the Water's view is usually sufficient in the evaluation of suspected acute sinusitis in children.

인터넷쇼핑의 혁신성과 위험지각에 따른 의류상품 구매행동

Various marketing strategies which enable both enterprises and consumers to internet are made possible in real time through a web of networks set up in cyberspace. Growth in the Interned market is feasible since this so-called "Internet marketing" provides both interacting parties with tangible benefits. The following will be discussed in detail. - the status of clothes purchase through e-commerce and the level of risk types perceived by consumers. - the difference of risk perception between a group with high purchasing intention and the other with low purchasing intention. - the perception of innovation in purchasing means during different purchasing intention groups and individual innovation. - the frequence in e-commerce based on purchasing intention groups and decision process of purchasing goods. Result from the study are as follows 1.72.8% of the respondents said they purchased goods on the Internet. Many of them did 3-5 times. Items purchased included CDs, computer gadgets and books as well as fashion - related goods. 2.56.4% of 송 respondents said they purchased clothes over the Net, many of whom did it once or twice. Goods included underclothes, pajamas and cheap miscellaneous goods that do not require and specific size. 3.Results show that socio-psychological risk was highly perceived among types of risk perceptions such as accomplishment, risk, socio-psycho-logical risk, time, convenience risk and economic risk 4.An analysis of the difference in risk perception between a group with high purchasing intention and the other with low purchasing intention showed the former had a low risk perception. 5.It turned out that consumers used the Internet more at the information serch and problem recognition stage than at any other stage after reviewing the influence of distinctions in e-commerce based on 5 stages of purchasing decision-making. 6. Factors affecting purchasing intention showed perceptive results in goods satisfaction, accomplishment risk, social risk and time risk. however, they did not show any sign of perceptive results in economic risk.

전위된 상악 견치 및 하악 측절치의 치험례

Tooth transposition is an anomaly of eruption characterized by the interchanged positions of two adjacent teeth, which is divided into complete and incomplete transposition. There are three common approaches for treating transposition: aligning the involved teeth in their transposed positions, moving them to their correct anatomic position in the arch and extracting one of the transposed teeth. Considerations in treatment plans are esthetic, function, risk of jeopardizing the roots and damaging the supporting structures, position of the root apex, developmental stages of teeth and expected compliance. The presented case reports described one maxillary canine-the first premolar transposition and two mandibular lateral incisor-canine transpositions. The former transposed teeth were arranged in their transposed position, and the latter transposed teeth were rearranged into their normal position. 치아전위란 인접한 두 치아의 위치가 서로 바뀐 치아 위치 이상을 말하며, 치관과 치근의 위치에 따라 완전전위와 불완전전위로 나눌 수 있다. 하악보다는 상악에 편측성으로 호발하며, 특히 상악 견치에서 자주 발생한다. 전위는 발견 시기와 전위된 양상에 따라 치료방법이 달라질 수 있다. 본 증례는 상악 견치와 제1소구치 간의 전위가 일어난 10세 7개월의 환아와 하악 측절치와 견치 간의 전위가 일어난 8세 11개월 환아와 8세 9개월 환아의 교정치료에 대한 것으로 양호한 치료 결과를 얻었고, 증례 별로 치료방법의 차이점에 관한 고찰을 하였기에 이를 보고하는 바이다.

조기분만 및 만기분만 산모 모유의 젖당, 단백질, 총지질 함량에 관한 비교 연구

The lactose, total nitrogen and total lipid contents of human breast milk were determined longitudinally after preterm and term deliveries. Milk samples were obtained from day 2 to 5, on weeks 1, 2, 4, and 6 after preterm (PM : n = 24) and term (TM : n = 22) deliveries. The lactose contents of PM and TM increased from 5.28 g/dl and 5.93 g/dl at days 2 to 5, to 7.95 g/dl, 7.87 g/dl at 6 weeks, respectively. The total nitrogen contents of PM and TM decreased significantly during the first 6 weeks after birth (from 374 to 220 mg/dl versus 382 to 220 mg/d, respectively). However the total lipid contents of the PM and TM increased during the same periods. Significant differences in the total lipid contents between the PM and TM were 2.15 versus 3.27 g/dl at 2 weeks, and 3.26 versus 2.52 g/dl at 6 weeks, respectively. The energy intakes of preterm infants was 134.8 kcal/kg/d during the 6 weeks of lactation, which satisfied with energy requirements (85-130 kcal/kg/d) of the preterm infants.
