여성 골반의 절단면영상은 여성 비뇨생식계통의 실감나는 3차원영상을 만들 수 있는 가장 좋은 재료이다. 이 연구의 목적은 여성 골반의 고화질 절단면영상과 테두리영상을 마련해서 여성 비뇨생식계통의 가상해부, 가상수술을 위한 3차원영상을 만드는 데 도움 주는 것이다. 한국 여성 시신의 골반을 대상으로 자기공명영상과 컴퓨터단층사진을 찍었다. 이 골반을 포매한 다음에 수평방향으로 연속절단하였고(간격 0.1 ㎜), 각각의 절단면을 사진 찍어서 절단면영상을 만들었다. 절단면영상에서 보이는 73개 구조물(비뇨생식기관과 주변 구조물)의 테두리를 그려서 테두리영상을 만들었다. 절단면영상과 테두리영상으로 각 구조물의 3차원부피영상, 3차원표면영상을 만들었다. 여성 골반의 자기공명영상 222개(간격 1 ㎜), 컴퓨터단층사진 222개(간격 1 ㎜), 절단면영상 2,220개(화적소 크기 0.1×0.1×0.1㎣, 빛깔 개수 48 bits color), 그리고 테두리영상 222개(간격 1 ㎜)를 만들었으며, 이 영상은 서로 들어맞았다. 절단면영상과 테두리영상으로 만든 3차원영상은 실감나는 가상현실 프로그램을 만드는데 도움 될 것이고, 나아가 여성 골반의 해부학과 임상을 익히는 데 이바지할 것이다. The sectioned images (SIs) of the pelvis from a female cadaver are the best source of realistic threedimensional (3D) models of the female urogenital system. The purpose of this research is to present SIs and outlined images of the female pelvis with improved quality, which may be used to produce 3D models to simulate virtual dissection or surgery of the female urogenital and adjacent structures. A pelvis of Korean female cadaver which preserved buttock curve was scanned with 3T MR and CT machines. The pelvis was embedded and milled at 0.1 ㎜ intervals. All sectioned surfaces were photographed to create horizontal SIs. On the Photoshop, 73 structures were outlined in the SIs to create outlined images. Once the structures were outlined, volume and surface models of the structures could be produced. A total of 222 MRIs and 222 CTs of a female pelvis were obtained. 2,220 SIs of the pelvis were obtained (0.1 ×0.1×0.1㎣-sized voxels; 48 bits color). 222 outlined images of 73 structures were prepared at 1㎜ intervals. Once the structures were outlined, 3D volume and surface models of the structures were produced without the help of the computer programmers using MRIcro and Maya software. We have produced high quality SIs of the female pelvis accompanied by corresponding MRI and CT images. 3D volume and surface models of the female pelvic structures have been constructed. These computerized models may serve as the basis for future realistic medical simulation programs that may enhance clinical understanding of pelvic anatomy.
Effects of Early Operation in Patients Who Diagnosed Complicated Parapneumonic Effusion

Purpose Thoracic empyema is defined as active suppuration within the pleural space and its treatment is controversial. Recently treatment of empyema is changing observation of empyema formation to early invasive treatment with the operation. This study compared the outcomes of early surgery and delayed surgery Method We retrospectively reviewed 40 patients who underwent surgery for complicated parapneumonic effusion from January of 2013 to May of 2020. The early group was defined as operation with empyema diagnosed within 1 week. Results Among 40 patients, 22 patients received early surgery, and 18 patients received delayed surgery. Stage III empyema was prevalent in the delayed group (27.3% vs 76.9%, p=0.004). Operative time was longer in the delayed group (87.5 vs 132.5 min, p=0.004). Intraoperative blood loss was more in the delayed group (200ml vs 750ml, p=0.009). The hospital stay was longer in the delayed group (18.5 days vs 30.0 days, p<0.001). The complication rate was not different between groups (31.8% vs 33.3%, p=0.919). The mortality rate was no significance between the two groups (4.5% vs 5.6%, p=0.884). Conclusion Delayed surgical treatment make operation difficult. Although inactive infected phase, early surgical treatment did not increase postoperative morbidity and mortality.

The effects of convection on the measurement of the diffusion coefficients of liquids by the pulsed gradient spin echo (PGSE) NMR method at low temperature are discussed. To examine the generation of convective flows, we used four different types of sample tubes in the diffusion measurements with temperature variation; a normal 5 mm NMR tube, a Shigemi tube, an ELISE type tube, and a capillary tube. Below room temperature, the calculated diffusion coefficients of chloroform in 5 mm o.d. type tubes increased with decreasing temperature, while those in the capillary tube decreased linearly. The convective flow was found to be significant even at low temperature and it seemed to be mainly induced by the transverse temperature gradient. It was also found that the capillary tube was most appropriate to measure the diffusion coefficients, since its small diameter is effective in suppressing the convective flows at both high and low temperatures.

Background: This study aimed to identify the clinical characteristics and prognosis of primary gastric lymphoma detected by screening upper endoscopy in high prevalence area of Helicobacter pylori ( H. pylori) infection. Methods: Between October 2003 and May 2013, consecutive subjects who were diagnosed with primary gastric lymphoma by screening upper endoscopy were retrospectively enrolled at Seoul National University Hospital Healthcare System Gangnam Center, Seoul, Korea. Results: During the study period, a total of 101,103 subjects received 207,114 screening upper endoscopy. Among them, primary gastric lymphoma was detected in 49subjects. They were 53.0 ± 10.5 years of age, and 72.9% (35/49) were female. Histologic type of primary gastric lymphoma was predominantly (98.0%, 48/49) extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma), and one remaining case (2.0%) was diffuse large B-cell lymphoma. Initial stage was IEa in 95.9% (47/49), and H. pylori was positive in 91.8% (43/49). Among 41 H. pylori-positive patients with gastric MALT lymphoma stage IEa, H. pylori eradication achieved complete remission in 95.1% (39/41). There were three cases of recurrence with (n = 2) or without (n = 1) reinfection of H. pylori. Conclusions: Upper endoscopy as a screening for upper gastrointestinal malignancymay detect gastric MALT lymphoma in early stage, which can be successfully treated with H. pylori eradication, especially in H. pylori endemic area. For those who achieved complete remission, it is needed to undertake surveillance for recurrence.

Introduction: Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening complication after solid organ transplantation. HLH has been reported as problem related with kidney and liver transplant, and there are limited reports of HLH after lung transplantation. Case: A 60-year-old man with idiopathic pulmonary fibrosis underwent bilateral lung transplantation. After lung transplantation, acute rejection was suspected and high dose steroid therapy was done. Since postoperative day(POD) 25, thrombocytopenia( platelet 112*1000/uL) and leukopenia(2530/uL)) were presented. The patient complained of intermittent symptom of low-grade fever, chest discomfort and dyspnea. Echocardiography showed stress induced cardiomyopathy and results of peripheral blood smear was nonspeci. c. Pneumonia was developed and patient was treated with antibiotics. Hyperbilirubinemia(total bilirubin 2.1 mg/dL) started to present at POD 50. Results of abdomen sonography was nonspeci. C except for mild splenomegaly (11.4cm). By POD 80, bilirubin was getting higher(total bilirubin 33.3mg/dL, direct bilirubin 25.7mg/dL, gamma glutamyl transpeptidase 260IU/L) and leukopenia and thrombocytopenia was getting aggravated (white blood cell 2080/uL and platelet 57* 1000/uL). The . brinogen was mildly elevated (4210m/dL), triglyceride was normal (127 mg/dL), the ferritin was elevated (4518 ng/mL) and soluble interleukin-2 receptor was elevated (8730U/ml). However, the finding of peripheral blood smear was still nonspeci. c. The cause of pancytopenia, low grade fever and hyperbilirubinemia was unclear, and we conducted bone marrow biopsy on POD 82. The finding showed that histiocytes were frequently seen with occasional hemophagocytes. Taken together, cytopenia, bone marrow hemophagocytes, elevated soluble interleukin-2 receptor and elevated ferritin were positive among laboratory tests listed in diagnostic criteria of HLH. We managed with etoposide and high dose steroid, but patient deteriorated and died on POD 87. Summary: HLH is a significant diagnostic and therapeutic challenge in lung transplantation and is potentially lethal complication. Therefore, clinicians should consider HLH as possible diagnosis in clinical context.

고온, 호알칼리성 Bacillus속 K-17 균주에서 $\beta$-xylosidase 유전자를 pBR322를 벡터로 이용하여 클로닝시켰다. p-Nitrophenyl-$\beta$-xylopyranoside를 함유하는 LB 한천배지에서 노란색을 형성하는 대장균 형질전환주에서 재조합 플라스미드 pAX278을 분리하였으며, 본 pAX278은 pBR322와 고온, 호알칼리성 Bacillus K-17 균주 염색체 DNA의 5.0 kb HindIII절편으로 구성되어 있었다. Biotin으로 로식된 pAX278을 probe로 하여 상동성 시험을 하여 본 결과, pAX278에 존재하는 5.0 kb HindIII 절편은 Bacillus K-17 균주의 염색체 DNA HindIII 절편 중에서 5.0 kb 분만 아니라 0.9 kb 절편과도 상동성이 있었다. pAX278의 5.0 kb 절편을 pGR71에 연결시켜 B. subtilis에서도 발현시켰다. pAX278을 가지는 E. coli 균주가 생성하는 $\beta$-xylosidase는 균체외에 존재하였으며 그 효소학적 성질은 Bacillus속 K-17의 $\beta$-xylosidase와 동일하였다. The chromosomal DNA fragments of thermophilic alkalophilic Bacillus sp, K-17, a potent xylanhydrolyzing bacterium, were ligated to a vector plasmid pBR322 and transformed into Escherichia coli HB101. The plasmid pAX278, isolated from a transformant forming yellow color on the LB agar plate containing 1 mM p-nitrophenyl- $\beta$-xylopyranoside, was found to enable the transformants to produce p-xylosidase. The 5.0 kilobase insert of pAX278 had single sites for EcoRI, PstI, XbaI, and PvuII, and 2 sites for BglII. Biotinylated pAX218 was hybridized to 0.9 kb as well as 5.0 kb fragment from Bacillus sp. K-17 DNA on nitrocellulose filter. pGX718 was constructed by inserting the 5.0 kb HindIII fragment of pGX278 at the HindIII site of pGR71, E. coli and B. subtilis shuttle vector. The enzymatic properties of $\beta$-xylosidase from E. coli HB101 carrying recombinant plasmid were the same those of $\beta$-xylosidase from Bacillus sp. K-17.

Background/Aims α-Synucleinopathy in the brain is the neuropathological hallmark of Parkinson’s disease (PD). However, the functional impact of α-synucleinopathy in the enteric nervous system remains unknown. We aim to evaluate the association between gastrointestinal (GI) dysfunction and α-synuclein (αSYN) pathology in the stomach and colon of PD patients and controls, as well as to investigate the association between the αSYN pathology in GI tract and future PD risk. Methods A total of 35 PD patients and 52 neurologically intact subjects were enrolled in this study. Endoscopic biopsies were performed, and then immunohistochemical staining for αSYN was performed. All subjects completed the validated Rome III questionnaire for the assessment of GI symptoms. The association between GI symptoms and the αSYN pathology in GI mucosa was evaluated. Incident PD cases were assessed during a median follow-up of 46 months. Results The proportion of self-reported constipation and functional constipation through the Rome III questionnaire was significantly higher in PD patients than in controls (P < 0.001 and P = 0.015). However, no significant association was found between the αSYN pathology in the stomach and colon mucosa and constipation, as well as other GI symptoms including dyspepsia symptoms and abdominal discomfort or pain, regardless of the presence or absence of clinical PD (P > 0.05). No incident PD cases were diagnosed during study period. Conclusions Our present study suggests that the deposition of αSYN in the mucosal enteric nervous system may not be reflected by functional impairment of the affected segment of the gut.
