      • 동일한 환자에서 조혈모세포이식 전후의 호중구감소 기간 중 감염양상에 대한 비교연구 : 중심정맥관 관련 패혈증을 중심으로 Based on Central Venous Catheter Related Septicemia

        연구배경: 동일한 환자를 대상으로 조혈모세포이식 전 일반 병실에서의 관해유도 항암제치료 및 강화요법으로 인한 호중구감소 기간의 감염양상과 고용량 항암제치료 후 무균실에서의 조혈모세포이식 시 호중구감소 기간의 감염양상을 중심정맥관 관련 패혈증을 중심으로 비교 연구함으로써 효율적인 감염관리지침을 마련하고자 하였다. 대상 및 방법: 1999년 1월부터 2001년 7월까지 2년 7개월간 동아대학교병원 일반병실에서 악성혈액질환으로 항암제 치료를 받다가 조혈모세포이식을 시행받았던 22명을 대상으로 중심정맥관 관련 패혈증의 형태를 비교분석하였다. 결과: 대상 환자들의 일반병실과 무균실에서의 입원기간은 각각 32±13일, 33±19일, 호중구 감소기간은 15±12일, 17±18일, 호중구 감소기간의 발열 횟수는 14회, 14회로서 입원병실의 종류에 따른 차이는 없었다. 환자들의 중심정맥관 사용일수는 일반병실에서 29일(7~545일), 무균실에서 100.5일(25~606일)이었으며, 총 사용일수는 각각 1,515일, 3,250일로서 무균실에서의 중심정맥관 사용일수가 의미 있게 길었다(P=0.001). 중심정맥관 사용 일수에 대한 감염률은 각각 5.28/1,000일, 3.08/1,000일로서 일반병실에서 그 빈도가 높았으나 통계적 유의성은 보이지 않았다(P=0.141). 결론: 중심정맥관 삽입 후 일반병실에서 항암제 치료를 받던 환자가 조혈모세포이식을 위하여 무균실에 입원하는 경우, 중심정맥관을 교체하는 것이 감염관리를 위하여 바람직할 것으로 생각된다. Background: We compared the incidence of central venous catheter (CVC) related septicemia during neutropenic period in the same patient, which developed following chemotherapy for remission induction and consolidation, or hematopoietic stem cell transplantation (HSCT). Methods: We retrospectively reviewed the medical records and laboratory data of 22 patients with hematologic malignancies who received anticancer chemotherapy followed by HSCT at Dong-A University Hospital between January, 1999 and July, 2001. We investigated the duration of hospitalization, duration of neutropenic period, duration of catheterization, microbiologically documented organisms and incidence of CVC related septicemia. Results: The data in general ward (before HSCT) and laminar air flow room (after HSCT) were as follows: duration of hospitalization was 32±13 days and 33±19 days, duration of neutropenic period was 15±12 days and 17±18 days, median duration of catheterization was 29 days (7~545 days) and 100.5 days (25~606 days) (P=0.001), and incidence of CVC related septicemia was 5.28/1000 days and 3.08/1000 days, respectively. In the CVC related septicemia, the most common etiologic organism was coagulase negative staphylococcus. Conclusion: We suggest that the exchange of CVC before admission to laminar air flow room could decrease the incidence of CVC related septicemia in HSCT recipients.

      • Characterization of Antihypertensive Angiotensin I-Converting Enzyme Inhibitor from Recombinant E. coli

        안지오텐신(ACE) 저해제는 항고혈 효과를 갖고 있으므로 오랫동안 고혈압의 예방이나 치료에 이용되어 왔다. 본 연구는 재조합 대장균으로부터 새로운 ACE 저해제를 생산하고 정제하며 나아가 이들이 구조-기능 관P를 규명하기 위해 수행되었다. Saccharomyces cerevisiase의 ACE 저해 펩타이드 유전자를 함유하고 있는 재조합 pGEX-4T-3을 대장균 BL21(DE3)로 형질전환 시켰다. 재조합 pGEX-4T-3을 갖고 있는 대장균 BL21(DE3)로부터 생산된 Glutathione-s 전이효소 (GST) 융합 단백질을 얻어서 그중 ACE저해 펩타이드를 Sephadex G-25 컬럼 크로마토그래피로 정제하였다. 정제된 ACE 저해 펩타이드는 타이로신-아스파틱엑시드-그리신-글리신-발린-패닐알라린-아르기닌-발린-타이로신-트레오닌의 서열을 가진 새로운 decapeptide이었고 ACE에 대하여 경쟁적으로 저해하였다. The angiotensin I-converting enzyme (ACE) inhibitor has anti-hypertensive effects and has long been used as prevention or remedy of hypertension. This study were carried out to produce and purify a new ACE inhibitor from recombinant E. coli and further elucidate its structure-function relationship. Recombinant pGEX-4T-3 containing ACE inhibitory peptide gene of Saccharomyces cerevisiae was transformed into E. coli BL21(DE3). Glutathione-S transferase (GST) fusion protein from E. coli BL21(DE3) harboring the recombinant pGEX-4T-3 was obtained and the ACE inhibitory peptide was purified with Sephadex G-25 column chromatography. The purified ACE inhibitory peptide was a novel decapeptide with sequence Tyr-Asp-Gly-Gly-Val-Phe -Arg-Val-Tyr-Thr which shows very low similarity to the other ACE inhibitory peptide sequence. The purified ACE inhibitor competitively inhibited ACE.

      • 살수여상공정을 이용한 돈사폐수의 처리에 관한 연구

        Tricking filter was not usually employed for the piggey wastewater treatment resulting from a high concentration of susfended solids(SS) and biochemical oxygen demand(BOD) in the piggery wastewater could hamper and efficient operation due to the frequent clogging problems. In order to minimize the clogging problem, the filtration process using strawcolumn was applied, as a pretreatment process, for SS redution. The prefiltered effluent was then treated in the laboratory tricking filters which used crused stone media. The experimental results showed that the overall removal efficiencies of BOD, chemical oxygen demend, and SS were 78, 75, and 90%, repectively, at the respective tricking filter gydraulic and organic loadings of 1.26㎡/㎡/day and 0.77kgBOD/㎡/day. The tricking filter constant, and reaction rate constant, k of the Eckenfelder equation were 0.6901 and 0.89, respectively, obtained from the experimental data.

      • 有害重金屬 産業廢水處理의 技術開發에 關한 硏究

        In recent year there has been considerable concern about waste discharges of chromium and cyanides to aquatic ecosystems. Chromium occurs in aqueous systems as both Cr (III)and Cr(VI).the Cr(VI)is of particular concern because this form has been demonstrated to be a public health hazard. The toxicity of cyanides to fish has long been recognized. Compounds containing the cyanides radical are freguently present in effluents of many industries, including electroplating plants, steel mills, petroleum refineries and gas works. The objective of this study is to remove the chromium and cyanides in the wastewater efficiently. In the experiment the amount of NaHSO₃used for treatment were 2,4,6,10 and 20 ml and the concen-tration of chromium was 100,200,300,500 and 1000 ㎎/1 wastewater. At that time efficiency was 99.1%-99.9% and the dfficiency of pH 1.5was better than pH 1, pH 2 and pH 3. The results of cyanides treatment was that concentration of residual CN^(-)ion was below 0.003㎎/1, treated efficiency was 99.9%-99.98%, and in case of pH 9 was better than pH 11.

        Recently, column-supported wall structural system is frequently adopted in mixed-use high-rise buildings. Due to the sudden change of stiffness at the transfer floor proper load transfer and avoiding stress concentration are very important in column-supported wall structural system. It is revealed by many investigators that 2-dimensional analysis is not reliable and inappropriate selection of element for modeling may lead to erroneous result for gravitational loading. In this study, structural behavior of column-supported wall structure at transfer floor subject to lateral loading is compared by changing modeling methods.

      • 운전자 건강과 교통사고 예방에 관한 연구

        The purpose of this research is to examine how driver's physical and psychological condition affects on traffic offence or accident and to find the reasons. 192 people was selected as sample by means of Intended Sampling method out of 220 traffic offenders who were participating in traffic education program after consulting with some specialist about offenders condition (health), a questionnaire composed of 40 questions was mode Then the data was analyzed by the SPSS PC program of SM-430 computer. Comprehensive examining the relationships between traffic offenders and their health, drivers who got traffic accidents and their health, and overspeeding drivers and their health deduced the following results. First, even though undesirable physical and psychological condition is a subject to high frequency of offence or accident, most of drivers used to ignore these factor. Second, driver's psychological condition affects overspeeding that lead big graffic accident. Temselves should recognize the importance of such factors but also many education institutes have to analyze the main reason. And educational programs about drivers physical and psychological condition ought to be designed in the long term view.

      • 쌀의 식이섬유함량이 정상인의 혈당에 미치는 영향에 관한 연구

        건강한 지원자를 대상으로 전기압력밥솥으로 조리한 수원 464 백미와 일반쌀 백미를 섭취시켜 혈당과 인슐린의 반응을 비교하여 다음과 같은 결과를 얻었다. 1. 식후 60분 후에서 수원 464 백미섭취군의 혈당이 일반쌀 백미섭취군에 비해 유의성 있게 더 감소하였다 (p<0.01). 2. 실험 당질에 대한 혈당반응면적 (Area Under the Curve : AUC)은 수원 464 백미군이 일반쌀 백미군에 비해 유의성 있게 낮았다 (p<0.05). 3. 혈청 인슐린치는 혈당반응과 유사한 패턴을 보였으며 식후 120분에서 수원 464 백미군의 인슐린치가 일반쌀 백미군의 인슐린치보다 더 유의성 있게 더 낮았다. 4. 일반쌀 백미의 혈당지수 (glycemic Index : GI)를 100%로 하였을 때 수원 464 백미의 혈당지수는 64.5%로 매우 낮게 나타났다. 5. 수원 464 백미의 혈당지수는 문헌상으로 조사된 일반쌀 현미의 혈당지수인 90%에 비해서도 현저하게 낮았다. 이상의 임상시험을 통하여 일반쌀에 비하여 식이섬유함량이 높은 수원464는 혈당지수가 매우 낮은 것으로 밝혀졌으며 수원 464 백미는 문헌상으로 조사된 일반쌀 현미의 혈당지수 보다도 낮음을 확인하였다 The purpose of this study is to investigate the effect of dietary fiber content of rice on the postprandial serum glucose and insulin responses in normal subject. Two rice varieties, Ilpum and Suwon 464 which are different in dietary fiber content, were cooked in pressure cooker and used for the test. The rice with a higher dietary fiber content gave a significantly lower glucose level (p<0.01) and insulin level (p<0.05) than did the normal rice variety. After a meal, the 60-min glucose levels of Suwon 464 and Ilpum were 90.3±4.8mg/dl and 111.6±2.7mg/dl, respectively. The glycemic index (GI) of Suwon 464 shows 64.5%, which was apparently lower than that of Ilpum. these results indicate that Suwon 464 high in dietary fiber can be useful in low-GI diets.
