N/A Background: A correct estimation of volume status and dry weight in dialysis patients remains a difficult clinical problem Clinical status and chest X-ray are not sensitive enough, while invasively measured central venous pressures are not routinely available. Recently, the ultrasonographic determination of the diameter and collapse index of the inferior vena cava has been proposed as a noninvasive method for estimating intravascular volume. We tried to evaluate the clinical relevance of this method in dialysis patients by comparing it with alphahuman-atrial natriuretic peptide (alpha-h-ANP) and cyclic guanosine 3:5-monophosphate (cGMP) levels. Methods: Using echocardiography, the diameter of the inferior vena cava (VCD) and its decrease on deep inspiration (collapse index : CI) were evaluated in 27 hemodialysis patients. Echocardiography of the inferior vena cava (IVC) was performed in the supine position after 10 minutes rest. The transducer was placed in the subxiphoid region and long and short axis views of the IVC were obtained just below the diaphragm in the hepatic segment. VCD was measured before the P-wave on the electrocardiogram to avoid interference with A-wave and V-wave on the venous pressure curve, and corrected for body surface area. Pre- and post-hemodialysis levels of the plasma alpha-h-ANP and cGMP were measured by radioimmunoassay. The relationship between VCD, CI determined by echocardiography, and alpha-h-ANP and cGMP concentrations were studied. Results: The levels of alpha-h-ANP and cGMP were markedly elevated before hemodialysis and significantly lower values were found after hemodialysis (alpha-h-ANP: 162.7 102.6 pg/ml vs 90.6 61.0 pg/ml, cGMP: 35.3 8.8 pmol/ml vs 21.3 6.2 pmol/ml). A significant correlation was found between VCD and alpha-h-ANP before (r=0.81, p<0.05) and hemodialysis (r=0.65, p<0.05). No such significant correlation was observed between CI and alpha-h-ANP levels. Furthermore, a significant correlation was observed between VCD before hemodialysis and the change in alpha-h-ANP during hemodialysis (r=0.64, p<0.05). The relationship between VCD and cGMP before hemodialysis was not significant (r=0.26, p>0.05) and also no relation was observed between the decrease of cGMP during hemodialysis and VCD before hemodialysis (r=0.12, p>0.05). A significant correlation between the percent change in body weight and the percent change in VCD during hemodialysis (r=0.91, p<0.05) and also significant relation was observed between the pecent change in body weight and the percent change in alpha-h-ANP levels (r=0.40, p, 0.05). Conclusion: Echocardiography of the inferior vena cava allow an estimation of changes of intravascular volume in ESRD patients without cardiac filling impairment as shown by the correlation to other indices of intravascular volume, such as alpha-h-ANP. In this study, CI and cGMP levels were less informative. Inferior vena cava echocardiography is noninvasive and easily available and serial measurements of VCD and alpha-h-ANP levels allow an estimation of chages of intravascular volume in FARD patients on maintenance hemodialysis.

In this paper, a small supersonic wind tunnel is designed and built to study the flow characteristics of a supersonic impulse turbine cascade. The flow is visualized by means of a single pass Schlieren system. The supersonic cascade with 3-dimensional supersonic nozzle was tested over a wide range of nozzle installation angle. Highly complicated flow patterns including shocks, nozzle-cascade interaction and shock boundary layer interactions are observed.
동시 간전이 직장 구불결장암 환자의 재발성 복수에 대한 SB 복강 주입의 복수 감소 효과 증례

Objectives : To report and demonstrate the effect of decreasing ascites volume by SB intraperitoneal injection to a refractory ascites patient with synchronous colorectal liver metastasis and metachronous peritoneal carcinomatosis. Methods : Two cycles of intraperitoneal and intravenous SB injection were conducted. Each injection cycle was made up of 4 days. Nine vials of SB were injected to the patient every day. To compare the volume of ascites between pret- and post-treatment, follow-up computed tomography was done on June 3, 2013. To observe other therapeutic effects of SB injection, laboratory tests were conducted periodically. Results : On the follow-up computed tomography images, the amount of ascites and pleural effusion had decreased compared to the April 30, 2013 computed tomography images. The levels of aspartate transaminase, alanine aminotransferase and lactate dehydrogenase decreased significantly from May 9, to May 30, 2013. The amount of oral intake increased constantly during hospitalization. The patient's symptoms such as abdominal distension, abdominal pain and dyspnea were improving until discharge. Conclusions : Even if thiese results cannot be applied to every synchronous colorectal cancer liver metastasis patient, we demonstrated that SB intraperitoneal injection has ascites-decreasing effect to refractory ascites patients with synchronous colorectal liver metastasis and metachronous peritoneal carcinomatosis.
부분입사형 초음속 터빈 익렬내 유동 특성에 관한 수치적 연구

Turbo-pump system, an essential component of liquid rockets and induced weapons, adopts a partial admission axial turbine which drives pump. And the turbine of a turbo-pump system is usually operated at supersonic condition due to its high loading chracteristics. Therefore, reseaches about flow and performance characteristics of a partial admission supersonic turbine must be preceeded to progress the aerospace and defense industries as well as the development of turbo-pump systems. In this study, flow characterisitics within blades of the partial admission supersonic turbine are numerically investigated by using Fine Turbo, a commercial CFD Code. Before performing the numercial analyses, to verify accuracy of the numerical result computed by Fine Turbo, I performed the comparison between the numerical results with J.J.Cho’s experimental results. It is found that the numerical results show good agreement with the experimental results. Computations about the partial admission supersonic turbine have been performed to investigate flow characteristics including shock patterns. It is also found that the flow and performance of partial admission supersonic turbine are largely depend on shocks ocurred in the nozzle and at the leading edge of blades, expansion or compression at exit of nozzle and separations occurred in passage.

Nowadays Passivehouse gets very interested because of energy saving. There are a lot of conditions to meet Passivehouse criteria. Air conditioning system is one of the most important systems because Passivehouse is a very air tightness building-in other words there is no exfiltration and infiltration in Passivehouse. Therefore Passivehouse can"t emit contaminated air in a house without air conditioning system.

This paper describes the concepts of Static Line Rating (SLR) and Dynamic Line Rating (DLR) and the computational methods to demonstrate them. Calculation of the line capacity needs the heat balance equation which is also used for computing the reduced tension in terms of line aging. SLR is calculated with the data from the worst condition of weather throughout the year. Even now, the utilization ratio is obtained from this SLR data in Korea. DLR is the improved method compared to SLR. A process for DLR reveals not only improved line ratings but also more accurate allowed line ratings based on line aging and real time conditions of weather. In order to reflect overhead transmission line aging in DLR, this paper proposes the method that considers the amount of decreased tension since the lines have been installed. Therefore, the continuous allowed temperature for remaining life time is newly acquired. In order to forecast DLR, this paper uses weather forecast models, and applies the concept of Thermal Overload Risk Probability (TORP). Then, the new concept of Dynamic Utilization Ratio (DUR) is defined, replacing Static Utilization Ratio (SUR). For the case study, the two main transmission lines which are responsible for the north bound power flow in the Seoul metropolitan area are chosen for computing line rating and utilization ratio. And then line rating and utilization ratio are analyzed for each transmission line, so that comparison of the present and estimated utilization ratios becomes available. Finally, this paper proves the validity of predictive DUR as the objective index, with simulations of emergency state caused by system outages, overload and so on.
