심실중격결손증-치험 1예-

A successful treatment of infants and children with congenital heart defects depends upon several factors: diagnosis must be accurate, the operative technique, including techniques of perfusion and myocardial preservation, must be suited to the needs of patients and intensive cares, including accurate anesthesia, must be applied through all stages of investigation and treatment, i.e. before, during and after the operation. Recently we performed a open heart surgery successfully for a six-years old girl who had suffered from frequent upper respiratory tract infection and mild to moderate exertional dyspnea with congenital heart defects (ventricular septal defect, Kirklin type II, with patent foramen ovale). The operation and postoperative course were very smooth. And now we report this with review of literatures.

비만 치료 및 예방에 대한 한약의 효능 연구(Ⅱ) : 4종 처방이 비만 흰쥐의 면역조직화학적 변화에 미치는 영향

We had experiment with several prescriptions in order to find cure and prevention effects for an obesity. We fed control gmup rats high fat diet and administered normal saline for Bweeb. And we fed experimental gmup ram high fat diet and administered exmct of four prescriptions for Bweek. The result were as follows: 1. The size of the epididymal fat cell of rats in Bikam-whan, Chekamhanghyuluiyiin-rang and Chekambohyulansin-tang groups showed a significant decrease in comparison with control group. 2. The fat drops area in hepatic lobule of rats in Bikam-whan, Cheknmhanghyuluiyiin-rang, Chekambohyulonrin-tang, and Chekambangpungrongseong-san groups showed a significant decrease in comparison with control group. 3. The area of the insulin granuls in islet of rats in Bikam-who4 Chekamhnnghyuluiyiin-tang and Chekambohyulonrin-tong group showed a significant decrease in comparison with control group. According to above these results, Bikam-whan. Cheknmhonghyuluiviin-tnng and Chekambohyulonrin-tang have shown to be capable of cure and prevent for an obesty.

Objects : We had experiment with Chekamhanghyuluiiin plus Zeae Stigma in order to find effects on cure and prevention of the obesity. Methods : We fed control group rats high fat diet(Harlan, U.S.A.) and administered normal saline during 8 weeks, and experimental group rats high fat diet(Harlan, USA) and administered Chekamhanghyuluiiin plus Zeae Stigma extract during 8 weeks. Results : 1. The body weight of all experimental group decreased than control group. 2. The average size of the epididymal fat cells in all experimental group decreased than control group but only 7CH5 group showed statistically significant difference with control group. 3. The fat drops area of hepatic lobule in all experimental group decreased than control group but 4CH10, 7CH5 and 7CH10 group showed statistically significant difference with control group. 4. The area % of the insulin granules in Langerhans islet of 7CH5 and 7CH10 group showed difference with control group. Conclusion : Chekamhanghyuluiiin plus Zeae Stigma could be effective for the obesity.

심내막상 결손증 : 3예 보고 A Report of 3 Cases

심내막상 결손증(Endocardial cushion defect)은 선천성 심장기형의 약 3% 정도를 차지하는 비교적 드문 질환이며, 복잡한 동반 기형을 나타내고 있고, 그 수술 방법에 있어서도 많은 논란이 있으며, 사망율도 높은 질환인 바, 저자들은 지난 1년간에 3예의 본 증례에 대한 수술 치험에서 술후 3∼6개월간에 만족할만한 임상적 추적 결과를 얻었기에 보고하는 바이다. In Pusan Paik Hospital, Inje Medical College, there were two complete endocardial cushion defects (ECD) and a partial one, which were successfully repaired in 1986. In a patient of partial ECD, primum atrial septal defect was closed with pericardial patch. In a patient of complete ECD, associated with secundum ASD, pulmonary stenosis and Down's syndrome, the atrial and ventricular septal defects were closed separately with bovine pericardium and Dacron patches respectively, and then pulmonary stenosis was relieved by transannular patch widening in addition to valvotomy and infundibulectomy. In another patient with complete ECD, small interventricular communication was closed with simple suture with pledget, and primum ASD closed with pericardial patch. In all patient, the mitral clefts were closed with three or four 5-0 Prolene interrupted sutures. Transient A-V dissociation developed postoperatively in two patients, and heart failure in complete ECD with Down's syndrome was overcome with medical treatment.

We had experiment with five herb in order to find cure and prevention effects on the obesity. We fed control group rats high fat diet(Harlan, USA) and administered normal saline during 8 weeks, and experimental group rats high fat diet(Harlan, USA) and administered five herbal extract during 8 weeks. The results were as follows; 1. The body weight of all experimental group decreased than control group. 2. The average size of the epididymal fat cells in all experimental group decreased than control group but only Zeae Stigma and Crataegi Fructus group showed statistically significant difference with control group. 3. The fat drops area of hepatic lobule in all experimental group decreased than control group but Plantaginis Semen, Zeae Stigma and Crataegi Fructus group showed statistically significant difference with control group. 4. The area % of the insulin granules in Langerhans islet of all experimental group showed no difference with control group.

황견을 이용한 심장이식술 및 심장관류 용액의 심보존 효과 비교

한국산 황견을 이용한 심장이식 수술을 11회 시행하여 수술 전후의 혈역학적인 변화를 관찰하고 수술 수기의 숙달 및 준비과정의 문제점을 파악하는데 도움이 되고자 하였다. 심장관류 용액의 심보존 효과를 비교 관찰하기 위하여 각기 다른 2 종류의 심장 정지액 주입으로 황견의 심장을 정지시킨 후 심장을 절제하여 0℃에서 24시간 보존하면서 좌심실 근육조직 절편을 채취하여 고속 액체 크로마토그라피를 이용한 방법으로 ATP, ADP, AMP를 측정하였다. University of Wisconsin(이하 UW) 용액은 상온 단순 심정지 방법으로, St. Thomas 용액은 인공 심폐기를 이용한 전신 저체온법을 함께 사용하였다. ATP 농도는 UW 용액에서 St. Thoms 용액보다 2, 4, 8, 12 시간대에서 높게 유지되었으며 ADP 농도도 역시 UW 용액이 4시간대 이후에 높은 수치를 보였다. AMP 농도는 두용액 사이에 의미있는 차이를 보이지 않았다. Eleven cases of orthotopic cardiac transplantations were performed using 33 mongrel dogs. The main causes of death were pulmonary edema and bleeding at anastomotic site. To compare the efficacy of cardiac preservation, purine metabolites(ATP, ADP, AMP) were examed during 24 hours of cold storage(0℃) of the mongrel dog hearts after using two different types of cardioplegic solutions. The hypothermic arrest with total cardiopulmonary bypass method was employed in St. Thomas solution preservation cases. Left ventricular myocardial specimens were obtained after 0.5, 2, 4, 8, 12, and 24 hours at 0℃, and stored at -75℃ deep freezer until analysis. Specimens were analyzed for levels of adenine nucleotides by high performance liquid chromatography. The ATP content in the UWS (University of Wisconsin solution) group tends to be higher than that of the combined hypothermic arrest STS(St. Thomas solution) group after 2, 4, 8, and 12 hours of preservation respectively. The ADP content in the UWS group was higher than that of the STS group at 4, 8, 12, and 24 hours. The AMP contents did not change significantly in the two groups. These results strongly suggests that although UW solution did not provide satisfactory long-term preservation of heart, UWS group is better than STS group with hypothermic arrest method.

심장 이식 2예 보고

97년 9월 이후 2예의 동소 심장이식수술을 시행하였는데 대상 환자들은 남녀 각 1명, 나이는 여자는 55세, 남자는 23세이고, 체표면적은 각각 1.54m2, 1.62m2였다. 대상환자들은 확장성 심근증과 제한성 심근증 환자로 NYHA IV 정도의 심한 심부전증 증상이 있었다. 수술 후 면역억제제로는 사이클로스포린, 아자티오프린, 스테로이드를 사용하였다. 첫 번째 환자에서는 수술 10일 후 심근조직검사에서 ISHLT 기준의 I 단계의 거부반응을 보였고, 이후의 조직검사에서는 거부반응이 없었으며 술후 2개월째에 대상포진, 8개월째에는 구강 칸디다증으로 입원 치료를 받았다. 두 번째 학자에서는 술후 2일째 사이클로스포린에 의한 신독증을 보였으나, 순조롭게 회복되어 퇴원하였다. Since September 1997, we have performed two cardiac transplantations in one dilated cardiomyopathy and one restrictive cardiomyopathy. The patients were a 55-year-old female and a 23-year-old male, and their body surface area(m2) were 1.51 and 1.62 respectively. Both patients were suffered from dyspnea as NYHA functional class IV. Postoperative immunosuppression was accomplished with cyclosporine, azathioprine and predinisolone. In case 1, there was an ISHLT grade IA rejection in endomyocardial biopsy at postoperative 9th day which disappeared at the tome of next biopsy 20 days later. The patients were treated for herpes zoster and oral candidiasis at postoperaive 2 months and 8 months, respectively. In case 2, nephrotoxicity related to cyclosporine developed 2 days after the surgery, but was soon resolved and the patient was discharged in good condition.
