<P>We evaluated the associations between potentially functional variants in a comprehensive list of cancer-related genes and lung cancer in a Korean population.</P><P>A total of 1969 potentially functional single nucleotide polymorphisms (SNPs) of 1151 genes involved in carcinogenesis were evaluated using an Affymetrix custom-made GeneChip in 610 nonsmall cell lung cancer patients and 610 healthy controls. A replication study was conducted in an independent set of 490 cases and 486 controls. 68 SNPs were significantly associated with lung cancer in the discovery set and tested for replication.</P><P>Among the 68 SNPs, three SNPs (corepressor interacting with RBPJ 1 (<I>CIR1</I>) rs13009079T>C, ribonucleotide reductase M1 (<I>RRM1</I>) rs1465952T>C and solute carrier family 38, member 4 (<I>SLC38A4</I>) rs2429467C>T) consistantly showed significant associations with lung cancer in the replication study. In combined analysis, adjusted odds ratio for <I>CIR1</I> rs13009079T>C, <I>RRM1</I> rs1465952T>C and <I>SLC38A4</I> rs2429467C>T were 0.69, 0.71 and 0.73, respectively (p=4×10<SUP>−5</SUP>, 0.01 and 0.001, respectively) under the dominant model. The relative mRNA expression level of <I>CIR1</I> was significantly associated with rs13009079T>C genotypes in normal lung tissues (ptrend=0.03).</P><P>These results suggest that the three SNPs, particularly <I>CIR1</I> rs13009079T>C, may play a role in the pathogenesis of lung cancer.</P>

We report a 19-year-old man with hyperkeratosis of nipple and areola which is a part of an epidermal nevus. He did not have a history of endocrinopathy or synthetic steroid therapy. The lesion, a fingertip sized verrucous dark browncolored macule on left upper back, had spread into left-anterior chest wall and had involved nipple and areola to make them hyperkerstotic and verrueous. Histologic section from the on left areola showed hyperkeratosis, papillomatosis, acanthosis and increased pigmentation in the basal cell layer. Chernical peeling with 30% trichloroacetic showed favorable response. (Kor J Dermetol 28(l): 90 83, 1900)

Purpose: This study was conducted to investigate whether polymorphisms of genes involved in immune checkpoints can predict the clinical outcomes of patients with advanced stage non-small cell lung cancer (NSCLC) after 1st line paclitaxel-cisplatinchemotherapy. Experimental Design: A total of 379 patients with advanced stage NSCLC were enrolled. Twelve single nucleotide polymorphisms (SNPs) of PD-1, PD-L1, and CTLA-4 genes were selected and genotyped. The associations of SNPs with chemotherapy response and overall survival (OS) were analyzed. Results: Among the 12 SNPs investigated, PD-L1 rs2297136T>C and rs4143815C>G were significantly associated with clinical outcomes after chemotherapy. The rs2297136T>C was significantly associated with both better chemotherapy response and better OS (adjusted odds ratio [aOR] = 1.82, 95% CI = 1.19-2.77, P = 0.006; adjusted hazard ratio [aHR] = 0.76, 95% CI = 0.61-0.94, P = 0.01), and the rs4143815C>G had a significantly better response to chemotherapy (aOR = 1.42, 95% CI = 1.05-1.93, P = 0.02) under additive model for the variant alleles. Consistent with the individual genotype analyses, rs2297136C-rs4143815G haplotype harboring variant alleles at both loci was significantly associated with better chemotherapy response and OS compared with combined other haplotypes (aOR = 1.92, 95% CI = 1.27-2.91, P = 0.002; aHR = 0.76, 95% CI = 0.61-0.94, P = 0.01, respectively). Conclusions: PD-L1 rs2297136T>C and rs4143815C>G polymorphisms may be useful for the prediction of clinical outcome of 1st line paclitaxel-cisplatin chemotherapy in NSCLC.

The purpose of this study was to find the difference in gait patterns when elderly and young people walk by analyzing COP, Gait Line, Foot pressure pattern, and ensuring the original biomechanics technology of developing high performance footwear for the elderly. The subjects who took part in the test consist of 20 elderly people and 20 young people. The physical features of the elderly people that were recruited for the study are as below: 20 healthy male subjects(elderly people) with an average age of 75.43 yrs(S.D 6.46 yrs), weight of 68.10 kg(S.D 0.94 kg) and a height of 168.65 cm(S.D 1.47 cm). Foot pressure pattern data was collected using a EMED-AT system(Novel Gmbh, Germany) operating at the 50 Hz during walking. The results are as follow: COP route of the elderly leans to lateral compared to the young, and Gait Line from heel to toe is not clear and laterally curved. At the same time, a contact are aonthe midfoot is high compared to the young, and maximum force of the forefoot is low. As a result of analysis, in order to develop high performance footwear for the elderly, it is necessary to develop lasts and soles reflecting the elderly`s gait patterns.

Background: Evidence indicates that the let-7 microRNA (miRNA) may be a prognostic factor in lung cancer. Genetic variation in miRNA precursors could influence the processing and expression of miRNAs, which could affect the prognosis of lung cancer. We aimed to investigate the impact of single nucleotide polymorphisms (SNPs) of pri-let-7 on the prognosis of non-small cell lung cancer (NSCLC). Methods: Seven hundred sixty-one patients with surgically resected NSCLC were included. The four SNPs (pri-let-7a-2 rs1143770 and rs629367, pri-let-7a-1 rs10739971, and pri-let-7f-2 rs17276588) were genotyped using a polymerase chain reaction-restriction fragment length polymorphism assay. The genotype associations with overall survival (OS) and disease-free survival (DFS) were evaluated. Results: Of the four SNPs analyzed, the rs1143770C>T was identified to be significantly associated with OS and DFS. The rs1143770 CT or TT genotype exhibited a significantly better OS and DFS compared with the rs1143770 CC genotype (adjusted hazard ratio for OS = 0.67, 95% confidence interval = 0.49―0.91, P = 0.01 and adjusted hazard ratio for DFS = 0.74, 95% confidence interval = 0.58―0.95, P = 0.02). Conclusions: This observation indicates that the pri-let-7a-2 rs1143770C>T may have a prognostic impact on surgically resected NSCLC.

Background: Recently, many institutions have acquired multi-detector computed tomography (MDCT) systems. This made it easier and more convenient to use MDCT as a initial diagnostic modality for hemoptysis. The purpose of this study was to evaluate the usefulness of MDCT before bronchoscopy and/or bronchial arterial embolization (BAE) for hemoptysis. Methods: We studied a total of 125 patients with hemoptysis who underwent, between 2006 and 2008, MDCT in a routine protocol before bronchoscopy and/or BAE. One hundred two patients underwent bronchoscopy and 29 patients underwent BAE. We compared the usefulness of MDCT and bronchoscopy for detecting the bleeding site and identifying the cause. We also evaluated our ability, using MDCT, to detect instances where the bronchial artery caused hemoptysis. Results: The rate of detection of a bleeding site was 75.5% on MDCT and 50.9% on bronchoscopy. MDCT and bronchoscopy detected the bleeding site in agreement in 62.7% of patients. MDCT alone found the bleeding site in 27.5% of cases. MDCT identified the cause of hemoptysis in 77.5% and bronchoscopy in 11.8%. In 29 patients who underwent BAE, we detected a total of 37 hypertrophied bronchial arteries that were causing hemoptysis. Of 37 bronchial arteries, 23 (62.2%) were depicted on MDCT. Conclusion: MDCT is superior to bronchoscopy for detecting the bleeding site and identifying the cause of hemoptysis. MDCT can also predict the side of affected bronchial artery with depiction of hypertrophied bronchial artery and localizing the bleeding site. Doing MDCT before bronchoscopy and BAE can provide a guideline for the next step.

Linear Angiokeratoma Circumscriptum Associated with Hemangiectatic Hypertrophy Min Seok Song, M.D., Yoo Deuk Lee, M.D., Soo II Chun, M.D. Department of Dermatology, Yonsei University College of Medicine Seoul, Korea A 28-year-old male presented with skin lesions over the left upper limb progressively increasing in size and number since birth. Physical and dermatologic examination revealed hemorrhagic papules and coalesced plaques with hyperkeratotic surface in linear arragement onthe left entire limb, and engorged superficial veins on the left arm. Two nevus flammeus lesions were found at the right hypochondriac and left lumbar region. The size of the left arm and hand was larger than the right though the length of both arm was equal. Histopathology showed the findings of angiokeratoma. Radiographic findings revealed no bony abnormality. Venogram and angiogram showed prominent venous system and increased perfusion of affected site. A diagnosis of linear angiokeratoma circumscriptum associated with hemangiectatic hypertrophy was perferred since there were no bony lesion of the classic Klippel-Trenaunay syndrome. <P>A nonsense mutation in cereblon (<I>CRBN</I>) causes a mild type of mental retardation in humans. An earlier study showed that CRBN negatively regulates the functional activity of AMP-activated protein kinase (AMPK) in vitro by binding directly to the α<SUB>1</SUB>-subunit of the AMPK complex. However, the in vivo role of <I>CRBN</I> was not studied. For elucidation of the physiological functions of <I>Crbn</I>, a mouse strain was generated in which the <I>Crbn</I> gene was deleted throughout the whole body. In <I>Crbn</I>-deficient mice fed a normal diet, AMPK in the liver showed hyperphosphorylation, which indicated the constitutive activation of AMPK. Since <I>Crbn</I>-deficient mice showed significantly less weight gain when fed a high-fat diet and their insulin sensitivity was considerably improved, the functions of <I>Crbn</I> in the liver were primarily investigated. These results provide the first in vivo evidence that Crbn is a negative modulator of AMPK, which suggests that <I>Crbn</I> may be a potential target for metabolic disorders of the liver.</P>
관절경적 정복술 및 경피적 유관나사 고정술을 이용한 슬개골 종 골절의 치료

A variety of surgical modalities for fractures of patella have been described. We used arthroscopic reduction and percutaneous cannulated screw fixation for six cases of longitudinal fracture of patella. Ages of the patients ranged from 25 to 33 years. The postoperative regimen was one week long leg splint for reducing the pain, followed by continuous passive range of motion exercise of the knee including active one and quadriceps strengthening exercise until the full range of motion was gained, with progressive partial to full weight bearing with crutches. The follow-up period was from 12 to 54 months. Results were assessed subjectively and objectively with retrograde study. The full range of knee motion was recovered from 20 to 35 days postoperatively. The radiographic bone union was achieved from 31 to 42 days. And all patient had good results according to Lysholm and Gillquist scoring system. We had no experience of complication except one which is prominence of screw end. So, we believed that the arthroscopic reduction and percutaneous cannulated screw fixation for longitudinal fractures of patella is the useful surgical method.
