      • Mito-Tempo Can Protect Against the Allergic Airway Inflammation through Reducing Mitochondrial ROS Generation

        Oxidative stress is well known to be implicated in the development of asthma. The mitochondrial respiratory chain is a major site of intracellular reactive oxygen species (ROS) generation and, at the same time, an important target for the damaging effects of ROS. Mito-Tempo is a specific mitochondrial ROS inhibitor and it is known to be associated with opening of mi-tochondrial permeability transition pore and inhibition of cell necroptosis or apoptosis. However, there is little information on the protective effects of Mito-Tempo on the inflammatory airway disorders including bronchial asthma and its acute exacerbation. We investigate the effects of Mito-tempo on the allergic airway inflammation and hyperresponsiveness using the mice sensitized with OVA and LPS and then challenged with OVA (OVALPS-OVA mice). The OVALPS-OVA mice showed the typical features of neutrophilic asthma; increased airway inflammatory cells, the pathologic changes, the increased levels of Th2 cytokines in lungs of OVALPS-OVA mice, increased mitochondrial ROS generation, and increased bronchial hyperresponsiveness. Interestingly, we found that in OVALPS-OVA mice, Mito-Tempo, a novel mitochondrial targeting agent significantly reduced the increases in inflammatory cytokines, mitochondrial ROS generation, airway inflammation, and bron-chial hyperresponsiveness. These findings indicate that mitochondrial dysfunction including oxidative damage may be im-plicated in the pathogenesis of bronchial asthma and provide the therapeutic potential of a mitochondrial targeting agent, Mito-Tempo, for bronchial asthma.

        국내 병원 별 방사선치료의 진료 구조 현황

        목 적:국내 방사선치료에 대한 전국현황의 통계를 1997년을 중심으로 한 병원 별 진료 장비, 인력현황을 조사하고 그에 대한 연간 신환자수의 분포를 검토하여 그 결과를 선진국의 예와 비교함으로써 현재의 국내 방사선치료의 진료 수준을 분석하고자 하였다. 대상 및 방법:1998년에 치료방사선과가 있는 전국의 42개 병원을 대상으로 설문조사를 통해 자료를 수집하였다. 통계자료는 1997년 1월 부터 12월 까지 총 1년으로 하였고 통계처리는 마이크로소프트사의 Excel 프로그램을 이용하였다. 결 과:1997년에 국내에서 42개의 병원이 71대의 외부 방사선치료장치, 100명의 치료방사선과 전문의, 26명의 의학물리학자, 205명의 방사선사가 있었고 연간 19,773명의 신환을 치료하였다. 전국 42개 병원의 89%는 최소한 6 MeV이상의 선형가속기를 보유하였고 95%는 모의 조사장치를 소유한 반면 5%는 모의 조사장치를 가지고 있지 않았다. 컴퓨터 치료계획을 할 수 있는 장비를 소유한 병원은 91%였고, 83%는 정도관리 지침서를 가지고 있다고 응답하였다. 병원별 치료방사선과 의사가 단지 1명인 병원이 36%이었으며 의학 물리학자가 없는 곳이 38 %였다. 한 병원당 년간 환자수 분포의 중앙값은 348명, 기계당 환자수의 중앙값은 263명, 방사선종양학과 전문의당 환자수의 중앙값은 171명, 방사선사당 환자수의 중앙값은 81명, 기계당 방사선사의 중앙값은 3명이다. 결 론:국내의 치료방사선과를 보유한 병원의 수, 방사선치료를 받는 환자수 및 치료장비수는 일본 및 미국과 비교하여 인구비례로 계산해 볼때 매우 적은 편이다. 치료장비면에서는 선형가속기, 모의 조사장치, 컴퓨터 치료계획시스템의 수준이 미국과 비교하여 크게 차이가 없었다. 의사, 방사선사에 대한 환자수의 부하(loading)는 미국과 비교하여 큰 차이는 없었다. 그러나 인력면에서 의학물리학자가 없는 병원의 수가 상당히 많은 관계로 이의 보완을 위해서 미국과 같은 part time 제도의 운영도 해 볼 수 있을 것이다 Purpose:To measure the basic structural characteristics of radiation oncology facilities in Korea during 1997 and to compare personnel, equipments and patient loads between Korea and developed countries. Method and Materials:Mail serveys were conducted in 1998 and data on treatment machines, personnel and performed new patients were collected. Responses were obtained from the 100 percent of facilities. The consensus data of the whole contry were summarized using Microsoft Excel program. Results:In Korea during 1997, 42 facilities delivered megavoltage radiation theraphy with 71 treatment machines, 100 radiation oncologists, 26 medical physicist, 205 technologists and 19,773 new patients. Eighty nine percent of facilities in Korea had linear accelators at least 6 MeV maxium photon energy. Ninety five percent of facilities had simulators while five percent of facilities had no simulator. Ninety one percent of facilities had computer planning systems and eighty three percent of facilities reported that they had a written quality assurace program. Thirty six percent of facilities had only one radiation oncologist and thirty eight percent of facilities had no medical physicists. The median of the distribution of annual patients load of a facility, patients load per a machine, patients load per a radiation oncologist, patients load per a therapist and therapists per a machine in Korea were 348 patients per a year, 263 patients per a machine, 171 patients per a radiation oncologis, 81 patients per a therapist, and 3 therapists per a machine respectively. Conclusion:The whole scale of the radiation oncology departments in Korea was smaller than Japan and USA in population ratio regard. In case of hardware level like linear accelerators, simulators and computer planning systems, there was no big diffrences bewteen Korea and USA. The patients loads of radiation oncologists and therapists had no significient differences as compared with USA. However, it was desirable to consider the part time system in USA because there were a lot of hospitals which did not employ medical physicists.

        식도암의 방사선치료 성적

        목 적 : 수술을 시행하지 않은 식도암 환자에서 방사선치료 후 생존율 및 예후 인자를 파악하여 향후 치료 방침 결정에 도움이 되고자 한다. 대상 및 방법 : 1992년부터 1996년까지 원자력병원 방사선종양학과에서 근치적 또는 고식적 목적으로 치료를 받은 식도암환자를 대상으로 하여 후향적 분석을 시행 하였다. 수술 또는 방사선치료 후 재발한 환자 및 방사선조사를 40 Gy 미만으로 받은 환자를 제외한 40명의 환자를 대상으로 하였다. 이 중 남자는 35명, 여자는 5명이었다. 2 명의 환자는 생검을 시행하지 않았고 그 외 환자는 모두 편평 상피암이었다. AJCC 1997 병기별 분포는 I 1명, ⅡA 6명, ⅡB 6명, Ⅲ 10명, ⅣA 12명, ⅣB 4명 이고 병기를 알 수 없는 경우가 1명이었다. 보조 항암화학요법을 시행한 환자는 모두 10명이었다. 조사선량의 범위는 40∼60 Gy 이고 중앙선량은 59.4 Gy 였다. 결 과 : 전체 환자의 중앙생존기간은 6.5개월이고 1년 생존율은 28.3%이다. 연령 및 병변위치, 방사선량, 항암화학요법 추가에 관한 생존율의 차이는 관찰되지 않았다. 각각의 병기에 따라 생존율의 차이는 관찰되지 않아 임상병기는 예후에 큰 도움이 되지 않았다. 그러나 병기 Ⅲ이내의 환자와 병기 Ⅳ이상의 환자 사이에는 통계학적으로 의미 있지는 않았지만 중앙생존기간이 각각 7.6개월 및 6.2개월로 차이를 보였다. 결 론 : 식도암의 생존율은 극히 불량하였다. 환자의 전신 상태 등을 고려하여 완치 및 고식적 목적을 구별하고 항암화학요법의 추가 및 스텐트(stent), 또는 근접 조사치료를 추가 함으로 생존율의 증가 및 고식적 치료 목적을 달성하여야 할 것이다. Purpose : To determine treatment protocol for inoperable esophageal cancer patients, we evaluated survival rate and prognostic factors. Materials and Methods : We evaluated esophageal cancer treated by curative or palliative aim in KCCH from 1992 to 1996, retrospectively. Recurrent or underdose case below 40 Gy were excluded. The number of male and female were 35 and 5, respectively. Thirty-eight patients were squamous carcinoma and 2 patients were not biopsy proven. Ten patients were treated with radiation therapy and chemotherapy. Median dose of radiation therapy was 59.4 Gy and the range was 40∼60 Gy. Results : The median survival is 6.5 months and 1-year survival rate was 28.3%. Age, location, radiation dose and chemotherapy were not significant prognostic factors. Median survivals of patients with below stage III and over stage IVA were 7.6 and 6.2 months respectively, but it is not significant. Conclusion : The survival for esophageal cancer is very poor. For patients with curative aim, chemotherapy must be considered. For patients with palliative aim, short-term external beam radiation therapy and/or brachytherapy must be considered.

      • Flashlight-Induced High-Performance Stretchable Cu conductoers

        Herein, we report a novel stretchable Cu conductors with significant conductivity and elasticity, which was allowed by the flash-induced multiscale interface interactions between Cu and a polymer. Microscale randomly wrinkled Cu was demonstrated on an elastomer by irradiating single pulse of millisecond flashlight, enabling stretchability of Cu conductor to exceeding 20% in all directions. Simultaneously, the enhanced adhesion force of the nanoscale interlocked interface between the Cu nanoparticles and the elastomeric substrate increased the reliability and elasticity of Cu. Finally, the fabricated crumpled Cu conductor exhibited outstanding electrode conductivity of 37 kS cm<SUP>-1</SUP> despite severe stress caused by the motion of human wrist, which was successfully utilized for wireless near-filed communication system.

      • Further Work on Characterizing a Synthetic Wastewater Solution

        A simple characterization procedure was further tested for removal of dissolved organic carbon (DOC) using granular and powdered activated carbons, The working solution was prepared by dissolving organic and inorganic matters in water. Several integral equilibrium experiments were conducted with different initial concentrations of DOC while keeping the concentration ratio between species remained unchanged. These experiments enable us to get information on the distribution function of species in the solution. In this study, a binomial distribution of DOC fraction in terms of the Freundlich coefficient, k, was obtained so that the solution encountered can be described by a finite number of pseudospecies, identified with a certain k value. Predictions based on this characterization procedure were in good agreement with experimental data.

        Vancomycin과 Heparin의 Compatibility 시험

        Vancomycin (VANC), a glycopeptide antibiotic used for the treatment of patients with infections caused by Staphylococcal species and Streptococcal species, is incompatible with heparin (concentration > 1.0 U/㎖). Even though heparin concentration in TPN solutions administered in AMC (Asan Medical Center) is 0.25 U/㎖, beyond 1.0 U/㎖, we have stopped administration of TPN during the VANC infusion time because of the absence of experimental sources. So, we studied the compatibility of VANC and heparin in TPN. We observed the variations before and after admixing VANC and heparin (in TPN) by examining with the naked eye under a microscope of 400 magnifications, measuring the concentration of VANC and the heparin activity. RESULT : ① We could not distinguish the variations under the microscope (× 400). ② As a result of measuring the concentration of VANC by HPLC (UV detector 225㎚, C_(18) Column, Mobile phase acetonitrile : 0.01 M 1-heptane sulfonic acid = 12 : 88, Flow rate 2.0 ㎖/min), there is no significant difference between before and after (% change of peak area from standard sample : 102.61%, 101.03% respectively / P-value = 0.464). ③ However, according to the results of measuring the heparin activity by APTT, anticoagulant action of heparin in TPN was reduced by VANC. Therefore, the simultaneous administration of TPN (heparin 0.25 U/㎖) and VANC via a Y-site is judged to be inadequate.

        Nonpalpable supraclavicular lymph nodes in lung cancer patients: preoperative characterization with 18F-FDG PET/CT.

        <P>OBJECTIVE: Our purpose was to evaluate the usefulness of integrated 18F-FDG PET/CT in the detection and characterization of nonpalpable supraclavicular lymph node metastasis in patients with the initial diagnosis of lung cancer. SUBJECTS AND METHODS: This study was conducted from May 2005 to May 2006 and included 32 consecutively registered lung cancer patients in whom supraclavicular lymph nodes were not palpable but were identified on contrast-enhanced CT or exhibited increased FDG uptake on integrated PET/CT. Three patients had bilateral nodes, for a total of 35 nodes in the 32 patients. Results of cytologic analysis of a specimen obtained with sonographically guided fine-needle aspiration (n = 27), normal initial and follow-up sonographic findings (n = 3), and no change in the size of supraclavicular lymph nodes on follow-up sonography (n = 2) were the reference standards. The presence of supraclavicular lymph node metastasis was determined with integrated PET/CT (uptake greater than that of surrounding tissue) and contrast-enhanced CT (node short-axis diameter of 5 mm or more). The diagnostic efficacies of these methods in the detection of supraclavicular lymph node metastasis were compared. RESULTS: Supraclavicular lymph node metastasis was diagnosed cytologically in 12 (34%) of 35 lesions. The diagnostic accuracies of integrated PET/CT and contrast-enhanced CT in the detection of supraclavicular lymph node metastasis were 71% and 66%, respectively; the difference was not statistically significant. Although the difference was not statistically significant, the sensitivity (92%) and negative predictive value (93%) of integrated PET/CT were higher than those of contrast-enhanced CT. CONCLUSION: Because of its high sensitivity and negative predictive value, integrated PET/CT is useful in the detection and characterization of nonpalpable supraclavicular lymph nodes in lung cancer patients.</P>

        The diagnostic role of $^{18}F$-fluorodeoxyglucose ($^{18}F$-FDG) positron emission tomography (PET)/computed tomography (CT) for gallbladder small cell carcinoma has not been reported. A knowledge of the imaging characteristic features of this malignancy can be useful. Here we report a rare case of a patient who had various diagnostic imaging modalities, including $^{18}F$-FDG PET/CT.

        최근 스마트폰의 대중화로 사용자들에게 개인화된 다양한 서비스들이 제공되고 있다. 스마트폰은 다양한 센서 디바이스들을 탑재하고 있어 상황인지를 하기에 적합한 환경을 가지고 있다. 이에 본 연구는 규칙기반 추론엔진을 이용한 스마트폰에 적용할 수 있는 상황인지 서비스 모델을 제안한다. 제안하는 모델은 스마트폰의 다양한 센서를 사용하여 사용자의 상황인지를 통해 폰의 상태를 변경시켜준다. 상황 변화를 감지하기 위해 사용되는 데이터로는 사용자의 입력이 들어간 일정정보와 실시간으로 변하는 시간, GPS, Bluetooth 값으로 부터 규칙기반 추론엔진을 이용하여 상황을 추론하며 그 상황에 맞도록 폰의 상태를 변화한다. 상황인지는 센서로부터 들어오는 실시간 정보와 저장되어 있는 일정정보와의 비교를 통해 하위 컨텍스트를 추출하며 추론엔진을 거쳐 상위 컨텍스트를 생성하게 된다. 제안하는 방법은 회의 등 벨소리가 울리지 말아야 할 장소에서의 모드 전환 서비스에 적용하여 타당성을 증명하였다.
