A 28-year-old primigravida at 33 weeks gestation was referred to our hospital because of aggravation of seizures which occurred intermittently for the past 2 years. A brain magnetic resonance imaging scan taken at 33 weeks gestation showed a large brain tumor (7 × 5 × 4 cm) in the left high frontal lobe. She had a cesarean section under general anesthesia at 38 weeks and 1 day gestation. General anesthesia was induced with thiopental 250 mg, succinyleholine 75 mg and lidocaine 60 mg after a vecuronium 0.8 mg intravenous injection. The hyperventilation and mannitol used to reduce increased intracranial pressure (ICP) in this patient may be harmful and detrimental to the fetus before delivery. Achieving both ICP control and fetal well being at the same time during a cesarean section can be challenging to anesthesiologists. We report the anesthetic management of a cesarean section in a patient with a large brain tumor. (Korean J Anesthesiol 2002; 43: 515~519)
2-Methylisoborneol(2-MIB)제거를 위한 산화 및 흡착공정의 특성

One of the Musty and earthy smell compounds in raw water is generally attributed to 2-methylisoborneol (2-MIB). It is well known that activated carbon and oxidants such as O_3, ClO_2 are effective ways to control 2-MIB. In isotherm equilibrium experiments, 2-MIB in distilled water was much more adsorbed to the activated carbon(A/C) than raw water containing dissolved organic carbon (DOC). The Freundlich constants(k) of distilled water and raw water were 3.36 and 0.049, and 1/n values were 0.80 and 0.42, respectively. The 2-MIB residual rate were Y = e^(0.55_x)~ e^(-0.54_x) with Ozone(O_3) dose by 5 minutes contact time at the 241 and 353 ng/L initial concentrations. The 2-MIB residual rate were Y = e^(-0.32_X)~ e^(-0.35_X) with Chlorine dioxide(ClO_2) dose by 15 minutes contact time at the 89 and 249 ng/L initial concentrations. 2-MIB was decreased from 1911 ng/L to 569ng/L by post-ozonation(70%removal efficiency) and removal efficiencies of 2-MIB by the following 4 kinds Granular Activated Carbon(GAC) process such as coal base, coconut base, wood base and zeolite+carbon base were 95.8, 89.5, 88.4, and 93.7% respectively.

Since the first successful replantation operation, several reports have been recorded replantated finger survival with high success rates. But ten and nine digits amputations are not common. It is even more unusual to have all digits suitable for replantation. 4 cases are presented where replantation of ten and nine digits were done at the department of PRS. Guro Hospital, Korea University from March 1987 to Jun 1988. In these cases, the patients were young and healthy, all of ten and nine digits were sharply amputed near the proximal phalanx, so a replantation team of 8 surgeons in alternating fashion replanted all ten and nine digits over 30 hours and each patient received 28 pints of whole blood during the procedure. Under general anesthesia, replantatin of both started simultaneously. All digits survived completely except partial necosis of right little finger in case 1. The ischemic time of each digit ranged from 4 hours and 45 minutes to 31 hours as shown in table 5. The long ischemic time did not criticaly affected the survival of amputated digit. Follow-up was from 12 months to 20 months postoperatively. The functional evaluation revealed as follows: 1. The grasping power ranged 13 lb to 65 lb(average;39.7lb) 2. The pinching power with index finger ranged 5 lb to 15 lb(average:10.2 lb) 3. The palm to pulp distance ranged 0 cm to 6 cm.(average;2.2cm) 4. The two point discrimination ranged 2.5mm to 22mm.(average;7.8mm) 5. The shape of digits were acceptable aesthetically. 6. All 4 patients were able to use both hands well in all activities of daily living.
Enhanced Sialylation by Knockout of Sialidase Genes in CHO Cells Using CRISPR/Cas9

Sialic acids present on non-reducing terminal of glycan is important for prolonged in vivo half-life of glycoproteins. Many efforts have been made to increase the sialic acid content in the manufacturing of therapeutic glycoproteins because glycoproteins containing non-sialylated glycans do not show effective in vivo efficacies owing to a short half-life. It was reported that knock down of sialidase expression using the RNA interference (RNAi) technology enhanced protein sialylation in CHO cells which are the most widely used for therapeutic glycoprotein production [1]. However, knock down strategy did not completely shut down the sialidase expression. In this study, we performed knockout of sialidase genes in CHO cells by using the CRISPR/Cas9 genome editing technology. Only three sialidases (Neu1, Neu2, and Neu3) were actively expressed in CHO cells when analyzing their expressions using real-time polymerase chain reaction. First, we disrupted Neu3 gene because it is located in the plasma membrane while Neu1 and Neu2 exist in lysosome and cytosol, respectively. After the transfection of vectors expressing Cas9 and sgRNAs, Neu1 gene-disrupted clones were selected by using the T7 endonucleases 1 (T7E1) assay and cell surface sialidase activity assay. Disruption of Neu1 gene did not affect the viability of CHO cells. The disruptions of Neu1 and Neu2 genes in CHO cells are currently in the progress.

Endocannabinoids can affect multiple cellular targets, such as cannabinoid (CB) receptors, transient receptor potential cation channel, subfamily V, member 1 (TRPV1) and peroxisome proliferator-activated receptor g (PPARg). The stimuli to induce adipocyte differentiation in hBM-MSCs increase the gene transcription of the CB1 receptor, TRPV1 and PPARg. In this study, the effects of three endocannabinoids, N-arachidonoyl ethanolamine (AEA), N-arachidonoyl dopamine (NADA) and 2-arachidonoyl glycerol (2-AG), on adipogenesis in hBM-MSCs were evaluated. The adipocyte differentiation was promoted by AEA whereas inhibited by NADA. No change was observed by the treatment of non-cytotoxic concentrations of 2-AG. The difference between AEA and NADA in the regulation of adipogenesis is associated with their effects on PPARg transactivation. AEA can directly activate PPARg. The effect of AEA on PPARg in hBM-MSCs may prevail over that on the CB1 receptor mediated signal transduction, giving rise to the AEA-induced promotion of adipogenesis. In contrast, NADA had no effect on the PPARg activity in the PPARg transactivation assay. The inhibitory effect of NADA on adipogenesis in hBM-MSCs was reversed not by capsazepine, a TRPV1 antagonist, but by rimonabant, a CB1 antagonist/inverse agonist. Rimonabant by itself promoted adipogenesis in hBM-MSCs, which may be interpreted as the result of the inverse agonism of the CB1 receptor. This result suggests that the constantly active CB1 receptor may contribute to suppress the adipocyte differentiation of hBM-MSCs. Therefore, the selective CB1 agonists that are unable to affect cellular PPARg activity inhibit adipogenesis in hBM-MSCs.

Radiofrequency ablation (RFA) is performed as an alternative to surgical resection for primary or secondary liver malignancies. Although RFA can be performed safely in most patients, early and late complications related to mechanical or thermal damage occur in 8-9.5% cases. Hemocholecyst, which refers to hemorrhage of the gallbladder, has been reported with primary gallbladder disease or as a secondary event associated with hemobilia. Hemobilia, defined as hemorrhage in the biliary tract and most commonly associated with accidental or iatrogenic trauma, is a rare complication of RFA. Here we report a case of hemocholecyst associated with hemobilia after RFA for hepatocellular carcinoma that was successfully managed by laparoscopic cholecystectomy.

Background: Previous reports with proximal junctional failure (PJF) included relatively young patients or deformity without sagittal imbalance. The present study focused on the two well-known risk factors for PJF, old age and severe sagittal imbalance. With these high-risk patients, the present study aimed to identify a strategy that could prevent PJF and to investigate whether the degree of correction would really affect the PJF occurrence. Methods: Patients who were ≥ 60 years of age and underwent long fusion (≥ 4) to the sacrum for severe sagittal imbalance (defined as pelvic incidence minus lumbar lordosis [PI–LL] ≥ 30°) were included. PJF was defined as a vertebral fracture at the uppermost instrumented vertebra (UIV) or UIV+1, failure of UIV fixation, myelopathy, or any need for proximal extension of fusion. Presumed risk factors were compared between the patients with and without PJF. Results: Total 146 patients (mean age, 68.4 years) with preoperative mean PI–LL of 46.8° were included. PJF developed in 39 patients (26.7%) at a mean of 18.1 months after surgery. Multivariate analysis showed that osteoporosis (odds ratio [OR], 2.812; p = 0.019) and UIV located below T10 (OR, 3.773; p = 0.010) were significant risk factors for developing PJF. However, the degree of correction did not affect PJF occurrence. Conclusions: The present study indicates that osteoporosis should be well corrected preoperatively and extending the fusion above T10 should be considered for severe imbalance in old patients. However, the amount of correction was not associated with PJF development.
Parasites of feral pigeon (Columba livia) by fecal examination from some areas in Seoul, Korea

This study was conducted to evaluate the prevalence of helminths parasites from collected fecal samples of 410 feral pigeons (Columba livia) in 3 different areas of Gwangjin-gu, Seongdong-gu and Dongdaemun- gu in Seoul from December, 2011 to July, 2012 in Korea. The fecal samples were examined through by the centrifugation method using formalin-ether solution. The overall prevalence of parasites in the pigeons was 29.27% (120/410). Two nematodes (Tetrameres spp. and Capillaria spp.) and one protozoa (Eimeria spp.) were identified. In the case of nematodes, Tetrameres spp. dominated numerically (58, 14.15%), followed by Capillaria spp. (10, 2.44%) and oocysts of Eimeria spp. were detected in 78 cases (19.02%). This investigation introduces the result of examination on the prevalence of parasites in feral pigeons from some areas in Seoul for the first time.
