Aortic transection or interruption is a rare condition which developed after an acute deceleration injury. Its occurrence depends on the location and direction of the force applied and is usually from motor vehicle accident or falling down. The exact incidence of aortic transection in trauma is not known but, when develops, only about 10-15% of the victims can survive and be transported to the hospital. Even in the survivors, majority of them will be fatal within a few days if a prompt diagnosis and surgical treatments are not made. Aggressive diagnostic work-up is recommended for the patients with high suspicious index, which would salvage the victims with this fatal condition. We report the experience of two cases of aortic transection or interruption following motor vehicle accidents.

Ssirum, our national sport required power and skill, makes our body robust and has a nation’s spirit in it. Also, the development of a nation is based on the peopie’s healthy mind and body, completed by the gymnastic activity. Therefore, Ssinun can be strechted itself as a pannational and public folk gymnastics. However, our Ssirum becomes gradually amused, professionalized, and exceedingly commercialized on the level of the folkgame according to the changes of the times. As a result of it, we seem to be forgetful of its value as an essential folk match. We should have the calling conscience to develop our cultural heritage on the basis of its essence. In this thesis, we suggest a new Satba and it’s holding before the match about the commonly called "the struggle of the Satba", argumented much during the match, to be part of the rational and systematic theory establishment. This study is based on the ground of the questionnaire to the athletes enlisted on the folk Ssirum committee. The content is summarized as follows. First, the folk Ssirum committee makes and controls a pair of the blue and red personal Satba for the enlisted athletes only, marks an athlet’s name according to the personal one, and draws a red line on the blue Satba and a blue one on the red by 1.5㎝ wide at the central line of the on left and right femoral region to clearly indicate the position of both athlet’s holding the Satba before the match. Secondly, when the committee enlists an athlete, he makes d Satba after weighing. And its size is by 114㎝ wide and 134㎝ plus the girth of the waist and the thigh long. Thirdly, they made a Satba suitable measuring its size, but they supplement its size in changing of the body class and reweighing per a quiter of half a year. Fourthly, the vice-umpire confirms to agree on athlete and its name specified on the Satba, and to agree the geight of th Satba and its line position to the central line of the left-right femoral region at the commence of the match, Fifthly, when holding Satba before the match, an athlete catches the thigh Satba first and the waist Satba next at the present practice, but he holdes the latter toward the abdomen first and the former next by the reverse order. Sixthly, when they finished all the matchs, the committee treturn and take charge of the Satba which are supplied to the atnlete after the match. In doing so, we could reduce psychological and practical fators caused by holding the Satba before the match, and enhance its value and import as a cultural heritage well-matched the aspect of the professionalized game an meeting the times. Also, we want to summarize the function of the Satba and its value as a symbolic meaning to do so, we think that it is possible to assciate the symbolic significance of our Taekeuk frag design and Jisinbalgi in the New Year manners with the Ssirum match. First, the circle of the Taekeuk frag design symbolizes the universe, which is equal to the circle in an arena. The blue and re line within the circle runs toward up and down, which symbolizes the meaning that winner or loser can be repeated and a permanent winner and loser can’t exist, I think. Also, the blue and red color is consistant with the same color Satba, which represents the harmony of power and skill. Second, the Jinsinbalgi among our New Year customs represents of expelling the minor demos to step down the chilliness of the earth. In this respect, the earth symbolizes the Yin as arena. We could understand the man and the man, doing Ssirum on it, as the Yang and the harmony of the Yin and the Yang. Therefore, we could have the significance when Ssirum makes a decision of the win or lose and it is conistent with the symbolic meaning of combination and symmetry as our proper naional sport. As mentioned above, we make an effort to proceed the practical game systematizing the size and control of Satba. We could enhance the value of our proper folk Ssirum if we had the correspondence between the Ssirum match the symbolic meaning of the Satba color.

진달래 꽃에 의한 Grayanotoxin 중독 3 례

Traditionally, the Rhododendron species has been used in gastrointestinal disorder or hypertension. Grayanotoxin exists in honey, flowers, pollen, and the nectar of the Rhododenron species. We experienced 3 cases of Grayanotoxin intoxication. The symptoms of intoxication were nausea, vomiting, hypotension, bradycardia, diplopia, dizziness, and chest discomfort. Generally, the treatment for Grayanotoxin intoxication is fluid resuscitation and injection of atropine sulfate. The patients who were intoxicated with Grayanotoxin were discharged without complication after supportive care.

시토크롬 C의 전기화학반응에 있어서 프로모터와 전해질과의 영향

The electrochemical reaction of cytochrome C varied with promotors and electrolytes. Among electron transfer carriers, 4-4' bipyridine(PyPy) showed good redox response using 20 mM phosphate solution as electrolyte and the case of Bis(4 pyridyl) bisulphide(PySSPy) as promotor showed same result using 0.2M Sodium cacodylate as electrolyte tested cyclic voltammetry method. Between electrodes, Au electrode is better behaviour than ITO electrode. 0.21M Tris, 0.24M Cacodylate acid showed the most effective electrochemical characteristics as electrolyte.

허혈 조건부여가 심근기능의 회복에 미치는 영향 : 적출 심장모델을 이용한 실험연구 Isolated heart experimental study

Background: Brief episode of coronary artery occlusion(i.e.,ischemic preconditioning) makes the heart more resistant to injury from a subsequent ischemic insult. Although a great deal of effort has been made in studying ischemic preconditioning, the underlying mechanism of ischemic preconditioning and its effect on hypothermic insult has not been elucidated. This study was performed to see whether ischemic preconditioning protects against the depression of cardiac contractility induced by hypothermic cardioplegic arrest/reperfusion. And recently, adenosine was known to have some correlation with the mechanism of preconditioning. If so, does this effect remain after the blockade of adenosine receptor by 8-phenyl theophyline? Method: Twenty-four Sprague-Dawley rat weighed 250-350g were used and divided into three groups. Rat hearts were removed rapidly, and each isolated heart paced with a rate of 180/min was perfused by modified Krebs-Hensleit buffer(KHB) solution on a Langendorff apparatus for an hour. After obtaining baseline data including left ventricular pressure(LVP), dp/dt, and coronary flow, cardiac arrest was induced by perfusion of 0℃ crystalloid cardioplegic(St Thomas) solution. After that, all hearts were stored in the same St Thomas solution at same temperature for 2 hours. In group Ⅰ(control group), the heart was reperfused by KHB solution. In group Ⅱ(preconditioning group), the heart was subjected to two 2-minute episode of global ischemia followed by 5 minute reperfusion with KHB solution(preconditioning) before cardiac arrest. In group Ⅲ(phenyl theophylline group), the heart was subjected to preconditioning procedure and 8-phenyl theophylline at 10μM in concentration was added to KHB solution at time of reperfusion. Observing parameter was obtained in each group at 10, 20, 40 and 60 minutes after starting reperfusion and compared statistically by use of one way ANOVA test(STASTICA, release 4.5). p-value less than 0.05 was considered significant. Results: Although depressed LVP, dp/dt, and coronary flow were seen in all groups during the reperfusion period, the preconditioned group showed more effective recovery of LVP than that of the control group, especially at 10, 20 and 40 minutes(p<.05). We failed to demonstrate the difference between the phenyl theophylline group and the control group(p=NS). Conclusion: These results suggest that ischemic preconditioning has protective effect on recovery state of hypothemic cardioplegic arrest/reperfusion. Its protective effect was limited during early reperfusion stage and was blocked by adenosine blocker.

표면 플라즈몬 공명형 Sucrose 센서의 제작 및 특성

A surface plasmon resonance (SPR) sensor for determination of sucrose concentration using gold thin film was constructed. The properties of gold thin film are very serious factors in exciting surface plasmon resonance. So the optimum condition of gold thin-film for the SPR phenomenon was investigated. The result is that the optimum thickness is 545Å at 43.75° resonance angle and roughness is about 3A at that time. Using this SPR system, we got the characteristic of the linear resonance angle shift from 10 wt% to 40 wt% sucrose concentrations.

This paper proposed a novel SIT high frequency resonant inverter having drive signal phase shift control function. Phase control type inverters using SIT can realize a power conversion at the high switching frequency with low switching loss. Especially, the high output power can be obtained by connecting the output terminal of two unit inverters in series. The stability of system using protection circuit for over current and the automatic follow-up control for load variation by using PLL are presented. This inverter produces approximately sinusoidal waveform at a high frequency, ranging from 180 Khz to 220 Khz, and is applied to the 2KW induction heating. The operating characteristics of this inverter circuit are discussed from a theoretical point of view and compared with experimental results.

교통사고후 발생한 심장파열 : 증례 A Case Report

Blunt cardiac rupture is a rare and lethal injury. Over 80% of patients with cardiac rupture caused by blunt chest trauma were died at scene and during transport to hospital. We treated a 38-year-old male patient who suffered cardiac rupture by steering-wheel injury during traffic accident. The patient was in profound shock on arrival at our hospital and the initial chest X-ray was normal. But we suspected cardiac tamponade caused by cardiac rupture with neck vein engorgement, profound shock, anterior chest wall bruise. Rapid diagnosis was done by chest CT and 2-D Echocardiography. The patient was sent to O. R and right atrial rupture was repaired with simple suture. The clinical course of the patient was uneventful and he was discharged without problems.

Diagnostic Value of Clinical T Staging Assessed by Endoscopy and Stomach Protocol Computed Tomography in Gastric Cancer: The Experience of a Low-Volume Institute

Purpose: Clinical staging of gastric cancer appears to be important more and more for tailored therapy. This study aimed to verify the accuracy of clinical T staging in a low-volume institute. Materials and Methods: We retrospectively reviewed prospectively collected data of gastric cancer patients who underwent resection. A total of 268 patients of gastric cancer were enrolled from March 2004 to June 2012. These demographics, tumor characteristics, and clinical stages were analyzed for identification of diagnostic value of clinical T staging. Results: The predictive values for pT1 of endoscopy and computed tomography were 90.0% and 89.4%, respectively. In detail, the predictive values of endoscopy for pT1a, pT1b, and pT2 or more were 87%, 58.5%, and 90.6%, respectively. The predictive values of computed tomography for pT1a, pT1b, and pT2 or more were 68.8%, 73.9%, and 84.4%, respectively. The factors leading to underestimation of pT2 or more lesions by gastroscopy were the middle third location, the size greater than 2 cm, and younger age. Those for overestimation of pT1 lesion by computed tomography were male, age more than 70 years, elevated type, and size greater than 3 cm. Conclusions: Diagnostic accuracy of early gastric cancer was 90%, which is comparable to those of high volume center. In patients with early gastric cancer, limited gastrectomy or minimal invasive surgery can be safely introduced at a low volume center also. However, the surgeon of low-volume institute should consider the accuracy of clinical staging before extending the indication of limited treatment.
