        “Meet the new boss / Same as the old boss”: The Politics of Replication and Conrad’s Politics of Humanity

        In contrast to the conventional political novel, which clearly promotes one political system over another, Joseph Conrad routinely rejects all political systems because they privilege ideas over people. In these works, Conrad consistently chronicles a political plot and a personal plot. These plots interact, and invariably Conrad focuses on the effect of the political plot on the personal plot, or more particularly on the damage the events of the political plot have on the individuals of the personal plot. Whether it be the deaths of Winnie and Stevie in The Secret Agent or the destruction of Razumov’s life in Under Western Eyes or Paul’s exile in “An Anarchist” or the death of Ruiz in “Gaspar Ruiz,” Conrad represents human beings not benefitting from political systems but being victims of them. Furthermore, Conrad regularly represents the competing political factions in his works as mirroring rather than opposing one another, which results in the end in their appearing to be little different from one another. The numerous revolutions in Nostromo are equally corrupt; the revolutionary movements and the established governments in The Secret Agent both believe that the ends justify the means; and the revolutionaries and authorities in Under Western Eyes are in the end indistinguishable from one another in their disregard for law and order. And yet Conrad does have hope, but that hope lies in human relationships, in such characters as Tekla in Under Western Eyes and Peyrol in The Rover, who choose people over politics, as we see Conrad time and again affirming those who affirm humanity and rejecting those who reject humanity.

        Roundtable on Peter J. Katzenstein's Contributions to the Study of East Asian Regionalism

        Over the past decade, Peter J. Katzenstein has made enormous contributions to our theoretical and empirical understanding of Asian economic and security regionalism, which has been manifested by the proliferation of intra- and extra-regional free trade agreements, regional financial institutions, and cooperative regional security dialogues. Katzensteins scholarly works on Asian regionalism and the changing role of Japan have set the pace for research in the field. In this article, a group of distinguished scholars in the field of Asian regionalismVinod K. Aggarwal, Min Gyo Koo, Amitave Acharya, Richard Higgott, and John Ravenhillcritically evaluate Katzensteins approach to the links among Japan, Asian regionalism, and global politics. In response, Katzenstein argues that Asian (and European) regionalism is linked to the American imperium and to core regional states and that regionalism is best studied with an eclectic approach. For him, regionalism is a force that defines security, economic, and cultural dimensions of world politics, thus bringing about a modicum of order therein.

        Primary versus revision total shoulder arthroplasty: comparing relative value and reimbursement trends

        Background: Total shoulder arthroplasty (TSA) has been demonstrated to be an effective treatment for multiple shoulder pathologies. The purpose of our study was to compare the relative value units (RVUs) per minute of surgical time for primary and revision TSA. Methods: The American College of Surgeons National Surgical Quality Improvement Program database was queried to identify patients that underwent primary TSA, one-component revision TSA, and two-component revision TSA between January 1, 2015 and December 31, 2017 using current procedure terminology codes. RVUs were divided by mean operative time for each procedure to determine the amount of revenue generated per minute. Rates were compared between the groups using a one-way analysis of variance with post-hoc Tukey test. Statistical significance was set at p<0.05. Results: When dividing compensation by surgical time, we found that two-component revision generated more compensation per minute compared to primary TSA (0.284±0.114 vs. 0.239±0.278 RVU per minute or $10.25±$4.11 vs. $8.64±$10.05 per minute, respectively; p=0.001). Conclusions: The relative value of revision TSA procedures is weighted to account for the increased technical challenges and time associated with these procedures. This study confirms that reimbursement is higher for revision TSA compared to primary TSA.

        Atom ingress from synthetic body fluid into nanoporous layers formed in titanium by helium ion-implantation

        Helium ion-implantation can be used to form a buried layer of nanoscale cavities in Ti. This oers the potential for diusing dopantatoms of interest for biomedical applications, into a Ti surface. Disc specimens were taken from a He-implanted foil and a small spot inNRA and PIXE) were used to determine the uptake of specic elements in two regions: the central eroded spot and the outer (un-eroded)region. The most signicant eect was for oxygen. A small amount of prior erosion resulted in athreefoldincrease in the oxygen inbothregions. Whereas, greater erosion caused a rapid reduction in the uptake in the centre, the strong enhancement in theouterregionremained a striking feature. This provides convincing evidence that oxygen can move laterally over large distances through the nanopor-ous layer.

      • A New Origin and Function of Poomsae in Taekwondo Training

        The objective of this paper is to establish a new paradigm for the understanding of poomsae in Taekwondo training. Until now poomsae have been considered as a system of connected offensive and defensive drills for teaching the fundamentals of combat. Hence, they are understood as a means and preparatory stages for attaining fighting skills, and consequently, they are treated as a secondary activity to kyorugi (sparring). Contrary to this understanding of poomsae, we want to argue that poomsae training has its own end, independent from kyorugi training. To demonstrate this point, we will focus on the relation between poomsae and the Chinese gymnastic exercises known as daoyin-sul or daoyin exercises (i.e., Daoist exercises of guiding and pulling), which are performed in conjunction with breathing techniques and the stretching of the body to accumulate qi (vital energy) in order to provide a foundation for the potential to realize the pure self.

        IBD2020 global forum: results of an international patient survey on quality of care

        Background/Aims: IBD2020 is a global forum for standards of care in inflammatory bowel disease (IBD). The aim of the IBD2020 survey was to identify and describe variations in quality care of IBD. Methods: Patients with IBD from Finland, Italy, France, Canada, Germany, UK, Spain and Sweden were surveyed during 2013 to 2014, covering: disease characteristics; impact on life and work; organization and perceived quality of care. Results: Seven thousand five hundred and seven patients participated (median age, 39 years [range, 10-103 years]; 2,354 male [31.4%]), including 4,097 (54.6%) with Crohn’s disease (CD) and 3,410 (45.4%) with ulcerative colitis (UC). Median time from symptom onset to diagnosis was 1 year for both CD (range, 0-47 years) and UC (range, 0-46 years), with no clear evidence of improvement in diagnostic delay over the preceding 24 years. Half of the patients (3,429; 50.0%) rated their care as “excellent” or “very good,” with similar results for CD and UC across countries. Five factors were significantly (P<0.01) associated with perceived good quality of care: quality of specialist communication; review consultation being long enough; failure to share information; no access to a dietician; speed of advice. Conclusions: The IBD2020 survey has highlighted areas related to quality of care of IBD from the patients’ perspective, with scope for improvement. (Intest Res 2018;16:537-545)

        Early Cardiac Dysfunction in Biopsy-proven Nonalcoholic Fatty Liver Disease

        Background/Aims: Nonalcoholic fatty liver disease (NAFLD) encompasses a range of diseases from nonalcoholic fatty liver (NAFL) to nonalcoholic steatohepatitis (NASH) and has been linked to cardiovascular disease and sub-clinical cardiac remodeling. This paper presents a retrospective study of biopsy-proven NAFL and NASH to examine the differences in subclinical cardiac remodeling. Methods: Patients were recruited from an institutional repository of patients with liver-biopsy-confirmed NAFLD. Patients with a transthoracic echocardiogram (TTE) within 12 months of the liver biopsy were included. The parameters of the diastolic dysfunction were reviewed for the differences between NAFL and NASH as well as between the stages and grades of NASH. Results: Thirty-three patients were included in the study, 17 with NAFL and 16 with NASH. The NASH patients were more likely to have lower platelets, higher AST, higher ALT, and higher rates of type 2 diabetes mellitus, coronary artery disease, and hypertension than the NAFL patients. The E/e’ ratio on transthoracic echocardiogram was significantly higher in NASH compared to NAFL, advanced-stage NASH compared to early stage, and high-grade NASH compared to low-grade. The E/e’ ratio was also significantly higher in NASH than NAFL in patients without diabetes mellitus. The presence of diastolic dysfunction trended toward significance. The other markers of diastolic dysfunction were similar. Logistic regression revealed a statistical association with E/e' and NASH. Conclusions: NASH patients had evidence of a higher E/e’ ratio than NAFL, and there was a trend towards a significant diastolic dysfunction. Patients with NASH compared to NAFL should be closely monitored for signs and symptoms of cardiac dysfunction. (Korean J Gastroenterol 2021;78:161-167)
