Background: Since the implementation of Well-dying Law in February 2018, there have been many clinical and administrative changes in the process of deciding either to give an intensive or end-of-life care. We intended to evaluate if there is any change in clinical aspects or adequacy of ICU admission of patients. before and after the law implementation. Methods: We performed a single-center retrospective cohort study involving patients admitted to medical ICU for each 3-month before and after the law implementation. Results: A total of 178 patients which were divided into 83 patients of pre- and 95 patients of post-legislative groups were reviewed. There were no significant differences in baseline characteristics including age, sex, education levels, religion, economic status, and ECOG at the time of ICU admission. There were no changes in the portion of patients with terminal comorbidities including any malignancy, liver cirrhosis, chronic lung disease, or pneumonia. There were also no differences in APACHE II score(mean +- SD, 15.4+-7.1 vs. 16.5+-8.6, p=0.37) at ICU admission, ICU prognosis including in-ICU mortality (31.5 vs. 30.5%, p=0.53), in-hospital mortality (37.4 vs. 39.0%, p=0.82), ICU length of stay ( IQR, 4-11.0 vs. 3-11.0 days, p=0.72), and hospital length of stay (IQR, 9-23.0 vs. 9-45.0 days, p=0.16). Adequacy of ICU admission, which was separately assessed by two intensivists, (73.8 vs. 74.7%, p=0.28) showed no difference before and after the law implementation. Conclusion: There was not any significant change in clinical characteristics, ICU prognosis, and the adequacy of ICU admission before and after the implementation of Well-dying Law.

Primary venous aneurysm (PVA) is a rare condition and in most cases of little clinical significance, whereas PVA's are potentially life threatening because they have been recognized to be a source of recurrent pulmonary embolism. Recently, we experienced a case of PVA without obvious pulmonary embolism that was managed by surgery. And we report rediologic findings of the venography, Magnetic Resonance Imaging (MRI) and operation.
Validation for models for tumor recurrence after liver transplantation in hepatectomy patients

Purpose: Early recurrence of hepatocellular carcinoma (HCC) remains a challenging issue after hepatic resection (HR) because of the associated poor prognosis. Models for tumor recurrence after liver transplantation (MoRAL) have been designed to predict tumor recurrence in HCC patients in the liver transplantation setting. This study aimed to validate the predictability of MoRAL for HCC recurrence or patient death and to evaluate the predictors of early HCC recurrence in hepatectomy patients with treatment-naïve solitary HCC. Methods: This study included 443 patients with HCC recurrence after HR from January 2005 to December 2011. Patients were stratified into early recurrence (n = 312) and late recurrence (n = 131) groups according to the development of recurrence either within or more than 2 years after hepatectomy. Results: The median levels of alpha-fetoprotein and protein induced by vitamin K absence-II and the median MoRAL score were significantly higher in the early recurrence group than in the late recurrence group. Regarding pathologic characteristics, the median tumor size, prevalence of tumor grade 3 or 4, microvascular invasion, presence of tumor necrosis, and macrovascular invasion in the early recurrence group were greater than those in the late recurrence group. Multivariate analysis showed that tumor grade 3 or 4, microvascular invasion, and high preoperative MoRAL score were predisposing factors for early HCC recurrence after HR. Conclusion: The MoRAL score can be used to predict early recurrence in patients with HCC who undergo curative HR. Using this model, other treatments could be considered for patients with early recurrence predicted after HR.

The cDNA nucleotide sequence of genome segment B encoding the VP1 protein was determined for the aquatic birnavirus GC1 isolated from the rockfish Sebastes schlegeli in Korea. The VP1 protein of GC1 contains a 2,538 bp open reading frame, which encodes a protein comprising 846 amino acid residues that has a predicted MW of 94 kDa. The sequence contains 6 potential Asn-X-Ser/Thr motifs. Eight potential Ser phosphorylation sites and 1 potential Tyr phophorylation site were also identified. GC1 contains the Leu-Lys-Asn (LKN) motif instead of the typical Gly-Asp- Asp (GDD) motif found in other aquatic birnaviruses. We also identified the GLPYIGKT motif, the putative GTPbinding site at amino acid position 248. In total, the VP1 regions of 22 birnavirus strains were compared for analyzing the genetic relationship among the family Birnaviridae. Based on the deduced amino acid sequences, GC1 was observed to be more closely related to the infectious pancreatic necrosis virus (IPNV) from the USA, Japan, and Korea than the IPNV from Europe. Further, aquatic birnaviruses containing GC1 and IPNV have genogroups that are distinct from those in the genus Avibirnaviruses and Entomo-birnaviruses. The birnavirusstrains were clustered into 5 genogroups based on their amino acid sequences. The marine aquatic birnaviruses (MABVs) containing GC1 were included in the MABV genogroup; the IPNV strains isolated from Korea, Japan, and the USA were included in genogroup 1 and the IPNV strains isolated primarily from Europe were included in genogroup 2. Avibirnaviruses and entomobirnaviruses were included in genogroup 3 and 4, respectively.
기후 변화의 생태계 영향에 대한 ABMS 연구 : 빙하감소와 북극곰의 모의실험을 바탕으로

최근 기후 변화에 따른 생태계의 변화에 대한 연구가 활발히 이루어지고 있다. 본 연구는 생태계 연구에 새로운 연구방법으로서 행위자 기반 모델(ABMS)을 소개하고, 사례연구를 통해 이 분야에 대한 활용 가능성을 제안하고자 한다. ABMS의 활용 가능성을 제안 · 평가하기 위하여, 기후 변화로 인한 빙하의 감소와 그에 따른 북극곰의 멸종 위기에 대해서 에이전트기반의 모형을 수립하고 모의실험을 실행하였다. 연구 결과는 중요 변수 간 상호작용에 신뢰할 만한 일관성을 발견할 수 있었다. 이러한 연구 결과는 제안한 빙하-북극곰 모델이 환경 변화에 따른 북극곰 멸종 시기 예측에 기여할 것을 기대할 수 있게 한다. 이에 따라, ABMS는 생태계의 다양한 변화 과정의 분석 일반에 적용이 가능할 것을 기대할 수 있다. It has actively advanced to study the impact of climate change on ecosystem. This study addresses ABMS (Agent Based Modeling and Simulation) as a methodology of ecosystem research. ABMS would suggest the possibility of practical use in this sector. This study would investigate how the melting speed of glacier in the arctic influences the extinction period of polar bears. The Polar Bears and Glacier Model in this study is expected to contribute to accurate prediction of the polar bear’s extinction period. The suggested ABMS could also be applied to the study of various factors of ecosystem in general.
Intraoperative indocyanine green angiography for the objective measurement of blood flow

Purpose: Intraoperative assessment of tissue perfusion is important to predict wound healing or improvement of symptoms in patients with peripheral arterial disease (PAD) or vascular trauma. There is no widely accepted standard for intraoperative measurement of tissue perfusion. Here, we report the use of indocyanine green (ICG)-based angiography to determine the blood flow in patients with PAD and vascular trauma. Methods: The SPY fluorescent imaging system was utilized. A dose of 3–5 mL of ICG (2.5 mg/mL) was injected intravenously followed by a 10-mL normal saline flush. The SPY imaging system was used to quantitatively assess perfusion. During the study period, the SPY imaging system was applied in 4 patients with PAD and one patient with vascular trauma. Results: In 3 patients with PAD associated with an ischemic wound, complete wound healing was achieved with the indication of viable tissue by the SPY system. In one patient with severe claudication in both lower extremities, the ICG angiography was used to determine the increased blood flow after revascularization. In the case of vascular trauma, this imaging system enabled the delineation of viability of the injured tissue. Conclusion: ICG angiography can determine the surface tissue viability in PAD patients. In cases of severe vascular trauma, the SPY system can be used to determine tissue perfusion. Further study is warranted to define the definite utility of this technology to assess perfusion, response to revascularization, and potentially, to predict the likelihood of wound healing.

본 논문은 한국어 표절 검사를 위해서 표절의 유형을 분석하여, 유형별 분석 결과를 기반으로 하여 유사 문장 판별 모델을 제안한다. 제안하는 방법은 한국어 문장에 대한 표절 유형 분석 결과를 토대로 LSA와 N-gram을 이용한 유사 문장 검색을 통하여 여러 유형의 표절로부터 견고한 유사 문장 판별 모델을 구현하였다. 제안한 모델의 성능 분석을 위해서 학생들이 인위적으로 작성한 표절 리포트와 표절한 첨부 문서로 실험 데이터를 구축하였다. 성능 비교를 위해서는 기존의 N-gram 모델, 벡터모델, LSA 모델이 사용되었으며, 실험 결과 제안한 모델이 정확률, 재현율, 그리고 F값 척도에서 우수한 성능을 보임을 알 수 있었다. In this paper, we proposed a method to find out similar sentences from documents to detect plagiarized documents. The proposed model adapts LSA and N-gram techniques to detect every type of Korean plagiarized sentence type. To evaluate the performance of the model, we constructed experimental data using students` essays on the same theme. Students made their essay by intentionally plagiarizing some reference documents. The experimental results showed that our proposed model outperforms the conventional N-gram model, Vector model, LSA model in precision, recall, and F measures.
