마우스에서 Cisplatin과 방사선조사로 유발된 구내염에 대한 재조합 표피성장인자의 효과

목 적: 마우스 모델에서 cisplatin과 방사선조사로 구내염을 유발시킨 후 재조합 표피성장인자 (recombinant human epidermal growth factor, rhEGF)을 처치하여 그 효과를 알아보고자 하였다. 대상 및 방법: 마우스 24마리를 대상으로 정상대조군 8마리와 실험군은 각각 rhEGF 처치군과 미처치군으로 8마리씩 분류하였으며 실험군은 10 mg/kg의 cisplatin을 방사선조사 첫날 동시에 1회 복강 내 투여하였고, 방사선조사는 5일간 1회 5 Gy씩, 5일간 25 Gy를 조사하였다. RhEGF 처치군은 방사선조사 2일 전부터 2일간 1 mg/kg의 rhEGF를 피하주사하였으며 방사선조사 3일째부터 3일간 1 mg/kg를 피하주사하여 총 5 mg/kg의 rhEGF를 투여하였다. 마우스의 체중변화, 먹이섭취 및 조직학적 변화를 평가하였다. 결 과: 마우스 체중변화는 rhEGF 처치군이 실험 3일째부터 5일간 rhEGF 미처치군과 비교하여 통계적으로 유의한 체중의 차이를 관찰할 수 있었다. 먹이섭취는 실험군(rhEGF 처치군과 미처치군)에서 실험 5일째까지 감소하였다가 13일째부터 먹이섭취의 증가를 보여주었다. Cisplatin과 방사선조사 후 7일째의 조직학적 검사에서는 rhEGF 처치군 에서 마우스의 점막 표피층의 변성이 관찰되었으나 rhEGF 미처치군에서는 점막층의 염증 반응이 관찰되었다. 결 론: Cisplatin과 방사선 조사로 마우스에서 발생한 구내염에서 rhEGF를 투여한 경우 체중 변화와 먹이섭취의 유의한 개선을 관찰 하였으며 조직학적 검사에서 점막 손상의 회복을 확인하였다. 향후 임상적으로 rhEGF가 항암화학요법과 방사선치료로 인한 구내염을 줄일 수 있는 가능성을 확인하였다. Purpose: To study the effect of recombinant human epidermal growth factor (rhEGF) on oral mucositis induced by cisplatin and radiotherapy in a mouse model. Materials and Methods: Twenty-four ICR mice were divided into three groups— the normal control group, the no rhEGF group (treatment with cisplatin and radiation) and the rhEGF group (treatment with cisplatin, radiation and rhEGF). A model of mucositis induced by cisplatin and radiotherapy was established by injecting mice with cisplatin (10 mg/kg) on day 1 and with radiation exposure (5 Gy/day) to the head and neck on days 1∼5. rhEGF was administered subcutaneously on days -1 to 0 (1 mg/kg/day) and on days 3 to 5 (1 mg/kg/day). Evaluation included body weight, oral intake, and histology. Results: For the comparison of the change of body weight between the rhEGF group and the no rhEGF group, a statistically significant difference was observed in the rhEGF group for the 5 days after day 3 of the experiment. The rhEGF group and no rhEGF group had reduced food intake until day 5 of the experiment, and then the mice demonstrated increased food intake after day 13 of the of experiment. When the histological examination was conducted on day 7 after treatment with cisplatin and radiation, the rhEGF group showed a focal cellular reaction in the epidermal layer of the mucosa, while the no rhEGF group did not show inflammation of the oral mucosa. Conclusion: These findings suggest that rhEGF has a potential to reduce the oral mucositis burden in mice after treatment with cisplatin and radiation. The optimal dose, number and timing of the administration of rhEGF require further investigation.

Purpose : To investigate safety of Ilzarov apparatus in 1.5T MRI system and prevent dangerous accident during performing MR imaging. Methods and materials : Foreign Ilizarov external fixator(half ring, threaded rod, wire with stopper, bolts nuts) and korean Ilizarov external fixator(half ring, threaded rod, wire without stopper, bolts, nuts) were tested. We measured force during exposure to magnetic field at 60cm from MRI potral, 30cm, at the portal and 30cm, 50cm inside the portal. Results : The force of Ilizarov apparatus except foreign half ring was below 0.3N per 100g. The force of foreign half ring was 2.6N, 6.02N, 14,3N at 60cm, 30cm and portal respecitivery. Conclusion : Ilizarov apparatus is safe except foreign half ring. We must checkout half ring before perfoming MRI.

<P>Copine3, a known calcium-dependent membrane binding protein, contains two tandem C2 domains and an A domain. This protein has been shown to interact with receptor tyrosine kinase 2 (ErbB2), but little is known concerning the physiological function of Copine3. To better understand its cellular function, we carried out a yeast two-hybrid screen to find Copine3 binding partners. Among the identified proteins, Jun activation domain-binding protein 1 (Jab1) appears to directly interact with Copine3. This physical interaction between Copine3 and Jab1 as well as the specific binding regions of both proteins were confirmed in vitro and in vivo. Our results also demonstrate that binding of Copine3 to ErbB2 is increased when Jab1 is overexpressed in SKBr3 breast cancer cells. Furthermore, two ErbB2 downstream signaling proteins [phosphatidylinositol 3 (PI3) kinase and protein kinase B (AKT)] were also activated by Jab1 overexpression in these cells. These data suggest that binding of Copine3 and Jab1 regulates, at least to some extent, the ErbB2 signaling pathway. Moreover, overexpression of both Copine3 and Jab1 in SKBr3 cells effectively increased cellular migration. Collectively, our findings indicating that Jab1 enhances the ErbB2 binding ability of Copine3, further activating the ErbB2 signaling pathways involved in breast cancer cell pathogenesis.</P>
Risk factors of chronic subscapularis tendon tear

Background: Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods: This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. Subscapularis tendon integrity was determined by MRI findings based on the thickness of the involved tendons. The association strengths of demographic, physical, social, and radiologic factors, comorbidities, severity of rotator cuff tear (RCT), and serologic parameters for SBT were evaluated using logistic regression analyses. The significance of those analyses was set at p<0.05. Results: The prevalence of SBT was 32.2% (302/939). The prevalence of partial- and full-thickness tears was 23.5% (221/939) and 8.6% (81/939), respectively. The prevalence of isolated SBT was 20.2% (190/939), SBT combined with supraspinatus or infraspinatus tendon tear was 11.9% (112/939). In multivariable logistic regression analysis, dominant side involvement (p<0.001), manual labor (p=0.002), diabetes (p<0.001), metabolic syndrome (p<0.001), retraction degree of Patte tendon (p<0.001), posterosuperior RCT (p=0.010), and biceps tendon injury (p<0.001) were significantly associated with SBT. Conclusions: Metabolic syndrome is a potential risk factor for SBT, as are these factors: overuse activity, diabetes, posterosuperior RCT, increased retraction of posterosuperior rotator cuff tendon, and biceps tendon injury.
Nodular Fasciitis with Cortical Erosion of the Hand

<P>Nodular fasciitis is a benign, reactive myofibroblastic tumor that is often mistaken for a sarcoma because of its histological appearance and rapid growth. Involvement of a finger is extremely rare. We report a case of nodular fasciitis of the thumb, accompanied by bone erosion. Magnetic resonance findings suggested the possibility of a malignancy, which could have led to misdiagnosis as a malignant soft tissue sarcoma. Instead, the lesion was treated by excisional biopsy, which confirmed nodular fasciitis. There has been no evidence of local recurrence at recent follow-up, 1 year after surgery. This case illustrates that, to avoid unnecessarily aggressive surgery, nodular fasciitis must be included in the differential diagnosis for any finger lesion that resembles a sarcoma, even if bone erosion is present.</P>

호산구성 근막염은 원인과 병인이 잘 알려지지 않은 미만성 근막염과 호산구 증가증을 보이는 드문 질환이다. 스테로이드 제제가 일차적인 치료제로 알려져 있으며 치료에 반응이 없는 경우 MTX와 같은 항류마티스 제제를 투여 할 수 있다. 하지만 이러한 약제에도 저항성을 보이는 경우 종양괴사인자 억제제가 치료에 사용될 수 있으며 치료 후 반응을 평가 하기 위해 자기공명영상이 유용할 수 있다. 이에 저자들은 스테로이드와 항류마티스 제제에 저항성을 보이는 호산구성 근막염 환자에게 종양괴사인자 억제제를 사용하여 치료 호전된 증례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Eosinophilic fasciitis (EF) is a rare fibrosing disorder characterized by painful swelling and induration of the limbs and trunk, characteristic histology with sclerosis and lymphocytic inflammation affecting the fascia. The cause and pathogenesis of EF are still unknown and current therapies include glucocorticoids with or without use of immunosuppressive agents. Recently, there have been several case reports documenting the efficacy of a TNF alpha inhibitor in EF following a steroid-resistant disease course. However, there has been no report on the experience in treatment of EF with a TNF alpha inhibitor in Korea. Hence, we report a case of steroid and methotrexate-resistant EF which was successfully treated with adalimumab, along with a review of the relevant articles.
