Henoch-Schonlein 자반증(HS 자반증)의 원인은 아직 잘 알려지지 않았으나, IgA를 함유하는 면역 복합체가 침착하는 전신 질환으로 세정맥, 세동맥, 모세 혈관과 같은 작은 혈관의 손상을 초래하는 전신성 혈관장애로 알려져 있다. 모든 연령에서 발생 할 수 있으며, 소아가 성인에 비하여 더 많이 발생하고 피부, 복부, 관절, 신장에서 증상이 나타나며, 뇌신경학적 증상은 드문 것으로 알려져 있으며 심각한 결과를 초래할 수 있다. 저자들은 전형적인 HS 자반증의 환아에서 뇌신경학적 증상을 보인 1례를 문헌 고찰과 함께 보고하는 바이다. Henoch-Schonlein purpura(HSP) is a multisystem disorder affecting predominantly the skin, joints, gastrointestinal tract and kidneys, although other organs are rarely involved. Neurological complications are rare during the course of HSP. We report a 8-year-old boy with HSP who presented with neurologic abnormality (neck stiffness, headache, vomiting, fever, increased WBC in cerebrospinal fluid, which dramatically resolved with clinical improvement.

Purpose: Angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers (ARBs) are frequently employed to counteract the detrimental effects of proteinuria on glomerular diseases. However, the effects of ARBs remain poorly examined in pediatric patients with immunoglobulin A (IgA) nephropathy. Herein, we evaluated the efficacy and safety of losartan, an ARB, in pediatric IgA nephropathy with proteinuria. Methods: This prospective, single-arm, multicenter study included children with IgA nephropathy exhibiting proteinuria. Changes in proteinuria, blood pressure, and kidney function were prospectively evaluated before and 4 and 24 weeks after losartan administration. The primary endpoint was the difference in proteinuria between baseline and 24 weeks. Results: In total, 29 patients were enrolled and received losartan treatment. The full analysis set included 28 patients who received losartan at least once and had pre- and post-urinary protein to creatinine ratio measurements (n=28). The per-protocol analysis group included 22 patients who completed all scheduled visits without any serious violations during the study period. In both groups, the mean log (urine protein to creatinine ratio) value decreased significantly at 6 months. After 24 weeks, the urinary protein to creatinine ratio decreased by more than 50% in approximately 40% of the patients. The glomerular filtration rate was not significantly altered during the observation period. Conclusions: Losartan decreased proteinuria without decreasing kidney function in patients with IgA nephropathy over 24 weeks. Losartan could be safely employed to reduce proteinuria in this patient population. ClinicalTrials.gov trial registration (NCT0223277)

Although ethylformate and phosphine fumigants are widely used for pest quarantine, studies related to their mechanism of action and metabolic physiological changes in Drosophila models are still unclear. In this study, we investigated how key metabolites altered by fumigants and cold treatment are associated with and affect insect physiology by comparative metabolome analysis. Fumigant treatment significantly altered cytochrome P450 and glutathione metabolites involved in the detoxification mechanism and showed lower expression of PGF2α involved in the immune response compared to the control. Additionally, most of the metabolites functioned in metabolic pathways related to the biosynthesis of amino acids, nucleotides and cofactors.

경기도 가축매몰지 주변 지하수의 계절별 변동 특성과 원인 분석을 위하여 가축매몰지 주변 60 m 이내에 위치한 관정(84개소)을 선별하여 오염물질들의 분포 및 특성과 그 원인에 대한 분석을 수행하였다. 오염원에 대한 지시인자(pH,DO, ORP, EC)의 경우, 대부분의 관정에서 계절에 상관없이 비교적 일정한 값을 유지하고 있는 것으로 나타나 침출수와 같은 오염원에 의한 계절적 영향이 뚜렷하지 않은 것으로 분석되었다. 질산성질소의 경우 대부분 30 mg/L 내외의 농도분포를 보이는 것으로 나타났으며 특히 가축매몰지로부터 20 m 이내에 위치한 관정에서도 15 mg/L 이하의 농도 분포를 보이는 것으로 분석되었다. 암모니아성질소와 염소이온의 경우 가축매몰지와 근접한 곳에 위치한 관정보다는 오히려 40 ~60 m 거리에 위치하는 관정에서 더 높은 농도 분포를 보이는 것으로 나타났다. 이러한 결과는 대부분 가축매몰지 침출수와 같은 높은 농도의 오염물 유입보다는 각 관정 주변에 위치한 기존의 오염에 의한 영향을 나타내주는 것이라 할 수 있으며 향후 가축매몰지 관련 지하수 보전 및 관리에 있어서 유용한 정보를 제공해 줄 수 있을 것으로 기대된다. Seasonal variations of major contaminants in groundwater around livestock burial areas in Gyeonggi province, Korea, were examined. Seven typical contamination indicators (NO3-N, NH3-N, chloride, pH, DO, ORP, and EC)were monitored in groundwater samples collected from 84 wells located within 60 m of livestock burial sites forthe leachate plume emanating from the livestock burial sites. The monitoring results of pH, DO, ORP, and EC revealed minimal seasonal variations, providing no evidence for leachate plumes. The NO3-N concentrations were below 30 mg/L and exhibited minimal seasonal fluctuations, even in the wells located close to (< 20 m) the burial sites; the NH3-N and chloride concentrations also showed similar results. The contamination indicators examined in this study indicate that the observed groundwater contamination is primarily from preexisting pervasive contamination due to agricultural activities and livestock farming, not leachates derived from nearby livestock burial sites.

The effect of the Pt electrode and the $Pt-IrO_2$ hybrid electrode on the performance of ferroelectric device was investigated. The modified Pt thin films with non-columnar structure significantly reduced the oxidation of TiN diffusion barrier layer, which rendered it possible to incorporate the simple stacked structure of Pt/TiN/poly-Si plug. When a $Pt-IrO_2$ hybrid electrode is applied, PZT thin film properties are influenced by the thickness and the partial coverage of the electrode layers. The optimized $Pt-IrO_2$ hybrid electrode significantly enhanced the fatigue properties with minimal leakage current.
