        The incidence of hematologic disorders in patients with Down's syndrome is significantly increased, about 14∼30 - fold higher than in general population and includes neonatal transient abnormal myelopoieis and acute leukemias. The age of onset of leukemia in Down's syndrome is peaking first in the newborn period and then under 4 years of age. Down's syndrome with acute leukemia above the age of 20 is very rare and it's treatment oucome is unclear. The treatment of Down's syndrome with leukemia has been controversial because of toxicity and associated congenital cardiac and other abnormalities. But if treated adequately, children with Down's syndrome show a favorable response to anti-leukemia therapy. A 24-year-old man with Down's syndrome was first seen for the evaluation of anemia and thrombocytopenia. The peripheral blood morphology and bone marrow study revealed acute myelogenous leukemia, cytogenetic study of bone marrow showed trisomy 21. Beacuse of his sicioeconomic condition and medical abnormalities including deafness, visual loss, he was treated with low dose subcutaneous cytosine arabinoside(Ara-C) for 11 days. Complete remission was obtained after 37 days. The complete remission lasted for 5 months. He subsequently relapsed, and died 6 months later.

        Most patients with aplastic anemia who do not respond to immunosuppressive treatment or are not candidates for bone marrow transplantation die of infection or bleeding. The neutropenia in acute leukemia, aplastic anemia, or occurring subsequently to chemotherapy and bone marrow transplantation increases susceptibility to infection. In general, the number of infectious episodes correlate with the degree and duration of neutropenia. Global immunosuppression produced by conditioning for bone marrow transplantation or graft-versus-host disease, is associated with unusual bacterial and fungal pathogens, or serious viral and protozoan infections. In addition, repeated treatment with broad-spectrum antibiotics is associated with the emergence of resistant organisms and fungal diseases because of the altered microbial microenvironment of the host. The incidence of invasive fungal infection caused by Saccharomycetes eerevisiae in immunosuppressed patients is very rare, compared with that of infection by candida or aspcrgillus species. Cases of Saccharomycetes cerevisiae fungemia occurring in the course of treatment with broad-spectrum antibiotics are reported in patients with extensive burn or with prosthetic valve endocarditis. We experienced a case of urinary tract infection by Saccharomycetes cerevisiae in a 27-year old female patient with severe aplastic anemia. We report the case with a review of relevant literatures.

        Rheumatoid arthritis frequently involves the cervical spine and may lead to neurologic impairement. However, direct involvement of CNS structures by inflammatory cells has been reported infrequently. The prevalence of this complication of rheumatoid arthritis is unknown. Inflammatory CNS involvement in rheumatoid arthritis reportedly occurs in the setting of longstanding, active, erosive articular disease and is accompanied by extracranial and extraspinal nodules and vasculitis. This is diagnosed by radiologic finding of CNS nodules or meningeal thickening and by biopsy or autopsy. Treatment with corticosteroid, cytotoxic agent or surgical decompression is helpful. But the majority of patients die within several months of onset of neurologic symptom. Recently, we experienced a case of pachymeningitis caused by rheumatoid arthritis, which resolved repeatedly with azathioprine treatment.

        The tidal water-levels, tidal ellipses, tidal currents and tide-induced residual currents in Mokpo harbour were simulated and reproduced by M₂tide using a three-dimensional numerical model in order to describe the M₂tidal phenomena of Mokpo harbour and to prepare the basic steps for the application to various real aspects such as the evaluations of coastal development, the constructions of breakwater and marine facilities, and navigation safety at sea, the prediction of pollutant distribution in water, and the monitoring or prediction of oil spill movement on sea surface.

        The tidal currents and residual flows in Ulsan Bay were simulated and reproduced by a hydrodynamic model in order to prepare for the application to various real aspects such as the evaluations of coastal development, the constructions of breakwater and marine facilities, and navigation safety at sea, the prediction of pollutant distribution in water, and the monitoring or prediction of oil spill movement on sea surface. Comparing the computed with the observed tidal currents and residual flows in Ulsan Bay, the results of simulation of simulation were found to be good enough to satisfy the real applications.

      • 한반도 서해 해역의 하계 수질 평가 : 부영양화를 중심으로

        Based on the data obtained through the collections and analyses of seawaters for the western water area of the peninsula of Korea in summer of the years 1993 and 1994, pollution indices and eutrophication indices were estimated and the water quality of the western water area of the peninsula of Korea was evaluated with relation to eutrophication. The water quality of mid-area located in the northward from the latitude of 36。 N was found to be deteriorated due to eutrophication and to fall under the regular grades, while that of southern area located in the southward from the latitude of 36。 N was found to be graded second class in the surface and middle layers of water column and to fall under the regular grades in the bottom layer. The entire western water area, including mid-area and southern area, of the peninsula of Korea was generally evaluated to be grade third class according to Korean criteria for seawater quality, in the light of pollution index, and to have the potentiality of red tide occurrence, in view of eutrophication index.

        Space Supervision을 이용한 치성 정중선 변위의 교정

        상하악 중절치 간의 정중선 변위는 골격성 변위,치성 변위 및 골격성과 치성의 복합적 변위 등으로 구분된다. 그 중 치성 정중선 변위는 치열의 발육시기에 따라 다양한 방법으로 치료 방법을 선택할 수 있다. 특히 혼합치열의 초기에는 치열 내에 존재하는 공간의 조건,인접 영구치의 맹출 시기 또는 맹출 방향에 따라 영구 절치들의 위치가 자발적으로 변화,개선될 수 있다. 따라서 이러한 치아의 생리적 이동을 이용하여 특별한 교정장치의 이용 없이 치열 내에서 치아의 배열을 개선시킬 수 있다. 본 증례보고에서는 특히 유견치의 발거 시기 및 좌우 유치의 발거 순서 등을 이용하여 치성 정중선 불일치를 개선시킨 증례틀을 보고하고자 한다. A midline deviation is divided into three types, such as a skeletal midline deviation, a dental midline deviation, and a combination. Among them, the midline deviation which appear in the early mixed dentition without any skeletal problem can be managed differently by the stage of dental development. The location of the permanent incisors can be improved without using artificial force in consideration of the condition of the space, the eruption timing or the eruption path of the adjacent permanent teeth in early mixed dentition, especially when the permanent incisors are erupting. Therefore. the space supervision technique, which can have the teeth align themselves utilizing physiologic events such as sequence or timing of the eruption of permanent teeth, can be an alternative to orthodontic movement. This case report describes midline deviation improvement by space supervision.

        We estimated the self purification capacity of tidal flats in KanJin Bay, southwestern coast of Korea during flood tide and ebb tide in September, December, 1997 and March, 1998. The net flux of 17.8 gDIN/m²/yr with tidal current outflow to sea was estimated in September and 16.0 gDIN/m2/yr outflow to sea in December, while 0.57 gDIN/m²/yr influx into the tidal flat in March. The flux of 1.29 gDIP/m²/yr outflow to sea was estimated in September, while 2.64 gDIP/m2/yr and 5.90 gDIP/m²/yr influx into the tidal flat in December and in March, respectively. The SS flux of 0.849 kgSS/m²/yr and 5.28 kgSS/m²/yr outflow to sea were estimated in September and in March, while 12.0 kgSS/m²/yr inflow into the tidal flat in December. The net flux of 339 gCOD/m²/yr was inflow into the tidal flat was estimated in September, 254 gCOD/m²/yr in December and 63.7 gCOD/m²/yr in March, respectively. The capacity of COD purification of the tidal flats was 219 gCOD/m²/yr for a period of the diurnal tide.
