Background: Our objective was to evaluate the location of popliteal artery (PA) in osteotomy planes during high tibial osteotomy (HTO) and to determine a safer angle for screw drilling to the tibial tuberosity during distal tuberosity osteotomy (DTO). Methods: Twenty knees in 20 patients who underwent contrast-enhanced computed tomography for cardiovascular diseases were examined. Osteotomy planes for open-wedge HTO (OWHTO) and hybrid closed-wedge HTO (hybrid CWHTO) were created using three-dimensional bone models. The distance from the posterior cortex of the tibia to the PA (dPC-PA) in the osteotomy planes was measured in the virtual osteotomy planes. The dangerous point (Point D1) was defined as the point 17.5 mm away from PA, setting the working length of the bone saw as 35 mm. The distance between the most medial point of the tibial cortex (Point M) and Point D1 in OWHTO and the most lateral point (Point L) and Point D1 in hybrid CWHTO were examined (dM-D1 and dL-D1, respectively). The location of Point D1 to the osteotomy line (%D1) was expressed as percentage, setting the start and end of the osteotomy line as 0% and 100%, respectively. To determine the safe angle for screw drilling in DTO, the angle between the line tangential to the medial cortex of the tibia and that passing through the center of the tibial tuberosity and PA were measured. Results: In OWHTO and hybrid CWHTO, the mean dPC-PA was 10.6 mm (6.9-16.5 mm) and 10.2 mm (7.3-15.4 mm), respectively. The mean dM-D1 in OWHTO was 25.9 mm (24.6-27.2 mm) and dL-D1 in hybrid CWHTO was 5.1 mm (2.9-7.4 mm). The mean %D1 was 47.6 ± 3.7% in OWHTO and 9.3 ± 4.1% in hybrid CWHTO, respectively. The minimal angle between the two lines in DTO was 35.2°. Conclusion: PAs could run within 10 mm from the posterior cortex in the osteotomy planes of HTO. Therefore, proper posterior protection is necessary when cutting posterior cortex. An angle of less than 35° against the medial cortex line would be safe for screw fixation to avoid vascular injury in DTO.

Behçet’s disease (BD) is a multisystem inflammatory disease of unknown origin. Rarely, BD occurs together with myelodysplastic syndrome (MDS). Interestingly, it is speculated that these are not simple coexistence but that the etiology of intestinal BD is at least partly derived from MDS itself. Furthermore, there is a relationship between MDS in patients with intestinal BD and trisomy 8. Immunosuppressive agents alone are insufficient to control MDS-associated BD, and many of these patients die of infection or hemorrhage. Surgery is considered for intestinal BD patients who are unresponsive to medical treatment or those with bowel complications such as perforation or persistent bleeding. We report a case of intestinal BD associated with MDS and trisomy 8. The patient was unresponsive to oral steroids and immunosuppressive treatment; the patient improved by surgical repair of a bowel perforation. Five years after the surgery, the patient is free of recurrence and not on medication. Our experience suggests that surgery may provide an effective therapeutic option for the treatment of MDS-related BD. (Intest Res 2020;18:469-475)

The fractional slot windings are widely used in rotating machine in order to increase the flexibility of design and improve the voltage waveform. However, the MMF wave of fractional-slot windings are found to contain unique harmonic component, which are designated as even order space flux harmonics, fractional number flux harmonics, or both. They may cause stray loss and stator core vibration. This paper proposes new winding methods "novel interspersed windings" and "expanded group windings" to reduce these harmonics. The advantages of two proposed windings are verified by using numerical analysis and measurement test of winding model.

A wide spectral coverage from near-infrared (NIR) to far-infrared (FIR) of {\it AKARI} both for imaging and spectroscopy enablesus to efficiently study the emission from gas and dust in the interstellar medium (ISM). In particular, the Infrared Camera(IRC) onboard {\it AKARI}offers a unique opportunity to carry out sensitive spectroscopy in the NIR (2--5\,$\mu$m) for the first time from a spaceborn telescope. This spectral range contains a number of important dust bands and gas lines, such as the aromatic and aliphatic emissionbands at 3.3 and 3.4--3.5\,$\mu$m, H$_2$O and CO$_2$ ices at 3.0 and 4.3\,$\mu$m, CO, H$_2$, and \ion{H}{I} gas emission lines. In this paper we concentrate on the aromatic and aliphatic emission and ice absorption features. The balance between dust supply and destruction suggests significant dust processing takingplace as well as dust formation in the ISM. Detailed analysis of the aromatic and aliphatic bands of {\it AKARI} observations for a number of\ion{H}{ii} regions and \ion{H}{ii} region-like objectssuggests processing of carbonaceous dust in the ISM. The ice formation process canalso be studied with IRC NIR spectroscopy efficiently. In this review, dust processing in the ISM divulged by recentanalysis of {\it AKARI} data is discussed.

A Valsalva aneurysm filled with thrombi can be difficult to diagnose, because it mimics a cardiac tumor. Both cardiac magnetic resonance imaging (MRI) and transesophageal echocardiogram (TEE) were performed on a patient who showed a low-echoic mass located between the atrial septum and the non-coronary sinus. Based on MRI findings allowing tissue characterization and the accurate location of the mass and the TEE findings of an irregular surface of the mass and a partial defect in the edge of the non-coronary sinus, we diagnosed the mass as a thrombosed Valsalva aneurysm that had perforated the inter-atrial septum. The operative findings coincided with the preoperative di-agnosis. Both MRI and TEE are useful for diagnosing this condition.
A Method for Reducing the Number of Metal Layers for Embedded LSI Package

We have successfully demonstrated a high-pin-count and thin embedded-LSI package to realize next generation's mobile terminals. The following three design key points were applied: (i) Using Cu posts, (ii) Using the coreless structure, (iii) Using a Cu plate as the ground plane. In order to quantitatively determine the contribution of the three points, the five-stage process for reducing the number of metal layers is described by means of the electrical simulation. The point-(i) and (ii) are effective from the viewpoint of the power integrity (PI); that is, these points play important roles in reducing the number of metal layers, and especially the point-(ii) contributes at least twice as the point-(i). The point-(iii) is not effective in the PI, but has a few effects on the signal integrity (SI). For reducing the number of metal layers, we should, at first, pay attention whether the PI characteristics fulfill the specification, and then we should confirm the SI characteristics.
