복강경을 이용한 부인과 수술의 임상적 고찰

Background and Objectives : To assess the advantages, disadvantages, appilcability, and complications of laparoscopy in gynecologic disease. Materials and Method: A retrospective study which includes 591 patients who underwent laparoscopic surgery from January, 1994 to February, 2005 for gynecologic diseases was done. Results: The patient's ages ranged from 19 to 65 years and their mean age was 40years. The indications of laparoscopy were myoma in 240 cases (40.6%), ovarian tumor, ectopic pregnancy, pelvic adhesion, primary infertility, secondary infertility, endometriosis, pelvic inflammatory disease, hemorrhagic corpus luteum and tubal obstruction in decreasing order. The operations performed were laparoscopic-assisted vaginal hysterectomy in 247 cases (41.8%), diagnostic laparoscopy, oophorectomy, salpingo-oophorectorny, ovarian cystectomy, pelvic adhesiolysis, salpingectomy, ovarian wedge resection and fulgarization in order. Operation times were from 10 to 240 minutes (mean 64 minutes). Postoperative hospital stays were from 0 day to 15 days (mean 3.9 days). There were 11 surgical complications including 1 trocar site hematoma, 11 incision site infection, 2 postoperative ileus and 1 patient underwent reoperation because of delayed ureteral fistula. Conclusion: We conclude that operative laparoscopy is useful in gynecologic diseases but need development of laparoscopic equipments, research of operative method, extension of operative indications, and skilled laparoscopic surgeons.

The preconception is socalled the naive theories was made according to the general experience, direct investigation, wrong explanation on textbook or bibliography material and different of culture. By Champagne and klopfer(1983) find out that the preconception are latent in those blains long time, so they cause serious influences to the general teaching method without special healing. The researches on the air unit in primary school are first, the concept of air volume and its weight, secondly the difference of concepts between urban and local area, and third the distinction of sex. In this work, there are formed preconception in the third grade children primary school, urban children thought are more than local, and last male children have more right thought than female both urban and local area.

King Richard Ⅱ의 극적기교

The play can be divided into two parts by the differences of the dramatic technique. In the first half (from the opening scene to Act Ⅲ, Scene ⅰ), the character of King Richard is depicted by the objective perspective. Through a series of events including the banishment of Bolingbroke and Mowbray, Richard's appropriation of the Lancaster property to finance his Irish expedition and his choosing of helpless Yoke as a regent, King appears arbitrary, inhuman, tyrannical and headstrong. While Shakespeare was true to the actual historical fact, he made his own inventions to get the heatrical effect. The exchange of retorts between King and his dying uncle, Gaunt, is one of Shakespeare's inventions in which Richard's unfitness for kingly office is fully exposed. The Duke of Gaunt makes King fly into a rage by using his name 'Gaunt' as a pun. Shakespeare compressed time between the events, though in actual fact some time must elapse. These dramatic techniques make the play what is above the mere chronicle play. In the second half (from King's return from Ireland to the end), Richard is no longer the arbitrary and tyrannical king, displaying his absolutism stubbornly against all advice. He returns to find himself with neither an army nor subjects. Scroop, one of his favorites, delays deliberately his report on the defection of Yoke on whom he laid his only remaining hope. In the deposition scene, King is baited by his courtiers like a bear is baited by a few dogs in the bear-baiting. King becomes a poet and player, singing and acting out his anguish to death. Now the audience feel pity for the same Richard they had been angry with in the first half. To get this dramatic effect, Shakespeare used the subjects around King as instruments for Richard to vent his sorrow naturally and lavishly. And the dramatist portrayed all the events in which King appeared through the eyes of Richard. So the audience can see the external world only through the eyes of Richard. These dramatic techniques have us share King's sorrow with ease and, on the other hand, listen to his cry: can it be possible for a subject to depose the anointed king by force? So we can say that the second half has a lot of elements to be found in the expressionistic play. In a sense, the play is a synthesis of chronicle play and tragedy, the realistic play and expressionistic one.

당귀의 품질평가법에 관한 연구

Angelicae gigantis radix is the root of the perennial plant, which belongs to family Umbelliferae. However, this herbal drug has quite different chemical components according to its different geographical origin though other herbal drugs (i.e. Leonuri Herba, Xanthii Fructus and so on) show similar major constituents. In other word, the root of Angelica gigas containing the coumarin compounds is commonly used in Korea as Angelicae gigantis radix, while Angelica sinensis and Angelica acutiloba including phthalide compounds are used in China and Japan, respectively. In this paper, Near-Infrared spectroscopy was developed to determine Genus name of Angelica spp., especially Angelica gigas and Angelica sinensis which are generally misused in the herbal market. Also X-ray fluorescence spectrometry was evaluated to analyze the differences between Angelica gigas and Angelica sinensis with their specific properties.

Timon of Athens의 극적기교와 인간혐오

Timon of Athens is an intellectual tragedy. After setting up his objectiives to write this work, Shakespeare chose his characters to fit the objectves. He had an intention to awaken his contempories by criticizing and berating the society oriented by Mammonism and corrupted by infidelity. He used his characters as instruments to accomplish his intention. So Timon is presented as a grotesque man, rather than a really tragic figure. The other characters are portrayed as types, not as 'real' men. His treatment of characterization originated from a dramatic technique used on purpose by Shakespeare. This is a forte of this play, not a weakness. We should not detach the characters from their stage. If we should separate the characters from their stage, we would find them dull and grotesque. When we watch the characters play their role on the stage, we will find the stage brisk and vigorous.

東大門 大規模 流通商業施設 立地地域의 步行環境類型에 따른 步行人의 認知特性分析

In this study, we investigated to understand characteristics of pedestrian environment in Dong-Daemun market area and cognitive characteristics of pedestrians moving path. And then, correlation between factors affecting on pedestrian environment and cognitive characteristics of pedestrians was analyzed. As the results, it was found that pedestrians had not any general evaluation criteria for pedestrian environment and pedestrian space and recognized as the various pedestrian environments by the convenience of the individual purpose. At the study area, variation of a floating population from the viewpoint of the time is larger. But, such variation of a floating population affected a slightly different on the pedestrian space of the place, and cognitive pedestrian evaluate it at various viewpoint.

근적외선 분광분석법을 이용한 LAS (linear alkyl benzene sulfonate)의 정량분석법

Linear alkyl sulfonates (LAS) constitute a large fraction of the surfactants used in cleaning processes in households, trade and industry. Despite the industrial significance and the possible environmental impact of these compounds, the fast and inexpensive determination of LAS concentrations is still a difficult task. In this study, near infrared (NIR) spectroscopy which is a rapid spectroscopic analysis method compared with a traditional analytical method for the measurement of LAS concentration such as HPLC, GC and standard wet chemistry method. NIR spectra of LAS between 0.313 and 25.0% (w/v) in water were utilized to develop a calibration model. The best results (R = 0.998, SEP = 0.244% (w/v)) obtained by using partial least-squares regression with spectral data treatment and 2nd derivatization were comparable to the results (SEC = 0.186% (w/v), SEP = 0.206% (w/v)) obtained by using multiple linear least-squares regression (MLR). However, models based on derivative spectra have no significant advantage with MLR.

HIV-1 Non-Nucleoside 역전사 효소 억제제의 생물학적 활성과 3차원 구조사이의 정량적 상관관계(3D-QSAR) 연구

본 연구에서는 AIDS라고 잘 알려진 후천성 면역 결핍(부전)증후군(Acquired Immuno Deficiency Syndrome)의 치료제로 사용될 수 있는 NNRTIs(Non-nucleoside Reverse Transcriptase Inhibitors)에 대해, 3D-QSAR방법으로 이들의 약리단(phrmacophore model)을 정의하고자 한다. NNRTIs는 HIV의 유전물질이 건강한 세포로 옮겨 갈 수 없도록 역 전사효소에 달라붙어 DNA합성을 방해함으로써 HIV의 증식을 막는다. 이들의 대표적인 화합물들은 HEPT(Hydroxyethoxymethylphenylthiothymine), TIBO(tetrahydroimidazo-benzodiazepinone), Nevirapine 등으로, 이들이 이번 연구에 사용된 화합물들이다. 지금까지의 연구에서는 이들의 결합 모형을 각각 연구 해 왔지만 X-ray 구조를 보면 NNRTI들이 역 전사 효소의 결합부위와 상호 작용할 때 유사한 입체구조를 갖는다고 할 수 있다. 따라서 이번 연구에서는 이러한 사실을 확인하기 위해 3D-QSAR 방법을 이용하여 NNRTIs 각각의 약리성 자리와 3차원 입체구조를 X-ray 구조와 비교, 분석하였다. 마지막으로, 이렇게 해서 얻은 활성가설을 하나의 '분자 체'로 이용하여 National Cancer Institute(NCI)의 AIDS관련 화합물들로 이루어진 데이터 베이스를 검색함으로써 선도물질이 될 수 있는 구조들을 검색해 보았다. A definition of pharmacophore for the NNRTIs(Non-Nuceoside Reverse Transcripase Inhibitors) was carried by considering a 3D-QSAR model. HEFT (hydroxyethoxy -methylphenylthiothymine), TIBO(tetrahydroimidazobenzodiazepinone) and Nevirapine among NNRTIs were used in this study. Pharmacophore models generated for the NNRTI binding pocket have been usually limited so far to a single class of compound. But NNRTIs have all been shown by X-ray crystallography to share a common binding location. Then, these inhibitors seem general similar conformation when bound to the pocket of RT. It has been confirmed that the similar interactions between the three class inhibitors have been shown by superimposing of pharmacophore models. The 3D-pharmacophore models as molecule sieve was applied in NCI database in order to find the potent inhibitors. The results provided a good basis to design new inhibitors of HIV-1 RT.

Poly-(acrylic acid-co-methylacrylate)합성조건에 따른 시멘트계 재료의 물성

The water soluble polymers based on polycarboxlates are come into the spot-light in the detergent fields of CaCO_3, Al(OH)_3 and leather industries. In this research, we synthesized molecular weights(Mn) by changing the concentrations of monomer(acrylic acid-methylacrylate mixture), chain transfer agent(butanethiol) and initiator(ammoniumpersulfate). We investigated the influences on the fluidities(the flow & the mini slump) and the setting times of cement material admixtured with the copolymers by the statistical method. The optimum synthesis formulation of the poly-(AA-co-MA) for cement materials is at the concentrations of monomer 50 M%, chain transfer agent 6.0 M% and initiator 2.0 M%. The number-average molecular weight of that was decided on 7,600 by the viscosity method.

복강경하 대장절제술을 시행 받는 환자에서 라모세트론과 온단세트론외 술 후 오심 및 구토 예방 효과 비교

Objectives : We evaluated the efficacy of ramosetron and ondansetron for preventing postoperative nausea and vomiting (PONV) in laparoscopic colectomy. Methods and Materials : Eighty patients who received laparoscopic colectomy were randomly divided into two groups: R group (ramosetron 0.1mg PO) and O group (ondansetron 4mg twice IV). Injection or oral medication was administered before the induction of anesthesia in each group. Injection was administered at the end of surgery in O group. General anesthesia was induced using propofol and rocuronium, and maintained with sevoflurane, remifentanil and air (FiO2 0.5), We investigated the incidences of PONV in each group by the Rhodes index of nausea, vomiting and retching (RINVR) at postoperative 6 and 24 hours. Results : The incidence of PONV was not different between group at each time points after surgery (at postoperative 6 hours: 20% in group R, 17.5% in group 0, at postoperative 24 hours; 12.5% in group R, 7.5% in group 0). There was no difference in the severity of PONV, satisfaction, rescue drug usage. Conclusion : Prophylactic therapy with ramosetron is as effective as conventional prophylactic therapy with ondansetron for preventing PONV in general anesthesia for laparoscopic colectomy.
