Culdocentesis is a procedure that checks for abnormal Quid in the space just behind the vagina (cul-de-sac). First, a pelvic examination is done. Then, after exposing the postehor vaginal fonix with a bivalve vaginal speculum the posterior lip of the cervix is grasped with a tenaculum. The cul-de-sac is then entered through the postehor vaginal wall with an 18-20 gauge needle with a syringe inserted. As the cul-de-sac is entered, suction is applied, and the intraperitoneal contents are aspirated. In the recent past, culdocentesis was commonly used in the evaluation of a vahety of common intraperitoneal conditions, principally, the hemorrhage associated with ectopic gestations and ruptured ovarian cysts the presence and/or drainage of purulent associated with pelvic inGammatory disease, and as a screening procedure to facilitate early diagnosis of ovarian neoplasms. Usually 16-18 gauge needle with a syhnge attached was used for culdocentesis. But we have experienced a case of therapeutic culdocentesis with 16 gauge angioneedle in patient of hemoperitoneum. So, report it with a bhef review of literature.

<P><B>Abstract</B></P> <P>AMP-activated protein kinase (AMPK) and its upstream mediators liver kinase B1 (LKB1) and sirtuin 1 (Sirt1) are generally known as key regulators of metabolism. We have recently reported that the AMPK pathway negatively regulates mast cell activation and anaphylaxis. Tanshinone IIA (Tan IIA), an active component of <I>Salvia miltiorrhiza</I> extract that is currently used for the treatment of cardiovascular and cerebrovascular diseases, shows anti-diabetic activity and improves insulin resistance in <I>db/db</I> mice through activation of AMPK. The aim of this study was to evaluate the anti-allergic activity of Tan IIA <I>in vivo</I> and to investigate the underlying mechanism <I>in vitro</I> in the context of AMPK signaling. The anti-allergic effect of Tan IIA was evaluated using mouse bone marrow-derived mast cells (BMMCs) from <I>AMPKα2</I> <SUP>−/−</SUP> or <I>Sirt1</I> <SUP>−/−</SUP> mice, or BMMCs transfected with siRNAs specific for AMPKα2, LKB1, or Sirt1. <I>AMPKα2</I> <SUP>−/−</SUP> and <I>Sirt1</I> <SUP>−/−</SUP> mice were used to confirm the anti-allergic effect of Tan IIA in anaphylaxis <I>in vivo</I>. Tan IIA dose-dependently inhibited FcεRI-mediated degranulation and production of eicosanoids and cytokines in BMMCs. These inhibitory effects were diminished by siRNA-mediated knockdown or genetic deletion of AMPKα2 or Sirt1. Moreover, Tan IIA inhibited a mast cell-mediated local passive anaphylactic reaction in wild-type mice, but not in <I>AMPKα2</I> <SUP>−/−</SUP> or <I>Sirt1</I> <SUP>−/−</SUP> mice. In conclusion, Tan IIA suppresses FcεRI-mediated mast cell activation and anaphylaxis through activation of the inhibitory Sirt1-LKB1-AMPK pathway. Thus, Tan IIA may be useful as a new therapeutic agent for mast cell-mediated allergic diseases.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Ethnopharmacological relevance: 15,16-Dihydrotanshinone I (DHT-I), isolated from the dried root of Salvia miltiorrhiza Bung, which is traditionally used to treat cardiovascular and inflammatory diseases agent in Chinese medicine. DHT-I has been reported to have a broad range of biological activities, including antibacterial activity, and has been used to treat circulatory disorders, hepatitis, inflammation, cancer, and neurodegenerative diseases. Aim of the study: The aim of this study was to evaluate the anti-allergic inflammatory effects of DHT-I on degranulation and on the generation of eicosanoids, such as, prostaglandin D2 (PGD2) and leukotriene C4 (LTC4), in IgE/Ag-stimulated bone marrow-derived mast cells (BMMCs). Materials and methods: The anti-allergic inflammatory activity of DHT-I was evaluated using BMMCs. The effects of DHT-I on mast cell activation were investigated by following degranulation and eicosanoid generation using ELISA and immunoblotting and immunoprecipitation techniques. Results: DHT-I at a concentration of 20 μM markedly inhibited degranulation and the generation of PGD2 and LTC4 in IgE/Ag-stimulated BMMCs (about 90% inhibitions, respectively). Analyses of FcεRI-mediated signaling pathways demonstrated that DHT-I inhibited the phosphorylations of spleen tyrosine kinase (Syk) and linker for activation of T cells (LAT), and inhibited downstream signaling process, including [Ca2þ]i mobilization induced by the phosphorylation of phospholipase Cγ1 (PLCγ1), and the activations of mitogen-activated protein kinases (MAPKs) and the Akt-nuclear factor-κB (NF-κB) pathway. Conclusions: DHT-1 inhibits the release of allergic inflammatory mediators from IgE/Ag-stimulated mast cells by suppressing a FcεRI-mediated Syk-dependent signal pathway. This result suggests DHT-I offers a novel developmental basis for drugs targeting allergic inflammatory diseases. & 2015 Elsevier Ireland Ltd. All rights reserved.

<P>We have synthesized ultra-small and uniform Fe(x)Co(1-x)/graphitic carbon shell (Fe(x)Co(1-x)/GC) nanocrystals (x=0.13, 0.36, 0.42, 0.50, 0.56, and 0.62, respectively) with average diameters of <4 nm by thermal decomposition of metal precursors in approximately 60 nm MCM-41 and methane CVD. The composition of the Fe(x)Co(1-x)/GC nanocrystals can be tuned by changing the Fe:Co ratios of the metal precursors. The Fe(x)Co(1-x)/GC nanocrystals show superparamagnetic properties at room temperature. The Fe(0.50)Co(0.50)/GC, Fe(0.56)Co(0.44)/GC, and Fe(0.62)Co(0.38)/GC nanocrystals have a single bcc FeCo structure, whereas the Fe(0.13)Co(0.87)/GC, Fe(0.36)Co(0.64)/GC, and Fe(0.42)Co(0.58)/GC nanocrystals have a mixed structure of bcc FeCo and fcc Co. The single bcc-phased Fe(x)Co(1-x)/GC nanocrystals functionalized with phospholipid-poly(ethylene glycol) (PL-PEG) in phosphate buffered saline (PBS) are demonstrated to be excellent T(1) MRI contrast agents.</P>

The purpose of this study is to identify the effect of service recovery efforts in the hotel business. This study tested if there is any significant effect of service recovery on consumers` revisiting intentions. Specifically this study identified the level of satisfaction for tangible and intangible rewards and its impact on service satisfaction, trust, and revisiting intentions. The intermediate effect of trust variable was also identified. In Busan, Ulsan, and Changwon 262 observations who have unsatisfied experiences were surveyed. Results show that there are several critical factors impacting on service recovery. First, both tangible and intangible rewards significantly influencing the service recovery but only intangible reward impact on the trust. It means that intangible rewards including kind attitude, sufficient apology, full understanding in the gust`s shoes are better effect on improving trust. This trust leads more intention to visit after assuring the service quality. This study found customers` trust impacts on their revisiting positively. They revisit and build another level of trust. According to this study the trust cannot be achieved without proper level of rewards program intangible. It is imperative to have progressive rewards and strong recovery strategies to retain regular customers when a company failed to meet their expectation.

<P>Serum antibodies and mannose-binding lectin (MBL) are important host defense factors for host adaptive and innate immunity, respectively. Antibodies and MBL also initiate the classical and lectin complement pathways, respectively, leading to opsonophagocytosis. We have shown previously that <I>Staphylococcus aureus</I> wall teichoic acid (WTA), a cell wall glycopolymer consisting of ribitol phosphate substituted with α- or β-<I>O-N</I>-acetyl-<SMALL>d</SMALL>-glucosamine (GlcNAc) and <SMALL>d</SMALL>-alanine, is recognized by MBL and serum anti-WTA IgG. However, the exact antigenic determinants to which anti-WTA antibodies or MBL bind have not been determined. To answer this question, several <I>S. aureus</I> mutants, such as α-GlcNAc glycosyltransferase-deficient <I>S. aureus</I> Δ<I>tarM</I>, β-GlcNAc glycosyltransferase-deficient Δ<I>tarS</I>, and Δ<I>tarMS</I> double mutant cells, were prepared from a laboratory and a community-associated methicillin-resistant <I>S. aureus</I> strain. Here, we describe the unexpected finding that β-GlcNAc WTA-deficient Δ<I>tarS</I> mutant cells (which have intact α-GlcNAc) escape from anti-WTA antibody-mediated opsonophagocytosis, whereas α-GlcNAc WTA-deficient Δ<I>tarM</I> mutant cells (which have intact β-GlcNAc) are efficiently engulfed by human leukocytes via anti-WTA IgG. Likewise, MBL binding in <I>S. aureus</I> cells was lost in the Δ<I>tarMS</I> double mutant but not in either single mutant. When we determined the serum concentrations of the anti-α- or anti-β-GlcNAc-specific WTA IgGs, anti-β-GlcNAc WTA-IgG was dominant in pooled human IgG fractions and in the intact sera of healthy adults and infants. These data demonstrate the importance of the WTA sugar conformation for human innate and adaptive immunity against <I>S. aureus</I> infection.</P>
