        전층각막이식 봉합사 제거 후 외상에 의해 발생한 지연성 축출성 맥락막상강출혈 1예

        Purpose: To report a case of delayed-onset expulsive suprachoroidal hemorrhage due to trauma after removal of a penetrating keratoplasty suture. Case summary: A 66-year-old man had penetrating keratoplasty for bullous keratopathy performed in his left eye. After 1 year, the continuous suture was removed for adjustment of astigmatism. Four days after removal of the suture, the patient struck his left eye with the back of his hand, although at the time of injury he had no specific symptoms. Two days later, the patient noticed abrupt pain, decreased visual acuity, and massive hemorrhage in his left eye. Examination revealed an inferior wound dehiscence of approximately 8 clock hours (2 thru 10 o’clock) with prolapsed intraocular contents such as the uvea and retina. The patient underwent cornea graft resuturing with resection of ocular contents, which could not be repositioned. Although bleeding was controlled after the operation, vision was lost in his left eye. After two months, according to decrement of intraocular hematoma, the patient had a phthisis bulbi with low intraocular pressure in his left eye. Conclusions: Persistent low intraocular pressure due to wound leakage through the graft-host junction likely resulted in delayed onset of expulsive suprachoroidal hemorrhage. The risk of traumatic corneal graft rupture after penetrating keratoplasty is significant and is associated with a poor visual outcome and eyeball viability. Therefore, patients should be cautioned.         백내장수술 중 낙하된 유리체내 수정체로 의뢰된 잔류수정체 제거술의 결과

        Purpose: To assess the clinical features and outcomes of patients referred for management of dropped lens fragments during cataract surgery. Methods: The medical records of 22 eyes from 22 patients who were referred to our hospital to undergo pars plana vitrectomy (PPV) for management of dropped lens fragments after phacoemulsification surgery at private clinics were reviewed. Data including patient demographics, preoperative and postoperative visual acuity, factors associated with dropped lens fragment, and postoperative complications were recorded. The factors were compared between 2 patient groups: postoperative Snellen acuity of 0.5 or better and acuity less than 0.5. The statistical significances of differences in factors between the 2 groups were calculated. Results: The mean interval between cataract surgery and PPV was 2.6 ± 3.7 days. At the final examination, the mean postoperative acuity was 0.57 ± 0.20, and 16 eyes (72.7%) had a visual outcome of 0.5 or better. After excluding 7 eyes with other pre-existing ocular co-morbidities, 14 eyes (93.0%) achieved a final visual acuity of 0.5 or better. Multivariate analysis showed that the predictor for visual outcomes of 0.5 or better was absence of preoperative eye disease (p = 0.007). Complications after PPV included 2 (9.0%) cases of cystoid macular edema and 1 (4.5%) case of retinal detachment. Conclusions: Prompt referral and surgical management within 1 week for dropped lens fragments during cataract surgery may achieve a better visual outcome in cases with no pre-existing eye disease.         눈꺼풀에 발생한 림프종모양 구진증 1예

        Purpose: Lymphomatoid papulosis (LyP) is one of the primary cutaneous CD30-positive lymphoproliferative disorders. LyP of the eyelid has rarely been reported. Herein, a case of typical LyP of the medial canthal area is reported. In addition, a literature review was performed. Case summary: A 40-year-old female presented with a skin mass in the medial canthal area of the left eye that developed 2 months earlier. Initially, a focal skin lesion developed, and even with conservative treatment at a local clinic, progressed to a mass lesion having a central ulceration and adjacent edema. After 6 weeks, the adjacent edema had gradually decreased. On ophthalmic examination, the left medial canthal lesion was a 6 x 6 mm sized elevated mass with a central crater covered by crust. The clinical impression was keratoacanthoma. The lesion was widely excised and reconstructed by a full-thickness skin graft after an incisional biopsy. Histopathologic findings showed dermal infiltration of various inflammatory cells with atypical lymphocytes showing positivity to the CD30 antigen, and LyP was diagnosed. Systemic evaluation showed no evidence of systemic lymphoma and the patient has remained free of recurrence or systemic disease after a 1-year follow-up.

        「ようだ·みたいだ」의 습득에 관한 연구 -L2학습자와 일본어모어화자의 발화에 주목하여-

        본 연구는 L2학습자가「ようだ·みたいだ」를 어떻게 습득하는가, 그 출현과 접속, 경합 등 습득의 제상을 밝히기 위한 것이며 그 경향을 일본어모어화자와 비교함으로써 L2학습자만의 특징을 밝히는 것을 목적으로 하고 있다. 본 연구의 결과 L2학습자는、①「ようだ·みたいだ」가 놓여지는 위치에 의해 서로간의 구별을 하고 있는 경향, ②접속하는 품사에 의해 「ようだ·みたいだ」를 구별하는 경향, ③회화 상대와의 친밀도에 의한 형식의 구별은 하고 있지 않는다는 것을 알았다. 그리고 ④그러한 경향은 일본어모어화자와 대부분일치하고 있다는 것이 밝혀졌다. 이러한 결과는 L2학습자의 「ようだ·みたいだ」의 사용에 일본어모어화자의 사용이 영향을 끼치고 있을 가능성과 문법적인 의미가 유의한 항목이라고 하더라도 학습자들은 각각 다른 것으로서 인식하며 다른 습득과정을 거칠 가능성을 시사하는 결과라 할 수 있다. 또한 L2학습자는 자신들이 만든 학습 전략을 가지고 두 항목을 구별하고 있으며 이러한 학습자들의 중간언어의 존재는 앞으로의 습득연구 재인식의 필요성을 보여주는 것이라 할 수 있다. In this study, I analyzed it for “Yoda and Mitaida” how L2 learner learned them. As the method, I check the appearance and connection, diverse aspects of the acquisition including the competition and compare the tendency with the tendency to use of the Japanese native speaker. It was thereby intended to clarify characteristics in the acquisition of the L2 learner. This result, I can focus as us to suggest that there is a possibility that the use of Japanese native speaker are affecting use of the L2 learners “Yoda and Mitaida”. In other words, Japanese native speaker Although an item that grammatical meaning is similar, it is recognized as another for L2 learners, likely to follow to learn different routes each can be considered. Such a result is meaning that L2 learners uses both items properly using his strategy, and it is thought that such existence of a student``s intermediate language is what shows the necessity of having a new appreciation of the state of future acquisition research.

        학습레벨에 따른 접속조사 「が・けど」의 습득에 관한 연구 -공기(共起)표현에 주목하여-

        This study aims to examine the pattern of creation of intermediate languages centered on units that appear in the learning process of reverse access survey Ga and kedo by large JFL environmental learners and their relationship with the type of language of learners. A brief summary of the study results is as follows. The tendency of JFL learners to acquire “Ga and kedo” can be summarized as follows: (1) The acquisition of “ga and kedo” is carried out on the axis of “kedo”. (2) “ga” shows little difference in use depending on the level of learning, but “kedo” changes the access item according to the level. (3) The form of the transfer of “ga and kedo” was concentrated in the midbody (“desu/masu type”). (4) “Ga” does not show the unit used in conjunction with a particular item, but kedo shows the use of a particular unit (“Ndesukedo”). (5) The unit “Ndesukedo” rarely appears at the primary and secondary levels, but it is apparent as the level rises. (Dongguk University)

        「からㆍので」の習得に関する研究

        This study is one that was analyzed for acquisition process of “kara node” of JFL learners. And I analyzed how the use of “kara node” about what is related by mother tongue and learning level. The result of this analysis is summarized as follows. (1) In JFL student’s use of kara and node, kara serves as an axis. (2) The politeness levels is related to a JFL student’s use of kara and node. (3) Language acquisition level is related to a JFL student’s use of kara and node. (4) The JFL student’s native language is related to his or her use of kara and node. In conclusion, the acquisition of “kara node” of JFL learners, in one form is used as an axis of learning. The strategy that is co-occurrence grammar other items in the process was observed. However, the tendency was different by factor of native language and level.

        推量,樣態表現の習得に關する硏究: L2學習者の 「ようだ,みたいだ,らしい,そうだ」 に着目して

        本硏究は推量·樣態を表す日本語の助動詞「ようだ·みたいだ·らしい·そうだ」を對象にL2學習者がそれらをどのように習得していくのか、その出現や接速、競合など習得の諸相を明らかにしすることを目的とした. L2學習者の結果は下記のようにまとめられる. (1)L2學習者の推量·樣態表現においては「みたいだ」の使用が最も多く、「らしい→ようだ→そうだ」の順で使用が多く見られる. これらの習得順序は、開ね「みたいだ(または「ようだ」)→そうだ→ようだ(または「みたいだ」)→らしい」の順になるが、學習者それぞれによってその順序は多少異なる. (2)L2學習者の推量·樣態表現の習得には「特定の使い分けの基準」が存在し、その基準は主に形式による使い分け(共起する品詞、置かれる位置)によって構成される. (3)L2學習者の推量·樣態表現の習得における「特定の使い分けの基準」は、學習初·中期段階に主に使用され、學習が進むにつれその基準は次第に緩くなる. (4)L2學習者の推量·樣態表現の習得における(1)~(3)の傾向は、母語が異なる學習者に共通して見られる. このように、L2學習者は推量·樣態表現を表す4つの形式を炳用·競合·變化·定着させるという一連のストラテジ一を用いながら習得していき、このプロセスの中で共起する品詞、置かれる位置という形式要素がその習得に强く關女していると言える. このような學習者の中間言語の存在は、今後の習得硏究の在り方を再認識する必要性を示してくれるものであると考えられる.

        日本語の反定·條件表現習得の普遍性と特殊性に關する硏究 ─ 大規模橫斷コ一パスから見られる學習者のユニット形成と母語との關連について ─

        In this study, I analyze the usage tendency of the condition expression of large-scale JFL environmental learners and analyze the factors that influence acquisition of condition expressions, such as what features are there (learners' mother tongue factor ·learning environment Factors) and the characteristics of use. The results from this study are summarized as follows. (1) In the case of JFL environmental learners, it was confirmed that “Tara” was the center of acquisition of condition expression, and that the condition form other than “Tara” was actively used. (2) From the JFL environmental learners, there was a difference in unit formation tendency by language type. (3) It is found that the unit formed from the learning process of condition expressions may vary depending on the learning environment. By focusing on the utterance of the learner, this study was able to draw out the relation with the new viewpoint of the characteristics of acquisition of condition expression of L2 learner. In the future, we will explore new factors that will affect learning from units and further advance research.

        심포지엄 : 共起關係から見た日本語の條件表現の習得硏究 -JSL學習者の使用と敎科書の分析, 比較から-

        고는 제2언어학습자에게서 보이는 조건표현의 습득 프로세스에는 어떠한 특징이 있는가, 학습자의 조건표현 습득과정의 한 부분을 명백히 하는 동시에 그 사용 원인의 하나인 학습교재(=교과서)와의 관련성에 대해 검토한 것이다. 선행논문에서 지적된 과제에 대해 본고에서는 발화데이터·일본어 교과서를 이용하여 분석·고찰한 결과, L2학습자는 조건표현마다 특정한 형식을 지닌 표현에 주목하여 그것을 사용함으로써 조건표현 간의 구별을 하고 있음을 알 수 있었다. 동시에 이러한 L2학습자의 사용(구별)에 교과서가 영향을 끼치고 있을 가능성은 낮다는 것을 알 수 있었다. 또한 상기의 결과로부터 명시적으로 문법항목을 제시하며, 풍부한 예문을 보여줌으로써 학습자 스스로가 언어형식에 주목하며 규칙을 유출해 가는 지도방법인「form-focused instruction」을 소개하고 보다 효과적인 조건표현의 지도 방법에 대해 제안하고 있다.
