Reconstruction of submental defects is a challenge that needs to be approached carefully, since many important anatomical structures are located in this small space. Both aesthetic and functional outcomes should be considered during reconstruction. In this report, we describe a case where a superficial branch of the transverse cervical artery (STCA) perforator propeller flap was applied for coverage of the submental area. An 85-year-old woman presented with a 3-cm ovoid mass on her submental area. We covered the large submental defect with a STCA rotational flap in a 180° propeller pattern. The flap survived well without any complications at 1 year of followup. A STCA propeller flap is a useful surgical option in reconstruction for defect coverage of the submental area.
Closed extensor tendon rupture caused by Kienböck disease: a case report

Kienböck disease, a rare disease that can cause chronic pain and motor dysfunction, occurs due to avascular necrosis of the lunate bone, which leads to dislocation of the carpal bone. Among various other etiologies, Kienböck disease can cause closed tendon rupture of the finger. In this report, we introduce a case of total rupture of the second extensor digitorum communis and the extensor indicis proprius tendons caused by undiagnosed Kienböck disease in an elderly female patient.
상이동 촉매 기법(phase transfer catalyst)을 이용한 rose bengal 시약의 최적 조성에 관한 연구

상이동촉매기법을기반으로 한rose bengal 시약은 중성이나염기성용액에서 녹지않는 난용성염을형성 하는칼슘을타겟팅하는 시약으로, 물에 젖은 검체에서 잠재지문 현출시 효과적일 것으로 기대되어 최근 연구가 이루어지고 있다. 그러나 그 동안 선행연구에서는 rose bengal 시약으로 현출된 지문의 형광 특성을 관찰하지 않았을 뿐만 아니라 제시된 시약의 제조법 또한 최적 현출 조건인지 검토되지 않았다. 이에 본 연구는 rose bengal 염료의 최대방출형광을 바탕으로 rose bengal염료와 상이동촉매제(Tetrabutylammonium)의 농도를 달리하여 최적의 시약 조성을 찾고자 하였다. 그 결과 rose bengal과 상이동촉매제의 농도가 각 0.01 M : 0.008 M일 때 가장 효과적인 것을 확인할 수 있었다. The rose bengal reagent, based on the phase transfer catalysis technique, is a calcium-targeting reagent that forms an insoluble salt that does not dissolve in a neutral or basic solution. It is expected to be effective in developing a latent fingerprint in a wet sample. However, many previous studies did not observe the fluorescence of the developed fingerprints, nor were the proposed methods of producing the reagents the optimal develop conditions. The aim of this study was to investigate the optimum reagent composition of rose bengal by varying the concentration of rose bengal dye and phase transfer catalyst based on maximum emission fluorescence of rose bengal. As a result, it was confirmed that rose bengal and surfactant concentration were the most effective when 0.01M: 0.008M, respectively.

A hemangioma in the knee joint of a 13-year-old girl was diagnosed by magnetic resonance imaging. There was pain and a tender localized swelling around to the patellar tendon. Arthroscopy verified the diagnosis but did not allow visualization of the full extent of the tumor. After arthroscopic excision of the associated hypertrophic medial plica, a miniarthrotomy was done, The tumor was attached to the periphery of the anterior horn of the medial meniscus, the anterior tibial plateau, and the fat pad. Its origin was from the venous plexus just below the anterior lip of the central tibial plateau. The patient was asymptomatic one and a half years after surgery. If the case is not amenable to arthroscopic surgery, open excision through miniarthrotomy ensures complete removal of the hemangiomas withont recurrence.

Most fractures of the shaft of the femur are caused by high-energy trauma. It would be expected that in many cases the ipsilateral knee ligaments are subjected to severe stress. In these days, early diagnosis and proper treatment of combined ligament injury in ipsilateral femoral shaft fracture become to be important and are possible by arthroscope and MRI. We retrospectively reviewed a series of 97 patients with 97 fractures of the femoral shaft from March 1995 to December 1997. Demonstrable ipsilateral knee ligament laxity was present in 10(l 1.3 per cent) of these patients. There were 7 males and 8 left femur fractures. Eight of them were injured by traffic accident. Ten patients were followed for an average months. PCL injuries were five cases and ACL and MCL injuries were two cases each and posterolateral instability was one case. Early diagnosis was possible in MCL and ACL cases but diagnosis was delayed to average 10 months post-accidentally in PCL injuries. MCL injuries and one ACL injuries were treated conservatively and one ACL and one PCL avulsion fracture were treated with pull-out suture technique and another 4 PCL injuries were treated with reconstruction using bone patella tendon bone. From this study, we advocate careful assessment of the knee, especially PCL injury in all cases of fracture of the femur caused by high-energy trauma.

현대사회에 앉아있는 사람들의 자세를 보면 거북목인 사람, 등이 굽어 구부정한 사람 등 앉아있을 때 바르지 못한 자세를 많이 볼 수 있고 이처럼 사람들의 자세는 올바르지 못한 형태를 띠고 있다. 앉아있는 시간이 길어질수록 자세는 더욱 불안정 해 질 것이고 허리디스크는 더욱더 쉽게 올 것이다. 더욱이 앉아있을 때 장시간 스마트폰사용과 컴퓨터사용으로 인해 올바르지 못한 자세는 더욱 악화되고 또한 불규칙적인 생활로 인해 질병 및 건강도 함께 나빠지고 있는 상태이다. 이러한 결과로 인해 스마트 웰니스 체어 시스템 개발을 고려하게 되었고, 이를 제작하여 사용한 결과 올바르지 못한 자세를 교정 및 개선 할 수 있었으며 또한 헬스케어기능을 부가함으로써 자신의 건강상태를 확인하고 악화되는 것을 예방할 수 있음을 확인하였다.
