        호기수소측정법을 이용한 Cisapride 와 Tiropramide 의 구강 - 맹장 배출시간의 영향

        N/A Objectives: A few gastroenterologists would underrate the importance of gastrointestinal motility in the pathogenesis of gut symptoms, particularly as the majority of clinic patients have functional symptoms. The aim of the present study was to determine whether changes in oral to cecal transit time affect the magnitude of the hydrogen (H2) excretion after ingestion of unabsorbable carbohydrate. The lactulose H2 breath test is in use as a simple non-invasive measurement of oral to cecal transit time and its reproducibility. The effect of tiropramide hydrochloride and cisapride on oral to cecal transit time have been compared in a placebo-controlled study. Methods: This studies including forty-one healthy volunteers by interval sampling of end expiratory H2 concentration after ingestion of: (1) 10 g lactulose with placebo; (2) 10 g lactulose with tiropramide hydrochloride (Tiropa®); (3) 10 g lactulose with cisapride (Prepulside®). A minimum of six days elapsed between the difference tests to avoid colonic bacterial metabolic changes. Results: The oral to cecal transit time (OCTT) measured by placebo ingestion, tiropramide and cisapride were 90.24±28.33, 102.44±31.37 and 83.41±27.17 minutes. A short of OCTT in cisapride ingestion showed significance compared with the placebo, tiropramide (p<0.0005). Area under curve (AUC) of the H2 concentration versus time curves from the OCTT after ingestion of placebo, tiropramide and cisapride were 35.50±24.35, 33.78±24.28, 45.65±26.04 (ppm×min×100), respectively. A rise of AUC in cisapride ingestion showed no significance compared with the placebo, tiropramide. Conclusion: The present findings indicate that changes of the OCTT do not significantly influence the H2 excretion as measured by interval sampling of end expiratory H2 concentrations after different drugs. In conclusion, within the ranges observed, the OCTT does significantly affect the shape of the H2 concentration after ingestion of differential drug.

      • 용혈성 빈혈과 흑수열을 동반한 Chloroquine저항성 수입 열대열 말라리아 1예

        저자들은 최근 아프리카를 여행하는 도중 말라리아의 예방약제인 chloroquine을 복용하였음에도 불구하고 용혈성 빈혈과 흑수열을 동반한 chloroquine 저항성 수입 열대열 말라리아를 Fansidar를 투여하여 치료가 된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. This previously healthy 26 year-old student was admitted to Cho Sun university hospital due to paroxysmal high fever, dizziness of 2 weeks duration and he also noted passage of dark brownish colored urine of 3 days duration. He had made journey to Africa for 2 months before admission. He had taken chloroquine 1 week before his jurney and continued this weekly throughout his stay. On admission, there was fever of 38.5℃ and numerous parasitemia of ring form of P. falciparum in the peripheral blood smear. On physical examination, he had jaundice and splenomgaly. On laboratory examination, he had jaundice and splenomegaly. On laboratory examination hemoglobin was 5.Og / dl, reticulocytecount was 11%, plasma hemoglobin was 7.5mg /dl and urine hemoglobin was 2.5mg /dl. He received Fansidar on 2nd day of admission. Five days later, his blood was completely clear of parasite and 7th day after admission, all symptoms were almost improved and then he was discharged.

        Toxic oxygen free radicals have been implicated as important pathologic mediators in many clinical disordesrs. Enhandng the intracellular content of antioxidant enzymes can provide means of limiting biological damage caused by oxygen free radicals. Many investigations have examined the potential of various oxyradical scavengers to modulate oxyradical injury. The oxygen free radicals play a role in the pathogenesis of chronic obstructive pulmonary disease. To investigate the pulmonary oxyradical injury and the protective role of antioxidant enzymes, author measured the amount of thiobarbituric acid reactants, the activiteies of antioxidant enzymes (superoxide dismutase, glutathione peroxidase, and catalase) and the sulfhydryl groups in serum and red blood cells from the chronic obstructive pulmonary disease groups and the normal controls. The thiobarbituric acid reactant in chronic obstructive pulmonary disease groups was increased than the controls in both serum and red blood cells, and the superoxide dismutase activity in red blood cells was no statistically difference in both groups. But, the glutathione peroxidase and catalase activities were lowered in chronic obstructive pulmonary disease groups than the controls. The sulfhydryl groups in serum and red blood cells were no statistically difference in both groups. These results suggest that the increased thiobarbituric acid reactant in chronic obstructive pulmonary disease groups mean oxygen radical toxicity, and the decreased glutathione peroxidase and catalase activities may play a role in cellular injury.

        발작성 야간혈색소뇨증에서 병발한 급성 신부전증 2 예

        Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired disorder involving multiple hematopoietic cell lines characterized by hemolytic anemia with accompanying hemoglobinuria usually occuring at night. exacerbation of the hemolytic process following the transfusion of whole blood or plasma, such as occurs in paroxysmal nocturnal hemoglobinuria has been reported to precipitate acute renal failure. We present here two cases of paroxysmal nocturnal hemoglobinuria developed acute renal failure (ARF) due to hemolytic transfusion reaction and treated by steroid, diuretics and fluid therapy. The first case was refered to the hospital that had been following up before and the second was discharged with symptomatic and hematologic improvement.
