        수정체유화술에서 프리챠퍼 사용이 초음파 에너지량의 감소에 미치는 영향

        Purpose: To compare the effectiveness and safety of the phaco prechopper technique dividing lenses into 2 or 4 pieces before phacoemulsification with the effectiveness and safety of conventional phacoemulsification during cataract surgery. Methods: This study included 360 eyes, which were divided into 4 groups according to nuclear opacity each group was subdivided into 3 groups according to the lens extraction technique (control, dividing the lens into two pieces using a phaco prechopper, and dividing it into four pieces) (Table 1). The following parameters were compared between the groups that had the same degree of nuclear opacity: changes in the endothelial cell count and hexagonality 2 months postoperatively, phaco time as well as the total phaco energy used for the operation, and the complication rates. Results: There were no intra- and post-operative complications in any group. The endothelial cell loss rate was significantly less in nuclear opacity groups 2 and 3 with the prechopper technique than in the control group. Hexagonality was significantly higher in the nuclear opacity group 3 with the prechopper technique than in the control 2 months after the operation. Phaco time was significantly less in the nuclear opacity groups 2 and 3 with the prechopper technique than in the control. Total phaco energy was significantly less in the nuclear opacity groups 2, 3, and 4 with the prechopper technique than in the control. Conclusions: Phacoemulsification using the prechopper technique is safe, resulting in significantly less endothelial cell damage, and requires less phaco time and less total phaco energy compared to those of the conventional phacoemulsification technique.

        Turbiscan<sup>®</sup>을 이용한 Glyceryl Monooleate(GMO) 함유 W/O 유제의 안정성 평가

        The main object of this study was to prepare of w/o emulsion including glyceryl monooleate(GMO) and to evaluate its stability by using the recently developed $Turbiscan^{(R)}LAB$. GMO is the polar oily surfactant with the low HLB value, and it forms the gel phase of cubic structures after dissolves in aqueous media. Phosphate buffer solution (PBS) of pH 7.4 was prepared as the water phase and Marcol 52(mineral oil) was used as the oil phase in this study. GMO was used as the surfactant of W/O emulsion. W/O emulsion using GMO alone as a surfactant was very unstable. But the emulsion using both GMO and poloxamer 407 was more stable. The stability of W/O emulsions was evaluated after centrifuging the emulsions. But it was difficult with naked eye because an opaque and concentrated system like W/O emulsion was very turbid. So $Turbiscan^{(R)}LAB$ was used to detect the destabilization phenomena in non-diluted emulsion. As a result, the W/O emulsion using the proper amounts of GMO and poloxamer 407 was more stable among them using GMO of various amounts. But it seems that the other element for the stability of W/O emulsion including GMO was required. Furthermore, the $Turbiscan^{(R)}LAB$ was a very efficient analyzer for evaluating the physical stability of emulsion.

        3년제 보건계학과의 효율적 운영방안

        Most of the Medical Junior Colleges faced new environmental changes: School years of some Health-related departments were extended from two-year program to three-year ones; The total high school graduates willing to apply to the colleges will fall short of the legal number limits of this country; Govorment's educational opening policies enables competitive foreign schools to land in our country more easily. In order to survive under these turbulant circumstances, any junior college has to cultivate it's own competitiveness establishing strategies and utilizing resources very efficiently. Some of the major points extracted from three-year junior college professors' opinions against the environmental changes are as follows, 1. Environmental changes should be fully appreciated. And junior colleges have to make haste to establish strategies placing priorities on specific fields which are their own strengths compared to others. 2. The crieria of provisions of miminstry of Education on the facilities and equipments should be enhanced. 3. The practical way to use available resources effectively is to make the best of human resources. 4. It is time to consider consolidating akin departments to one. 5. The school year of the health-related departments should be extended to four years to meet the contemporary needs in the medical fields.

        3D CAD 데이터 기반의 효율적 철근 요소 생성 알고리즘

        본 논문에서는 철근콘크리트 구조물의 3차원 디지털기술 기반 설계에 필요한 CAD/CAE 데이터 연동을 위하여 3D CAD 데이터로부터 철근 요소를 생성하는 2단계 알고리즘을 제안하였다. 1단계 과정은 STEP 표준형식의 3D CAD 데이터를 분석하여 여러 형태의 철근 객체 정보를 추출하는 알고리즘이며, 2단계 과정은 추출된 객체 형식의 철근 정보로부터 유한요소해석에 필요한 1차원 철근요소를 생성하는 알고리즘이다. 개발된 알고리즘을 구현하여 프로그램을 작성하였으며, 이를 사용하여 실제 3D CAD 소프트웨어로부터 생성된 철근콘크리트 구조물 데이터를 유한요소해석 데이터로 성공적으로 변환하여 제안된 알고리즘이 유효함을 보였다. In this paper a two-step algorithm is proposed to efficiently generate rebar elements from 3D CAD data in the context of CAD/CAE data transfer. The first step is an algorithm to identify various type rebar objects and their attributes by analyzing 3D CAD data in STEP format, which is one of the international data standards. The second algorithmic step is a procedure to generate one-dimensional rebar elements from the object data made through the first step for finite element analysis or other CAE tasks. Successful rebar element data generation from real 3D CAD data for a reinforced concrete structure shows the efficacy of the proposed algorithm.

        철결핍성빈혈 진단과 RDW, MCV

        자동혈구계산기에서 산출되는 RDW나 MCV가 철결핍성빈혈의 구분진단에 어떠한 도움이 되는지를 보기 위하여 어느 대학병원의 최근 5년 동안의 의무기록과 건강진단결과를 이용하여 227명의 빈혈환자와 143명의 건강인을 선정하고 그 들의 혈액검사 결과를 비교분석해 보았다. 분석결과 빈혈환자로서는 철결핍성빈혈과 재생불량성빈혈 환자가 많았으며, 그 외에 만성질환과 관련된 빈혈환자도 많은 것으로 나타났다. 정상인 들에 비하여 빈혈환자에서 RDW는 높게, 그리고 MCV는 낮게 나타났는데 특히 철결핍성빈혈에서는 다른 빈혈에 비하여 19.3±4.8로 현저하게 높게 나타난 반면, MCV는 62.9±13.7fl로 현저하게 낮게 나타났으며, 그 빈혈의 정도에 따라 그 변화가 크게 나타났다. 빈혈관련수들을 이용하여 D.F.=0.26-0.012MCV-0.130MCH+0.073MCHC+0.052RDW+0.003PLT와 같은 판별함수가 도출되었고, 이를 토대로 철결핍성빈혈과 다른 빈혈을 구분하는데 있어서는 MCV, MCH, MCHC, RDW등이 판별력이 있다는 것을 확인할 수 있었다. RDW and MCV are thought to be the highly sensitive blood cell parameters in the differentiation of iron deficiency anemias. Through the medical records of 227 anemic patients and the physical checking results of 143 healthy persons in a General Hospital during the recent five years, the authors evaluated various blood cell parameters including RDW and MCV.Iron deficiency anemia, aplastic anemia and other anemias associated with chronic disease were shown as the three major causes of anemias in Korea. In the patients of iron deficiency anemia MCV was very low(62.9±13.7fl), while RDW was very high(19.3±4.8) showing much lower MCV and much higher RDW in severe IDA compared with in mild IDA. To differentiate iron deficiency anemias form other anemias, a discriminant function was developed from some blood cell parameters like MCV, MCH, MCHC, RDW and platelets(D.F.=0.26-0.012MCV-0.130MCH+0.073MCHC+0.052RDW+0.003PLT).

      • 예술적 환영의 지위와 그 근거

        본고는 예술적 환영의 존재론적 지위와 그것의 근거에 대해 답하고 있다. 이 논문의 주된 참조 지점은 셔번의 명제 미학이다. 셔번은 화이트헤드의 명제 개념을 빌려, 예술적 창조, 예술작품의 존재론적 지위, 미적 경험 등을 일관되고도 탁월하게 설명해 낸다. 특히 그는 예술적 대상은 화이트헤드적인 의미에서 명제의 존재론적 지위를 가졌다고 말한다. 구체적인 예술작품은 객체화된 명제이며, 미적인 경험은 이 객체화된 명제를 수용자가 자신의 경험에서 주체적 지향의 요소로 받아들여 그 명제를 재창조함으로써 이루어진다고 본다. 그러나 나는 셔번의 명제 미학에서 힌트를 얻어 예술작품에서 파생되는 예술적 환영이 곧 명제라고 주장한다. 그리고 그것의 근거로서 셔번이 말하는 수평적 변환과 이면성의 느낌을 제시하며, 나아가 신체를 예술적 환영의 중요한 하나의 근거로서 부각시킨다. In this thesis, I purposed to answer to the ontological status of artistic illusion and its reason. The theory which I mainly refer to is Sherburne`s aesthetics of proposition. He has explained the nature of artistic creation, the ontological status of work of art, and artistic experience coherently and prominently with Whitehead`s concept of proposition in his book, A Whiteheadian Aesthetic. Especially, he argued that the artistic object has an ontological status of proposition in Whiteheadian sense. According to his theory, a concrete work of art is an objectified proposition, and aesthetic and artistic experience is an experience of recreation of that objected proposition in a receiver`s experience. However, I argue that artistic illusion derived from the work of art is the proposition. And I adduce the horizontal transmutation and the sense of ulteriority. Add to this, I emphasize the human body as an important reason in the artistic experience of artistic illusion.

      • 대학생의 우울취약성 성격양식이 진로결정수준에 미치는 영향 : 불확실성에 대한 인내력 부족의 매개효과를 중심으로

        본 연구의 목적은 대학생의 우울취약성 성격양식이 진로결정수준에 미치는 영향을 알아보고, 대학생의 우울취약성 성격양식과 진로결정수준 간의 관계에서 불확실성에 대한 인내력 부족의 매개역할을 검증하는 것이다. 연구대상은 서울, 경기, 인천 소재의 대학교에 재학 중인 남녀 대학생 450명이며, 회수된 설문지 중, 응답이 불성실하거나 누락된 23부를 제외하고, 총 437부(남자 176명, 여자 261명)의 자료를 최종 분석에 사용하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 진로결정수준에 대해 불확실성에 대한 인내력 부족이 가장 영향력이 높은 변인으로 나타났다. 둘째, 불확실성에 대한 인내력 부족이 우울취약성 성격양식과 진로결정수준 간의 관계를 부분 매개하는 것으로 나타났다. This Study aims to examine the effect of personal pattern of vulnerability to depression on career decision level in undergraduates and to test the role of lack in patience due to uncertainty as a medium between two variables. This study distributed and retrieved surveys to 450 male and female undergraduate students who are in four-year universities which locate in Seoul, Gyeonggi, and Incheon area. Among retrieved surveys, 23 unreliabel response along with missing answers were excluded. in total, 437 surveys(176 males and 261 females) were analyzed as a final research. The results came out as below. First, career decision level was mostly accounted fro by lack of patience due to uncertainty. Second, lack of patience due to uncertainty had partial mediating effects in influences of personality pattern of vulnerability to depression on career decision level in undergraduates.
