Objective : The purpose of this paper is to report the effect of Daehamhyung-tang(DHHT) on tinnitus with panic disorder from the disease pattern identification diagnostic system based on Shanghanlun provisions. Methods : According to a disease pattern identification diagnostic system based on Shanghanlun provisions, the patient was diagnosed with 'Taeyang-byung Gyeolhyung'. He took DHHT for 82 days. This paper evaluated the results of treatment on tinnitus by Tinnitus Handicap Inventory(THI) and Verbal Numercial Rating Scale(VNRS). To assess the mental state of anxiety, this paper used Hamilton Anxiety Rating Scale(HAM-A) Results : The THI score was declined from 58 to 6 And the VNRS score was declined from 6 to 1. HAM-A score was declined from 28 to 3. Conclusions : This case study suggests that DHHT has effectiveness on a patient suffers from tinnitus with panic disorder and diagnosed with 'Taeyang-byung Gyeolhyung'.

An analysis on Landscape Structure and Biodiversity of the Bokha Stream as a Model to Restore the Degraded Urban Stream

Landscape structure, habitat types, vegetation structure and biodiversity in the Bokha streamchosen as a reference stream were investigated to get ecological information necessary for restoration of urban stream degraded by excesive artificial interference. Landscape structure showed a slight change betwen before and after flooding. Habitat types of nine sorts were identified based on ecological information obtained from field survey such as micro-topography, hydrological characteristics, disturbance regime, and so on. Each habitat holds specific organisms to each site. Consequently, the number of plant comunities, and species of benthos and fish increased as the kinds of habitat type increase. Ordination of habitat types based on vegetation, benthos, and fish data reorganized them into three groups of pool types of two kinds depending on whether they are connected to the water course or not and riffle one. Vegetation showed different stratification and species composition depending on topographical position in relation to disturbance cycle. Based on the results from this study, relationship between environmental heterogeneity and biodiversity was discussed and a restoration plan was suggested in a viewpoint of vegetation.

대승기탕(大承氣湯)으로 호전된 조현병 환청에 대한 증례 1례

Objective : We report efficacy of Daeseunggi-tang on auditory hallucination in schizophrenia. From this, we can expect therapeutic possibility of herbal medicine chosen by 'A disease pattern identification diagnostic system based on Shanghanlun provisions' in schizophrenia. Methods : Daeseunggi-tang written in YangMyeong-JiWiByeong is applied to 31-year-old patient who diagnosed as schizophrenia and hospitalized for 3months taking antipsychotics. The patient was treated for a year. Results : Scores in Auditory Hallucination appended to PANSS and K-BDI were highly decreased. The patient takes no antipsychotics anymore for now. Conclusions : Auditory hallucination in Schizophrenia which needs antipsychotics can be managed by herbal medicine chosen by 'A disease pattern identification diagnostic system based on Shanghanlun provisions'.

황련아교탕(黃連阿膠湯)으로 호전된 3례의 병인(病因) 분석 : 분노와 수면장애에 대한 『상한론(傷寒論)』 기록의 이해

Objective : We analyzed 3 clinical cases to estimate the possibility of shanghanlun as a completed clinical text which has its own diagnostic system. Methods : We collected 3 cases treated by huang lian e jiao tang conducted by a practiced shanghanlun doctor and defined the conceptional meaning of each provisions by analyzing the application of them in each case. Results : Shanghanlun provisions dualized with Je Gang(提綱) and Jo Mun(條文) is the record of preceding factors which describe the phenomenon initiating the diseases. Conclusions : Rudimentarily a factor can bring about many outcomes, so if shanghanlun is structured record of the factors which can make a human sick from many diseases, it can be used as the completed clinical text only with those 113 prescriptions. Factors that shanghanlun recorded seem somewhat different from western conventional medicine because the field of each factors are very varied from biological phenomenon to psychological phenomenon. However it is not that weird because psychosomatic medicine in western conventional medicine also point out the factors come from mental, society and behavior.

대승기탕의 사하작용(瀉下作用)에 대한 약리학적 연구 분석

Objective : This study's purpose was to review the Daeseunggi-tang as the typical purgative. Method : We searched papers about Daeseunggi-tang about the gastrointestinal dysfunction caused by constipation, surgery and MODS. In total, 31 papers were analyzed in this study. Results : In matter of subject, researches on the gastrointestinal dysfunction caused by constipation were 16 cases, researches on the gastrointestinal dysfunction caused by surgery were 8 cases, researches on gastrointestinal dysfunction caused by MODS were 7 cases. Conclusion : In conclusion, the traditional use of Daeseunggi-tang on constipation, frequent flatus, and stomachache has proven to have sufficient evidences through the experimental and clinical evidences.

간 전이 소견없이 유암 증후군을 동반한 십이지장 구부의 유암종 1예

A primary duodenal carcinoid tumor causing carcinoid syndrome is rare. In case of accompanying carcinoid syndrome in a primary duodenal carcinoid tumor, it mostly suggests massive liver metastasis. In rare case, venous drainage of carcinoid tumor and systemic venous drainage are directly connected without passing through the portal system. Therefore, it is rare and interesting case which a primary duodenal carcinoid tumor accompanies carcinoid syndrome without liver metastasis. We experienced an occasion of a primary carcinoid tumor located in duodenal bulb in a 56 year-old woman. This patient came to our hospital because of intermittent diarrhea, epigastric pain and facial flushing. And it was surely diagnosed as carcinoid tumor by gastrointestinal endoscopic biopsy. She was hospitalized for surgery and we were able to confirm that carcinoid syndrome was accompanied through biochemical test. However, metastatic lesion was not found at liver, small and large intestine. We report this case with reference to documents due to rarity.

Vibrio fluvialis에 의한 위장관염 1 예

V. fluvialis는 해양 및 담수 서식 세균으로서 드물게 인체에서 위장감염을 일으킨다. 현재까지 외국에서 보고된 예는 있으나 국내에 보고된 예는 없었다. 저자들은 여름철에 조개(고막)를 먹은 후 심한 복통과 설사 및 탈수증 등의 증상이 1주일간 지속된 55세의 간경변 남자 환자에서 대변배양 검사상 확진된 V. fluvialis에 의한 위장관염 1예를 적극적인 항생제 치료를 하였으나, 간경변 합병증으로 사망한 증례를 경험하였기에 이에 보고하는 바이다. V. fluvialis is a gram-negative, oxidase-producing, halophilic bacterium. It is normally found in coastal waters and seafoods. There have been a few reports on Vibrio fluvialis gastroenteritis in other countries, whereas as there has been no previous report of V. fluvialis infections in Korea. Reports from other countries showed that V. fluvialis was isolated mostly from infants and children. We experienced a race case of gastroenteritis due to V. fluvialis in a 55-year-old man with liver cirrhosis who ate an ark shell. He was admitted due to mild abdominal pain and severe watery diarrhea followed by rapidly progressive dehydration, electrolyte imbalance, and hepatorenal syndrome for a week. This patient was not improved by intensive care and antibiotic therapy.
