Adenosquamous cell carcinoma of tonsil is a rare lesion of head and neck and is often misdi-agnosed as squamous cell carcinoma. It has a very aggressive clinical pattern. We encountered a patient with an adenosquamous cell carcinoma of tonsil and performed various treatment mo-dalities including surgical resection, radiation therapy, chemotherapy but the patient expired two years after the first diagnosis. Such case has never been reported earlier in Korea. Herein, we present this rare case with a review of related literature.

Oncycytoma is a tumor which is abundunt with granular and eosinophilic cytoplasm and mitochondria. It can be found in various organs such as the kidney, thyroid gland, pancreas, and breast. Salivary gland is reported to be involved with oncocytoma in less than 1% among the salivary gland tumors. A 65-year-old female visited presenting with recurrent lump of the left parotid gland. She had been diagnosed with oncocytoma after left superficial parotidectomy 8 years ago. Physical examination revealed multiple painless tumors in the left parotid gland. Ultrasound and computed tomography showed multiple mass in the left parotid gland. The patient had a revision parotidectomy. Pathology confirmed an oncocytoma and nodular oncocytic hyperplasia. We report a rare case of recurrent oncocytoma and nodular oncocytic hyperplasia from the parotid gland and report the case with related literature.

Schwannoma is a benign solitary neoplasm emerging from the Schwann cells of the peripheral, cranial and autonomic nerves. Approximately 25 to 45% of schwannomas occur in the head and neck region. However, schwannoma in the subfrontal area, nasal cavity or paranasal sinus is very rare and accounts for only 4% of these neoplasms. We experienced a case of schwannoma in the subfrontal area and left nasal cavity in a 74-year-old man who complained of recurrent rhinorrhea. 만성 중이염으로 단순유양동삭개술을 시행 받은 환자에서 유양동폐쇄술의 효과

Background and Objectives:This study is designed to examine the efficacy of mastoid obliteration (MO) in pa- tients who received canal wall up mastoidectomy (CWUM) for chronic otitis media (COM). Postoperative tym- panic membrane examination and hearing gain were taken into account. Materials and Methods:69 patients who underwent CWUM and tympanoplasty were included in this study. 44 patients underwent CWUM, tympa- noplasty, and MO (MO group) and 25 patients underwent CWUM and tympanoplasty (CWUM group), These two groups were compared with the postoperative finding of tympanic membrane such as perforation and retrac- tion of tympanic membrane, and recurrence of otorrhea. Preoperative and postoperative air conduction pure tone audiometry were used to compare the postoperative hearing gain of the two groups. To lessen the effect of other factors, 41 patients who underwent CWUM and tympanoplasty type 1 were included in comparing postoperative hearing gain. Results:There was no difference in postoperative finding of tympanic membrane or hearing gain whether MO was performed or not after CWUM and tympanoplasty in COM patients. Conclusions:In COM patients, introducing MO after CWUM and tympanoplasty showed no efficacy in preventing postoperative per- foration and retraction of tympanic membrane and recurrence of otorrhea or yielding better result in postopera- tive hearing gain. However, this study has several demerits such as small number of enrolled patients, short fol- low up period, and to being unable to exclude other factors that might have affected the results. Therefore, I presume collecting larger number of patients and considering more factors might lead to different result and plan to design a study with a significant result.

Woakes’ syndrome is a group of disease which include recurrent nasal polyps resulting in the broadening of the nasal pyramid, the onset of hypoplasia of frontal sinus and bronchiectasis, as well as the production of mucous discharge. Children and young adults are mostly susceptible to Woakes’ syndrome due to the plasticity of the bone. Even though the exact etiology is unknown, genetic factor is thought to be influential because it is often diagnosed in siblings. Otolaryngologically, the mainstream method of removing nasal polyp by endoscopic sinus surgery as well as topical or systemic treatment can be helpful. We report two siblings who visited our clinic both complaining of nasal obstruction. The patients presented with recurrent nasal polyps and showed signs of bronchiectasis, which led to the diagnosis of Woakes’ syndrome. These rare cases are presented here with a review of related literature.

비중격만곡증과 이관기능의 연관성에 대한 연구

Background and Objectives: This study is designed to examine the correlation of the direction of the nasal septum deviation (NSD) and Eustachian tube dysfunction (ETD). Since the Eustachian tube lies between the middle ear and the nasopharynx, we presumed that the direction of the NSD causes narrowing of certain parts of the nasal cavity and can cause the ETD. Materials and Methods: We recruited 50 NSD patients who were in need of septoplasty. We run the test of nasal endoscopy, oto-endoscopy, computed tomography, and Eustachian function test and looked into the correlation of the direction of the NSD and ETD. Results: There were no correlation between the direction of NSD and the side effected by ETD. Conclusion: Narrowed nasal cavity due to NSD is thought to take effect to the pathogenesis of ETD. However, this study has several shortcomings such as small number of enrolled patients and lack of diversity in measuring of Eustachian tube function. Further studies with more patients and detailed considerations are needed, and we plan to design a study that might yield different results.

자치단체의 성공적 복식부기 지식이전에 조직 환경적/지식전달자 특성의 영향

The Korean government is planning to adopt double entry bookkeeping in the local government accounting system. The primary objective of this study is to examine the factors affecting the knowledge transfer and success of double entry bookkeeping in local government accounting system. Success of implement in double entry bookkeeping depends on consultant's ability to transfer skillful knowledge and organizational administration ability.This study based on survey questionnaire data collected from 63 local government, and analyzed the cause and effect relationships between the degree of double entry bookkeeping knowledge transfer and two independent variables such as organization-related characteristics (governor's support, education and training), transferor-related characteristics (task commitment). The influence of the degree of double entry bookkeeping knowledge transfer and two independent variables on the success of double entry bookkeeping was also analyzed.The finding suggests that governor's support and task commitment are important factors to transfer of knowledge in the context of double entry bookkeeping. The degree of double entry bookkeeping knowledge transfer and education and training, and task commitment directly influenced and governor's support indirectly influenced on the success of double entry bookkeeping in local government. IMF 관리체제 이후 투명한 정부회계제도의 개혁이 요구되면서 정부기관에 복식부기 도입의 필요성을 제기되고 있으며, 행정자치부는 1999년 복식부기 도입 추진을 위한 기본계획을 수립하고 강남구와 부천시를 대상으로 복식부기 회계기준 및 전산프로그램 개발과 시험운영을 실시한 이후 2007년까지 전국의 모든 지방자치 단체에 도입을 추진 중에 있다. 본 연구에서는 성공적인 복식부기 제도 구현을 위해, 복식부기 정부회계 컨설턴트로부터 자치단체 공무원에게 복식부기의 성공적 지식이전에 조직 환경적 및 지식전달자 특성이 어떠한 영향을 미치는지를 분석하고자 하였다.분석을 위해 설문에 응답한 전국 63개 자치단체로부터 회수된 216부의 설문지를 활용하여 AMOS 4.0을 활용한 구조방정식 모형을 사용하였다. 변수로는 지식이전요인으로서 조직환경적 특성(자치단체장 지원과 교육 및 훈련)과 지식전달자 특성(지식전달자 헌신), 지식이전 정도로서의 복식부기 지식이전 및 복식부기 성과로서 복식부기 정보 만족도와 복식부기 도입성과를 사용하였다.분석결과, 복식부기 지식이전에는 자치단체장 지원과 지식전달자 헌신이, 복식부기 성과에는 복식부기 지식이전이 유의적인 영향을 미치며, 복식부기 지식이전 요인 중에서 교육 및 훈련은 복식부기 정보 만족도와 복식부기 도입성과 모두에, 지식전달자 헌신은 복식부기 도입성과에 유의적인 영향을 미치는 것으로 나타났다. 또한 자치단체장 지원은 복식부기 성과에 직접적인 영향을 미치지는 않으나, 복식부기 지식이전을 통해 간접적인 영향을 미치는 것으로 나타났다.

정부회계 지식이전 요인, 지식이전 및 성과간의 관계에 관한 연구
