N/A Objectives: Sweet`s syndrome(acute febrile neutrophilic dermetosis) is characterized by fever, neutrophilia, erythematous painful cutaneous plaques, a dense dermal infiltrate onsisting of mature neutrophils, and a rapid response to steroid therapy, This condition is clinically important because it is associated with malignancy and it should be one of the differential diagnosis in the febrile immunocompromized patients with skin lesion, In spite of its clinical importance, there was no report on the epidemiologic characteristics of Sweets syndrome in Korean patients so far because of the rarity of Sweet`s syndrome, This study was performed to investigate characteristics and clinical significance of Sweet`s syndrome in Korean patients. Methods : The authors report a case of Sweet`s syndrome associated with acute myelogenous leukemia, and review the other 23 Korean patients with Sweet's syndrome that have been published in the literature. The characteristics of malignancy-associa- ted Sweet`s syndrome are compared with those of Sweet`s syndrome without malignancy association. Results: 1) Twenty-four cases of Sweet`s syndrome were analysed. Male to female ratio was 8:16 and the mean age was 44.8± 11.7 years. In 79% of patients, fever was noticed during the episodes of Sweet's syndrome. The most frequent sites of skin lesions were the upper extremities(83%), followed by face and head(75%), the lower extremities(75%), neck(46%), and trunk (13%). 2) Associated underlying systemic disorders were present in 15(62%) of the patients. The most frequent associations were chronic inflammatory bowel disease in seven patients, hematologic neoplasia in five, and solid neoplasia in one patients. 3) Leukocyte counts(mean 13,400±8,000/mm3) were higher than 10,000/mm3 in 16(67%) of patients. Erythrocyte sedimentation rates(mean 66.8±44.4mm/ hour) were elevated in 18(82%) of patients. 4) Skin lesion improvements were observed within 2 to 5 days from the initiation of steroid therapy. 5) Malignancies were associated in 6 patients. The initial appearance of Sweet`s syndrome lesions preceded or occurred concurrently with the detection of previously unsuspected malignancy in 67%(4 of 6 patients), In malignancy associated Sweet's syndrome, relative male preponderance, older age, and higher leukocyte counts were observed compared with Sweet's syndrome which is not associated with malignancy. There were no statistical significances, however. The erythrocyte sedimentation rate was the only statistically significant parameter. Conclusion: The characteristics of Sweet`s syndrome in Korean patients were similar to the characteristics of their western counterparts. Workup for malignancy seems appropriate far newly diagnosed Sweet`s syndrome patients in case of elevated erythrocyte sedimentation rate, leukocytosis, old age, and male sex

Performing a brachial plexus block is very useful for shoulder arthroscopic surgery. Several techniques for blocking the brachial plexus have been described with the purpose of improving the efficacy and minimizing the risk. The parascalene approach was introduced in 1979. This block approaches at the lateral border of the anterior scalene muscle and superior to the clavicle. At this level, the incidences of phrenic nerve paralysis and spinal or epidural anesthesia should be minimized. Previous studies have reported on ultrasound-assisted brachial plexus blocks, but few studies have applied this imaging technology to the parascalene region. We report here on 8 cases of parascalene brachial plexus block with using ultrasound guidance to show the clinical usefulness of this technology for conducting arthroscopic shoulder surgery. Ultrasound technology is valuable to anesthesiologists to localize nerves and the needle placement during the parascalene approach to block the brachial plexus for conducting arthroscopic shoulder surgery. 한국영화산업에 있어서 지식기반자원이 성과에 미치는 영향에 관한 실증적 연구

본 연구는 자원준거관점에 입각하여 지식기반자원이 성과에 미치는 영향에 관해, 한국영화산업을 대상으로 하여 실증적인 이해를 도모해보고자 하였다. 본 연구에서는 선행연구에 기초하여 영화의 흥행성과에 영향을 미칠 수 있는 지식기반자원을 지식을 보유하고 있는 인적자원인 제작자, 감독, 작가, 배우로 정의하고 검증하였는데, 그 결과, 이러한 인적자원들이 영화의 흥행성과에 유의한 영향을 미친 것으로 나타나, 이들이 영화산업에서의 경쟁우위를 창출할 수 있는 중요한 지식기반자원임을 확인할 수 있었다.<br/> 본 연구에서는 또한 이들 지식기반자원의 우수성을 예술적 측면과 사업적 측면으로 구분하여 측정하고 영화의 흥행성과에 미치는 영향을 검증한 결과, 제작자의 경우는 사업적 우수성이 성과에 긍정적인 영향을 미치고, 감독과 배우의 경우에는 예술적 우수성이 성과에 긍정적인 영향을 미치는 것으로 나타났다. 이는 영화산업에서 수행하는 역할에 따라서 지식기반자원의 예술적 우수성 및 사업적 우수성이 영화의 흥행성과에 미치는 영향이 다르다는 것을 의미한다. 즉 영화의 흥행성과라는 관점에서 보면, 영화제작에 있어서 사업적 영역의 역할을 하는 제작자는 사업적 우수성이 예술적 우수성보다 중요하고, 예술적 영역의 역할을 하는 감독 및 배우는 예술적 우수성이 사업적 우수성보다 중요하다고 할 수 있다. 한편, 작가의 경우에는 예술적 우수성뿐만 아니라 사업적 우수성도 영화의 흥행에 유의한 긍정적인 영향을 미치는 것으로 나타났다. 이는 영화의 흥행성과라는 관점에서 보면, 작가와 예술적 우수성과 사업적 우수성이 모두 중요하다는 것을 의미한다.<br/> 본 연구의 결과에 나타난, 역할에 따른 예술적 우수성과 사업적 우수성의 중요성의 차이는 영화산업에서의 예술적 가치와 사업적 가치간의 균형이 중요함을 보여주고 있다. Based on the resource-based view of the firm, this study empirically examined the effect of knowledge-based resources on performance. The Korean film industry during 1981-2000 was chosen for the purpose of empirical examination because knowledge-based resources are critical in the film industry.<br/> This study operationally defined knowledge-based resources of film industry as producers, directors, screenwriters, and actors. And it was hypothesized that the superiority of those knowledge-based resources was associated with superior performance.<br/> Empirical results showed that both the artistic and business superiority of knowledge-based resources were indeed associated with superior performance in Korean film industry. However, it was also found that for the case of producers who played commercial roles in film making, only the business superiority was associated with better performance; and reversely for the cases of directors and actors who played artistic roles, only the artistic superiority was associated with better performance. These results suggest that the balance between artistic value and business value is important in cultural industries.

오른쪽 새끼 손가락 말단의 연부조직으로 전이한 유방암 1예

전이암이 사지 말단으로 전이되는 경우는 매우 드물다. 임상적으로 전이성 병변의 마지막 단계로 발생하는 경우가 흔하다. 흔히 병변이 양성종양이나 감염증과 비슷한 양상으로 오진되기도 한다. 드물게 사지 말단의 뼈로 전이된 예를 보고하지만 국내에서는 보고가 드물다. 본 환자는 손가락 끝 연부조직에 국한된 종괴로 발견되었으며 처음에는 유방암의 전이로 생각하지 않았던 증례로 조직검사를 통해 유방암의 전이로 확인되어 보고하는 바이다. Acral metastasis to the finger is a very rare phenomenon. We report herein a case of cutaneous acrometastasis to the right 5th finger tip in a 36-year-old woman with metastatic breast cancer. The patient underwent a right modified radical mastectomy for T3N3 invasive ductal carcinoma and received adjuvant chemotherapy and radiotherapy. After 2 years, she developed metastasis to the brain, bones, and lungs. She was found to have a growing tender mass on the tip of right 5th finger. A well-demarcated, soft tissue mass was identified on sonography. Under the clinical impression of a possible benign process, the nodule was surgically removed. The pathologic finding was consistent with metastatic breast cancer. Immunohistochemical staining for estrogen receptor, progesterone receptor, and HER2 were all negative, as in the primary tumor. Radiation was given to the finger tip and systemic chemotherapy with capecitabine was tried for systemic metastatic disease. (Korean J Med 75:112-114, 2008)

Primary craniocerebral plasmacytomas are uncommon and they account for 0.7% of all plasmacytomas (PCMs) They comprise two groups : PCM of the skull and dural PCM. We report one case of solitary dural PCM with calvarial involvement and discuss the clinical features and treatment of this uncommon tumor. A 24-year-old man was referred under impression of malignant tumor after biopsy of scalp mass in the left frontal area at a private clinic. According to radiological findings(skull X-ray, bone scan, brain computed tomography & magnetic resonance seans), our preliminary diagnoses included a meningioma or a metastatic. The epidural and subdural tumor as well as the infiltrated skull with scalp mass wee extensively resected. Histological examination with im-munohistochemical and ultrastructual study confirmed a PCM. There was no evidence of systemic myeloma. The patient received postoperative radiotherapy. Seventeen months after surgery, the patient is still in good health without mani-festations of multiple myeloma. We think differential diagnoses of epidural mass involving overlying skull should include PCM as well as meningioma and metastatic tumor.
