        지역사회 노인의 신체기능 평가도구 개발

        Objectives : To develop a physical functioning instrument for older adults living in the community. Methods : A representative sample of 979 people aged 65 years or over were interviewed in-person. Of these, 199 people also completed a detailed in-hospital examination. The scale items were selected based on the frequency of endorsement, along with the item-total and inter-item correlations. The associations of the scale with their physical performance and clinical examination were analyzed to evaluate the criterion-related validity. Construct validity was assessed using factor analysis, and internal consistency through Cronbach's alpha and item-total correlations. Test-retest reliability was measured by agreement between the household survey and the repeat survey at the in-hospital examination. Results : Initially, 23 items on the level of difficulty, ranging from no difficulty to an inability to complete a task, with the specific mobility and self-care tasks were included. Those with a high frequency of endorsement and a low inter-item or item-total correlations were excluded, resulting in a 10-item Physical Functioning (PT) scale. Equal weights were given to each item and a summated score was calculated. Significant associations were found between the PF scores and the physical performance, surrey and clinical data. The scale revealed a 2-factor (mobility and self-care) structure. Cronbach's alpha was 0.92 and the item-total correlations were in the 0.63 to 0.78 range. Pearson's correlations for the test-retest ranged between 0.56 and 0.61. Conclusions : The newly developed Physical Functioning (PF) scale showed good psychometric properties in older people. Further work, however, is needed to improve its sensitivity to discriminate higher levels of functioning, in addition to assessing its predictive value in detecting changes in health.

        The skip-lot concept is very useful in the design of sampling plan for destructive Inspection because of reduced average sample size. An economic design which minimizes The average cost per unit is made of skip-lot sampling plan for destutive testing. Therefore this paper aims to propose the economic design of skip-lot sampling plan for Lot-inspection designated as type SKSP-2, where provision is made for skipping inspection of some of the lots when the quality of the submitted product is good.

        The purpose of this study was to compare the occupational health concerns and opinions among 4 groups : workers, employers and managers, government officials, and health & safety managers. It could help establishing occupational health plans efficiently and providing the way to solve health problems in workplaces in the Inchon area. The delphi technique which is used for deciding group opinion was adopted for this study. Questionnaires regarding health problems and their priorities in the workplaces were sent to four groups three times. All items were measured by five degree ordinal scales. The four groups agreed with questionnaire items, improvement of working environment, occupational health concerns of the employers, the health concerns of workers, and measurement and analysis of working environment, as the upper five priorities for solving the occupational health problems. Besides with the first five priorities, health examinations, health education, and occupational diseases were suggested as important health problems in workplace.

        Background and Objectives: We evaluated the utility of a combined approach using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) and transesophageal bronchoscopic ultrasound-guided fine-needle aspiration (EUS-FNA-B/E) for mediastinal staging of lung cancer. Methods: An EBUS-TBNA database was analyzed retrospectively. EUS-FNA-B/E was performed after EBUS-TBNA when mediastinal lymph nodes were not accessible using EBUS-TBNA or when tissue sampling using EBUS-TBNA alone was inadequate. Results: During the study period, 44 patients were enrolled. EBUS-TBNA and EUS-FNA-B/E were performed on 79 and 52 lymph nodes, respectively. The sensitivity, specificity, and accuracy of mediastinal N staging using EBUS-TBNA alone were 79%, 100%, and 84%, respectively. The sensitivity, specificity, and accuracy of mediastinal N staging using a combination of EBUS-TBNA and EUS-FNA-B/E were 100%, 100%, and 100%, respectively. Significant differences in sensitivity (P=0.008) and accuracy (P=0.004) of mediastinal N staging were evident when EBUS-TBNA alone and the combined procedure were compared. The nodal stage shifted higher after use of the EUS-FNA-B/E procedure in six cases (13%). No serious complication associated with the procedures was noted. Conclusions: Use of a combination of EBUS-TBNA and EUS-FNA-B/E can afford better sensitivity and accuracy of mediastinal N staging compared with use of EBUS-TBNA alone.         Since the 100PPM Quality Certification System has been established by the Industrial Advancement Administration in 1995, The 100PPM Quality Innovation Movement has contributed much to the enhancement of quality competitiveness of Korean parts industry. In 2000, raised to a higher level, The Single PPM Quality Certification System has been established by the Small and Medium Business Administration. The Single PPM Quality Innovation Movement seeks to reduce the number of defects to below 10, out of one million industrial items produced. Ultimately, The Single PPM Quality Innovation Movement aims at achieving perfection, by enabling a zero defect rate in industrial production through the participation of all members of an organization. The Movement was mainly focused on parts suppliers of electronic, electric, automotive, mechanical industries. However, this movement should expand to all the industries. So this study aims to suggest a methodology to expand current of the Single PPM Quality Certification Scope.
