Background: Methods of estimating the erythema and pigmentation reactions, such as MPE (Minimal Perceptible Erythema), MED(Minimal Erythema Dose) and MMD(Minimal Melanogenic Dose), are vulnerable to the variations of subjective visual measurements. Objective : By using objective and quantitative methods, the erythema and pigmentaton responses were investigated. Methods : A sunlight, fluorescent lamp(Waldmann UV 800) was used as a UVB light source. Multiple sites of the lower back were irradiated with an increasing dose by a constant amount, With a reflectance spectrophotometer (DermaSpectrometer), erythema and melanin indices at control, MPE, MED, MMD sites were assessed. The dose-response angle of erythema and pigmentation were calculated. Results : The erythema and pigmetation indices at the control, MPE, MED and MMD sites were 8.7+2.0AU/30.5+2.6AU, 10.5+2.7AU/32.6+3.6AU, 13.2+3.5AU/32.6+3.3AU, and 10.4+3.0AU/ 33.5+2.7AU, respectively. The calculated dose-response angle accurately measured the response of skin to UVB irradiation. Conclusion . In this study, use of a reflectance spectrophotometer appeared to be a more sensitive and effective method than visual assessment, in addition, dose-response angles may be an objective index for determination of skin responses to UVB irradiation. 접촉피부염 환자에서 화장품 및 향료 항원군 이외의 부가항원 첩포시험 결과에 대한 고찰

Background: There are few epidemiological studies on patch test with additional batteries in Korea. Objective : We report the results of patch tests with additional batteries which are relatively uncommonly used in contact dermatitis patients. Methods : Standard patch testing was performed with Finn chamber on Scanpor tape. We used additional batteries including cosmetic and fragrance series which are commonly used, and the others including 14 batteries(so-called other batteries). Results : Over the last 8 years, 719 patients(male 184, female 535) were tested in our contact, dermatitis clinic. 178 patients(24.8%) were tested with both Standard and other batteries, In other batteries, rubber additive series were most frequently used(9.2%), and textile colours finish(4.3%) and hairdressing series(3.6%) were used in order of frequency. Rarely performed batteries were(meth) acrylate ser ies, photographic chemicals series, wood set, oil & cooling fluid series, and various allergens. Patients tested with steroid batteries did not show positive reactions. Conclusion : We suggest that 6 batteries((meth)acrylate series, photographic chernicals series, wood set, oil & cooling fluid series, various allergens, steroid battery) have law testing frequencies and positive rates. Background: A number of in vitro skin models have been developed for the purpose of the screening of cosmetics, pharmaceuticals, and environmental chemicals. To mimic the skin in vivo, a model should resemble morphologically and biochemically the parent, tissue. Objective : The purpos of this study is to study the differentiation and organization of the artificial epidermis in comparsion with epidermis in vivo based on the expression of epidermal protein antigens and basement membrane components. Methods : Human keratinocytes were cultured on deepidermidized dermis (RE-DED) or on fibroblast-populated collag-,n matrix (LSE). After 10 days culture, the sections of RE-DED and LSE were stained with hematoxylin and eosin. An immunohistochemical study was also performed with the sections of RE-DED and LSE using antibodies recognizing proliferating cell nuclear antigens (PCNA), epidermal growth factor receptor (EGFR), keratin 1, involucrin, filaggrin, loricrin, keratin 13, type IV collagen, and laminin. Results : In both culture systems(RE-DED and LSE) a multilayered epidermis with a horny layer was observed. In the human epidermis reconstructed by both culture systems, differentiation markers appeared but with a topography slightly different from that of epidermis in vivo, and components of the basement membrane was also expressed. Conclusion : Our findings suggest the epidermis obtained in both culture systems(RE-DED and LSE) resembled in vivo epidermis morphologically and biochemically, although it was not the same. The cases of bullous eruption of systemic lupus erythematosus in this study were two females. They were the age of 18 and 15. Both of them met ARA criteria for a diagnosis of SLE, and the eruptions developed long after the diagn- osis. In both cases bullae were presented on the face, neck and upper extremities. Administration of dapsone showed complete clearing of lesions within several days. Routine histology of the lesion showed subepidermal blisters, an intact epider- mis, ancl neutrophilic microabscess in dermal papillae. Direct munofluorescence of perilesional and normal forearm skin showed coarse linear pattern of immune deposits at the basement membrane zone in both cases. Circulating anti-basement membrane zone IgG auto-antibodies were detected at a titer of 1: 32 and 1: 16 in each case on NaC1 separated skin substrates.

11,609 of childhood new outpatients seen in Department of Dermatology of Seoul National University Hospital from 1975 to 1984 were analyzed statistica,lly. The study results were summarized as follows: l. 11, 609 out of 48, 188 new outpatients were under 15 years old age, amounting to 24. l %. 2. The 10 most common dermatoses were atopic dermatitis, vitiligo, urticari, hemangiorna, contact dermatitis, fungal disease, molluscum contagiosum, scabies, impetigo and nevocellular neuvs. The most freguent age groups were 2 to 6 years amounting to 47 2p, af total outpatients. 4. The peak incidence occurred in August, while the lowest incidence occurred in November and October. 5. Dermatoses which showed increasing tendency were nevocellular nevus and vitiligo, those of decreasing tendency were contact dermatitis and impetigo,
자외선 조사 후의 인체 각질형성세포에서의 IL-1 생산 및 Retinoid가 이에 미치는 영향

The purpose of this study is to observe the effect of UVB and retinoid on the production of IL-1 in cultured human keratinocytes and to determine whether retinoid can influence the effect of UVB on the IL-1 production. Human keratinocytes derived from normal foreskin were treated with various dose of UVB(0-100mJ/cm2) or retinol acetate(0-5 x 10-6M) and cultured for 1-3 days. IL-i levels in the culture supernatants and cell extracts of the cultured keratinocytes were measured by thymocyte costimulation assay. We demonstrated that UVB and retinol acetate increased the production of IL-1 by human keratinocytes, and the stimulatory effect of UVB on the production of IL -1 in keratinncvt.es was also increased by retinnl acetate.

Background: It has been widely accepted that the prevalence of tinea pedis in soldiers may be higher due to their occlusive army boots and communal baths. Objective : Our study was designed to investigate the prevalence of tinea pedis in Korean soldiers monthly throughout the year by the rank. of the soldiers and also to verify whether tinea pedis is one of the occupational diseases in Korean soldiers. Methods : We examined the feet of soldiers in a battalion monthly throughout the year. On those with suspected skin lesions, microscopic examinations along with fungus cultures were performed in July, 1996. Results: Average prevalence of tinea pedis throughout the year was 51.5% on clinical examination. The highest prevalence was 74.0% in August and the lowest in February by 36.7% The prevalence of tinea pedis by rank was 38.3%(18/47) in the new private group, 61.7%(29/47) in the private group, 84.8%(67/79) in the private 1st class group, 72.4%(55/76) in the corporal group and 70.9%(39/55) in the sergeant group in July, 1996. Dermatophytes were demonstrated microscopically in 59.1%(123/208) of the specimens examined. Cultures were positive in 93 cases, i.e., from 44.7% of those examined. Conclusion: The major factor contributing to the highest prevalence of tinea pedis in August among the months of the year seems to be the hot and damp weather conditions(especially in July and August). Considering the fact that the prevalence of tinea pedis in winter season was also higher by 36.7-40.2%, Korean soldiers seem to possess higher risk of tinea pedis due to their tight-fitting army boots. The spread of infection among non-infected soldiers might have resulted from the communal bath and Korean traditional habit of taking-off their shoes in a large dormitory where they sleep together. Moreover, the fact that the prevalence of tinea pedis increases markedly with their ranks suggests that tinea pedis should be considered as an occupational disease among Korean soldiers.
표재성 피부 진균증의 치료에서 미코스퍼® 크림의 치료효과에 관한 임상연구

Bifonazole(Mycospor®, an imidazole derivative, is a recently developed broad spectrum antifungal agent. This clinical and mycological study was made to evaluate the therapeutic effect of Bifonazole in superficial fungal diseases. It was made with 84 outpatients of the Dermatologic Clinic of Seoul National University Hospital, suffering from superficial fungal infections. The period of this study was April to September, 1986. These patients were treated with Mycospor® cream(1% bifonazole cream) once daily for three weeks, and evaluated on clinical symptoms and signs, and mycological test results before and after therapy. The results were as follows: 1. After 2 weeks of treatment, 92.6% of the 68 patients with dermatophytosis revealed therapeutic improvement, based on marked clinical improvement and negative mycology. The results in each dermatophytic infection were as follows: tinea pedis-90.3%(28/31), tinea cruris-95.7%(22/23), tinea corporis-90.9%(10/11), tinea manus-100%(3/3); The average improvement rate was 92.6%(63/68). 2. After 3 weeks of treatment, 100% of the 16 patients with tinea versicolor revealed negatiye conversion, based on KOH examination and Wood's lamp examination results. 3. No serious side effects were noted, with the exception of one case of mild irritation and erythema on the application site.
