        중간대뇌동맥과 두개외 속목동맥의 급성 일렬폐색에 의한 초급성 뇌경색 환자에서 성공적인 혈관내 물리적 혈전제거치료 성공 증례

        Previous studies have shown that patients with tandem occlusions involving extracranial internal carotid artery (ICA) and middle cerebral artery (MCA) have lower likelihood of recanalization by standard intravenous thrombolytic therapy. A 70-years-old man with a history of hypertension was admitted because of left hemiplegia and drowsiness which developed 47 minutes ago. On neurologic examination, he was drowsy and showed neglect syndrome as well as left hemiplegia. Brain CT angiography showed tandem occlusions at right extracranial ICA and proximal MCA. The administration of intravenous (IV) tissue plasminogen activator (tPA) did not improve his symptoms. We performed angioplasty and stenting for proximal ICA occlusion followed by mechanical thrombolysis for MCA occlusion. After the endovascular treatment, the MCA was recanalized and the patient recovered to show only mild left side weakness. This case shows successful treatment of hyperacute ischemic infarction from tendem occlusion of right MCA and proximal ICA with endovascular treatment following IV tPA thrombolysis.         초미분말 애시를 혼합한 시멘트의 물성

        Efects of ultrafinely ground ash on the rheological properties of cement paste were investigated. Also compressive strengthasincluded for comparison. According to the results of ultrafine ash blended cement paste in the lower W/B ratio, the fluidity were high,and the setting time was a little retarded. And the compressive strength of ultrafine ash blended mortar was increased in the long term.In the case of hardened cement paste at 28 days, Ca(OH)2 contents was decreased in order of control, ultrafine ash, silica fume blendedcement due to diference of the pozzolanic reaction.

        Effects of D-Serine and MK-801 on Neuropathic Pain and Functional Recovery in a Rat Model of Spinal Cord Injury

        Objective: Neuropathic pain is a common secondary complication of spinal cord injury (SCI). N-methyl-D-aspartate (NMDA) receptor activation is critical for hypersensitivity in neuropathic pain. This activation requires the binding of both glutamate and the D-serine co-agonist to the NMDA glycine site. We evaluated the effects of D-serine on neuropathic pain after SCI and explored the underlying molecular mechanisms. Methods: Anesthetized rats underwent T9 spinal cord contusion (130 kdyn). D-serine (500 and 1,000 mg/kg) and MK-801 hydrogen maleate (2.0 mg/kg) were injected daily for 2 weeks, starting the day after SCI. Functional outcomes were assessed according to the Basso, Beattie, and Bresnahan scale, while histological outcomes were evaluated based on lesion volume and spared tissue area. Mechanical allodynia and thermal hyperalgesia were evaluated by measuring the withdrawal threshold of a von Frey filament and hot/cold plate latency. Western blotting was performed to determine the expression levels of Trpv1, Nav1.9, calcitonin gene-related peptide (CGRP), and β-actin in damaged tissue. Results: The withdrawal threshold values and latency of the D-serine group were significantly lower than those of the noninjection group. The MK-801 group showed higher threshold values and latencies than the other groups. Western blotting showed increased Nav1.9 and Trpv1 levels and lower CGRP levels in the D-serine group, whereas the MK-801 group showed the opposite results. Conclusion: D-serine increases neuropathic pain after traumatic SCI by mediating the NMDA receptor. NMDA receptor antagonists alleviate neuropathic pain after traumatic SCI.

      • Effect of Surface Finishing Materials on the Moisture Conditions in Concrete: Vapor and water permeability of finishing materials under changing environmental conditions

        Permeability to vapor and water among other performances required for finishing materials is dealt with in this study. The relative moisture content of concrete coated/covered with a finishing material was experimentally investigated while changing the environmental conditions including temperature, relative humidity, and rainfall. An organic paint (water-based urethane), organic synthetic resin emulsion-type film coating (film coating E), and inorganic porcelain tiles were selected as the finishing materials. When compared from the aspect of vapor and water permeability, the vapor permeability and water permeability of water-based urethane were high and low, respectively; those of film coating E were high and high, respectively; and those of porcelain tiles were low and low, respectively. This means that the moisture state of concrete structures is governed not only by the environmental conditions but also by the performance of finishing materials. It is therefore of paramount importance to appropriately select a finishing material to address the specific deteriorative factors involved in the concrete structure to be finished.

        Subarachnoid Hemorrhage with Negative Baseline Digital Subtraction Angiography: Is Repeat Digital Subtraction Angiography Necessary?

        Introduction:Patients with negative initial digital subtraction angiography (DSA) are at significant risk for re-bleeding, which can lead to severe disability and death. The purpose of this study was to evaluate the necessity of repeat DSA in subgroups of patients with subarachnoid hemorrhage (SAH) with negative initial DSA. Methods:A total of 904 spontaneous SAH patients were admitted to our department between May 2005 and May 2012. Twenty eight patients were selected for inclusion in this study because repeated DSA performed due to the etiology of the SAH could not be demonstrated on the initial DSA. According to the SAH pattern on initial computed tomography scans, patients were divided into perimesencephalic nonaneurysmal SAH (PN-SAH) and non PN-SAH (NPN-SAH) groups. Repeat DSA was performed in all patients, and two of these patients underwent a third DSA. Results:Of the 904 patients, 28 patients (3.1%) had no vascular abnormality on initial DSA. Sixteen PN-SAH patients underwent a repeat DSA; however, no aneurysms were found. In contrast, 12 patients with NPN-SAH underwent repeat DSA, with detection of two cerebral aneurysms. Overall, the false-negative rate of the initial DSA was 7.1% (2/28 patients). No significant differences in false-negative results on initial DSA were observed between the PN-SAH and NPN-SAH groups. Conclusion:In the line with the results of the current study, we should be highly suspicious of patients with a nonaneurysmal SAH, especially those with a NPN-SAH pattern. In order to reduce the morbidity and mortality resulting from a misdiagnosis, repeat DSA is necessary, and exclusion of an aneurysm is important.

        The Impact of Modern Property Law on Female Property Rights in Korea: Toward a Fair Inheritance System

        Japan enacted German-style inheritance laws in its Asian colonies of Taiwan,Korea, and Micronesia after it colonized these areas. Before colonization, the areas had different inheritance laws. Land had been passed from a father to a son in Taiwan and Korea. In Micronesia, land had been passed from a father to a daughter (subject to the father's revocation). In principle, the newly adopted law allowed a spouse, sons, and daughters to receive a relatively fair inheritance. Facing the conflicts between traditional customs and newly introduced law, locals circumvented the law. This paper investigates how the new inheritance law impacted female property rights that were different from customary practice. In order to distinguish“customary rights of the oldest son as a clan leader” from “gender discrimination,”this paper studies the “difference in differences” in inheritance between “the oldest son and the youngest son” and “the oldest son and the oldest daughter.” A comparative analysis of the differences suggests that the new inheritance law is fair and that it improves female property rights even though gender discrimination remains substantial.

        석탄재의 함량변화가 시멘트 클링커의 소성성 및 상변화에 미치는 영향

        Coal ash generated from thermal power plants using briquettes contains Si, Al, and Fe components. These components are the main components required for the manufacture of cement clinker. In particular, Al and Fe components form the interstitial phase of cement clinker and have an important effect on the sintering of cement clinker. In this study, a large amount of coal ash was applied as a raw material for cement clinker by content, and the mineral formation process of cement clinker to which coal ash was applied was confirmed by sintering temperature. It was confirmed that the intermediate phase was generated in the sintering temperature range of 1050 ~ 1150 °C in the cement clinker to which a large amount of coal ash was applied. As the content of coal ash increased, the production amount of the intermediate phase increased. The phase produced by the addition of coal ash is expected to be converted to calcium silicate phase and interstitial phase and disappear above 1350 °C. The cement clinker applied with a large amount of coal ash at 1450 °C formed well-developed minerals equivalent to the standard cement clinker. 미연탄을 사용하는 화력발전소에서 발생되는 석탄재는 Si, Al, Fe 성분을 포함하고 있으며, 이들 성분은 시멘트 클링커에제조에 필요한 주요성분이다. 특히 Al, Fe 성분은 시멘트 클링커의 간극상을 형성하는 성분으로 시멘트 클링커의 소성에중요한 영향을 미친다. 본 연구에서는 시멘트 클링커의 원료로서 다량의 석탄재를 함량별로 적용하여, 석탄재를 적용한 시멘트클링커의 소성온도별 광물형성 과정을 확인하고자 하였다. 석탄재를 다량 적용한 시멘트 클링커는 1050∼1150 ℃의 소성온도구간에서 중간상이 생성되는 것을 확인하였고, 이는 석탄재의 함량이 증가할수록 생성량이 증가하였다. 석탄재의 첨가로생성된 상은 1350 ℃ 이상에서 칼슘실리케아트상과 간극상으로 전환되어 소멸되는 것으로 예상된다. 시멘트 클링커의 일반적인 소성온도인 1450 ℃에서는 다량의 석탄재를 적용한 시멘트 클링커는 순수원료를 사용하여 제조한 기준 시멘트 클링커와동등수준의 잘 발달된 광물을 형성하였다.

        The Kyoto Protocol, Competitiveness, and International Trade: A View through the Lens of a Geographically Weighted Regression Study

        Purpose The Kyoto Protocol imposes greenhouse gas emission ceilings on developed countries. However, on account of there being different reduction targets and conditions, a loss of competitiveness is expected among developed countries. The main objective of this study is to examine the impact of the Kyoto Protocol on international trade. Design/Methodology/Approach Geographically Weighted Regression (GWR) allows the coefficients of explanatory variables to differ by locality by assigning relatively more weight to geographically close observations. By studying the spatial patterns this paper investigates whether the competitiveness is observed or not. Findings GWR results show that while a loss of competitiveness can be observed among non-European developed countries (i.e., Canada, the United States, Japan, and New Zealand), no loss of competitiveness is clearly seen in the European Union. Research Implications The results based on the global gravity model indicate that there is no clearly observable loss of competitiveness among developed countries, but the spatial pattern shows the loss of competitiveness across regions. The results imply that spatial patterns need to be considered in international trade.
