        Early Results of Endovenous Ablation with a 980-nm Diode Laser for an Incompetent Vein of Giacomini

        Objective: We wanted to evaluate the effectiveness of endovenous ablation of the incompetent vein of Giacomini using a 980-nm diode laser. Materials and Methods: A total of 18 patients (18 limbs, 4%) had the incompetent vein of Giacomini. Retrograde reflux originating from the great saphenous vein was noted in sixteen limbs and paradoxical diastolic anterograde reflux from the saphenopopliteal junction was observed in two limbs. After tumescent anesthesia, laser ablation using a 980-nm wavelength laser fiber was performed under ultrasound and/or fluoroscopic guidance. Patients were evaluated clinically and with duplex ultrasound at one week and at one, three, six and twelve months after laser ablation for the technical and clinical success. Results: In the 18 limbs, the technical success rate was 100%. Continued closure of the vein of Giacomini was seen in 18 of 18 limbs after one month, in 12 of 12 limbs after three and six months and in six of six limbs after twelve months. No recanalization of the vein and no major complications occurred. Conclusion: Endovenous laser ablation with a 980-nm wavelength is an effective and safe procedure for treating an incompetent vein of Giacomini.         Malignant mesothelioma (MM) is a rare tumor that associated with asbestos exposure. For the reliable diagnosis, the adequate representative tissue samples are essential for the histology and immunohistochemical staining. We report a case of desmoplastic malignant mesothelioma (DMM), accounting for only 5 to 10% of all MM and having poor prognosis. A 76-year-old male visited emergency room presenting with chest pain. Chest computed tomography showed focal lobulated pleural enhancing mass in posterior aspect of left upper lobe. After video-assisted thoracoscopic surgery and several immunohistochemical stains, we could diagnose as DMM. The patient refused chemotherapy, but received analgesics and palliative radiation for the painful back area. Subsequently,he was transferred to hospice clinic after 17 months from appearance of the initial symptom.

        Fluoroscopy-Guided Endovenous Sclerotherapy Using a Microcatheter Prior to Endovenous Laser Ablation: Comparison between Liquid and Foam Sclerotherapy for Varicose Tributaries

        Objective: To compare the efficacy and adverse effects of endovenous foam sclerotherapy (EFS) and liquid sclerotherapy (ELS) using a microcatheter for the treatment of varicose tributaries. Materials and Methods: From December 2007 to January 2009, patients with venous reflux in the saphenous vein were enrolled. The foam or liquid sclerosant was injected through a microcatheter just before endovenous laser ablation (EVLA). Patients were evaluated for the technical success, clinical success, and procedure-related complications during the procedure and follow-up visits. Results: A total of 94 limbs were included: 48 limbs (great saphenous vein [GSV], 35; small saphenous vein [SSV], 13) were managed using EFS and EVLA (foam group; FG), and 46 limbs (GSV, 37; SSV, 9) were treated by ELS and EVLA (liquid group; LG). Varicose tributaries demonstrated complete sclerosis in 92.7% with FG and in 71.8% with LG (p = 0.014). Bruising (78.7% in FG vs. 73.2% in LG, p > 0.05), pain or tenderness (75.6% in FG vs. 51.2% in LG, p = 0.0237) were noted. Hyperpigmentation (51.2% in FG vs. 46.2% in LG, p > 0.05) was found. Conclusion: Endovenous foam sclerotherapy using a microcatheter is more effective than ELS for eliminating remnant varicose tributaries prior to EVLA. However, EFS is more commonly associated with local complications such as pain or tenderness than ELS. Furthermore, both techniques seem to prolong the duration of hyperpigmentation along with higher costs.

        대구경 현장타설 에너지파일의 열교환 성능과 시공성 및 경제성 분석

        에너지파일이란 새로운 형식의 지중열교환기로서, 건물의 기초 구조물인 말뚝 내부에 열교환 파이프를 삽입하고파이프 내부로 유체를 순환시켜 지반과 말뚝 매질사이의 열교환을 유도한다. 에너지파일은 기존의 기초 구조물을활용하여, 구조물의 지지 기능과 지중열교환기로서의 기능을 동시에 수행하는 에너지 구조체이다. 본 연구에서는에너지파일의 실증연구를 위하여 총 6가지 형태의 열교환 파이프가 삽입된 실규모 현장타설 에너지파일을 시험 시공하였다. 열교환 파이프의 형태는 다양한 형태별 성능 및 시공성 비교를 위해 병렬 U형 3본(5쌍, 8쌍, 10쌍), 코일형2본(피치간격 200mm와 500mm), S형 1본으로 선정하였다. 총 6가지 형태의 열교환 파이프를 시공하면서 시공 소요시간, 인력 소요, 필요 부대시설, 특이사항 등을 정리하여 각 형태별 시공성을 평가하였다. 또한 시공된 현장타설에너지파일에 대한 열교환 성능 평가시험을 수행하였다. 열교환 성능 평가시험은 실제 상업 건물의 냉방 부하를 모사하기 위하여 8시간 가동 16시간 휴지의 간헐적 가동을 통하여 수행하였다. 열교환 성능 평가시험 결과를 통하여 각 현장타설 에너지파일의 열교환량을 산정하였으며, 이를 에너지파일 근입 깊이 및 열교환 파이프 길이로 정규화하여각 형태별 열교환 효율을 평가하였다. 마지막으로 현장타설 에너지파일의 경제성을 분석하기 위하여 각 형태별 에너지파일의 시공비와 에너지파일 단위 길이 당 열교환 성능을 통해 1W/m 당 소요 비용을 산정하였다. An energy pile is a novel type of ground heat exchangers (GHEX’s) which sets up heat exchange pipes inside apile foundation, and allows to circulate a working fluid through the pipe for exchanging thermal energy with thesurrounding ground stratum. Using existing foundation structure, the energy pile can function not only as a structuralfoundation but also as a GHEX. In this paper, six full-scale energy piles were constructed in a test bed with variousconfigurations of the heat exchange pipe inside large-diameter cast-in-place piles, that is, three parallel U-type heatexchangers (5, 8 and 10 pairs), two coil type heat exchangers (with a 500 mm and 200 mm pitch), and one S-typeheat exchanger. During constructing the energy piles, the constructability of each energy pile was evaluated withconsideration of the installation time, the number of workers and any difficulty for installing. In order to evaluate thethermal performance of energy piles, the thermal performance tests were carried out by applying intermittent (8 hoursoperating-16 hours pause) artificial cooling operation to simulate a cooling load for commercial buildings. Through thethermal performance tests, the heat exchange rates of the six energy piles were evaluated in terms of the heat exchangeamount normalized with the length of energy pile and/or the length of heat exchange pipe. Finally, the economicfeasibility of energy pile was evaluated according to the various types of heat exchange pipe by calculating demandedexpenses per 1 W/m based on the thermal performance test results along with the market value of heat exchange pipesand labor cost.

        유기성폐자원의 에너지 회수: EU의 바이오가스 생산과 정책 동향

        Results of reviews on bio-gas production and policy trends in the European Union (EU) are as follows. In the EU,Germany leads in bio-gas production with 29 TWh of energy produced through energy crops as of 2013. This could beachieved through renewable energy laws and increases in feed in tariff (FIT) schemes in Germany. In the EU, bio-gashas been verified to play an important role and contribute to greenhouse gas reduction. However, it is necessary to providea measure to improve sustainability criteria and decrease the consumer's share of expenses. If bio-gas is produced usingorganic wastes instead of energy crops, this problem can be solved. If the bio-gas production policies in the EU are appliedin South Korea, bio-gas market will be promoted and greenhouse gas emission can be reduced in the future.
